Description
Die Erfindung betrifft einen mindestens partiellen Gelenk flächenersatz der im Oberbegriff des Anspruchs 1 angegebe nen Art.The invention relates to an at least partial joint area replacement specified in the preamble of claim 1 a kind.
In the known joint prostheses are usually both articular surfaces by coordinated artificial che joint parts replaced to a suitable pairing of the Ensure joint surfaces. It differs here also the layer of one bearing material especially designed for anchoring in the bone Area. This is the sliding surface of the joint replacement but only through a foreign material educated.
Another disadvantage is that in many cases also the healthy one Counter surface of a diseased joint part to be replaced must do that by using full stem prostheses large parts of healthy bone tissue can also be removed must and that then also an unphysiological introduction the forces occurring in the bearing fabric during loading The result is.
The invention has for its object a joint surface substitute for an at least partially cartilaginous Ge steering surface of a particularly chronically degenerative state the affected joint at which the joint surface the cartilaginous area is relieved at the same time the joint surface essentially preserved or again is made to physiologically pair the glide surfaces - and thus optimal storage conditions - upright to obtain.
This task is carried out with the characteristic features of the Claim 1 solved.
The invention includes the finding that a thin walled Be with recesses or openings stratification directly on the partially cartilaginous Ge Steering surface is arranged, the waxing and / or Growth of cartilage or a cartilage generation by the perforated wall allows so that a Slide pairing of two at least partially cartilaged Articular surfaces, even when the articular surface is inserted replacement, can be maintained. Besides that is too grafting in the manner of a dowel is possible. Important here is in particular that the cartilage and the the joint capsule producing the joint lubricant if possible be fully preserved. The joint surface replacement leads to relieve the joint surface, because the one above him acting forces are largely derived. Of the This also relieves over-cartilage of the joint surface can regenerate and a cartilage regenerate can pass through the pores to the surface of the Ge steering surface replacement grow.
Due to the inner openings and the porous structure fin an integration into the natural bone substance instead of. There is also a supply of bil end the cartilage structure by connecting to the body gene vascular system.
In particular, abrasion elements of the knot can also be used here pels in the joint surface according to the invention such as kör the patient's own tissue remains in the joint space. It represents with no long-term impairment for the Ge steering function.
The invention takes into account in particular that the natural joint from articulating joint surfaces stands with a pressure-resistant support tissue, the cartilage, are covered in the support area, from a joint capsule, that of an outer fibrous layer of tight colla gene connective tissue, which is on the edge of the cartilage Areas are continued in the periosteum, and from a joint inner skin, which a joint lubrication, a river that nourishes the superficial cartilage layer liquid (synovia), secreted. By the invention Measures will take these conditions for the sick and functionally largely to replace joint surface like derManufactured. The layers of the joint surface replacement are those of the neighboring bone / cartilage tissue customized.
The joint replacement according to the invention can preferably be used find in osteoarthritis, a chronic degenerative Ge joint disease in which the mobility of the joint simultaneous permanent pain is significantly reduced. In the course of osteoarthritis an imbalance between stress and condition or performance ability of the individual joint parts and the joint tissue adjust, with an inferior quality of the biological materials involved in the articular apparatus can be born or acquired. The functionality the joint capsule as a producer of joint lubricant which diffuses the superficial cartilage layers sion is also of considerable importance, because the insufficiently nourished cartilage layers dege nerieren and no longer serve as a pressure-resistant support fabric can. Wear damage occurs with long-term malfunctions on the power consumption balance. Direct transfer damage caused by heavy work, sports or excessive body weight. Indirect overbean Damage to stress is caused by a reduction in lei Stability of the joint capsule grace caused by endogenous disorders, in particular due to increasing age or metabolism Changes and inflammation can occur.
Through the measures according to the invention, the original largely restored joint function and are also maintained permanently because through the targeted partial replacement of joint areas, the centers responsible for the complaints eliminated with the result that the function of the joint is harmonized again, so that further pathological Changes in response to the previous malfunction there is no longer a reason.
The erosion of the degenerate due to overuse Articulated cartilage continues to cause Pressure and friction particles from the pathologically changing articular cartilage in the moving parts of the joint long, the biologically active components of which then Inflammation of the membrane producing the synovial fluid cause synovialis and thus cause pain.
Furthermore, this inflammation leads to fibrillation and de markation of the cartilage substance. The cartilage substance becomes often up to complete abrasion under load and / or movement of the joint ground, whereby it for Narrowing of the joint space in the area of the strongest Pressure and bulge in the stress-free edge zones can come.
Initially, feelings of tension and stiffness occur in the affected joints, which then contribute to initial pain Override joint movements. These are probably up a sticking of the articulated partners at not enough to reduce the amount of joint lubrication. After all stress and permanent pain can occur.
The joint surface replacement according to the invention is special applicable in the area of a concave outer surface of a Ge steering surface, such as pan-like in a Acetabular cup and / or in the area of a convex outer surface of a joint surface, such as bowls be arranged on a femoral head. The joint replacement according to the invention in particular a semi-rigid or flexible lattice or rusty or spy, d. H. spongy, structure and persists preferably from a base body made of titanium sheet, which can serve as a carrier material for both layers.
There are also other body-compatible metals or me metal alloys into consideration. Another preferred Embodiment of the invention is a felt or lamella rer polymer or a composite used, the surface in terms of pore size, on the one hand, a good bone binding and on the other hand allows over-cartilage.
In another preferred embodiment, it is Joint replacement according to the invention in the manner of a stretch trained metal, which is exclusively in a Direction extending slots. With an expansion expansion of the expanded metal in a cross to the slot the direction extending extends Slot width in the direction of expansion.
In another advantageous development, the (Micro) pores at least partially with a reactive and filled with absorbable material, which with time when the joint surface replacement grows onto the joint surface by cartilage generation in the cartilaginous part or through bone tissue in deeper areas of the sub chondral joint region is displaced.
The joint surface replacement preferably has La on its edge that open up both the cartilage and the ar gripping over the articulating joint surfaces. By means of special resorbable pens through these tabs inserted into the bone, the Ge Steering surface replacement on the prepared joint surface attached.
When using resorbable pens, the with time, at a time when it no longer as fasteners are needed by the body completely absorbed. The time has come, so soon enough bone tissue or cartilage generation in the Wed cropores of the joint surface replacement have grown to the joint surface replacement a secure hold on the subchondra len bones.
In a further advantageous embodiment of the inventions In accordance with the joint surface replacement, pins are inside ver the joint surface in corresponding recesses lowers.
Even if a joint surface is arranged on both sides The cartilaginous articular surfaces act on other, so that always physiological storage conditions at can be kept with a simultaneous upright maintenance of lubricating lubricant production in the joint capsule, which continues through the microporous crossings of the joint surface replacement according to the invention up to the can penetrate cartilaginous joint surfaces.
That of the at least partially cartilaged joint surface che of the bone facing side of the invention Joint replacement is preferably with hydroxyapatite, bioactive glass or glass ceramic coated to Ver to favor anchoring of the joint surface replacement. The Ge Steering surface replacement can also be made from a composite material exist material, which is a biomecha adapted to the bone situation and the growth of cartilage or of a regenerated cartilage.

Fig. 4a shows a detail of the embodiment of FIG. 4 in plan view.
Figs. 1 and 2 show a section in the region of the femur, wherein in Fig. 1, both parts (femoral component 1 and acetabulum 2) of the hip joint are displayed together.
The hip bone 3 has a hollow spherical acetabular cup 2 , of which only one zone 4, which is crescent-shaped in section, is cartilaginous. The femoral head 5 , which has cartilage 6 on its entire articular surface, is supported against an over-cartilage zone 4 , which is also crescent-shaped in section. The non-cartilaginous cup depth 7 is filled with loose connective and fatty tissue 8 and blood vessels and also bears the attachment point of the femoral headband 9th This not cartilaginous cup depth 7 is covered by a synovial membrane, which rejects the synovial fluid, synovia, and which continues ventrally on the head band 9 . The fat and connective tissue body 8 have the function of a shock absorber and are depending on the prevailing pressure conditions under the femoral headband 9 turned up or pulled.
An embodiment of the joint surface chen replacement 10 according to the invention for the femur ( Fig. 1) is highlighted in its position by a frame II.
The partial joint surface replacement 1 has a multilayer structure, as can be seen from the illustration in FIG. 2, which forms an enlarged representation. In Fig. 2, the section is limited to that of the frame II.
In this case, a first layer 11 , which faces the articular surface and provides contact in the region of the articular movement, has a structure with relatively narrow pores or openings 12 , which allows over-cartilage, while a further surface facing the adjacent bone having layer 13 has a cancellous structure designed for growing in or growing on the adjacent bones with relatively larger openings 14 . The joint surface replacement in particular forms a columnar insert which tapers slightly towards the bone side.
The joint surface replacement according to the invention has in particular re a thin-walled cap-shaped outer layer or or Coating on which the corresponding natural Ge steering layer replaced. This is the nature, Ab rub resistance and lubricity of the opposite surface facing the articular surface of the body's own NEN cartilage substance in the relevant articular area adjusted so that the action on the natural Ge articular surface of that of a natural joint surface area corresponds essentially.
The partial replacement of the articular surface - as in the following figures is shown in more detail - corresponds with regard to the articular surface of a (convex or concave) bowl-shaped contour which differs from the previous concepts have no continuous wall, but one that is distinguished by holes or open spaces perforated membrane or cancellous structure. Here is in the perforations the possibility of cartilage regeneration is given and thus for the convex shell of the head and for the concave pan a pairing of cartilage given regenerates.
The perforated shell or pan is special designed such that they are trimmed accordingly The bone fits, and that also a plug is possible with cartilage material. That to the bone facing side of the implant enables anchoring of bone material and thus contributes to a stable Si tuation of the prosthesis. One is particularly favorable Coating - preferably with calcium phosphate with bio active glass or glass ceramic. Another advantage The prosthesis consists of a compo siten material, which is a biomecheni adapted to the bone simulated the situation and thereby the growth of Knor fel or a cartilage regenerate favored.
In Fig. 3, another preferred embodiment of the joint surface replacement 20 of the present invention is shown in a partially cut side view for use in the area of the knee joint. This is formed as a joint surface replacement for the tibia 5 in a cap-shaped manner. The thin-walled joint surface replacement 20 is preferably made of titanium sheet in its basic structure and has wall 21 through micro-pores or cancellous structure 22 over its entire surface. In order to bring about permanent anchoring of the joint surface replacement 20 in the joint head 5 , this facing con cave inner surface of the joint surface replacement 20 is provided with a coating 25 which favors the growth of bone material on the joint surface replacement 20 . Appropriate materials for such coatings 25 are, for example, calcium phosphates, bioactive glass or glass ceramics.
The joint surface replacement 20 is first inserted by flush insertion into a correspondingly created recess by pressing in with a firm fit in the damaged area of the joint surface. This is followed by additional connections by the growth of bone material on the one hand and by cartilage in the area of the joint on the other hand, so that overall a function of the defect and the structure of the natural bone and articulated joint is created. By filling pores with resorbable material, the growth can be promoted by replacing this material with newly added bone substance.
In another advantageous further development, not shown, of this preferred embodiment, some micropores 22 can also be filled with a resorbable material 24 , which is displaced when cartilage 6 or bone material ingrows.
The articulated surface elements shown, which in the Embodiments shown so far are solid are by drilling out or clearing out one corresponding volume used to hold them, such as Dowels inserted into the bore or recess created in this way adds. This is initially for one primary fixation provided, which already already Can bear pressure loads due to bearing forces. The fixation is then supported by the other se customer fixation, which is due to the growth and ingrowth The inner pores and openings are exploited det.
In Figs. 4 and 4a, a further preferred form of the guide from the joint surface replacement 30 of the invention for application in the field of Schultereckgelenks shown 31 (part of the upper arm). In the top view shown in FIG. 4a, the round shape of the dowel-like insert can be seen, as shown in FIG. 4 in section Darge.
A likewise thin-walled, pan-like and perforated titanium sheet body here forms the joint surface replacement 30 . A large part of the crescent-shaped cartilaginous Ge joint surface 34 is preserved or is newly formed as a natural substitute. The attachment is carried out by driving in this version, the sharp-edged To the cup-shaped bone-side area ensure a secure attachment. Thus, when the bone is inserted, it penetrates the existing bone material into the "cup" and ensures fixation and gradual growth there.
In other - not shown - embodiments can an attachment if necessary with - in particular resor bbaren - pins are made, which according to their holding force lose after the implant is fixed by waxing is.
In all embodiments of the joint according to the invention areas are replaced after waxing and over-knotting peln created in the joint storage conditions that the phy largely correspond to the ecological storage conditions, because the cartilaginous articular surface in its physiological Corresponds to the properties of the original articular surface and constantly regenerating itself like this.
Although the hip joint is the main application for it inventive joint surface replacement, the joint surface replacement according to the invention also in connection with all other articular surfaces of conventional articular Endoprostheses are used, preferably by one of the Articular surfaces is replaced while the opposite Articular surface in its natural function lead can. If necessary - when replacing Be range of both articular surfaces - the replaced articular surfaces but also in whole or in part overlay and interact.
The invention is not restricted in its implementation to the preferred embodiment given above game. Rather, a number of variants are conceivable which of the solution shown also in principle makes use of different types.
