背景技术
在小切口手术过程中,特别是在眼科手术过程中,小探针被插入手术部分以切割、摘除或其它方式操作组织。在这些外科手术过程中,典型地要将流体注入眼睛中,然后从手术部分吸出注射流体和组织。
在眼科手术期间维持最佳的眼内压力是当前面临的一个难题。在不进行抽吸操作时,眼睛中的压力变成注入眼内的流体的压力。这种压力典型地被叫做“零流量压力”。然而,在实施抽吸操作时,由于与抽吸流动有关的抽吸线路中的所有压力损失,眼内压力从零流量压力明显下降。因此,为了在抽吸作用以其它方式将眼内压力降至软眼状态的时候进行补偿,眼外科医生目前容忍使用比所需要的零流量压力高的压力。然而在临床上,这种眼睛过压是不理想的。
因此,仍然需要寻求一种在眼睛外科手术期间对眼内压力进行控制的改进方法。
发明内容
在一个方面,本发明为一种利用显微外科系统控制眼内压力的方法。提供了容纳冲洗流体的注射室,并选定了所需要的眼内压力。用加压气体对注射室进行加压以向外科设备提供冲洗流体。测量与外科设备流体连通的流体管道内的流体流速。向计算机提供与所测量的流速相对应的信号。由计算机响应该信号计算出预测的眼内压力。响应于来自计算机的第二信号调节加压气体的水平,以将预测的眼内压力维持在所需要的眼内压力附近。
在本发明的另一个方面提供了一种注射室。该注射室具有用于保持冲洗流体的第一腔室和用于保持冲洗流体的第二腔室。第一腔室与第二腔室之间没有流动连通。从注射源向第一腔室和第二腔室提供冲洗流体。在显微外科手术进程中,由第一腔室向外科设备提供冲洗流体,在第一腔室内的冲洗流体的液位达到下限值时,该步骤结束。在这种结束过程中,从第二腔室向外科设备提供冲洗流体,由注射源向第一腔室重新充注冲洗流体。

通过参考图1-2,能最佳地理解本发明的优选实施例及其优点,其中同样的数字用来表示各个附图中的相同和相应的部件。如图1所示,眼睛显微外科系统10包括:压力袖带12;注射源14;双注射室16,其具有腔室16a和腔室16b;液位传感器18和20;流速传感器22;过滤器24和26;外科设备29;计算机或微处理器28;气体歧管30和32;加压气源34;比例电磁阀36、38和40;“开/关”电磁阀42、44、46、48、50、52、54;致动器56、58、60和62;以及压力传感器64、66和68。双注射室16、液位传感器18和20、注射流体管道70、72、74、76、78和80的一部分以及气体管道84和86的一部分优选地设在外科手术盒27中。注射源14、双注射室16、流速传感器22、过滤器24和26以及外科设备29经由注射流体管道70-80流动地连通。注射源14、双注射室16、气体歧管30和32、加压气源34以及致动器56、58、60和62经由气体管道82、84、86、88、90、92、94和96流动地连通。注射源14、液位传感器18-20、流速传感器22、微处理器28、比例电磁阀36-40、开/关电磁阀42-54、致动器56-62以及压力传感器64-68经由接口100、102、104、106、108、110、112、114、116、118、120、122、124、126、128、130和132电连接在一起。
注射源14优选地为柔性注射源。液位传感器18和20分别可以是任何一种适于测量注射室16a和16b的液位的设备。液位传感器18和20优选地能够以连续的方式测量注射室16a和16b的液位。流速传感器22可以是任何一种适于测量流体管道80内的流体流速的设备。流速传感器22优选地为非侵入式流速传感器。过滤器24和26为疏水性微生物过滤器。过滤器优选New York的East Hills的Pall公司销售的Versapor薄膜过滤器(0.8微米)。微处理器28能够执行反馈控制,优选地能够执行PID控制。外科设备29可以是任何一种适于向眼睛提供外科手术冲洗流体的设备,但优选为注射插管,冲洗机头,或冲洗/抽吸机头。
在操作过程中,通过对注射源14进行加压使流体管道70、72和74、注射室16a和16b、流体管道76、78和80以及外科设备29装填有外科手术冲洗流体140。外科手术冲洗液体140可以是任何一种适用于眼睛的外科手术冲洗流体,诸如(为举例的方式)由AlconLaboratories有限公司销售的BBS PLUS

优选地通过压力袖带12向注射源14加压。更具体地,微处理器28分别通过接口106和接口108、110发送控制信号以打开电磁阀42和关闭电磁阀44、46。微处理器28还通过接口104发送控制信号打开比例电磁阀40,使得歧管30供应合适数量的加压空气以致动压力袖带12。压力传感器68感测气体管道82内的压力,并通过接口126向微处理器28提供相应信号。电磁阀48-54开始处于打开状态,以便歧管32提供加压空气以致动致动器56-62,从而关闭流体管道72-78。微处理器28通过接口114-120发送控制信号关闭电磁阀48-54。电磁阀48-54的关闭致动致动器56-62以打开流体管道72-78。在所有腔室和流体管道都得到充注之后,微处理器28关闭致动器56-62,并因此关闭流体管道72-78。或者,可以完全依靠重力作用来对注射源14进行加压。
在装填之后,用户然后通过输入件134向微处理器28提供所需要的眼内压力。输入件134可以是任何一种合适的输入设备,但是优选为触摸显示屏或真实旋钮。腔室16b优选为最初的主动注射室。微处理器28发送合适的控制信号以打开电磁阀44和打开比例电磁阀36(通过接口100),从而向腔室16b提供合适的加压空气水平。压力传感器64感测气体管道84内的压力,并通过接口124向微处理器28提供相应的信号。微控制器28还发送合适的控制信号以打开致动器60,并因此打开流体管道78。腔室16b通过流体管道78、80和外科设备29将加压流体140供应到眼睛。流速传感器22测量流体140的流速,并通过接口132向微处理器28提供相应的信号。微处理器28利用来自流速传感器22的信号计算预测的眼内压力,并且凭经验确定显微外科系统10的阻抗信息。微处理器28随后将合适的反馈控制信号发送至定量电磁36,以在整个外科手术期间将预测的眼内压力维持在所需要的眼内压力或接近所需要的眼内压力。
液位传感器20在外科手术期间连续地监视腔室16b内的流体液位140的下降,并通过接口130向微处理器28提供相应的信号。微处理器28对提供给腔室16b的空气压力实施调节,以在流体140液位下降时适应流体高度差。当腔室16b内的流体140液位达到下限液位时,微处理器28关闭电磁阀44和致动器60,并打开电磁阀46和致动器58和62。现在腔室16a成为主动注射室。微处理器28通过接口102向比例电磁阀38提供合适的控制信号,以向腔室16a提供合适的加压空气水平。压力传感器66感测气体管道86内的压力,并通过接口122向微处理器28提供相应的信号。腔室16a通过流体管道76、80和外科设备29将加压流体140供应到眼睛。流速传感器22测量流体140的流速,并通过接口132向微处理器28提供相应的信号。如上所述那样,微处理器28计算预测的眼内压力,并向比例电磁阀38提供合适的反馈信号,以在整个外科手术期间将预测的眼内压力维持在所需要的眼内压力或接近所需要的眼内压力。一旦腔室16b重新装满了流体140,那么微处理器28就关闭致动器58和流体管道74。
液位传感器18在外科手术期间连续地监视腔室16a内的流体140液位的下降,并通过接口128向微处理器28提供相应的信号。微处理器28对提供给腔室16a的空气压力实施调节,以在流体140液位下降时适应流体高度差。当腔室16a内的流体140液位达到下限液位时,微处理器28将腔室16b转换成主动注射,使腔室16b处于待用状态,并通过流体管道72重新向腔室16a充注流体140。腔室16b和16a之间的这种循环在整个外科手术期间持续发生。
优选地通过流体液位传感器(未示出)监视注射源14,其中所述流体液位传感器能够在注射源14达到几乎空的界限时通过接口112向微处理器28提供信号。腔室16a和16b各自的容积优选地要满足:在注射源14快要空了的时候能够在不中断外科手术进程的情况下更换注射源14。更具体地,腔室16a和16b各自的容积优选地约为30cc。这样的容积能够在最大流动状态(例如瞳孔玻璃体切除)期间提供约两分钟时间用于更换接近空了的注射源14。另外,一旦注射源14被更换,在待用腔室16a或16b重新充注时,流体管道70、72和74内的所有空气泡将自动地被“清除”,而不需要预装填。
在腔室16a或16b中的任何一个出现故障的情况下,微处理器28优选地能够利用单独一个主动腔室继续进行外科手术。在腔室16a和16b均出现故障的情况下,微处理器28优选地能够利用单独的注射源14继续进行外科手术。
图2示出了改进的眼睛显微外科系统10a。显微外科系统10a类似于显微外科系统10,其不同之处在于:除了上述用于系统10的注射系统外,还具有冲洗系统。更具体地,除了系统10a还包括冲洗源200、流体管道202和206、气体管道208和216、电磁阀210和218、致动器214和222、电子接口212和220以及外科设备224之外,系统10a与系统10相同。如图2所示,冲洗源200完全通过重力作用来加压。本领域普通技术人员将能理解,显微外科系统10a允许独立地通过流体管道80将外科冲洗流体140输送到外科设备29(注射),以及通过流体管道206将外科冲洗流体140输送到外科设备224(冲洗)。微处理器28能够通过连续地监视腔室16b内部流体的体积变化来计算流体管道206内的流体140的流动信息,这由流速传感器20指示的。
从上述内容中可以意识到,本发明提供了一种利用显微外科系统控制眼内压力的改进方法。在此是以举例的方式对本发明进行说明的,本领域普通技术人员可以进行各种改进。例如,虽然在上文中是针对眼睛显微外科系统中的眼内压力控制来描述本发明,但是它也适用于在其它类型的显微外科手术期间对实施手术的组织的内压进行控制。

Claims (8)

1.一种利用显微外科系统控制眼内压力的方法,其包括以下步骤:
提供容纳冲洗流体的注射室;
选定所需要的眼内压力;
利用加压气体对所述注射室加压以向外科设备提供冲洗流体;
测量与所述外科设备流体连通的流体管道内的所述流体的流速;
向计算机提供与所述测量流速相对应的信号;
响应于所述信号,利用所述计算机计算预测的眼内压力;
响应于来自所述计算机的第二信号,调节所述加压气体的水平,以将所述预测眼内压力维持在所述所需要的眼内压力附近。
2.如权利要求1所述的方法,其中所述调节步骤包括:响应于所述第二信号,致动与所述加压气体的源流体连通的定量阀。
3.一种利用显微外科系统控制眼内压力的方法,其包括以下步骤:
提供注射室,所述注射室具有用于容纳冲洗流体的第一腔室和用于容纳冲洗流体的第二腔室,所述第一腔室与所述第二腔室之间没有流体连通;
从注射源向所述第一腔室和所述第二腔室提供冲洗流体;
在显微外科手术进程中,从所述第一腔室向外科设备提供所述冲洗流体;
在所述第一腔室内的所述冲洗流体的所述液位达到下限值时,结束所述第三提供步骤;
在所述结束步骤中,从所述第二腔室向所述外科设备提供所述冲洗流体;以及
在所述第四提供步骤期间,从所述注射源向所述第一腔室重新充注所述冲洗流体。
4.如权利要求3所述的方法,还包括以下步骤:
在所述第二腔室中的所述冲洗流体的所述液位达到第二下限值时,结束所述第四提供步骤;
重新启动所述第三提供步骤;以及
在所述第三提供步骤的所述重新启动期间,从所述注射源向所述第二腔室重新充注所述冲洗流体。
5.如权利要求3所述的方法,还包括以下步骤:
检测所述注射源达到第三液位下限值的时刻;以及
在不中断所述第三提供步骤或所述第四提供步骤中的任何一个的情况下更换所述注射源。
6.如权利要求3所述的方法,其中所述结束步骤包括用液位传感器检测所述下限值。
7.如权利要求4所述的方法,其中所述结束步骤包括用液位传感器检测所述第二下限值。
8.如权利要求3所述的方法,还包括以下步骤:
从冲洗源向所述第一腔室或所述第二腔室之一提供所述冲洗流体,所述冲洗源与所述注射源之间没有流体连通;以及
从所述第一腔室或所述第二腔室之一向第二外科设备提供所述冲洗流体。
