- PRIORITÄTSANSPRUCH
- Die vorliegende Anmeldung beansprucht die Vorteile gemäß 35 U. S. C § 119(e) aus der vorläufigen US-Anmeldung Nr. 61/077.648, eingereicht am 2. Juli 2008, mit dem Titel ”Treatment of Tinnitus with Vagus Nerve Stimulation”; der vorläufigen US-Anmeldung Nr. 61/078.954, eingereicht am 8. Juli 2008, mit dem Titel ”Neuroplasticity Enhancement”; der vorläufigen US-Anmeldung Nr. 61/086.116, eingereicht am 4. - Hintergrund
- Die vorliegende Offenbarung bezieht sich im Allgemeinen auf die Therapie, die Rehabilitation und das Training einschließlich induzierter Plastizität. Insbesondere bezieht sich die Offenbarung auf Verfahren und Systeme zur Verbesserung der Therapie, der Rehabilitation und des Trainings unter Verwendung der Nervenstimulation gepaart mit Trainingserfahrungen.
- ZUSAMMENFASSUNG
- Um die Erfindung zusammenzufassen, sind hierin bestimmte Aspekte, Vorteile und neuartige Merkmale der Erfindung beschrieben worden. Es ist selbstverständlich, dass nicht notwendigerweise alle derartigen Vorteile in Übereinstimmung mit irgendeiner speziellen Ausführungsform der Erfindung erreicht werden können. Folglich kann die Erfindung auf eine Art verkörpert sein oder ausgeführt werden, die einen Vorteil oder eine Gruppe von Vorteilen, wie sie hierin gelehrt werden, erreicht oder optimiert, ohne notwendigerweise andere Vorteile, wie sie hierin gelehrt oder vorgeschlagen werden können, zu erreichen.

- Die zahlreichen erfinderischen Lehren der vorliegenden Anmeldung werden unter besonderer Bezugnahme auf gegenwärtig bevorzugte Ausführungsformen (beispielhaft und ohne Einschränkung) beschrieben. Die vorliegende Anmeldung beschreibt mehrere Erfindungen, wobei keine der Darlegungen im Folgenden als die Ansprüche im Allgemeinen einschränkend angesehen werden soll. Wo Blockschaltpläne verwendet worden sind, um die Erfindung zu veranschaulichen, sollte erkannt werden, dass der physische Ort, an dem die beschriebenen Funktionen ausgeführt werden, nicht notwendigerweise durch die Blöcke dargestellt ist. Ein Teil der Funktion kann an einem Ort ausgeführt werden, während ein weiterer Teil der gleichen Funktion an einem anderen Ort ausgeführt wird. Mehrere Funktionen können am gleichen Ort ausgeführt werden.
- In
1 ist ein Paartrainingssystem gezeigt. Ein Zeitsteuersystem106 ist mit einem Nervenstimulatorsystem108 und einem Trainingssystem110 kommunikationsfähig verbunden. Wenn das Nervenstimulatorsystem108 einen Zeitsteuerungsbefehl vom Zeitsteuersystem106 empfängt, stellt es einem Nerv104 eine Stimulation bereit. Ähnlich erzeugt das Trainingssystem110 beim Empfang eines Zeitsteuerungsbefehls vom Zeitsteuersystem106 oder bei der Bereitstellung eines Zeitsteuerungsbefehls für das Zeitsteuersystem106 gewünschte geistige Bilder, Ideen, Formungen oder Zustände im Gehirn102 . Die Stimulation des Nervs104 beeinflusst das Gehirn102 durch das Induzieren von Plastizität. Die vorübergehende Paarkombination von Training und Stimulation erzeugt Manifestationen der Plastizität im Gehirn102 , die durch ein Plastizitätsmesssystem112 gemessen werden können. - Das Zeitsteuersystem
106 stellt im Allgemeinen die gleichzeitige Art der Paarung bereit. Die Stimulation und das Training sind gleichzeitig, weil sie zum gleichen Zeitpunkt auftreten, d. h., es gibt wenigstens etwas Überlappung bei der zeitlichen Steuerung. In einigen Ausführungsformen kann die Stimulation dem Beginn des Trainings voreilen, während in anderen Ausführungsformen die Stimulation dem Beginn des Trainings folgen kann. In vielen Fällen besitzt die Stimulation eine kürzere Dauer als das Training, so dass die Stimulation in der Nähe des Beginns des Trainings auftritt. Es ist gezeigt wurden, dass die Plastizität, die sich aus der Stimulation ergibt, Minuten oder Stunden dauert, so dass ein einziger Stimulationsimpuls für die gesamte Dauer des ausgedehnten Trainings ausreichen kann. - Bei der Behandlung von Tinnitus kann das Training z. B. aus kurzen hörbaren Tönen bestehen, einschließlich ausgewählter therapeutischer Frequenzen, gepaart mit Stimulationen. Weil die Dauer der Töne kurz sein kann, kann die zeitliche Steuerung sehr genau gesteuert werden, so dass der Ton vorübergehend mit der Stimulation zusammenfällt. Diese Art der Genauigkeit kann typischerweise irgendeine Form der Computer-Steuerung erfordern. In anderen Formen der Rehabilitation oder Ausbildung kann die zeitliche Steuerung des Trainings und/oder der Stimulation manuell gesteuert werden. Weitere Therapien und weiteres Training können eine durch das Training ausgelöste zeitliche Steuerung oder die Rückkopplung physischer Zustände enthalten, um ein Regelsystem zu schaffen.
- Das Nervenstimulationssystem
108 kann die Stimulation des Nervs104 unter Verwendung elektrischer Stimulation, chemischer Stimulation, magnetischer Stimulation, optischer Stimulation, mechanischer Stimulation oder irgendeiner anderen Form der geeigneten Nervenstimulation bereitstellen. Gemäß einer Ausführungsform wird eine elektrische Stimulation dem linken Vagusnerv bereitgestellt. In einem elektrischen Stimulationssystem können die geeigneten Stimulationsimpulse verschiedene Signalformen umfassen, einschließlich Konstantstromimpulsen, Konstantspannungsimpulsen, exponentiellen Impulsen oder irgendeiner anderen geeigneten Signalform. Ein elektrisches Stimulationssystem kann einen einzigen Stimulationsimpuls oder einen Zug von Stimulationsimpulsen verwenden, um den Nerv104 zu stimulieren. Die Stimulationsparameter werden bezüglich der beeinflussten Gehirnbereiche oder Systeme, der Plastizitätsmaße, der Desynchronisation oder irgendeines anderen geeigneten Maßes der Stimulationsparameter ausgewählt, um das Gehirn102 geeignet zu beeinflussen. Es ist ein halbsekündiger Zug aus zweiphasigen Stimulationsimpulsen mit einer Impulsbreite von 100 Mikrosekunden, mit 0,8 Milliampère und mit 30 Hz bei der Behandlung von Tinnitus effektiv verwendet worden. - Die Paarstimulation könnte unter Verwendung der Tiefenhirnstimulation, der kortikalen Stimulation, der transkraniellen Magnetstimulation oder irgendeiner anderen geeigneten Nervenstimulation ausgeführt werden.
- Eine Indikation der geeigneten Stimulation kann die Desynchronisation des kortikalen EEG sein. Es ist gezeigt worden, dass ein 0,8-Milliampère-Impuls eine kortikale Desynchronisation bei Frequenzen zwischen 30 und 150 Hz verursacht. 0,4-Milliampère-Impulse desynchronisieren den Kortex bei höheren Frequenzen von 100 bis 150 Hz. Es ist gezeigt wurden, dass die Desynchronisation in Reaktion auf die Stimulation des Vagusnervs wenigstens vier Sekunden dauert.
- Das Simultantrainingssystem
110 erzeugt die Sensoreingabe, die motorischen Folgen, die kognitive Eingabe, die geistigen Bilder, die Ideen, die Formungen oder die Zustände, die durch das Gehirn102 beizubehalten sind. Ein Trainingssystem110 kann Sinnesinformationen, wie z. B. Seh-, Hör-, Geruchs-, Tast- oder irgendwelche anderen geeignete Sinnesinformationen bereitstellen. Das Trainingssystem110 kann physische Therapien, kognitive Therapien, emotionale Therapien, chemische Therapien oder irgendwelche anderen geeigneten Therapien umfassen. Das Trainingssystem110 kann pädagogische Informationen darstellen. Das Trainingssystem110 kann die Versuchsperson physisch, geistig, emotional oder in irgendeiner anderen geeigneten Weise enthalten. Das Trainingssystem110 kann Lehrer, Ärzte, Therapeuten, Berater, Ausbilder, Trainer oder irgendwelche anderen geeigneten Trainingsanbieter enthalten. Das Trainingssystem110 kann spezifische Muster der Nervenaktivität durch direkte Gehirnsstimulation, z. B. durch elektrische, magnetische, optische oder irgendwelche anderen geeigneten Systeme zum Hervorrufen von Mustern, hervorrufen. Das Trainingssystem110 kann spezifische Gehirnbereiche über chemische Wirkstoffe, Kühlung, magnetische Stimulation oder andere geeignete Verfahren inaktivieren. - Das Paartrainingssystem nach
1 beeinflusst das Gehirn102 , um Plastizität zu erzeugen, die durch ein Plastizitätsmesssystem112 gemessen werden kann. Bei der Behandlung von Tinnitus kann eine kortikale Karte verwendet werden, um die Kartenverzerrung und die Korrektur, die die erfolgreiche Behandlung des Tinnitus begleitet, zu messen. Die Plastizität kann weniger invasiv durch Verhaltensreaktionen auf Reize, wie z. B. einen Stelltest (Startle-Test) für Tinnitus, gemessen werden. Ferner kann die Plastizität gemessen werden, indem die subjektive Erfahrung einer Versuchsperson erfragt wird. Falls ein Tinnitus-Patient nicht länger das anhaltende Geräusch erleidet, ist die Plastizität gemessen worden. - In
2 ist ein Paartrainingssystem, das einen subkortikalen Bereich114 des Gehirns102 beeinflusst, gemäß einer Ausführungsform gezeigt. Die Stimulation des Nervs104 beeinflusst den subkortikalen Bereich114 . Der subkortikale Bereich114 wiederum beeinflusst das Gehirn, um die Plastizität zu induzieren. Es ist bekannt, dass die Stimulation der Nerven104 , wie z. B. des trigeminalen Nervs und anderer cranialer Nerven, den subkortikalen Bereich114 beeinflusst. - In
3 ist ein Paartrainingssystem, das den Nucleus basalis116 beeinflusst, gemäß einer Ausführungsform gezeigt. Die Stimulation des Nervs104 beeinflusst den Nucleus basalis116 . Der Nucleus basalis116 wiederum beeinflusst das Gehirn102 , um die Plastizität zu induzieren. - In
4 ist ein Paartrainingssystem, das den Locus caeruleus118 beeinflusst, gemäß einer Ausführungsform gezeigt. Die Stimulation des Nervs104 beeinflusst den Locus caeruleus118 . Der Locus caeruleus118 wiederum beeinflusst das Gehirn102 , um die Plastizität zu induzieren. - In
5 ist ein Paartrainingssystem, das die Amygdala120 beeinflusst, gemäß einer Ausführungsform gezeigt. Die Stimulation des Nervs104 beeinflusst die Amygdala120 . Die Amygdala120 wiederum beeinflusst das Gehirn102 , um die Plastizität zu induzieren. - In
6 ist ein Paartrainingssystem, das den NTS122 beeinflusst, gemäß einer Ausführungsform gezeigt. Die Stimulation des Nervs104 beeinflusst den NTS122 . Der NTS122 wiederum beeinflusst das Gehirn102 , um die Plastizität zu induzieren. - In
7 ist ein Paartrainingssystem, das das cholinergische System124 beeinflusst, gemäß einer Ausführungsform gezeigt. Die Stimulation des Nervs104 beeinflusst das cholinergische System124 . Das cholinergische System124 setzt Acetylcholin (ACh) in das Gehirn102 frei, was die Plastizität induziert. - In
8 ist ein Paartrainingssystem, das das noradrenergische System126 beeinflusst, gemäß einer Ausführungsform gezeigt. Die Stimulation des Nervs104 beeinflusst das noradrenergische System126 . Das noradrenergische System126 setzt Noradrenalin (NE) in das Gehirn102 frei, was die Plastizität induziert. - In
9 ist ein Nervenstimulatorsystem gemäß einer Ausführungsform gezeigt. Eine Nervenstimulatorsteuerung109 ist mit einem Neurostimulator128 kommunikationsfähig verbunden. Der Neurostimulator128 stellt einen Stimulationsimpuls über ein Paar Elektroden130a und130b einem Nerv104 bereit. Die Elektroden103a und103b könnten Manschettenelektroden, leitende Platten oder irgendeine andere geeignete Nervenstimulationselektrode sein. Der Neurostimulator kann sowohl durch ein piezoelektrisches Energieversorgungssystem als auch durch eine induktive Nahfeld-Leistungsübertragung, eine induktive Fernfeld-Leistungsübertragung, eine Batterie, eine wiederaufladbare Batterie oder irgendein anderes geeignetes Neurostimulator-Energieversorgungssystem mit Energie versorgt werden. Wenn die Nervenstimulatorsteuerung109 die Zeitsteuerungsbefehle von einem (nicht gezeigten) Zeitsteuersystem empfängt, leitet die Nervenstimulatorsteuerung109 einen Stimulationsimpuls vom Neurostimulator128 über die Elektroden130a und130b ein. - In
10 ist ein drahtloses Nervenstimulatorsystem gemäß einer Ausführungsform gezeigt. Der Neurostimulator128 kommuniziert unter Verwendung einer induktiven Transponderspule132 mit dem Nervenstimulationssystem109 . Das Nervenstimulatorsystem109 enthält eine externe Spule134 . Die Informationen können zwischen dem Nervenstimulatorsystem109 und dem Neurostimulator128 übertragen werden. Die Leistung kann durch das Nervenstimulatorsystem zum Neurostimulator128 übertragen werden. - In
11 ist ein Doppelneurostimulatorsystem gemäß einer Ausführungsform gezeigt. Zwei Neurostimulatoren128 können den Nerv104 stimulieren. Die Neurostimulatoren können gesteuert werden, um als Redundanz einander zu verstärken oder um zu verhindern, dass efferente Signale weg vom Gehirn projiziert werden. - In
12 ist ein Mehrfachneurostimulatorsystem gemäß einer Ausführungsform gezeigt. Mehrere Neurostimulatoren128 können den Nerv104 stimulieren. Die Neurostimulatoren können gesteuert werden, um als Redundanz einander zu verstärken oder um zu verhindern, dass efferente Signale weg vom Gehirn projiziert werden. - In
13 zeigt eine graphische Darstellung einen Konstantstrom-Stimulationsimpuls gemäß einer Ausführungsform. - In
14 zeigt eine graphische Darstellung einen exponentiellen Stimulationsimpuls gemäß einer Ausführungsform. - In
15 zeigt eine graphische Darstellung einen Zug von Konstantstrom-Stimulationsimpulsen gemäß einer Ausführungsform. - In
16 ist ein synchronisiertes Zeitsteuersystem gemäß einer Ausführungsform gezeigt. Das synchronisierte Zeitsteuersystem umfasst eine Synchronisations-Zeitsteuerung186 . Die Synchronisations-Zeitsteuerung186 ist mit dem Nervenstimulationssystem108 und dem Trainingssystem110 kommunikationsfähig verbunden. Die Synchronisations-Zeitsteuerung136 stellt die Zeitsteuerungsbefehle dem Nervenstimulationssystem108 und dem Trainingssystem110 bereit, so dass die Stimulation und das Training gleichzeitig auftreten. Bei der Behandlung des Tinnitus kann die Stimulation des Nervs jedem Trainingston etwas vorausgehen, um der Stimulation Zeit zu geben, um das Gehirn zu beeinflussen, wenn der Trainingston angeboten wird. Weitere Ausführungsformen können andere geeignete Zeitsteuerungsvariationen enthalten. - In
17 zeigt eine graphische Darstellung eine mögliche Zeitsteuerungsbeziehung zwischen einem Ereignis und einer Stimulation für ein synchronisiertes Zeitsteuersystem. - In
18 ist ein Reaktions-Zeitsteuersystem gemäß einer Ausführungsform gezeigt. Das Reaktions-Zeitsteuersystem umfasst eine Reaktions-Zeitsteuerung138 . Die Reaktions-Zeitsteuerung138 ist mit dem Nervenstimulationssystem108 und einer Simultanereignis-Überwachungseinrichtung140 kommunikationsfähig verbunden. Die Reaktions-Zeitsteuerung138 empfängt die Zeitsteuerungsbefehle von der Ereignis-Überwachungseinrichtung140 und stellt die Zeitsteuerungsbefehle dem Nervenstimulationssystem108 bereit, so dass die Stimulation und das Training gleichzeitig auftreten. Weil die Stimulation in Reaktion auf ein Ereignis erzeugt wird, eilt die Stimulation im Allgemeinen dem Ereignis um irgendeine endliche Zeit Delta t nach. In den Fällen, in denen es einen Ereignisvorläufer gibt, der überwacht werden kann, kann die zeitliche Steuerung genauer ausgeführt werden. - In
19 zeigt eine graphische Darstellung eine mögliche Zeitsteuerungsbeziehung zwischen einem überwachten Ereignis und einer Nervenstimulation. - In
20 ist ein manuelles Zeitsteuersystem gemäß einer Ausführungsform gezeigt. Das manuelle Zeitsteuersystem enthält eine Reaktions-Zeitsteuerung138 . Die Reaktions-Zeitsteuerung138 ist mit dem Nervenstimulationssystem108 und Mitteln142 für die manuelle Eingabe kommunikationsfähig verbunden. Die Reaktions-Zeitsteuerung138 empfängt die Zeitsteuerungsbefehle von den Mitteln142 für die manuelle Eingabe und stellt die Zeitsteuerungsbefehle dem Nervenstimulationssystem108 bereit, so dass die Stimulation und das Training gleichzeitig auftreten. - In
21 zeigt eine graphische Darstellung eine mögliche Zeitsteuerungsbeziehung zwischen einem Ereignis, einer manuellen Eingabe und einer Nervenstimulation. - In
22 ist ein Regelungs-Zeitsteuersystem gemäß einer Ausführungsform gezeigt. Das Regelungs-Zeitsteuersystem enthält eine Regelungs-Zeitsteuerung144 . Die Regelungs-Zeitsteuerung138 ist mit dem Nervenstimulationssystem108 und einem Sensor146 kommunikationsfähig verbunden. Die Regelungs-Zeitsteuerung144 empfängt die Zeitsteuerungsbefehle von dem Sensor146 und stellt die Zeitsteuerungsbefehle dem Nervenstimulationssystem108 bereit, so dass die Stimulation und das Training gleichzeitig auftreten. - In
23 zeigt eine graphische Darstellung eine mögliche Zeitsteuerungsbeziehung zwischen einem abgetasteten Trainingsereignis und einer Nervenstimulation. - Der Sensor

24 ist ein initiiertes Zeitsteuersystem gemäß einer Ausführungsform gezeigt. Das initiierte Zeitsteuersystem enthält eine initiierte Zeitsteuerung148 . 25 zeigt eine graphische Darstellung eine mögliche Zeitsteuerungsbeziehung zwischen einer Nervenstimulation und einer Ereigniserzeugung. - In
26 ist ein Zeitsteuerungssystem für eine verzögerte Reaktion gemäß einer Ausführungsform gezeigt. Das Zeitsteuerungssystem für eine verzögerte Reaktion enthält eine Zeitsteuerung152 für eine verzögerte Reaktion. Die Zeitsteuerung152 für eine verzögerte Reaktion ist mit einem Nervenstimulationssystem106 und einem Sensor154 für vorbereitende Ereignisse kommunikationsfähig verbunden. Der Sensor154 für vorbereitende Ereignisse erfasst ein vorbereitendes Ereignis, das ein Paarungsereignis vorwegnimmt. Die Zeitsteuerung148 für eine verzögerte Reaktion empfängt die Zeitsteuerungsinformationen von dem Sensor154 für vorbereitende Ereignisse, die angeben, dass ein vorbereitendes Ereignis erfasst worden ist. Die Zeitsteuerung148 für eine verzögerte Reaktion stellt die Zeitsteuerungsbefehle dem Nervenstimulationssystem106 bereit, um eine Nervenstimulation einzuleiten. In der dargestellten Ausführungsform leitet die Zeitsteuerung152 unter der Voraussetzung eines negativen Delta t die Stimulation vor dem Beginn des Paarungsereignisses ein. Ein Zeitsteuerungssystem für eine verzögerte Reaktion kann die Stimulation zum gleichen Zeitpunkt wie dem Beginn des Paarungsereignisses oder nach dem Beginn des Paarungsereignisses einleiten. - In
27 zeigt eine graphische Darstellung eine mögliche Zeitsteuerungsbeziehung zwischen einer Nervenstimulation, einem vorbereitenden Ereignis und einem Paarungsereignis. - Die durch die Nervenstimulation induzierte Plastizität kann mit verschiedenen Therapien, der Rehabilitation, dem Training oder anderen Formen der persönlichen Verbesserung gepaart werden. Jede Therapie wirkt als eine Trainingsquelle. Die spezifischen Zeitsteuerungsanforderungen, die jeder Therapie zugeordnet sind, werden aus den Spezifikationen der Therapie abgeleitet, so dass die Stimulation während des Trainings auftritt, wobei sie in der Nähe des Anfangs des Trainings am effektivsten ist. Einige mögliche Therapien können Verhaltenstherapien enthalten, wie z. B. sensorische Unterscheidung für sensorische Defizite, motorisches Training für motorische Defizite, mit oder ohne Roboterunterstützung, und kognitives Training/kognitive Rehabilitation für kognitive Defizite. Die Übung und die motorische Therapie könnten gepaart werden, um motorische Defizite zu behandeln, die sich aus einer traumatischen Gehirnverletzung, einem Schlaganfall oder der Alzheimer-Krankheit und Bewegungsstörungen ergeben. Eine zwangsinduzierte Therapie könnte gepaart werden, es zu unterstützen, die Verwendung alternativer Strategien zu verhindern, um die Verwendung der nicht gepaarten Verfahren zu erzwingen. Die Sprachtherapie könnte für Sprach- und Ausdrucksdefizite gepaart werden. Kognitive Therapien könnten für kognitive Probleme gepaart werden.
- Sensorische Therapien, wie z. B. Töne, könnten gepaart werden, um sensorische Leiden, wie Z. B. Tinnitus, zu behandeln. Bei der Behandlung von Tinnitus könnten die paarweisen Töne bei Frequenzen liegen, die von den Frequenzen verschieden sind, die durch den Tinnitus-Patienten wahrgenommen werden.
- Die Bestrahlungs- oder Extinktionstherapie könnte damit gepaart werden, Phobien oder eine posttraumatische Belastungsstörung zu behandeln.
- Computerbasierte Therapien, wie z. B. FastForward für Dyslexie, Brain Fitness Program Classic oder Insight, könnten gepaart werden, um ihre Wirkungen zu verbessern. Sowohl die Psychotherapie als auch andere therapeutische Aktivitäten bei der Behandlung einer zwanghaften Verhaltensstörung, einer Depression oder einer Sucht könnten gepaart werden.
- Die Biofeedback-Therapie könnte gepaart werden. Temperaturmesswerte oder galvanische Hautreaktionen könnten gepaart werden, um Angst oder Diabetes zu behandeln. Ein Elektromyograph könnte gepaart werden, um die motorische Steuerung nach einer Gehirn-, Rückenmarks- oder Nervenschädigung zu verbessern. Ein Pneumograph könnte gepaart werden, um die Atmungskontrolle in einem gelähmten Patienten zu verbessern. Eine Echtzeit-fMRI könnte gepaart werden, um die Schmerzbehandlung zu verbessern oder die OCD zu behandeln. Ein Elektrodermograph, ein EEG, ein EMG oder ein Elektrokardiograph könnte gepaart werden, um Krankheiten, wie z. B. Angst, zu behandeln. Ein Elektroenzephalograph könnte gepaart werden, um Epilepsie zu behandeln. Eine Hämoenzephalographie könnte gepaart werden, um Migräne zu behandeln. Ein Photoplethysmograph könnte gepaart werden, um Angst zu behandeln. Ein Kapnometer könnte gepaart werden, um Angst zu behandeln. Die Therapie mit virtueller Realität könnte gepaart werden, um Krankheiten, wie z. B. eine Sucht, eine Depression, Angst oder eine posttraumatische Belastungsstörung, zu behandeln. Die Therapie mit virtueller Realität könnte außerdem gepaart werden, um die kognitive Rehabilitation oder Leistung zu verbessern. Arzneimitteltherapien könnten gepaart werden, um verschiedene Zustande zu behandeln. Amphetaminartige Verbindungen könnten gepaart werden, um die Neuromodulatoren und die Plastizität zu verbessern. SSRIs könnten gepaart werden, um die Neuromodulatoren und die Plastizität zu verbessern. MOA-Hemmstoffe könnten gepaart werden, um die Neuromodulatoren und die Plastizität zu verbessern. Gerinnungshemmende Mittel könnten gepaart werden, um während eines akuten Schlaganfalls als Mittel zum Auflösen eines Gerinnsels zu wirken. Verschiedene Arzneimittel, wie z. B. das Botulinumtoxin, Lidocain usw., könnten gepaart werden, um Krämpfe nach einer Nerven- oder Gehirnschädigung zu stoppen. Kleine Dosen von Missbrauchs-Arzneimitteln könnten gepaart werden, um ein starkes Verlangen bei Süchtigen zu löschen.
- Die Hormontherapie könnte gepaart werden. Progesteron, Östrogen, Stress-, Wachstums- oder Schilddrüsenhormone usw. könnten z. B. gepaart werden, um eine traumatische Gehirnverletzung oder die Alzheimer-Krankheit zu behandeln. Die Glucosetherapie könnte gepaart werden, um Angst zu behandeln. Die elektrische oder die magnetische Stimulation des zentralen oder peripheren Nervensystems könnten gepaart werden. Die transkranielle Magnetstimulation könnte z. B. verwendet werden, um die Aktivität in einem spezifischen Gehirnbereich zu verbessern oder zu verringern und dadurch die gerichtete kortikale Plastizität zu fokussieren. Die transkutane elektrische Nervenstimulation könnte gepaart werden, um chronische Schmerzen, Tinnitus oder andere Erkrankungen zu behandeln. Die subkutane elektrische Nervenstimulation könnte gepaart werden, um chronische Schmerzen zu behandeln. Die Stammzellentherapie könnte gepaart werden, um Krankheiten, wie z. B. die Parkinson-Krankheit, zu behandeln. Die Gentherapie könnte gepaart werden, um Zustände zu behandeln, wie z. B. das Down-Syndrom, die Huntington-Krankheit oder das Fragiles-X-Syndrom. Die hyperbare Sauerstofftherapie könnte gepaart werden, um eine Kohlenmonoxidvergiftung zu behandeln.
- Mehrere Therapien könnten gleichzeitig oder sequentiell gepaart werden.
- Keine der Beschreibungen in der vorliegenden Anmeldung sollte so gelesen werden, dass sie bedeutet, dass irgendein spezielles Element, irgendein spezieller Schritt oder irgendeine spezielle Funktion ein wesentliches Element ist, dass im Umfang der Ansprüche enthalten sein muss: DER UMFANG DES PATENTIERTEN GEGENSTANDES IST NUR DURCH DIE ERLAUBTEN ANSPRÜCHE DEFINIERT. Außerdem ist keiner dieser Ansprüche vorgesehen, um sich auf Paragraph sechs des 35 USC, Abschnitt 112, zu berufen, es sei denn, dass den genauen Wörtern ”Mittel zum” ein Mittelwort folgt.
- Die Ansprüche, wie sie eingereicht sind, sollen so umfassend wie möglich sein, wobei auf KEINEN Gegenstand absichtlich verzichtet wird, auf KEINEN Gegenstand zugunsten der Allgemeinheit absichtlich verzichtet wird und KEIN Gegenstand absichtlich aufgegeben wird.
- Zusammenfassung
- Die vorliegende Anmeldung beansprucht die Vorteile gemäß 35 U. S. C § 119(e) aus der vorläufigen US-Anmeldung Nr. 61/077.648, eingereicht am 2. Juli 2008, mit dem Titel ”Treatment of Tinnitus with Vagus Nerve Stimulation”; der vorläufigen US-Anmeldung Nr. 61/078.954, eingereicht am 8. Juli 2008, mit dem Titel ”Neuroplasticity Enhancement”; der vorläufigen US-Anmeldung Nr. 61/086.116, eingereicht am 4. August 2008, mit dem Titel ”Tinnitus Treatment Methods and Appatus”; und der vorläufigen US-Anmeldung Nr. 61/149.387, eingereicht am 3. Februar 2009, mit dem Titel ”Healing the Human Brain: The Next Medical Revolution”. Die vorliegende Anmeldung enthält die vorhergehenden Offenbarungen durch Literaturhinweis. Hintergrund: Die vorliegende Offenbarung bezieht sich im Allgemeinen auf die Therapie, die Rehabilitation und das Training einschließlich induzierter Plastizität. Insbesondere bezieht sich die Offenbarung auf Verfahren und Systeme zur Verbesserung der Therapie, der Rehabilitation und des Trainings unter Verwendung der Nervenstimulation gepaart mit Trainingserfahrungen.
Claims (20)
- Therapeutisches System, das umfasst: einen Nervenstimulator, der während einer Stimulationsperiode einem Nerv eine Stimulation bereitstellt; eine Trainingsquelle, die während einer Trainingsperiode ein Training bereitstellt; und eine Zeitsteuerung, die mit dem Nervenstimulator und der Trainingsquelle kommunikationsfähig verbunden ist, wobei die Zeitsteuerung den Nervenstimulator anweist, einem Nerv eine Stimulation bereitzustellen, so dass die Stimulationsperiode während der Trainingsperiode liegt.
- System nach Anspruch 1, bei dem die Stimulationsperiode vor dem Beginn der Trainingsperiode beginnt.
- System nach Anspruch 1, bei dem die Stimulationsperiode nach dem Beginn der Trainingsperiode beginnt.
- System nach Anspruch 1, bei dem der Nerv ein Vagusnerv ist.
- System nach Anspruch 1, bei dem die Stimulation des Nervs den Nucleus basalis beeinflusst.
- System nach Anspruch 1, bei dem die Stimulation des Nervs die Amygdala beeinflusst.
- System nach Anspruch 1, bei dem das Training einen hörbaren Tom umfasst.
- System nach Anspruch 1, bei dem das Training visuelle Informationen umfasst.
- System nach Anspruch 1, bei dem das Training körperliche Bewegung umfasst.
- System nach Anspruch 1, bei dem das Training eine chemische Therapie umfasst.
- Trainingsverfahren, das umfasst: Bereitstellen von Training während einer Trainingsperiode; Stimulieren eines Nervs während einer Stimulationsperiode, wobei die Stimulationsperiode während der Trainingsperiode auftritt; und Erzeugen einer messbaren Plastizität.
- Verfahren nach Anspruch 11, bei dem das Training therapeutisch ist.
- Verfahren nach Anspruch 11, bei dem das Training pädagogisch ist.
- Verfahren nach Anspruch 11, bei dem der Nerv elektrisch stimuliert wird.
- Verfahren nach Anspruch 11, bei dem der Nerv optisch stimuliert wird.
- Stimulationspaarungssystem, das umfasst: einen Neurostimulator, der einem Nerv eine Stimulation bereitstellt; und eine Zeitsteuerung, die mit dem Neurostimulator kommunikationsfähig verbunden ist, wobei die Zeitsteuerung die Stimulation des Nervs während einer Trainingsperiode einleitet.
- System nach Anspruch 16, bei dem die Zeitsteuerung durch Drähte mit dem Neurostimulator kommunikationsfähig verbunden ist.
- System nach Anspruch 16, bei dem die Zeitsteuerung durch induktive Spulen mit dem Neurostimulator kommunikationsfähig verbunden ist.
- System nach Anspruch 16, bei dem die Stimulation ein Zug aus elektrischen 0,8-Milliampère-Impulsen ist.
- System nach Anspruch 19, bei dem die elektrischen Impulse 30 Hz besitzen.
