Eine der zentralen Aufgaben sowohl der orthopädischen als auch der unfallchirurgischen Versorgung ist die Rekonstruktion des menschlichen Bewegungsapparates. Dabei ist neben dem knöchernen Aufbau die funktionelle Wiederherstellung der muskulären Anbindung von entscheidender Bedeutung. Die Problematik ergibt sich aus den wirkenden Zugkräften der Sehnenansätze, die relativ kurzfristig, und zwar vor dem Abschluss von Einwachsvorgängen, postoperativ auftreten und eine erneute Destruktion verursachen können.One of the central tasks of both orthopedic and trauma care is the reconstruction of the human musculoskeletal system. Die Knochenfragmente mit gegebenenfalls daran angebundenen Sehnenansätzen oder isolierte Sehnenansätze werden durch die Klemmvorrichtung geklemmt und können dann an Implantaten (wie Osteosyntheseplatten) oder direkt am Knochen refixiert werden. Die Klemmvorrichtung ist bevorzugt eine flächige Klemmvorrichtung, die zu einer flächigen Klemmung des betreffenden Frakturelementes mit einer vielfach höheren Festigkeit führt. Von besonderem Augenmerk ist die osteoinduktive Oberfäche der Klemmvorrichtung zur ossären Integration der Klemmvorrichtung. Im Laufe von ca. 6 bis 8 Wochen durchwächst natürliches Knochenmaterial (Trabekel) die zumindest auf der Oberfläche vorhandenen Strukturen und sichert die Klemmvorrichtung dauerhaft. Dies bedeutet, dass in dauerhafter Refixierung der Fragmente die Klemmvorrichtung im Wege einer Nachoperation nicht entfernt wird, sondern dauerhaft im Körper des Patienten verbleibt. Diese kann auch dadurch erhöht werden, dass gemäß einer bevorzugten Weiterbildung die Klemmvorrichtung ein osteoinduktives Volumen zur vollständigen ossären Integration der Klemmvorrichtung aufweist. In diesem Falle wird nicht nur die Oberfläche der Klemmvorrichtung von Trabekeln durchwachsen, sondern die Klemmvorrichtung als ganze Einheit. Dies resultiert in einer faktisch unlösbaren Langzeitfixation. Die osteoinduktiven Strukturen sind besonders bevorzugt aus einer dreidimensionalen, offenmaschigen Raumnetzstruktur aufgebaut. In dem Falle, dass das Implantat direkt mit dem Knochen verbunden werden soll, wird es sich hierbei bevorzugt um eine Knochenschraube handeln.
