Abstract
Description
The invention relates to a surgical instrument for endoscopic end-to-side connection of two hollow organs, especially a vascular prosthesis with a blood vessel, best consisting of a trocar-like and with a conical tip provided, sleeve - shaped shaft with a handle and an operating mechanism.
To hollow organs, such as blood vessels, together instruments have become known with which it is possible, blood vessels only in their longitudinal direction connection to each other. Such instruments that work with brackets, but cannot be used then, if a hollow organ, for example a vascular prosthesis or a vein graft to another hollow organ, for example a blood vessel to be connected laterally. Such vascular operations are still done manually taken, depending on the dexterity of the surgeon when handling tweezers, needles and thread with end-to-side Sewing arrives. Such operations are then special lengthy and exhausting for the patient when a Ge graft prosthesis connected to the abdominal aorta, for example shall be. Such operations do it - since the belly aorta runs behind the peritoneum near the spine - required that the abdominal wall be opened and the closure place for the vascular prosthesis is exposed so that the chir urgently in the so-called open intervention the sewing process at all can perform. Patients who have such an open undergo surgery are often multimorbile and suffer from various diseases (e.g. coronary Heart disease, hypertension, diabetes mellitus, hyperlipopro teinemia and the like) on restricted organ functions. Des half of such interventions are significant in these patients intra- and postoperative risks as well as with increased Related mortality.
The invention is therefore based on the object, a chir Urgent instrument for gentle endoscopic end-to-end Side connection of two hollow organs, especially a vessel to create a prosthesis with a blood vessel which it does not requires more that the abdominal wall or the thorax cut open the wall and the connection or interface wide must be exposed.
To solve this problem, according to the invention in a surgical instrument of the type described above suggested that the sleeve-shaped shaft follow the conical tip is a circumferential, groove-like constriction to accommodate the side connectable to a hollow organ and at least one circumferential and expandable thickening having end of the hollow which can be pushed onto the shaft organs and several with a small distance next to each other orderly, approximately C-shaped pre-formed brackets and is at least one end of all parentheses through the shaft created by the tapered tip opening of the wall enclosing the constriction the closing organ moving the hollow organ.
The instrument according to the invention makes it possible to chi surgical vascular surgery, such as the anastomosis of a vessel prosthesis on a blood vessel, for example the abdominal aorta or the thoracic aorta, endoscopically and without manual Sewing in a relatively short time Ren. So such a vascular surgery can also with such Patients are carried out, their age or health long operating times. The crane You can stay in a clinic or clinic by using the instrument according to the invention can be shortened.

In Figs. 1 to 3 of the drawing of two hollow organs Although a vascular prosthesis 2 with a interpret th aorta 3, for example, the abdominal aorta presented to-side connection end and only the front portion of a surgical instrument 1 for endoscopic. It is expressly pointed out here that such an end-to-side connection can also take place over the entire course of the aorta, that is to say also in the thoracic region. This instrument 1 , which is designed as a trocar, has a sleeve-shaped and therefore hollow, cylindrical shaft 4 made of metal, which can be designed as a kind of flexible shaft and in whose distal, front end a head with a tip 5 is inserted. At the proximal, rear end of the shaft 4 , a known handle, not shown, is arranged which is equipped with an actuating mechanism, which will be discussed further below. The head 4 a with the tip 5 can be screwed or soldered into the sleeve-shaped shaft 4 . The tip 5 is ausgebil det in a conventional manner and can be equipped with a stylet 6 if necessary. Furthermore, it is possible to insert a known optic into a bore 5 of the shaft 4 leading into the tip 5 , which is passed axially through the entire shaft 4 and which ends at the proximal, rear end, for example in an eyepiece.
At a distance from the head 4 a, the sleeve-shaped shaft 4 has a circumferential, groove-like constriction 7 , into which one end of the sleeve prosthesis 2 pushed onto the sleeve-shaped shaft 4 is inserted. This vascular prosthesis 2 is normally an artificial graft. However, a human or animal graft can also be used. However, this presupposes that the locking mechanism for the clips described below is designed differently. In this end of the vascular prosthesis 2 , a thickening 9 is incorporated, which is advantageously designed to expand by itself. This thickening 9 can consist, for example, of an expansion ring, an annular spring or the like. In the illustrated and explained exemplary embodiment, a further thickening 10 is provided at a distance from the thickening 9 , which is formed in the same way as the thickening 9 , but is not incorporated into the wall of the vascular prosthesis 2 here. Both thickenings 9 , 10 are enclosed by a large number of preformed, C-shaped brackets 11 , which consist, for example, of titanium or another suitable material. The se brackets 11 are arranged a short distance from each other umlau fend around the end of the vascular prosthesis 2 and not yet closed, that is, the free, for example pointed ends of the brackets 11 have a predetermined distance from each other ( Fig. 1).
The end of the vascular prosthesis 2 thus formed is now ent pressed together radially so far in the manufacture or in the preparation of the instrument 1 for a vascular operation that it completely, that is to say including the clips 11 , disappears in the groove-like constriction 7 of the shaft 4 . In order to ensure that this end of the vascular prosthesis 2 maintains its position in the constriction 7 , an outer sleeve 12 is pushed over the constriction ( FIG. 1), which is guided on the outside of the shaft 4 . The outer sleeve 12 can be designed to be articulated. The remaining area of the vascular prosthesis 2 is now in an annular space 13 between the outer sleeve 12 and the sleeve-shaped shaft 4 , for example in a wave shape.
The instrument 1 prepared in this way is now guided, for example using a further sleeve, through the abdominal or breast cover and the peritoneum or the pleura to the indicated aorta 3 . The wall 3 a of the aorta 3 is now pierced or cut open extremely carefully via the tip 5 , so that the tip 5 of the head 4 a and a small part of the shaft 4 with the outer sleeve 12 can penetrate into the aorta 3 ( FIG. 1 ). The pierced loading area of the wall 3 a of the aorta 3 lies on the outer side of the outer sleeve 12 at. Now the outer sleeve 12 is carefully pulled back or moved outwards by a predetermined amount via the actuating mechanism provided on the handle. Initially the pierced wall 3 a of the aorta 3 maintains its position on the outer sleeve 12th However, as soon as the outer sleeve 12 has moved through the opening 15 between the ends of the clips 11 , the wall 3 a penetrates due to its elasticity into the space between the ends of the clips 11 , as shown in FIG. 2. During this return movement of the outer sleeve 12, the elastically formed thickenings 9 and 10 , which form the end of the vascular prosthesis 2 , gradually spring outwards, so that the end of the vascular prosthesis 2 when the outer sleeve 12 assumes its drawn position, the position in the constriction tion 7 has left completely and occupies the position shown in FIG. 2.
In the head 4 a of the shaft 4 , a part 16 a of a clamp closer 16 is provided, which consists, for example, of a plurality of radially extendable segments. The other part 16 b of the clamp closer 16 is formed by an outer sleeve 12 on the outer sliding ring. When opening the wall 3 a of the aorta 3 , the part 16 a of the clamp closer 16 occupies a position within the head 4 a ( FIG. 1). However, as soon as the end of the vascular prosthesis 2 has left the constriction 7 , the part 16 a of the clamp closer 16 is moved outwards by the actuating mechanism mentioned on the handle of the instrument 1 ( FIG. 2). Now the front part of the shaft 4 , which carries the head 4 a with the tip 5 , is moved in the direction of the arrow 17 . This movement now leads to the fact that all clips 11 are closed simultaneously by an inclined end face 14 , the part 16 b of the clip closer 16 which forms the rear or proximal part of the clip closer 16 ( FIG. 3). This deformation of the brackets 11 can be promoted by appropriate selection of different cross sections of the brackets 11 over their length.
The anastomosis between the vascular prosthesis 2 and the aorta 3 is thus established. The instrument 1 is then moved out of the aorta 3 and out of the abdominal or chest cavity ( FIG. 4). The anastomosis produced in this way can be reached in a relatively short time and therefore does not require a lengthy and difficult operation. The vascular prosthesis 2 is first disconnected above the anastomosis prior to the final removal of the instrument 1 from the vascular prosthesis. 2 The other end of the vascular prosthesis 2 is then guided, for example, retroperitonally to an arterial inguinal vessel and connected there in the usual way.
If necessary, depending on the design of the locking mechanism, the outer sleeve 12 can be omitted. However, it must always be ensured that the at least one thickened portion 9 pointing towards the end of the vascular prosthesis 2 either itself or by special mechanical means radially outwards. The clamp grip 16 has to be designed so that it does not interfere with insertion of the instrument 1 into the aorta 3, but a proper closing of the clamps 11 in the wall 3a of the aorta 3 ensures. When using a human graft as a vascular prosthesis 2 , only at least one thickening 9 is prepared with the clips 11 and only connected to the vascular prosthesis 2 during the operation for later connection to the aorta 3
