背景技术

发明内容

具体实施方式
依照本发明的棘突植入物的实施例包括间隔器和从间隔器向外延伸的延伸部。该棘突植入物可以被构造为用于插入到颈椎、胸椎、和/或腰椎的相邻棘突之间。间隔器可被提供为各种尺寸,以适应病人之间的身体构造上的差异和间隔校正的不同角度。间隔器可包括开口,以便于组织生长从而将间隔器锚固到椎骨本体上,例如从棘突产生的组织生长。间隔器可以被构造为便于组织从上棘突和下棘突向内生长(in-growth),从而促成相邻棘突的融合。开口可以是相对较大的,并且/或者连通到间隔器的中空内部。中空内部可构造成接纳骨生长促进物质,例如将促进物质装在中空内部内。开口可以是相对较小的,并且/或者包括在间隔器表面的至少一部分上的细孔(pore)或互连的细孔。开口可被填充骨生长促进物质。
间隔器可具有任何合适的横截面形状。例如,它可以是圆柱形的、D形的、C形的、H形的,包括分离的悬臂梁,和/或任何其它合适的形状。所述形状可包括倒角、倒圆、平面、缓和切口(relief cut)、和/或其它特征,以适合身体构造上的特征,例如板和/或小面。间隔器可以具有侧壁,所述侧壁具有大体上平行的、锥形的或不规则的形状。间隔器可以具有固定高度或者它可以具有允许外科手术时进行调节的可变高度。为了单个程度的脊柱校正可以提供单个间隔器或者为了单个程度或多个程度的脊柱校正可以提供多个间隔器。如果提供多个间隔器,那么它们可以由相同的或不同的材料制成。
延伸部可相对于间隔器纵轴线从间隔器横向地延伸,以将间隔器保持在相邻棘突之间。单个延伸部可以在一个或多个方向上延伸,或者可以提供在多个方向上延伸的多个延伸部。一个或多个延伸部可以是相对于彼此和/或相对于间隔器可纵向地调节的,以允许延伸部相对于棘突被定位。可以提供可相对于间隔器和另一延伸部轴向移动的可移动的延伸部。可选地,可以提供多个可移动的延伸部,例如,延伸部可以夹靠在棘突的侧面上,以使棘突相对于彼此固定不动,并促进相邻椎骨之间的融合。延伸部可包括能与棘突接合的紧固件。紧固件可包括缝合线、线、销、条带(strap)、夹具、销钉(spike)、螺钉、齿、粘结剂、和/或其它合适的紧固件。紧固件可以整合到延伸部中,或者可以是模块化的。模块化的紧固件可以是可调节的、可替换的、和/或可拆卸的,使得能够适应从刚性固定到不固定的固定的种类和特性。
可以提供这样的延伸部,所述延伸部允许一个植入物的延伸部位于或者交叠在相邻植入物的延伸部之上。例如,延伸部可以类似于木瓦(shingle)交叠地交叠。延伸部可以偏移,以进一步促进相邻延伸部的交叠。延伸部可以具有促进延伸部的交叠部分之间的相对运动的光滑表面。延伸部可以具有彼此接合以抵抗相对运动的表面;例如,交叠的延伸部的相对表面可以包括垫、钩、销、齿、刺毛、表面粗糙度、粘合剂、孔、环、螺钉、螺栓、和/或允许一个延伸部抓持另一个延伸部的其它特征。
植入物可以被成形为适合小的或者消失的棘突,诸如病人的骶骨上的棘突。例如,一个或多个延伸部的一部分可以向外张开以置于骨例如骶骨的相对较宽和/或较平的部分上。这种延伸部可以包括更长的、更尖的和/或以其它方式适合刺入和抓持骨的紧固件。延伸部可以是相对于彼此角度可变以适应下面的骨的形状。
间隔器可以具有固定高度或可变高度。可变高度的间隔器可以包括具有可以被锁定在所期望的相对间隔的可变高度间隔的第一部分和第二部分。高度间隔可以是可与延伸部的水平骨抓持间隔同时地或者独立于延伸部的水平骨抓持间隔地调节的和/或锁定的。通过用可拆除的器具将间隔力施加在第一和第二部分上并且然后锁定在所期望的间隔,高度间隔可以是可调节的。通过结合到植入物本身中的机构的操作,高度间隔可以是可调节的。可以通过单个机构调节高度间隔并且锁定所期望的间隔。间隔器的高度间隔的调节可以用来使相邻棘突彼此分离开。
植入物可以包括当它们与相邻棘突的骨接合时朝向彼此压缩延伸部或者使延伸部远离彼此的机构,使得相邻棘突可以类似地被压缩或者远离彼此。
植入物可以包括与多于两个棘突接合的间隔器和/或延伸部,以治疗多个脊柱节段(spinal level)。
间隔器、延伸部、和/或紧固件可以有利地由不同材料制成。例如,间隔器和延伸部可以由相对较软的材料制成,而紧固件可由相对较硬的材料制成。例如,间隔器和/或延伸部可以由聚合物和/或其它相对较软的材料制成,而紧固件可以由金属和/或其它相对硬的材料制成。不同材料可以具有不同的透射特性,使得一种材料可以很好地出现在医学图象上,而另一种材料仅仅朦胧地出现或者根本就不出现。例如,植入物的金属部分将清晰地显示在X-射线上,而聚合物部分将是模糊得多。这些特性能用来允许外科医生看到某些部分,举例来说,与骨接合的紧固件,同时允许清楚地看透其它部分以看得见治疗部位,举例来说,骨之间的间隔。
可以使用环扎来稳定所述棘突植入物,和/或提供其它益处。例如,线、条带、带子、缆索、绳、和/或其它细长构件可以环绕椎弓根、板、棘突、横突、和/或其它脊柱结构。环扎可以是相对不可延伸的,以便为脊柱弯曲提供硬阻止,或者环扎可以是相对可延伸的,以便为弯曲提供逐渐增加的阻力。环扎可以是相对柔性的和起皱的(drapeable),例如为织造织物,或它可以是相对刚性的,例如为金属带。环扎可以具有在被植入后使它恢复到先前设定形状的形状记忆性能。环扎可以独立于棘突植入物,或者可以接合该棘突植入物。例如,环扎可以穿过棘突植入物的中空内部,并且/或者接合延伸部。环扎可以与间隔器偏离开,并且提供一拉紧力,该拉紧力利用间隔器作为一支点,以卸下盘和/或打开盘间隔。
植入物可被补充有骨生长促进物质,以促进棘突、板、横突、小面和/或其它脊柱结构之间的相邻椎骨的融合。骨生长促进物质可以是与植入物间隔开的,放置在邻近植入物之处的,夹在植入物和下面的骨之间的,放置在植入物内部的,涂覆在植入物上的,和/或相对于植入物以其它方式放置的。如果骨生长促进物质被涂覆在植入物上,它可以覆盖整个植入物,或者仅仅覆盖植入物的所选部分,例如延伸部、紧固件、棘突接触的间隔器部分,和/或其它部分。
此外,骨生长促进物质可以包括直接有助于相邻椎骨之间的间隔的支撑的结构构件。例如,结构骨移植片可以结合在间隔器和/或延伸部中、上,围绕间隔器和/或延伸部,和/或以其它方式与间隔器和/或延伸部相联接,以为新骨形成物提供结构支撑和骨架。例如,结构骨件可以与间隔器相接合并且延伸超过间隔器,使得相邻棘突搁置在结构骨上。
如同这里所使用的那样,骨生长促进物质可以包括骨泥、骨碎片、骨条、结构骨移植片、从血小板(platelet)中得到的生长因子、骨髓抽出物、干细胞、骨生长蛋白质、骨生长肽、骨连接蛋白质、骨连接肽、羟磷灰石、磷酸钙、其它陶瓷、和/或其它合适的骨生长促进物质。
植入物和任何相关环扎或其它部件可以由任何合适的生物相容的材料制成,该生物相容的材料其中包括金属、可再吸收陶瓷、不可再吸收陶瓷、可再吸收聚合物和不可再吸收聚合物。一些具体的例子包括:不锈钢、钛和它的合金(包括镍钛合金)、钽、羟磷灰石、磷酸钙、骨、氧化锆、铝、碳、生物玻璃、聚酯、聚乳酸、聚乙醇酸、聚烯烃、聚酰胺、聚酰亚胺、聚丙烯酸脂类、聚酮、含氟聚合物,和/或其它合适的生物相容的材料以及它们的组合。
棘突植入物可以在包括棘上韧带去除后进路(posterior approach)、棘上韧带保留后进路、侧进路(lateral approach)、和/或其它合适的进路(approach)的多种外科手术技术中使用,以用于治疗脊柱疾病。棘突植入物可以被用于通过融合相邻椎骨或通过保留相邻椎骨之间的运动来治疗脊柱疾病。棘突植入物可以仅包括例如间隔器的伸展限位件,仅包括例如柔性环扎元件的弯曲限位件,或者既包括弯曲限位又包括延伸限位件。棘突植入物可被用来减小作用在小面关节上的负荷,增大棘突间隔,减小作用在盘上的负荷,增大前部盘间隔,和/或以其它方式治疗脊柱疾病。前部作用可通过将脊柱组成元件拉紧到间隔器的后面以对脊柱构造施加一有利的机械作用来实现。用于棘突植入物的技术可包括不改变外科手术处的组织,或改变组织例如修剪,挫、变粗糙、和/或以其它方式改变植入物处的组织。
图1和2示出了腰椎10的一对相邻椎骨的后视图和侧视图。上椎骨12通过盘16与下椎骨14相分离。每个椎骨包括一对横突18、19,一向后突出的棘突20、21,和一对将横突18、19连接到棘突20、21的板22、23。除了通过盘16连接外,椎骨12、14在一对小面关节24处关节连接。
图1-9示出了代表性的棘突植入物100。植入物100包括定位在棘突20、21之间的间隔器102。间隔器102的高度104限定了棘突20、21能向一起移动到的接近程度。这样,间隔器102保持在棘突20、21之间的最小距离。在包括相邻椎骨后部下沉的脊柱疾病的情况下,将间隔器102插入棘突20、21之间将分开椎骨,并减轻作用在神经组织和小面关节24上的压力。
如同在图3中所示的那样,间隔器102包括第一端106、第二端108、和从第一端延伸到第二端的纵轴线110。在所示的实施例中,间隔器102具有大体上平行于纵轴线110的侧壁112,该侧壁112包括上外表面114和下外表面116。横向开口118(也参见图6)从上外表面114和下外表面116向内地贯通,以促进组织生长。代表性的间隔器102包括由内表面122界定的中空内部120,使得开口118从外表面连通到中空内部120。在图1和2中,显示将骨生长促进物质124装入到中空内部120内,以便通过棘突20之间的骨生长促进椎骨12、14的融合。
棘突植入物100进一步包括第一延伸部126,该第一延伸部126横向于纵轴线110地从间隔器102向外伸出,以大体上位于上棘突的侧面上。第一延伸部126与棘突20的邻接帮助将间隔器102保持在棘突20之间。在代表性的棘突植入物100中,第一延伸部126相对于间隔器102是固定的,并且植入物包括第二延伸部128,该第二延伸部128可安装到间隔器上,以能相对于第一延伸部126轴向移动。可以将第二延伸部128朝向第一延伸部126移动至大约棘突20的宽度,并更好地稳定植入物100。通过将固定螺钉130拧靠在间隔器102上,第二延伸部被固定在合适的位置。延伸部126、128包括紧固件132、134、136,这些紧固件从延伸部126、128伸出以接合棘突20,进而将间隔器102固定到棘突20上。图1示出了骨条125形式的附加骨生长促进物质,该骨生长促进物质沿着棘突20的侧部夹在延伸部126、128之间,以促进沿着棘突的侧部的骨生长,从而进一步增进椎骨12、14的融合。延伸部126、128优选为向下延伸并同时向上延伸(如同所示的那样),以可选地连接到下棘突,从而使棘突20在进行融合时相对彼此固定不动。
紧固件132、134和136可以采用任何合适的形式。紧固件可以与延伸部126、128一体地制成,例如通过紧固件与延伸部一起机加工或铸造,或者紧固件可以被单独地形成并被永久连接到延伸部126、128。紧固件132是与延伸部126螺纹地啮合的尖锐销钉。螺纹啮合允许用不同的紧固件132来替换紧固件132。例如,紧固件132可以用一个具有不同形状、不同尺寸、不同材料、或不同表面涂层的紧固件替换。螺纹啮合也允许紧固件132被调节,以通过改变紧固件从延伸部126延伸的延伸量来改变紧固件与骨相接合的程度。这样,紧固件132能被调节成适应不同形状的骨,或以不同的量刺入骨中。例如,多个带螺纹的紧固件132可被调节成延伸不同的量,从而适应弯曲的或倾斜的骨。最后,当不期望固定时,例如当期望在非融合方法中使用植入物100作为延伸限位而不限制弯曲时,螺纹啮合允许使用者拆除紧固件132。
紧固件134和136是允许多个销钉被快速地调节、改变或去除的多销钉组(multi-spike pod)。紧固件134包括与延伸部126的非圆形的开口140相接合的非圆形的突部138。非圆形的接合阻止了紧固件134的转动。突部138可以与开口140形成压配合、卡扣配合、或其它合适的接合。可以用辅助螺钉142进一步固定突部138。紧固件136包括带螺纹的轴144,该带螺纹的轴144与底座构件146螺纹地接合,从而使紧固件136的长度能够被调节。轴144以转动的和枢转的方式接合延伸部126,使得能转动地和倾斜地调节紧固件136以接合骨表面。在所示的实施方式中,轴144末端带有球148,球148与开口140接合形成球窝结构,从而具有三个自由度。然而,可以使用允许任何数量的自由度的任何机构。紧固件136可被允许在使用中可以移动,使得当延伸部126被压向骨时,紧固件136调节到骨表面的角度。紧固件136也可以例如通过螺钉142被固定,以调节接头(joint)的拉紧力和/或将紧固件136锁定在一预定的方位。
图4示出了在相对的延伸部126、128上的紧固件的轴向关系。在所示的植入物100内,在植入物100的顶部上的紧固件132被示为沿着一共同轴线150相对齐。在植入物100的底部上的紧固件134被示为是偏移的,使得当它们被压到骨内时,如果需要的话,它们能相互交错。在植入物100上可提供紧固件类型、数量和对齐方式的任何组合。
如同在图5和6中所看到的那样,间隔器102的端106、108包括前斜面152。这些斜面152允许端106、108间隔开地(clear)向后地面对椎骨12、14的结构,例如小面关节24。而且,如同在图5和6中所看到的那样,间隔器102相对于延伸部126、128向前偏移,使得间隔器102的纵轴线110在延伸部126、128的中线154的前面。在延伸部126、128安装在棘突20、21的侧面上时,间隔器120的向前的偏移允许它深深地安装在棘突20、21之间。
如同在图3和图8中能最清楚地看到的那样,第二延伸部128限定一大体上与间隔器102的横截面形状相一致的孔155。在图1-图9所示的实施方式中,孔155向前开口,从而形成“C”形状。突部156从孔的上部和下部向内延伸,以滑动地接合间隔器102的上表面和下表面上的细长槽158。第二延伸部128能朝向第一延伸部126和远离第一延伸部126地纵向平移。将固定螺钉130拧靠在间隔器102的后侧面160上,迫使突部156向后靠在槽158的侧面上并且将第二延伸部128纵向地锁定在合适的位置上。如同所示的那样,可以使间隔器102的后侧面160变粗糙,以更好地啮合固定螺钉130。当拧紧时固定螺钉130也可以钻到间隔器102的表面中,从而充分啮合间隔器102。孔155可以与间隔器102精密地一致,以强制第二延伸部128相对于第一延伸部126作大体上平行的运动。可选地,孔155可以大于间隔器102一预定量,以允许第二延伸部128相对于第一延伸部126调节一预定大小的角度,如同在图7中所示的那样,以允许延伸部128适应下面的骨表面。
如同在图8中能最清楚地看到的那样,第二延伸部128包括具有第一凸耳中心线162的第一凸耳161和具有第二凸耳中心线166的第二凸耳164。在所示的实施方式中,第一凸耳中心线162和第二凸耳中心线166是平行的并且是间隔开的,使得第二延伸部128具有大体为“Z”形的平面形状。如图9所示,在多级外科手术中,在需要时,这种形状允许一个植入物100的延伸部与另一个植入物100相互交错,从而允许植入物间的间隔较小,并且/或者允许有更长的延伸部凸耳以便能实现更大面积的骨接合。在图1-图9所示的实施方式中,中心线162和166是从第二延伸部128的中线154等距离地偏移的。中心线162和166可以不平行,并且它们可以不对称地偏移,以形成不同的形状,进而适应不同的椎骨构造。例如,形状可以是为脊柱10的不同部分特制的。在图1-图9所示的实施方式中,第一延伸部126的形状与第二延伸部128相同。然而,第一和第二延伸部126、128可以具有不同的形状。
图10示出了具有间隔器202以及第一和第二延伸部204、206的植入物200。间隔器202包括细孔208,以便于组织生长到其中。细孔208可以是彼此间隔开的单独的开口、互联的开口,或者是单独的开口和互联的开口的组合。间隔器202可以是全部具有均匀孔隙的整体块。可选地,间隔器202可以包括外多孔层210和不同组成的内层212。例如,内层212可以是实体的(solid)、多孔的、中空的、或一些其它构造。多孔的内层可以具有尺寸和/或分布与外层210不同的细孔。类似地,任何多孔部分可以具有均匀的孔隙或者孔尺寸或密度变化的孔隙。多种细孔构造是合适的。优选地,细孔大小在1μm到2mm的范围内。更优选地,细孔大小在1μm到500μm的范围内。仍然更优选地,细孔大小在75μm到300μm的范围内。可通过多种方法产生细孔,例如颗粒烧结、从材料中滤去可溶成份;缠结、编织或以其它方式组合纤维;和/或通过任何其它已知的方法。细孔尺寸可以被调整成,优选情况下促进硬组织生长、软组织生长,或硬组织和软组织组合的生长。延伸部204、206可以是实体的,或者它们可以具有大的开口和/或小的开口,以促进骨在延伸部204、206内生长和/或围绕延伸部204、206生长。如同前面所描述的那样,也可以涂覆间隔器202和/或延伸部204、206。
延伸部204、206可以是固定的和/或可调节的。在图10所示的植入物200中,第一延伸部204被固定到间隔器202的一端,并且第二延伸部206是可沿着间隔器202平移的,以允许延伸部被放置为邻近棘突。延伸部204、206被示为具有可以接合棘突20、21以使棘突20、21相对彼此被固定的可选的销钉214。
图10还示出了与植入物200结合的环扎的使用。例如,围绕板22、23放置一个或多个柔性带216,以提供弯曲限位。在脊柱弯曲期间,带216可以帮助承担施加在销钉214上的负载。替代带216或者在带216以外,一个或多个带218、220可以围绕横突18、19设置。
图11-图13示出了根据本发明的一起使用棘突植入物300和环扎的其它实施例。植入物包括用于放置在相邻棘突20、21之间的间隔器302,和延伸部304。在图11的实施例中,柔性材料带310环绕在棘突20、21周围。通过将带310放置在棘突接触间隔器302的区域312、314的后面,形成偏移318。带310的拉紧在每个椎骨12、14上产生力矩320、322,该力矩320、322卸去施加在相邻椎骨12、14之间的盘16上的一部分压力。随着带310的拉紧的增加,可实际上增大椎骨12、14的前部间隔324。这样,通过使用棘突植入物300和带310的组合,可以抬开椎骨12、14,其中植入物300被用作支点。除了已经提及的益处,这种结合用后部棘突疗法产生前部盘间隔的效果,这与典型的盘间隔疗法相比具有更小的侵入性。
在图12和13的实施例中,植入物300包括用于将环扎带310连接到植入物300的机构。在图12的实施例中,机构包括在延伸部304的上端和下端上的开口330、332。通过将带310连接到延伸部304,带310和延伸部304帮助稳定彼此以防止前后移动。这种连接也帮助将带310定位在距离间隔器302的预定偏移318处。在图13的实施例中,带310环绕通过间隔器302本身的中空内部。在这个实施例中,带是不偏移的,并且在椎骨上产生最小的力矩或者不产生力矩。
图14-图24示出了用于将可移动的延伸部连接到图1的植入物的其它机构。参考图14,植入物400包括间隔器402、第一延伸部404和第二可移动的延伸部406。可移动的延伸部406包括环408形式的主体,环408具有大体上与间隔器402的外表面相一致的内表面410,使得环被可滑动地接纳在间隔器402上。固定螺钉412被拧靠在间隔器402上,以将可移动的延伸部406固定在间隔器402上的所期望的位置处。固定螺钉412的拧紧将可移动的延伸部406相对于间隔器402向后地偏置。环的前部部分414压靠在间隔器402的前部部分416上,以反抗这种向后偏置并允许固定螺钉412锁定延伸部406。间隔器402可包括多个凹痕418,以在预定的轴向位置处产生与固定螺钉412的牢固接合(positive engagement)。可以将环408的尺寸设定为允许延伸部406相对于间隔器402作预定量的倾斜。

可以使用多种外科手术方法和技术植入依照本发明的植入物。外科手术方法可以包括棘上韧带去除后部进路、棘上韧带保留后部进路、侧部进路、和/或其它合适的方法。技术可以包括不改变外科手术处的组织,或改变组织,例如修剪,挫、弄粗糙、和/或以其它方式改变它们。例如,在图1中,使用侧部进路并且下棘突在它的上表面26上被切除,从而增大椎间间隔,以便接纳植入物100。准备好椎间间隔之后,间隔器102被插入到椎间间隔内。如果存在第一延伸部126,它可以被向内压靠以位于一个或多个棘突的附近或邻接一个或多个棘突。如果使用第二延伸部128,它与间隔器102接合,并且也可选地被向内压靠。在图1中,使用了具有向内定向的骨紧固件的相对的延伸部126、128,并将它们向内压靠,使得紧固件132接合棘突20、21。因为延伸部被如图3、图8和图9中所示那样的上下偏移,图1中未示出紧固件132与下棘突21的接合。
参考图25和图26,提供一组器具1300,以便于将植入物侧插入到椎间间隔内。该组器具包括多个插入器1302、1303,其中每个插入器1302、1303具有第一部分或把手部分1304和第二部分或工作部分1306。工作部分1306可插入到椎间间隔内。优选地,把手部分1304横向于工作部分1306延伸,以使得在工作部分1306位于椎间间隔内时易于握持和操作插入器1302、1303。把手部分1304和工作部分1306可以限定弯曲的、倾斜的、偏移的、和/或任何其它合适的横向朝向。在图25的实施例中,插入器1302、1303是大体上“L”形的。工作部分1306在与它的自由端1308间隔开的第一部分1307处的横截面尺寸相对较大,而在它的自由端1308处的横截面尺寸相对较小,并且工作部分1306的横截面尺寸从第一部分1307到自由端1308逐渐变小。在所示的实施方式中,工作部分是圆锥形的,并且从较大的直径逐渐变小到较小的直径。端部1308限定具有开口1310的中空末端。该组器具1300配备有多个类似地构造的插入器,这些插入器具有不同大小的工作部分1306,使得一个插入器1302的端部1308可配合在另一个插入器1303的末端的开口1310内。可选地,工作部分1306可以被分离成连接到相对的把手1314、1316上的相对的半件。当相对于彼此移动相对的把手1314、1316时,工作部分1306的相对的两个半件相对于彼此移动。在所示的实施方式中,朝向彼此紧握把手1314、1316使得工作部分1306扩张,并使工作部分1306的相对的半件远离彼此地向外打开。
在使用中,第一插入器1302被插入到椎间间隔内。第一插入器1302是相对小的,从而易于插入。当端部1308被进一步插入时,锥形的工作部分1306扩张椎间间隔。可选地,当工作部分在椎间间隔内时,通过扩张工作部分,例如通过紧握把手1314、1316,可进一步扩张椎间间隔。通过将第二插入器1302的中空末端放置在第一插入器1303的末端上,使得第二较大的插入器1302与第一插入器1303接合,然后使将交叠的器具往回移动通过所述椎间间隔,而去除第一插入器1303,并插入第二插入器1302。当进一步插入第二插入器1303的端部时,锥形工作部分扩张椎间间隔。可选地,当工作部分在椎间间隔内时,通过扩张工作部分能进一步扩张椎间间隔。可以这种方式插入逐渐增大的插入器,直到椎间间隔已经被扩张到所期望的大小。一旦达到了所期望的大小,通过注意到最后的插入器的尺寸,可以确定合适的植入物的尺寸。插入器可以任选地包括在它的锥形工作端上的标记1320,该标记1320对应于不同间隔器大小,以便进一步易于选择植入物的大小。通过如图26所示那样将间隔器1402与插入器的工作端接合起来,插入植入物。植入物可以被接合在插入器的中空末端内,或者插入器的末端可以接合植入物的中空末端,如所示的那样。当取出插入器时,间隔器1402被压入到椎间间隔内。

在图30的示例性的实施例中,由于延伸部和间隔器之间的可变的角度,在延伸部中不需要偏移以允许交叠的关系。图27的植入物可以同样允许延伸部和间隔器之间的可变的角度。然而,因为延伸部的偏斜,延伸部将呈现更平行的方位。图27-30的植入物可以包括贯穿这个说明书所描述的多个植入物中的任一个的特征。
上面所描述的叠加植入物有助于通过允许延伸部交叠并且因此延伸部在棘突的侧部上需要更少的间隔,从而将植入物放置在相邻脊柱节段处。此外,如果在交叠的延伸部之间形成刚性连接,那么多个植入物的整个脊柱构造的刚性是增加的。在上面所描述的叠加植入物中,交叠的植入物的相对的表面可以包括垫、钩、销、齿、刺毛、表面粗糙、粘合剂、孔、环、螺钉、螺栓、和/或允许一个延伸部抓持另一个延伸部的其它特征。

在使用时,定位植入物,其中间隔器在相邻棘突2650之间并且延伸部在棘突2650的各侧上。骨夹2602被夹持到椎骨上。例如,它们被夹持到相邻棘突2650上。压缩器/分开器的臂夹2620被连接到骨夹2602。然后转动突起部2618,以压缩或分开臂2612,并且通过扩展骨夹2602,直到椎骨处于所期望的相对间隔。然后使植入物压缩器2624与植入物100相接合并且被压缩,以使植入物扩展,从而接合棘突,进而固定椎骨之间的所期望的间隔。

参考图57,植入物插入器2800用于以直接后进路(posterior approach)插入类似于图1-9的植入物的植入物100。插入器2800包括以剪刀类的布置连接在一起、并具有把手端2806和工作端2808的第一臂2802和第二臂2804。各臂2802、2804的工作端包括可响应于臂的工作端的朝向和远离彼此的移动而朝向和远离彼此移动的夹持面2810、2812。夹持面2810、2812可操作地夹持,并且因此抓持植入物100的第一延伸部126。第一臂2802包括邻近夹持面2810、2812地从第一端2816向中间地伸出到与夹持面2810、2812间隔开的第二端2818的足部2814。当夹持面2810、2812与第一延伸部126夹持接合时,足部2814可定位在间隔器102之上。槽2820形成在足部2814的第二端2818附近并且从足部2814的前边缘2822向后延伸。槽2820的大小被设定成接纳第二延伸部128。
在使用时,第二延伸部128放置在间隔器102上并且通过将足部定位在间隔器102上而使插入器2800与植入物相接合,使得槽2820接纳第二延伸部128,并且夹持面2810、2812在第一延伸部126的相对侧上。操作插入器把手2806,以夹持第一延伸部126。这样被夹持着,第一和第二延伸部126、128被固定地保持为处于预定间隔开的关系。考虑到最后要拆掉插入器2800,如果需要,可以插入和拧紧固定螺钉130(图3),以进一步固定第二延伸部128。从后到前地通过足部2814的去除切口2824与固定螺钉孔相对准,以在植入物100与插入器相接合时有助于操作固定螺钉。插入器2800可以用来在从后指向前的方向上将植入物100插入在相邻棘突之间。足部2814的上方和下方2826、2828可以被去除掉,以避免组织碰撞足部2814并且提高可视性。
参考图58,提供用于在准备插入植入物100时挫掉相邻组织,例如,用于去除掉相邻棘突之间的组织的锉刀2900。锉刀2900包括头部2901,该头部2901具有有助于插入的圆锥形末端2902和齿2904。沿着它的纵轴线插入和移除锉刀头2901并且围绕它的纵轴线2906转动锉刀头2901将去除掉邻接组织。在它用来准备相邻棘突的情形中,倾斜的把手2908,例如,形成135到45度的范围内的,或者更优选为120到60度的范围内的,更优选地大约90度的角2910,可用来将头部2901定位在棘突之间并且围绕它的轴线前后转动它,以磨除组织。齿2904朝向把手2908倾斜,以易于头部2901的最初的插入。当插入时,倾斜齿2904更少倾向于阻碍。

Claims (1)
1.一种用于放置在脊柱的相邻椎骨的棘突之间的植入物,所述植入物包括:
间隔器,该间隔器具有第一端、第二端和从所述第一端延伸到所述第二端的纵轴线,所述间隔器具有侧壁,所述侧壁纵向地延伸并具有上表面和下表面,所述上表面和下表面可操作地邻接所述棘突并将所述棘突保持为处于间隔开的关系,所述上表面和下表面被间隔开的距离对应于所述棘突之间的预定最小间隔;和
第一延伸部,该第一延伸部横向于所述纵轴线地从所述间隔器伸出,以大体上位于相邻椎骨的棘突的侧面上,所述第一延伸部能与所述棘突相接合以限定所述棘突之间的最大间隔。
