Abstract
Description
The invention relates to a catheter with a front balloon the preamble of claim 1.
Die perkutane transluminale Angioplasie insbesondere der oft durch Arteriosklerose eingeengten Herzkranzschlagadern gewinnt zunehmend an Bedeutung (Vollbracht, C., et. al." "Statt Bypass anlegen, die Koronarien weiten", Praxis Kurier 30, 24. Juli 1985 Seite 25). Percutaneous transluminal angioplasia in particular coronary arteries often narrowed by arteriosclerosis is becoming increasingly important (Vollbracht, C., et. al. " "Instead of bypassing, the coronaries widen", Praxis Kurier 30, July 24, 1985 page 25). The prerequisite for this is that the balloon the stenosis on the catheter advanced in the conduit or vasoconstriction not only reached, but the first balloon also collapsed into the conduction stenosis Longitudinal penetrates to a real approach to dilation to have when the balloon is exposed to fluid. For this are Devices with balloons are required, initially very have small dimensions of their diameter. With the conventional Catheters like the balloon catheter according to Grüntzig the balloon is already outside on the front Part of the shaft of the two-lumen catheter (A.R. Grüntzig, et. al., "Transluminal Coronary Dilatation - Inventory and Outlook ", Deutsches Ärzteblatt, issue 38, from Sept. 23, 1983, pp. 27-30), which takes up space, especially since the guide wire is also in the lumen of the catheter located.
With patent P 33 26 648 "balloon catheter" is already a Front balloon in connection with a nozzle-carrying body described, the connection to the front nozzle of a catheter wins for fluid that the nozzle-carrying body with the balloon in front on / into the front nozzle of the catheter a wire is pulled out by the examiner. This illustrated The front balloon is not supported in the axial direction and therefore cannot be guided by the catheter.
From this prior art there is the task that the front balloon in front of the catheter in collapsed condition only has a very small diameter and that it also also controllable, at least axially.
This task is carried out with the catheter mentioned at the beginning Front balloon through in the characterizing part of the claim 1 included features solved.
The advantages achieved with the invention are in particular in that extremely small balloons in terms of diameter at the front of the catheter, for example, for percutaneous transluminal angioplasia penetrating very fine stenoses can be used, whereby it can be estimated that stenoses are sometimes less than 1 mm in diameter from the front balloon can be really penetrated. The Grüntzig catheter already has a diameter of 1 mm. According to a foreman of a manufacturing company Medical devices could use latex for such balloons are used, there would be foils for a balloon with the thickness of only 1/10 mm. The leadership of the front balloon on the front part of the guide wire might appear ideal but still needs testing.
Those placed around the guidewire in a stocking or tent-like manner Balloons can be made very long, so that at the same time several stenoses present in the body's conduction path can be expanded at the same time. Several totaling Expansions due to fluid introduced into the balloon are likely be gentler than a single very strong angioplasty Dilatation. Compared to the two-lumen catheter according to Schnepf or Grüntzig, the breakthroughs are for Fluid to the balloon short and wide, so that even larger front balloons according to its own patent claims, much more pressure-sensitive should be. Represents the front pole of the front balloon one unit with the ball tip of the guide wire , it will often be possible to intermittently advancing with balloon expansion, a stenosed To expand the vessel from the proximal side, for example, from the examiner. Other angiologists also wish during balloon dilation in the vessel to be expanded that the balloon if possible in the original - if narrowed vascular clearing - Longitudinally supported / secured, this would be then the front balloon is suitable, which is only a few cm behind the Tip is attached to the guide wire. That according to claim 1 noted principle of the front nozzle on the catheter is Modifiable: instead of the body of the front nozzle Breakthroughs for fluid to the balloon can be a piston in the lumen of the catheter can be used or even one to the front in Slide-through catheter slide-through ring, likewise Attached transversely to the guide wire and thus by the examiner can also be moved by means of fluid. Passive and active control the front part of the guide wire and thus the Front balloons are possible. Passive control results when the front part of the guidewire is bent pre-tensioned is active when on one side in the envelope of the front balloon there are nozzles, by their admission the front part of the guide wire is bent becomes what is recognizable on the X-ray machine. Also X-ray contrast media could be clogged in the fluid. Through crystalline Such nozzles of the balloon envelope would be reversible closable, so that then the expansion of the stenosis Conduction path of the body would have to take place. The third question Hand for the operation of such a device is relevant to answer that a foot switch to operate the fluid to be inserted into the catheter. Not always like everyone Vibrations of intermittent fluids up to the front Penetrate the front balloon, so result advantageously Vibrations on the catheter to the balloon in general, and this will undoubtedly improve the sliding of the catheter along the length of the balloon, and the expansion of the stenosed conduction pathway of the body should be the same "physiological", more gentle than with a single strong jerky expansion of the anyway wall-damaged vessel, which is not uncommon for vascular rupture led.
An embodiment of the invention is shown in the drawing shown and is described in more detail below.
