发明概述
本发明的一个实施方式涉及用于在椎骨元件间定位的体间间隔物。间隔物可包括一主体,主体具有前壁和后壁以及相对的第一和第二面。间隔物可还包括许多齿,其取向允许间隔物插入椎骨元件间的间隙,并在间隔物定位后防止或限制移动。
齿可位于第一和第二面中的一个或两个上。齿可位于有限区段内,或者横跨整个面。

发明详述
本发明的一个实施方式涉及用于插入椎骨元件间的体间间隔物。间隔物10包括一主体,主体具有下表面18和上表面17。齿30被设置在至少沿表面17,18中的一个,以维持相对于椎骨元件的位置。齿30以覆盖表面17,18中至少一个的全部或部分的模式排列。
图1显示了一个实施方式的间隔物10,它具有环形形状,形成开口12。间隔物10由相对的后壁和前壁13,14形成,并由相对的两侧壁15完成环形状。前壁14比后壁13具有更大的高度,使得间隔物10具有楔形形状。壁中可具有一个或多个孔19。孔19提供了在外科手术期间用器械抓握或移动间隔物10的途径。孔19可带有螺纹以匹配器械。孔19延伸通过壁,进一步提供通往开口12的通路,例如用于插入骨生长物质。
图2显示了大致D形的间隔物10的顶视图。该形状由圆形前壁14和基本上直的后壁13所形成。开口12的内壁大致匹配外壁形状,也为大致D形。开口12外壁间形成的间隔物壁的厚度在整个间隔物10中基本一致。然而,应理解,间隔物10和开口12可具有许多不同的形状,间隔物壁的厚度可独立地改变,各自取决于使用参数。
横平面被定义为沿间隔物侧向对齐并且延伸通过侧壁15。横平面基本上平行于横向中心线T-T。纵平面被定义为沿间隔物10对齐并且延伸通过前壁和后壁14,13。纵平面基本上平行于纵向中心线L-L。
图3显示了间隔物10的侧视图,更清楚地描绘了由较高的前壁14和较矮的后壁13形成的楔形形状。当从侧面观察间隔物10时,上表面17和下表面18通常为凸面。最大高度从前壁14向内(即,在前壁14和后壁13之间的点)。在图3所示实施方式中,最大高于更接近前壁14。间隔物10可围绕侧中心线99基本均一。因此,中心线99和下表面18间的距离与中心线99和上表面17间的对应距离相同。线98与中心线99相切形成角Z。角Z可约为2°-7.5°。因为间隔物10围绕中心线99均一,上表面17和中心线99间形成互补且相等的第二角(未示出)。因此,上下表面17,18的切线间形成的总的间隔物角度约为4°-15°。
许多齿30位于下表面和上表面18,17中的一个或两个上。图4显示了一个实施方式的齿30的视图,它们各自具有面向间隔物10前壁14的两个前侧表面31。前侧表面31相互倾斜并沿共用前缘36相连。各个齿30还包括面向间隔物10后壁13的两个后侧表面32。后侧表面32相互倾斜并沿共用后缘37相连。在图4所示实施方式中,各个齿30大致为矩形,由在各个面上被路径34限定的四个面所形成。各个齿30还包括在前侧表面和后侧表面31,32相交处的顶点33。
图5是在横平面中沿线5-5切割(参见图3)的截面图。视图贯穿第一行齿30,显示了第一行和相邻的第二行的顶点33。齿30的相邻两行发生偏移,使得第一行的顶点33对准相邻的第二行的谷。谷的底部位于在齿30间延伸的路径34中。后侧表面32弯曲形成圆形后缘37。
图6是在纵平面中沿线6-6切割(参见图2)的截面图。视图贯穿第一列齿30,显示了第一列和相邻的第二列的顶点33。齿30的相邻列发生偏移,使得第一列的顶点33对准相邻的第二列的谷。谷的底部对齐在齿30间延伸的路径34内。
沿纵平面看,由于后侧表面32具有比前侧表面31更小的角,齿30在前侧方向上倾斜。而且,前侧表面32弯曲,下凹的前侧表面31可使顶点33面向前侧方向。这种构型提供了利用前路插入间隔物10。后侧表面32的较小角和顶点33的前角在插入过程期间不会卡住。一旦间隔物10插入椎骨元件间的盘间隙,前侧表面31的较大角和面向前侧的顶点33阻止或限制了间隔物10的前侧移动。图1-6中所示实施方式包括用于前路插入途径取向的齿30。其它实施方式可包括用于不同插入途径的沿不同方向取向的齿30。
路径34在间隔物10的表面以方格图案延伸。路径包括一系列在第一方向上横跨表面延伸的第一平行路径,和一系列在第二方向上延伸的第二平行路径。各个系列的路径34基本上是直的,并且相对于纵平面和横平面形成一定角度。在图2所示实施方式中,第一路径相对于纵平面呈角度θ。第二路径相对于纵平面呈镜像角θ’。镜像角是相对于第一角为负。以例子的方式,各自相对于纵平面,第一路径呈约45°,第二路径呈约-45°。
路径34可以是平面,或倾斜入间隔物10的面,或者这两种形状的组合。特定的实施方式包括U-形路径和J-形路径。在一个实施方式中,第一路径具有第一形状,第二路径具有不同于第一形状的第二形状。
许多齿30和路径34形成均一模式,如图2所示。齿30被设置为沿纵平面基本对齐的一系列列。经列中各个齿30的顶点33画一直线C。在一个实施方式中,线C基本上平行于纵向中心线L-L。齿30还被设置成沿横平面基本对齐的一系列行。经行中各个齿30的顶点33画一直线R。在一个实施方式中,线R基本上平行于横向中心线T-T。第一和第二路径沿齿各边缘相交。在图2所示实施方式中,各交点与横跨间隔物10延伸的线C和R对齐。
齿30各自面向同一方向对齐,并使后侧表面32和共用后缘37具有相同取向。这很好地在图2中显示。随着间隔物10移动进入盘间隙,各个齿30联合运作。若横跨表面的各个齿30具有不同的取向,插入期间齿30不会协同运作。以例子的方式,如果表面第一区段上的齿30与第二区段上的齿30取向不同,间隔物10将较难插入,并可从椎骨间隙中脱出。
第二个实施方式的齿30如图7和8所示。与第一个实施方式相同的标号标号用于表示类似的元件。各个齿30具有朝间隔物10的前侧14对齐的一对前侧表面31,和一对朝后侧13对齐的后侧表面32。齿30各自具有大致矩形形状。前侧表面和后侧表面31,32各自向外延伸并由外表面39所覆盖。路径34沿第一和第二方向延伸并围绕齿30。在一个实施方式中,外表面39各自符合间隔物10表面的凸度。在这种情况下,外表面39相对于总的凸面为平面。
齿30的各齿需按共同的后缘37要面向同一方向来对齐。。而且,齿30的各齿要按共用的前缘36沿相同方向取向来对齐。由于总的凸度和上下表面17,18的角度,齿30可以不相互平行。在一个实施方式中,间隔物10沿各个方向大致均一,可以多种插入角度使用。
图9-11显示了第三个齿实施方式。同样,与第一个实施方式相同的标号用于表示类似的元件。前壁和后壁14,13被侧壁15隔离,形成开口12。齿30可以与许多列(例如沿线C)和行(例如沿线R)对齐的均一模式位于表面上。
每个齿30均具有四个面,形成总的矩形形状。后侧表面37相对于前侧表面36,而两个侧面38分别沿各侧延伸。后侧表面37和前侧表面36基本上宽度相同,两侧面38的宽度也相同。后侧表面37以比后侧表面37小的角度从间隔物10向外延伸。在一个实施方式中,后侧表面37的角度约为70°,前侧表面36的角度约为0°(即,前侧表面36基本上垂直于路径)。在一个实施方式中,侧面38以约0°从间隔物向外延伸。在这些实施方式中,相对于基本平的路径34测定表面的角度。
齿30列对齐,如图9中的线C所示。齿30对齐,使得第一齿30的前侧表面36与相邻第二齿的后侧表面37之间没有空隙。如图11所示,这种间隔形成基本上锯齿状取向。间隔物10表面上的各列沿相同方向对齐。在一个实施方式中,各列基本上平行于纵向中心线L-L。齿30还行对齐,如图9中心线R所示。在一个实施方式中,行基本上垂直于列,并可垂直于纵向中心线L-L。
路径34位于齿30各列之间。在一个实施方式中,路径34宽度小于齿30的宽度(即,小于前侧表面36和后侧表面37的宽度)。
第三个实施方式的齿30的尺寸和构型适合前路插入途径。后侧表面37的角度被设定成便于间隔物10从前侧移动进入盘间隙。前侧表面36具有较陡的角度,以防止间隔物10在插入后从盘间隙脱出。与其它实施方式一样,齿30可以其它取向对齐,用于其它插入途径。
间隔物10的外缘可为圆形或弯曲构型,以防止尖锐角度使插入间隔物10变得较为困难,或者对患者造成损伤。在一个图3所示实施方式中,间隔物10具有沿外缘延伸的圆角92,而在外缘处上下表面17,18与侧壁15以及后壁和前壁13,14相交。特定的实施方式包括约2-3mm的圆角。邻近外缘的齿30可落在圆角区域内,并可倒圆或弯曲。如图7所示的例子,与表面边缘稍微重叠的齿30a在间隔物10的边缘内具有倒圆的部分。齿30b更接近边缘,因而具有更大的倒圆部分。齿30c几乎完全位于边缘内,因而几乎被完全倒圆。沿开口12的齿30也包括倒圆部分,如齿30d所示。
间隔物10可由单块材料构成,在一个或两个表面17,18上形成齿30。或者,齿30可由单独的材料构成并附连于表面17,18。
一个实施方式的间隔物(例如图3)显示,下表面和上表面各自为凸形。其它实施方式还包括基本上平的表面,或者凹的表面。而且,间隔物可具有不同形状的表面,下表面具有第一形状而上表面具有第二形状(例如,凸形下表面和基本上平的上表面)。
术语椎骨元件通常用于描述椎骨几何结构,包括椎体、蒂、椎板和棘突。可调节间隔物10的尺寸和形状,使其具有足够的强度条件而用于椎骨不同区域中,包括颈椎、胸椎和腰椎。
开口12中可具有骨生长物质以促进骨生长穿过间隔物10。骨生长物质可包括浸渍有蛋白质如骨形态发生蛋白(BMP)、LIM矿化蛋白(LMP)等的海绵、基质和/或其它载体。
可以除本文所述之外的其它具体方式实施本发明而不背离本发明的范围和基本特征。齿30还可向内间隔离开表面17,18的边缘。沿表面边缘可存在不包括任何齿30的空隙55。间隔物10可具有多种形状和尺寸。在一个实施方式中,间隔物包括闭合开口12的内壁。在另一个实施方式中,间隔物不包括开口12。在上下表面17,18上都具有齿30的实施方式中,齿30可在各个表面上沿一共同方向对齐。因此,认为本发明实施方式在所有方面是示例性而非限制性的,旨在包括所附权利要求书的涵义和等价范围内的所有改变。
Claims (12)
1.一种体间间隔物,所述间隔物包括:
主体,它具有前侧和后侧以及第一和第二表面;
在所述主体第一表面上的齿,各齿大致为矩形形状并具有面向前侧的两个前表面和面向后侧的两个后表面,所述前表面和后表面相交而形成峰,沿延伸通过所述主体的前侧和后侧的纵平面看,所述后表面具有比所述前表面更小的角,并且所述前表面弯曲并下凹而使所述峰面向前侧方向。
2.如权利要求1所述的间隔物,其特征在于,所述峰从前侧看大致呈V形。
3.如权利要求2所述的间隔物,其特征在于,所述各齿朝前侧倾斜。
4.如权利要求1所述的间隔物,其特征在于,所述各齿还包括位于两个前表面和两个后表面之间的基本上平的外表面。
5.如权利要求1所述的间隔物,其特征在于,所述两个前表面在共用前缘处相交,所述两个后表面在共用后缘处相交,所述共用前缘和后缘对齐并形成基本上直的线。
6.如权利要求1所述的间隔物,其特征在于,所述前侧和后侧具有倒圆的构型。
7.如权利要求6所述的间隔物,其特征在于,所述邻近前侧和后侧的齿中至少一个具有匹配所述倒圆构型的倒圆取向。
8.如权利要求1所述的间隔物,其特征在于,所述主体是具有中央开口的环形形状。
9.如权利要求1所述的间隔物,其特征在于,所述主体具有前侧大于后侧的大致楔形形状。
10.如权利要求9所述的间隔物,其特征在于,所述第一和第二表面为凸形。
11.如权利要求9所述的间隔物,其特征在于,所述第一和第二表面相对于间隔物的中线基本上相等地对齐,所述第一表面和所述中线间形成的第一角基本上等于所述第二表面和所述中线间形成的第二角。
12.如权利要求9所述的间隔物,其特征在于,所述主体具有中央开口,在所述前侧和所述开口之间具有最大高度。
