Description
- Die Erfindung bezieht sich im allgemeinen auf ein Wirbelsäulenimplantat zum Gebrauch als Bandscheibenersatz und betrifft im besonderen ein gelenkiges Implantat bzw. Gelenkimplantat, das sich mit den benachbarten Wirbeln durch Knocheneinwüchse vereinigt, um den eigentlichen Raum zwischen den Wirbeln wiederherzustellen und eine Kompression von Nervenwurzeln und/oder des Rückenmarks auszuschließen, und gleichzeitig die Wirbelsäulenflexibilität zu erhalten.
- Die Wirbelsäule wird aus einer Anzahl von Wirbeln gebildet, die in ihrem normalen Zustand durch knorpelige Bandscheiben voneinander beabstandet sind. Diese Scheiben bilden ein Kissen zwischen benachbarten Wirbeln, das axialem Druck bzw. axialer Kompression entlang einer Stützachse der Wirbelsäule Widerstand entgegensetzt, jedoch eine begrenzte Bewegung zwischen den Wirbeln erlaubt, um die charakteristische Flexibilität des gesunden Rückgrats bereitzustellen. Verletzungen, Krankheit oder andere degenerative Funktionsstörungen können die Ursache dafür sein, dass eine oder mehrere Bandscheiben schrumpfen, kollabieren bzw. zusammenfallen, verfallen oder verschoben werden, eine Hernie verursachen oder auf andere Weise beschädigt werden. Dies kann zu einer Kompression einer benachbarten Nervenwurzel (oder Nervenwurzeln) oder des Rückenmarks führen, wodurch chronische und oftmals lähmende Schmerzen und in fortgeschrittenen Stadien irreversible Behinderungen der oberen und/oder unteren Glieder verursacht werden.
- Eine Anzahl von Vorrichtungen und Verfahren wurde zur Ersetzung von beschädigten oder verschobenen Bandscheiben vorgeschlagen. Ein gemeinsamer Ansatz ist die permanente Stabilisierung oder Vereinigung bzw. "Fusion" der benachbarten Wirbel, um den eigentlichen intervertebralen Raum bzw. den Raum zwischen den Wirbeln zu erhalten und eine relative Bewegung zwischen den Wirbeln auszuschließen. Verschiedene Verfahren der Wirbelstabilisierung wurden entwikkelt. Zum Beispiel wurden autogene Späne aus stiftförmigen Knochenteilen zwischen die Wirbel implantiert, um ein Knochenwachstum über den intervertebralen Raum zu bewirken, wodurch die benachbarten Wirbel zu einem Knochenstück vereinigt werden. Dieses Verfahren weist den Nachteil auf, dass es die Entnahme eines Spenderknochens für den Span aus Anderen Teilen des Palienlenkörpers benötigt, die üblicherweise einen separaten chirurgischen Arbeitsschritt erfordert und daraus resultierend die Komplikationen und den Kostenaufwand erhöht. Eine alternative Quelle stellen Leichenknochen dar, mit potentiellen Komplikationen von übertragbaren Krankheiten, beeinträchtigter Spanaufnahme, Zerfall oder Verschiebung. Eine weitere Entwicklung dieses Verfahrens der vertebralen Stabilisation umfasst die Implantation eines perforierten zylindrischen Knochenkorbes zwischen benachbarten Wirbeln. Knochenstücke, die bei der Vorbereitung der Wirbel für die Implantation erzeugt werden, werden in den Knochenkorb eingefügt, um ein Knochenwachstum in, durch und um den Korb zu begünstigen.
- Die Wirbelstabilisierung durch die Vereinigung von benachbarten Wirbeln hat sich für die dauerhafte Erhaltung des intervertebralen Raumes als erfolgreich erwiesen, jedoch wurde herausgefunden, dass sie eine Vielzahl von Nachteilen aufweist. Eine Vereinigung von benachbarten Wirbeln eliminiert zwangsläufig einen Teil des normalen Bewegungsumfanges der Wirbelsäule, wodurch die Wirbelsäulenflexibilität der Testperson reduziert wird. Zusätzlich erhöht die Vereinigung der Wirbel die Belastungen, die den benachbarten mobilen Bereichen der Wirbelsäule auferlegt werden, wodurch häufig das nachfolgende Versagen von normal verbundenen Oberflächen oberhalb und unterhalb der vereinigten Wirbel beschleunigt wird.
- Es wurde außerdem vorgeschlagen, eine verletzte Bandscheibe durch eine Prothese zu ersetzen, die "gelenkig" ist, um eine relative Bewegung zwischen den Wirbeln zu erlauben. Es wurde festgestellt, dass bisher bekannte Vorrichtungen dieser Art im allgemeinen unter mangelhafter Anhaftung zwischen der Prothese und den Wirbeln leiden. Die beabsichtige Bewegung zwischen den Komponenten bei bisher bekannten gelenkigen Prothesen kann eine relative Bewegung zwischen der Prothese und einer benachbarten Knochenoberfläche bzw. benachbarten Knochenoberflächen verursachen. Da eine solche Bewegung ein Knocheneinwachsen stören würde, wurden im allgemeinen gelenkige Prothesen als unvereinbar mit der Anhaftung mittels Knocheneinwüchsen betrachtet. Da des weiteren die Gelenkelemente dieser Vorrichtungen üblicherweise eine erhebliche vertikale Ausdehnung in Anspruch nehmen, um den gewünschten Bewegungsumfang zu erreichen und aber jedoch in den intervertebralen Raum passen müssen, wurde die Anhaftung solcher Vorrichtungen im allgemeinen durch Verwendung von flachen Platten oder Oberflächen erreicht, die auf jeder Seite der Gelenkelemente als Befestigungspunkte an die Wirbel bereitgestellt werden. Ein Beispiel für eine prothetische Vorrichtung, die dieses Verfahren der Anhaftung verwendet, ist in
WO 98/14142 - Daher ist es ersichtlich, dass noch immer ein Bedarf für ein Wirbelsäulenimplantat besteht, das einen dauerhaften Erhalt des intervertebralen Raumes bewirkt, um eine Nerven- oder Rückenmarkskompression zu verhindern und den natürlichen Bewegungsumfanges zwischen den betroffenen Wirbeln so weit wie möglich zu bewahren. Des weiteren besteht ein Bedarf für solch eine Vorrichtung, die in der Lage ist, eine dauerhafte, starke Anhaftung an die Wirbel zu bilden und dennoch nicht über die externen bzw. äußeren Oberflächen hervorzuragen, auf die sie befestigt ist. Ein weiterer Bedarf besteht für eine Vorrichtung zur Ersetzung einer beschädigten oder verschobenen Scheibe, die den intervertebralen Raum aufrechterhält, um eine Nerven- und Rückenmarkskompression zu verhindern und dennoch die natürliche relative Bewegung zwischen den Wirbeln zu bewahren. Die vorliegende Erfindung richtet sich in erster Linie auf die Bereitstellung einer Vorrichtung, die diese und andere Anforderungen erfüllt. Dementsprechend stellt die Erfindung ein Wirbelsäulenimplantat entsprechend Anspruch 1 bereit.
- Das Implantat umfasst ferner interne Gelenkmittel, die das erste Element an das zweite Element koppeln, um eine Relativbewegung zwischen dem ersten Element und dem zweiten Element zu ermöglichen. Der Begriff "Interne Gelenkmittel" beschreibt eine Vorrichtung, die eine Relativbewegung zwischen Komponenten der Vorrichtung erlaubt, im Gegensatz zu Bewegungen zwischen der Vorrichtung und einer externen Struktur, wie bspw. einem benachbarten Knochen oder anderem Gewebe.
- In besonderen Ausführungsformen der betreffenden Erfindung kann das erste und zweite Element entweder halb zylindrisch oder halb elliptisch geformte äußere Oberflächen aufweisen. Das erste und zweite Element weist jeweils eine äußere Wand auf, die mit einer oder mehreren Öffnungen perforiert ist, die darin bereitgestellt werden, um Knocheneinwüchse in die Fusions- bzw. Verbindungskammern zu erlauben, und weisen angrenzende Gelenkoberflächen auf, die die internen Gelenkmittel bilden. In einer weiteren bevorzugten Ausführungsform werden die Gelenkoberflächen als ineinandergreifende konkave und konvexe Oberflächen ausgebildet, um ein Kugelgelenk (oder eine Nut und einen Kanal oder andere derartige Gelenke) zu bilden, das eine relative Schwenkbewegung zwischen den Wirbeln erlaubt, jedoch einer Kompression zwischen diesen standhält bzw. Widerstand entgegensetzt.
- Das erste und zweite Element können miteinander verbunden werden, um ein einzelnes Element zu bilden, das unter Verwendung von Verfahren implantiert werden kann, die ähnlich zu jenen sind, die in den Implantationen der bisher bekannten ungelenkigen vertebralen Vereinigungsimplantate verfolgt werden. Vorübergehende stabilisierende Mittel können für eine starre Kopplung des ersten und zweiten Elements durch Einfügen von biologisch resorbierbaren Elementen bereitgestellt werden, um die Implantation zu erlauben und Knocheneinwüchse in die Fusionskammern und eine Vereinigung des ersten und zweiten Elements mit den benachbarten Knochen während einer anfänglichen Stabilisationsperiode zu ermöglichen, nach der die vorübergehenden Mittel biologisch zerfallen bzw. abgebaut werden, um die Gelenkbewegung zwischen den ersten und zweiten Elementen zu erlauben.
- Die vorliegende Erfindung umfasst vorzugsweise erste Verbindungsmittel, um die erste Gelenkoberfläche mit einem ersten Wirbel zu verbinden, und zweite Verbindungsmittel, um die zweite Gelenkoberfläche mit einem zweiten Wirbel zu verbinden. Die erste und zweite Gelenkoberfläche setzen einer axialen Kompression zwischen dem ersten und zweiten Element in der Richtung der Stützachse Widerstand entgegen, erlauben jedoch eine relative Schwenkbewegung zwischen dem ersten und zweiten Element. Mindestens eine, vorzugsweise beide, der ersten Verbindungsmittel und der zweiten Verbindungsmittel umfassen eine Fusionskammer, die mindestens eine Öffnung darin aufweist, um Knocheneinwüchse zu ermöglichen, und damit das Implantat dauerhaft und sicher an der entsprechenden Position zu befestigen.
- Die vorliegende Erfindung kann zum Aufrechterhalten bzw. Erhalt eines intervertebralen Raumes zwischen einem ersten Wirbel und einem zweiten Wirbel eingesetzt werden. Die Vorrichtung wird verwendet, indem ein Teilbereich des ersten Wirbels entfernt wird, um darin eine Kavität zu bilden, und ein erstes Element eines Wirbelsäulenimplantates in der in dem ersten Wirbel ausgebildeten Kavität befestigt und ein zweites Element des Wirbelsäulenimplantats mit dem zweiten Wirbel verbunden wird. Das erste Element umfasst eine erste Gelenkoberfläche und eine erste Fusionskammer, die mindestens eine Öffnung darin aufweist, um Knocheneinwüchse von dem ersten Wirbel zu ermöglichen. Das zweite Element umfasst eine zweite Gelenkoberfläche, wobei die erste und zweite Gelenkoberfläche aneinandergrenzen, um ein Gelenk zu bilden, das eine relative Schwenkbewegung zwischen dem ersten und zweiten Element erlaubt, jedoch eine relative Kompression zwischen dem ersten und zweiten Element verhindert. In einer weiteren bevorzugten Ausführungsform kann ein Teilbereich des zweiten Wirbels entfernt werden, um eine Kavität zur Aufnahme des zweiten Elements auszubilden. Das zweite Element kann ebenfalls eine zweite Fusionskammer umfassen, um Knocheneinwüchse des zweiten Wirbels zu ermöglichen. Knochenteile, die bei der Entfernung von Wirbelbereichen gebildet werden, um die Implantation oder das Knochenwachstum stimulierende Verbindungen oder Vorrichtungen aufzunehmen, können in die Fusionskammer bzw. Fusionskammern eingebracht werden, um einen Knocheneinwuchs zu fördern.

Die
1 bis3 zeigen ein Wirbelsäulenimplantat10 , das im wesentlichen ein erstes Element20 und zweites Element22 aufweist. Das erste Element20 ist an das zweite Element22 mittels eines internen Gelenkmittels24 gekoppelt, um eine relative Schwenkbewegung zwischen dem ersten und dem zweiten Element zu erlauben. - Die
4a und4b zeigen das Wirbelsäulenimplantat10 der vorliegenden Erfindung, das in situ zwischen einem er sten Wirbel12 und einem zweiten Wirbel14 eingebaut ist. Das Implantat10 ist in einem vorderen Bereich des Wirbelkörpers zwischen dem fünften und sechsten Halswirbel implantiert dargestellt. Die Vorrichtung und das Verfahren der vorliegenden Erfindung sind jedoch für vordere (anterior), seitliche (lateral) und hintere (posterior) Eingriffe an den Wirbeln einsetzbar. - Wie am besten in den
2 bis3 dargestellt, umfasst das erste Element20 des Implantats10 erste Verbindungsmittel in Form einer ersten Fusions- bzw. Verbindungskammer 30, um den ersten Wirbel12 zu beaufschlagen. Mindestens eine Öffnung32 ist in der ersten Fusionskammer30 ausgebildet, um Knocheneinwüchse in, durch und um die erste Fusionskammer30 von dem ersten Wirbel12 zu ermöglichen, um das erste Element mit dem ersten Wirbel12 zu vereinen. Vorzugsweise ist eine Vielzahl von Öffnungen32 vorgesehen, um ferner Knocheneinwüchse zu fördern. Es wurde festgestellt, dass die Vereinigung durch Knocheneinwüchse eine dauerhaftere steife Verbindung zwischen dem Implantat10 und der Skelettstruktur bereitstellt, als sie von anderen Verbindungsmittel bereitgestellt wird. - Das zweite Element
22 des Implantats10 weist ein zweites Verbindungsmittel zur Beaufschlagung des zweiten Wirbels14 auf. Das zweite Verbindungsmittel weist vorzugsweise eine zweite Fusionskammer34 auf, die im wesentlichen ähnlich in ihrem Aufbau zu der vorstehend beschriebenen ersten Fusionskammer30 ist und mindestens eine darin ausgebildete Öffnung36 aufweist, um Knocheneinwüchse von dem zweiten Wirbel14 in, durch und um die zweite Fusionskammer34 zu ermöglichen. Alternativ kann das zweite Verbindungsmittel eine Klebeverbindung, eine Schraubverbindung, eine Nagelverbindung oder jedes andere wirksame alternative Verbindungsmittel aufweisen. Jedoch wird die Bereitstellung einer zweiten Fusionskammer30 bevorzugt, da sie eine dauerhaftere und sicherere Kopplung bereitstellt als andere bekannte Befestigungsmittel. Obwohl das erste Element20 und der erste Wirbel12 in den Figuren oberhalb des zweiten Elements22 und des zweiten Wirbels14 dargestellt sind, ist es verständlich, dass die vorliegende Erfindung ebenso die umgekehrte Konfiguration einschließt, in der das erste Element, wie hierin beschrieben, an einem ersten Wirbel befestigt ist, der sich auf der Wirbelsäule unterhalb des zweiten Wirbels befindet, an dem das zweite Element befestigt ist. Das erste und zweite Element können aus biologisch verträglichen Materialien hergestellt werden, die ohne Einschränkung Titan, chirurgische Legierungen, rostfreien Stahl, Chrommolybdänlegierungen, Kobaltchromlegierungen, Zirkoniumoxidkeramik, nicht absorbierbare bzw. nicht resorbierbare Polymere und andere zu erwartend biologisch verträgliche Metall- oder Polymermaterialien umfassen. - Unter Bezugnahme auf die
2 bis3 weisen die Gelenkmittel24 der vorliegenden Erfindung eine auf dem ersten Element20 bereitgestellte erste Gelenkoberfläche40 und eine angrenzende auf dem zweiten Element22 bereitgestellte zweite Gelenkoberfläche42 auf. Die erste und zweite Gelenkoberfläche40 ,42 sind vorzugsweise aus abnutzungs- und stoßresistenten, biologisch verträglichen Materialien mit niedrigem Reibungsbeiwert, wie bspw. Titan, rostfreiem Stahl, chirurgischen Legierungen, Chrommolybdänlegierungen, Kobaltchromlegierungen, Zirkoniumoxidkeramik, nicht resorbierbaren Polymeren und anderen biologisch verträglichen Metall- oder Polymermaterialien hergestellt oder beschichtet. Die Gelenkmittel24 halten axialer Kompression zwischen dem ersten und dem zweiten Element20 ,22 Stand bzw. setzen dieser Widerstand entgegen, erlauben jedoch eine relative Schwenkbewegung zwischen diesen. Somit setzen die Gelenkmittel24 , wie in4 dargestellt, einer axialen Kompression zwischen einem ersten und einem zweiten Wirbel12 ,14 entlang einer sich gewöhnlich entlang der Wirbelsäule erstreckenden Stützachse44 , wie bspw. in4 dargestellt, Widerstand entgegen, aber erlauben eine Schwenkbewegung zwischen den Wirbeln. Der Begriff des "Schwenkens" ist dazu vorgesehen entweder eine Rotations- oder Drehbewegung um die Stützachse44 (bspw. eine Rotation zwischen Halswirbeln durch Drehen des Kopfes nach rechts oder links) und/oder eine Kippbewegung mit einem Neigungswinkel in eine beliebige Richtung relativ zu der Stützachse44 (bspw. durch Nicken des Kopfes nach vorn oder hinten und/oder Kippen des Kopfes nach unten auf die rechte oder linke Seite) oder beide dieser Bewegungen zu umfassen. - Einer axialen Kompression sowie einer lateralen Translation senkrecht zu der Stützachse
44 wird zwischen dem ersten und zweiten Wirbel12 ,14 Widerstand entgegengesetzt, indem die erste interne Gelenkoberfläche40 mit einem Hohlraum, wie bspw. einer konkaven Oberfläche46 , versehen ist, die eine von der zweiten internen Gelenkoberfläche42 auskragende Protuberanz bzw. Ausstülpung, wie bspw. eine konvexe Oberfläche48 , aufnimmt. Diese Kugelgelenkanordnung (eine sog. "ball-and-socket"-Anordnung) erlaubt eine relative Rotation um die Stützachse44 zwischen dem ersten und zweiten Wirbel12 ,14 . Die Gelenkmittel24 können mit einem oder mehreren Stops bzw. Anschlägen ausgebildet werden, um den erlaubten Umfang der Rotationsbewegung zu beschränken. - Zusätzlich zu der Kugelgelenkanordnung für die Gelenkmittel
24 sind andere Konfigurationen möglich. Wie in den8 und10 dargestellt, umfassen die Gelenkmittel eine Wippe bzw. einen Kipphebel100 und einen Kanal101 . Die zweite Gelenkoberfläche42 kann einen im wesentlichen U-förmigen Kanal101 aufweisen, der sich entlang einer Länge des zweiten Elements22 erstreckt. Die erste Gelenkoberfläche40 kann, jedoch nicht in Übereinstimmung mit der vorliegenden Erfindung, einen entsprechend geformten Kipphebel100 umfassen, der bspw. halbelliptisch geformt entlang der Länge des ersten Elements20 verlaufen kann. Eine solche Anordnung erlaubt lediglich eine Kippbewegung in einer Ebene. Als Alternative kann, wie in9 dargestellt, ein zweiter Bewegungsumfang zu den Gelenkmitteln24 hinzugefügt werden, indem die Form des Kanals102 modifiziert wird. Der modifizierte Kanal102 kann bspw., jedoch ebenfalls nicht in Übereinstimmung mit der vorliegenden Erfindung, im wesentlichen sanduhrförmig in der Tiefenrichtung des Kanals102 ausgeformt sein. Die Kanalbreite auf dem Kanalboden103 und am Eingang des Kanals104 sind größer als die Kanalbreite an dem Mittelpunkt des Kanals105 . Der sanduhrförmige Kanal102 erlaubt eine Kippbewegung entlang zweier senkrechter linearer Ebenen. - Wie in den
11a und11b dargestellt, kann ein alternativer zweiter Bewegungsumfang bzw. -bereich zu den Gelenkmitteln24 durch Modifikation des Querschnitts des Kanals106 hinzugefügt werden. Der modifizierte Kanal106 kann bspw. im wesentlichen sanduhrförmig über die Länge des zweiten Elements22 ausgebildet sein. Die Kanalbreite an dem Mittelpunkt107 des Kanals in Längsrichtung ist geringer als die Kanalbreite an dem vorderen Ende108 und dem hinteren Ende109 des Kanals. Ein Fachmann, der die Unterstützung der vorliegenden Offenbarung nutzt, wird erkennen, dass die hierin beschriebene "Sanduhr"-Form verschiedene Kurven oder gerade Kanten umfasst bzw. umfassen kann, solange die Breite an oder nahe der Endpunkte des Kanals (oder dem oberen Ende und dem Boden des Kanals) breiter ist als die Breite an einem beliebigen Punkt zwischen den Endpunkten. Der sanduhrförmige Kanal106 erlaubt zwei Bewegungsumfänge: eine Kippbewegung und eine Schwenkbewegung um die Mittelachse. Diese Bewegungen können für einige Patienten wünschenswert sein. - Gemäß der vorliegenden Erfindung und wie in den
15a und15b dargestellt, wird ein zweiter Bewegungsumfang zu den Gelenkmitteln24 hinzugefügt. Dies kann durch Modifizieren der Form des Kanals110 bereitgestellt werden. Der modifizierte Kanal110 kann bspw. im wesentlichen rauten- bzw. diamantförmig über die Länge des zweiten Elements22 ausgeführt sein. Die Kanalbreite an dem Mittelpunkt111 des Kanals in Längsrichtung ist größer als die Kanalbreite an dem vorderen Ende112 und dem hinteren Ende113 des Kanals. Ferner weist die erste Gelenkoberfläche40 einen entsprechend geformten Kipphebel115 auf, der bspw. rauten- bzw. diamantförmig entlang der Länge des ersten Elements20 ausgebildet sein kann. Dem Fachmann, der die Unterstützung der vorliegenden Offenbarung nutzt, wird erkennen, dass die hierin beschriebene Rauten- bzw. "Diamant"-Form verschiedene Kurven und gerade Kanten umfasst, solange die Breite an einem beliebigen Punkt zwischen den Endpunkten des Kanals (oder dem oberen Ende und dem Boden des Kanals) breiter ist als die Breite an oder nahe den Endpunkten. Der rautenförmige Kanal110 erlaubt drei Bewegungsumfänge: eine Kippbewegung und eine Schwenkbewegung um die Mittelachse. Diese Bewegungen können für einige Patienten wünschenswert sein. - Die Gelenkmittel
24 weisen außerdem vorzugsweise eine oder mehrere eckige hervorstehende Fasen50 auf, die in der ersten internen Gelenkoberfläche40 und/oder der zweiten internen Gelenkoberfläche42 ausgebildet sind, um eine relative Kippbewegung zwischen dem ersten und zweiten Wirbel12 ,14 in eine oder mehrere Richtungen zu erlauben. Gemäß der in den Figuren dargestellten Ausführungsform sind die erste und zweite interne Gelenkoberfläche40 ,42 jeweils mit einer eckigen hervorstehenden Fase50 ausgestattet, in einer im wesentlichen pyramidenförmigen Konfiguration, die am besten in9 dargestellt ist und wodurch eine Kippbewegung in alle Richtungen (360 °) ermöglicht wird. Eine im wesentlichen konische Konfiguration ist ebenso möglich und würde gleichermaßen eine Rotationsbewegung und eine 360°-Kippbewegung erlauben. Zusätzlich können, wie in den10 und11 dargestellt, die erste und zweite Gelenkoberfläche40 ,42 jeweils mit einer im wesentlichen bogenförmigen Konfiguration bereitgestellt werden, die einen größeren Bewegungsumfang bereitstellt. - Der natürliche Bewegungsumfang der Wirbelsäule kann durch eine Bereitstellung von Fasen
50 von näherungsweise 5° rund um den Rand von jeder der ersten und zweiten Gelenkoberfläche40 ,42 , angenähert werden und erlaubt dadurch ein Kippen in alle Richtungen von näherungsweise 10° zwischen benachbarten Wirbeln. Wie in den Figuren dargestellt ist, ist der Schwenkpunkt oder die Schwenkachse der Gelenkmittel24 im allgemeinen mittig auf der ersten und zweiten Gelenkoberfläche40 ,42 angeordnet und wird während dem Implantieren, wie in4 dargestellt, mit der regulären Rotationsachse der Wirbelsäule ausgerichtet. Diese Anordnung kann jedoch wahlweise verändert werden, um den Rotationsmittelpunkt der Gelenkmittel24 mittig (zentral), vor (anterior), hinter (posterior), links, rechts oder exzentrisch (außermittig in beiden der Vorwärts-/Rückwärtsrichtung und der Links-/Rechtsrichtung) zu bzw. von der eigentlichen Rotationsachse der Wirbelsäule anzuordnen, um eine exakte Ausrichtung der Wirbelsäule zu erreichen, wodurch eine optimale sagittale und koronale Wirbelsäulenbalance und – ausrichtung wiederhergestellt wird. - Wie am besten in
1 dargestellt ist, weisen das erste und zweite Element20 ,22 des Implantats10 der vorliegenden Erfindung vorzugsweise im wesentlichen aneinandergren zende halbzylindrische Außenwände60 ,70 auf, um einen im wesentlichen zylindrischen Körper zu bilden. In ihrer zusammengesetzten Konfiguration stoßen das erste Element20 und das zweite Element22 mit ihren jeweiligen ersten und zweiten Gelenkoberflächen40 ,42 angrenzend und ineinandergreifend, wie oben beschrieben, aneinander. Das erste Element20 weist vorzugsweise ferner eine erste gekrümmte Außenwand60 auf. Die eine Öffnung oder mehrere Öffnungen32 zur Erleichterung von Knocheneinwüchsen sind in dieser ersten gekrümmten Außenwand60 vorgesehen und stehen mit einer ersten Fusionskammer62 in Verbindung, die zwischen der ersten gekrümmten Außenwand60 und der ersten Gelenkoberfläche40 gebildet wird. In gleicher Weise weist das zweite Element22 vorzugsweise eine zweite gekrümmte Außenwand70 auf, die eine oder mehrere Öffnungen36 zur Erleichterung von Knocheneinwüchsen begrenzt. Die Öffnungen36 stehen in Verbindung mit einer zweiten Fusionskammer72 , die zwischen der zweiten gekrümmten Außenwand70 und der zweiten Gelenkoberfläche34 gebildet wird. Die erste und zweite gekrümmte Außenwand60 ,70 können mit Gewinden80 versehen sein, um den Vorschub des Implantats in den intervertebralen Raum während der Implantation zu ermöglichen und um zu helfen, das Implantat10 in der implantierten Position zu befestigen. Die Gewinde80 auf jeder der ersten und zweiten gekrümmten Außenwand60 ,70 sind vorzugsweise so ausgerichtet, dass sie durchgängige Gewinde bilden, wenn das erste und zweite Element20 ,22 ineinander eingreifen. Unter gewissen Umständen kann es wünschenswert sein, selbstschneidende Gewinde80 vorzusehen und/oder die Gewinde80 so zu gestalten, dass Knochenstücke, die bei der Implantation erzeugt werden, in die Öffnungen32 geführt werden, wie dies von demUS-Patent Nr. 5,489,308 , das für Kuslich et al. erteilt wurde, gelehrt wird. In anderen weniger bevorzugten Ausführungsformen können die Gewinde80 durch eine konturierte äußere Oberfläche ersetzt werden, die glatte, ver zahnte, angeflanschte, mit Nadeln oder mit Wülsten versehene Oberflächeneigenschaften aufweist. Das Implantat10 kann außerdem einen oder mehrere Unterstützungsflansche85 in dem ersten und/oder zweiten Element20 ,22 für eine zusätzliche Festigkeit umfassen. - Wie am besten in den
6a ,6b und12 dargestellt wird, umfassen das erste und zweite Element20 ,22 des Implantats10 der vorliegenden Erfindung vorzugsweise im wesentlichen halbelliptische Außenwände97 ,99 , die aneinandergrenzen, um einen im wesentlichen elliptischen Körper zu bilden. In ihrer zusammengesetzten Konfiguration stoßen das erste Element20 und das zweite Element22 mit ihren jeweiligen ersten und zweiten Gelenkoberflächen40 ,42 angrenzend und ineinandergreifend, wie oben beschrieben, aneinander. Das erste Element20 umfasst vorzugsweise ferner eine erste gekrümmte Außenwand97 . Die eine Öffnung oder mehrere Öffnungen32 zur Ermöglichung von Knocheneinwüchsen sind in dieser ersten gekrümmten Außenwand97 vorgesehen und stehen mit einer ersten Fusionskammer62 in Verbindung, die zwischen der ersten gekrümmten Außenwand97 und der ersten Gelenkoberfläche40 gebildet wird. In gleicher Weise weist das zweite Element22 vorzugsweise eine zweite gekrümmte Außenwand99 auf, die eine oder mehrere Öffnungen36 zur Ermöglichung von Knocheneinwüchsen begrenzt. Die Öffnungen36 stehen in Verbindung mit einer zweiten Fusionskammer72 , die zwischen der zweiten gekrümmten Außenwand99 und der zweiten Gelenkoberfläche42 gebildet wird. In alternativen Ausführungsformen können die konturierten äußeren Oberflächen des ersten 20 und zweiten 22 Elements glatte, verzahnte, angeflanschte, mit Nadeln oder mit Wülsten versehene Oberflächeneigenschaften aufweisen, um das Element in der implantierten Position zu befestigen. Das Implantat10 kann außerdem einen oder mehrere Unterstützungsflansche85 in dem ersten und/oder zweiten Element20 ,22 für eine zusätzliche Festigkeit umfassen. - In einer weiteren bevorzugten Ausführungsform, wie sie in den
7 und12 dargestellt ist, kann sich das Element10 entlang der horizontalen Achse konisch verjüngen. Das hintere Ende92 weist einen vertikalen Radius auf, der größer ist als der vertikale Radius des vorderen Endes93 . Die konische Form verringert die Materialmenge, die entfernt werden muss und erleichtert das Einfügen des Implantats10 zwischen dem ersten und zweiten Wirbel. - In einer weiteren bevorzugten Ausführungsform kann die vorliegende Erfindung ferner die Bereitstellung eines oder mehrerer Stabilisierungsmittel umfassen, um vorübergehend das erste Element
20 steif bzw. fest an das zweite Element22 zu koppeln, um eine Relativbewegung zwischen diesen zu verhindern. Beispielsweise wird es bevorzugt, dass das erste und zweite Element20 ,22 während des Einbaus des Implantats in den intervertebralen Raum steif an ihrer Position gehalten werden. Zusätzlich sollte das erste und das zweite Element20 ,22 für eine ausreichende Zeitdauer nach der Implantation steif gekoppelt bleiben, um ausreichende Knocheneinwüchse in die Fusionskammern zu erlauben, um eine Relativbewegung zwischen dem Implantat10 und den Wirbeln12 ,14 während normalen Tätigkeiten des Patienten zu verhindern. Üblicherweise reichen näherungsweise acht bis zwölf Wochen für die Knocheneinwüchse aus, bevor das erste und zweite Element20 ,22 entkoppelt werden und es ihnen ermöglicht wird, eine Gelenkbewegung auszuführen. Diese vorübergehende Stabilisierung des ersten und zweiten Elements wird durch die vorliegende Erfindung, ohne den Bedarf für eine zweite chirurgische Maßnahme durch die Verwendung von mittelfristig stabilisierenden Mitteln erreicht, die aus biologisch resorbierbarem Material gebildet werden. Bei spiele für biologisch resorbierbare Materialien umfassen Polyglykolatpolymere oder vergleichbares, Laktide, Polydioxanone, Polyglykonate, laktide/glykolide Kopolymere. Durch eine geeignete Wahl des Konstruktionsmaterials bzw. der Konstruktionsmaterialien kann die Zeitdauer, die benötigt wird, um die Stabilisierungsmittel biologisch abzubauen, wirksam kontrolliert werden. Nachdem die Stabilisierungsmittel aufgelöst und durch den Körper resorbiert wurden, werden das erste und zweite Element des Implantats10 entkoppelt und die Bewegung des Gelenks ermöglicht. In einer beispielhaften Ausführungsform, wie sie in3 dargestellt ist, können die Stabilisierungsmittel einen oder mehrere biologisch abbaubare Keile82 umfassen, die zwischen dem ersten Element20 und dem zweiten Element22 verkeilt sind, um eine Relativbewegung zwischen diesen zu verhindern. In einer anderen beispielhaften Ausführungsform werden das erste und zweite Element20 ,22 in ihrer gekoppelten Konfiguration eingesetzt, wie in den2 bis3 dargestellt, wobei in die Räume zwischen den Gelenkoberflächen40 ,42 anschließend ein biologisch abbaubares Polymer eingespritzt oder eingefüllt wird, das die mittelfristigen, vorübergehenden Stabilisierungsmittel bereitstellt, um die Elemente in ihrer Position zu koppeln. Dabei muss große Sorgfalt angewandt werden, um ein Befüllen der Fusionskammern30 ,34 und der Öffnungen32 ,36 der Fusionskammern mit dem Polymer zu verhindern, wodurch Knocheneinwüchse verhindert werden könnten. Die Gewinde82 bleiben ebenfalls offengelegt, um die Implantation der Vorrichtung zu unterstützen. In einer anderen beispielhaften Ausführungsform, wie sie in2 dargestellt ist, umfassen die Stabilisierungsmittel eine entfernbare und/oder biologisch resorbierbare Abschlusskappe90 , die lösbar das hintere Ende92 des Implantats umschließt, um das erste und zweite Element20 ,22 miteinander zu koppeln. Die Abschlusskappe kann einen oder mehrere Clips94 umfassen, um das Implantat zu um schließen, und eine oder mehrere Aussparungen96 zur Aufnahme eines Schraubenschlüssels, eines Schraubendrehers oder anderer Betätigungsvorrichtungen, die dazu benutzt werden das Implantat10 in den intervertebralen Raum zu schieben. Eine zweite entfernbare und/oder resorbierbare Abschlusskappe kann auf dem vorderen Ende93 des Implantats10 befestigt werden. Die Abschlusskappen können zusätzlich dazu benutzt werden, die Knochenstücke in den Kammern62 ,72 der Fusionskammern30 ,34 zu halten, wie im folgenden noch näher beschrieben wird, bis ausreichende Knocheneinwüchse und eine Vereinigung aufgetreten ist. - Eine weitere bevorzugte Ausführungsform der vorliegenden Erfindung ist in
5 dargestellt und zeigt ein Implantat10 , das im allgemeinen ein im wesentlichen wie oben dargestelltes erstes und zweites Element20 ,22 umfasst und ferner ein dazwischen liegendes Gelenkelement aufweist, das in der dargestellten Ausführungsform eine im wesentlichen halbkugelförmige Kappe umfasst, die zwischen die konkave Oberfläche46 des ersten Elements20 und die konvexe Oberfläche48 des zweiten Elements22 eingesetzt ist. Die Kappe90 ist vorzugsweise aus abnutzungsresistentem Konstruktionsmaterial mit niedrigem Reibungsbeiwert gefertigt, das einigermaßen belastbar sein kann, um Stoßbelastungen zu absorbieren. Die Bereitstellung der Kappe90 erlaubt eine Gelenkbewegung an der Verbindungsstelle zwischen der oberen Oberfläche der Kappe und der konkaven Oberfläche46 des ersten Elements20 sowie zwischen der unteren Oberfläche der Kappe und der konvexen Oberfläche48 des zweiten Elements22 , wodurch sich ein bipolares Gelenk ergibt. Diese Anordnung reduziert eine reibungsbedingte Abnutzung auf den Gelenkoberflächen durch eine Verteilung der Abnutzung auf zwei Gelenkverbindungsstellen. Die Beaufschlagung der unteren konkaven Oberfläche der Kappe90 mit der konvexen Oberfläche48 des zweiten Elements22 und die Beaufschlagung der oberen konvexen Oberfläche der Kappe90 mit der konkaven Oberfläche46 des ersten Elements20 verhindert, dass die Kappe beim Einbau des Implantats entfernt wird. - Die vorliegende Erfindung kann dazu genutzt werden, einen gewünschten intervertebralen Raum zwischen einem ersten Wirbel
12 und einem zweiten Wirbel14 zu erhalten. Das verwendete Verfahren umfasst vorzugsweise die Entfernung eines Teils des ersten Wirbels, um darin eine Kavität auszuformen und ein erstes Element eines gelenkigen Wirbelsäulenimplantats in der Kavität anzubringen und ein zweites Element des Implantats mit einem zweiten Wirbel zu verbinden. Das gelenkige Wirbelsäulenimplantat, das zur Ausführung dieses Verfahrens verwendet wird, ist vorzugsweise im wesentlichen ähnlich zu der vorstehend beschriebenen beispielhaften Ausführungsform, die eine erste und zweite innere Gelenkoberfläche aufweist, die aneinandergrenzen, um ein Gelenk zu bilden, das eine relative Schwenkbewegung zwischen dem ersten und zweiten Element ermöglicht, jedoch eine relative axiale Kompression zwischen diesen verhindert. Mindestens eines dieser Elemente weist ferner vorzugsweise eine Fusionskammer auf, die eine Kammer und mindestens eine Öffnung umfasst, die in Verbindung mit der Kammer steht, um Knocheneinwüchse in, durch oder um die Fusionskammer von dem jeweiligen Wirbel zu ermöglichen. In einer bevorzugten Form umfasst das Verfahren zur Benutzung einer Vorrichtung entsprechend der vorliegenden Erfindung eine Befestigung sowohl des ersten als auch des zweiten Elements des Implantats an den jeweiligen Wirbel, indem Fusionskammern für Knocheneinwüchse verwendet werden. In dieser Ausführungsform wird ein Teil des zweiten Wirbels vorzugsweise entfernt, um darin eine Kavität zur Aufnahme des zweiten Elements des Implantats zu bilden. Das Verfahren kann ferner das Einfügen von Knochenstücken, die die in der vorstehend beschriebenen Entfernung von Wirbelteilen hergestellten Stücke darstellen können, in die Fusionskammer bzw. – kammern des Implantats umfassen. Es wurde festgestellt, dass die Anwesenheit dieser Knochenstücke die Knocheneinwüchse in die Fusionskammern stimuliert und fördert. In einer weiteren Ausführungsform setzt das Verfahren die Anwendung von Mitteln zur Stimulation bzw. Anregung des Knochenwachstums in, durch und/oder um die Fusionskammer bzw. die Fusionskammern voraus. Die Mittel zur Stimulation des Knochenwachstums können bspw. elektrische oder magnetische Knochenwachstumsstimulatoren, morphogenetische Knochenproteine (BMP), Hydroxylapatit-Verbindungen, eine demineralisierte Knochenverbindung und/oder andere knochenwachstumsstimulierende Vorrichtungen oder Verbindungen umfassen. Es wird ebenfalls vorausgesetzt, dass der Einsatz von Cyanoakrylat-Material, allgemein als chirurgischer Klebstoff bezeichnet, auf der äußeren Oberfläche der Fusionskammer bzw. der Fusionskammern verwendet werden kann, um das Implantat in einer optimalen Position zu befestigen, um Knocheneinwüchse abzuwarten. - Die Implantation des gelenkigen Wirbelsäulenimplantats entsprechend der vorliegenden Erfindung kann in einer Art und Weise ausgeführt werden, die im wesentlichen den bisher bekannten Verfahren für die Implantation von nichtgelenkigen, steifen Verbindungsimplantaten mit den vorstehend dargestellten Ausnahmen gleicht. Beispielsweise kann das Verfahren gewöhnliche chirurgische Eingriffe an der Wirbelsäule, entweder über einen vorderen (anterior) Eingriff an dem Halswirbel und den Brustwirbelsäulensegmenten, oder über einen vorderen (anterior) oder hinteren seitlichen (posterior lateral) Eingriff an den Lendenwirbelsäulensegmenten anwenden. Die Entfernung von Wirbelteilen kann mittels Bohren oder Abspanen einer im wesentlichen kreisrunden Bohrung ausgeführt werden, die einen Durchmesser und eine Tiefe aufweist, die im wesentlichen übereinzustimmend dem verwen deten Implantat angepasst ist bzw. sind, um den ersten und zweiten Wirbel zu überbrücken. Zusätzlich kann die kreisrunde Bohrung konisch verjüngt ausgeführt werden, wobei der vordere Enddurchmesser kleiner ist als der hintere Enddurchmesser. Die Bohrung ist vorzugsweise mittig in dem intervertebralen Raum, näherungsweise in der Mitte zwischen dem ersten und zweiten Wirbel, angebracht. Gewinde können anschließend in die Bohrung eingeschnitten werden, um das Einsetzen des Implantats zu erleichtern.
- In einem alternativen Verfahren kann die Entfernung von Wirbelteilen bzw. -abschnitten unter Verwendung eines einzigartigen chirurgischen Instruments, wie es in
14 dargestellt ist, ausgeführt werden, um eine elliptische Bohrung zu bohren, die einen großen und kleinen Durchmesser und eine Tiefe aufweist, die angepasst sind, um im wesentlichen mit dem verwendeten Implantat übereinzustimmen, das den ersten und zweiten Wirbel verbindet bzw. überbrückt. Vorzugsweise ist das chirurgische Instrument an einem Bohrer befestigt, um für den Schneideteil Rotationsenergie bereitzustellen. Der Schneideteil des chirurgischen Instruments besteht aus einer umlaufenden flötenartigen Bohrspitze, die mit in einer elliptischen Form ausgebildeten Schneidklingen ummantelt ist. Die flötenartige Bohrspitze erstreckt sich über die Schneidklingen, um einen kreisförmigen Ausschnitt zwischen dem ersten und zweiten Wirbel zu schneiden. Sobald bzw. während die Bohrspitze in den Wirbel schneidet, entfernen die Schneidklingen das verbleibende Material, um eine elliptische Bohrung zu erstellen, die einen großen und kleinen Durchmesser und eine Tiefe aufweist, die angepasst sind, um im wesentlichen mit dem verwendeten Implantat übereinzustimmen, das den ersten und den zweiten Wirbel verbindet. - Alternativ dazu kann das Implantat mit selbstschneidenden Gewinden versehen sein. Das Implantat wird mit der miteinander in Eingriff stehenden ersten und zweiten Gelenkoberfläche eingebaut, wobei das erste und zweite Element des Implantats steif miteinander mittels Stabilisierungsmitteln gekoppelt sind, die im wesentlichen wie vorstehend beschrieben ausgeführt sind. Das stabilisierte Implantat wird in die Bohrung bis in die gewünschte Tiefe vorgeschoben. Vorzugsweise wird das Implantat so weit vorgeschoben bis das hintere Ende
92 mit der äußeren Oberfläche des Wirbels fluchtet oder leicht vertieft dazu liegt, wodurch die Möglichkeit ausgeschlossen wird, dass äußerlich vorstehende Teile des Implantats benachbarte oder lokale Gefäß-, Nerven- oder viszerale Strukturen beeinträchtigen oder verletzen könnten. - Im Lendenbereich, in dem ein größeres Lastlager benötigt wird, können zwei gelenkige Implantate in parallelen Positionen plaziert werden, wobei die jeweilige längsgerichtete Achse benachbart zu jener des anderen in einer im wesentlichen parallelen Anordnung liegt, die eine Krümmungs/Ausdehnungsbewegung ermöglicht. Diese Anordnung benötigt üblicherweise die Entfernung von Wirbelmaterial, um zwei Öffnungen oder eine ausgedehnte Öffnung zu bilden, um beide Implantate aufzunehmen. Alternativ kann ein einzelnes Implantat geeigneter Größe in den Zwischenraum unter Verwendung des transabdominalen offenen chirurgischen Eingriffs, des transabdominalen laparoskopischen Eingriffs oder des retroperitonealen offenen vorderen Eingriffs eingesetzt werden, das eine Schwenkbewegung um den einzelnen Gelenkpunkt erlaubt, wie vorstehend für die Verwendung in den Hals- und Brustbandscheibenräumen beschrieben.
- Postoperativ treten Knocheneinwüchse auf, die das erste und/oder zweite Element des Implantats mit dem jeweiligen Wirbel vereinen bzw. mit ihnen fusionieren. Nach einer ausreichenden Zeitdauer sind die stabilisierenden Mittel aufgelöst und im Körper resorbiert, wodurch die Gelenkmittel entkoppelt werden, um eine Relativbewegung zwischen dem ersten und zweiten Element zu ermöglichen, wodurch die Wirbelsäulenflexibilität erhalten wird und destruktive Belastungen auf die benachbarten intervertebralen Gelenke minimiert werden. Das Implantat setzt einer axialen Kompression zwischen dem ersten und dem zweiten Element Widerstand entgegen und verhindert dadurch eine relative Kompression zwischen dem ersten und zweiten Wirbel, wodurch der gewünschte intervertebrale Raum erhalten wird. Ferner ist das Implantat in die aufnehmende Wirbelseite versenkt, um einen Zusammenstoß mit den benachbarten Gefäß-, Nerven- oder viszeralen Strukturen auszuschließen.
Claims (19)
- Wirbelsäulenimplantat (
10 ), mit: einem ersten Element (20 ), das eine erste Gelenkoberfläche (40 ), eine erste Außenfläche (60 ,97 ) und ein erstes Verbindungsmittel zur Beaufschlagung eines ersten Wirbels (12 ) aufweist, einem zweiten Element (22 ), das eine zweite Gelenkoberfläche (42 ), eine zweite Außenfläche (70 ,99 ) und ein zweites Verbindungsmittel zur Beaufschlagung eines zweiten Wirbels (14 ) aufweist, und einem Gelenkmittel, das die erste Gelenkoberfläche an die zweite Gelenkoberfläche koppelt, um eine Relativbewegung zwischen dem ersten Element und dem zweiten Element zu ermöglichen, wobei das Gelenkmittel axialem Druck bzw. axialer Kompression in der Richtung einer Stützachse standhält bzw. Widerstand entgegensetzt, dadurch gekennzeichnet, dass das Gelenkmittel (40 ) einen Kanal (111 ) entlang einer Länge des zweiten Elements (22 ) und eine Wippe entlang einer Länge des ersten Elements (20 ) aufweist, wobei mindestens ein Teilabschnitt der Wippe (113 ) in Eingriff mit dem Kanal (111 ) steht, wobei die Wippe (113 ) und der Kanal jeweils ein erstes und zweites Ende aufweisen, wobei die Wippe (113 ) und der Kanal jeweils eine Breite an irgendeinem Punkt zwischen dem ersten und dem zweiten Ende aufweisen, die größer ist als eine Breite an oder nahe dem ersten und dem zweiten Ende. - Wirbelsäulenimplantat nach Anspruch 1, wobei die Wippe und der Kanal (
113 ) im wesentlichen rauten- bzw. diamantförmig sind. - Wirbelsäulenimplantat nach einem der vorstehenden Ansprüche, wobei die Wippe (
113 ) eine konvexe Gelenkoberfläche definiert, die mit einer Gelenkoberfläche zusammenwirkt, die durch den länglichen Kanal (111 ) definiert ist, um die Relativbewegung zwischen dem ersten Element (20 ) und dem zweiten Element (22 ) zu ermöglichen. - Wirbelsäulenimplantat nach einem der vorstehenden Ansprüche, wobei die konvexe Gelenkoberfläche eine sich über die Breite der Wippe erstreckende laterale Krümmung aufweist.
- Wirbelsäulenimplantat nach einem der vorstehenden Ansprüche, wobei ein zwischenliegendes Gelenkelement (
90 ) zwischen der ersten und der zweiten Gelenkoberfläche (40 ,42 ) positioniert wird. - Wirbelsäulenimplantat nach Anspruch 5, wobei die erste Gelenkoberfläche (
40 ) eine konkave Oberfläche (46 ) aufweist und die zweite Gelenkoberfläche (42 ) eine konvexe Oberfläche (48 ) aufweist und das dazwischenliegende Gelenkelement (90 ) eine im wesentlichen halbkugelförmige Kappe (90 ) aufweist. - Wirbelsäulenimplantat nach einem der Ansprüche 1 bis 6, das ferner ein vorderes Ende (
93 ) und ein hinteres Ende (92 ) aufweist, wobei das Wirbelsäulenimplantat eine Verjüngung zwischen dem vorderen und dem hinteren Ende (93 ,92 ) definiert. - Wirbelsäulenimplantat nach einem der vorstehenden Ansprüche, mit einem hinteren Ende (
92 ) und einem vorderen Ende (93 ), wobei ein erster Radius des hinteren Endes (92 ) größer ist als ein zweiter Radius des vorderen Endes (93 ). - Wirbelsäulenimplantat nach einem der vorstehenden Ansprüche, wobei mindestens eines der ersten und zweiten Elemente (
20 ,22 ) eine Fusions- bzw. Verbindungskammer (30 ,34 ) und mindestens eine Öffnung (32 ,36 ) aufweist, die sich durch die Außenfläche (60 ,70 ,97 ,99 ) erstreckt und in Verbindung mit der Verbindungskammer (30 ,34 ) ist, um Knocheneinwüchse in die Verbindungskammer (30 ,34 ) zu ermöglichen. - Wirbelsäulenimplantat nach einem der vorstehenden Ansprüche, wobei die erste Außenfläche (
60 ,97 ) eine erste abgerundete Außenwand (60 ,97 ) und die zweite Außenfläche (70 ,99 ) eine zweite abgerundete Außenwand (70 ,99 ) aufweist. - Wirbelsäulenimplantat nach Anspruch 10, wobei die erste abgerundete Außenwand (
60 ,97 ) und die zweite abgerundete Außenwand (60 ,99 ) Sicherungs- bzw. Befestigungsmittel zur Sicherung des Wirbelsäulenimplantats (10 ) innerhalb einer Kavität zwischen dem ersten Wirbel und dem zweiten Wirbel aufweisen. - Wirbelsäulenimplantat nach einem der vorstehenden Ansprüche, wobei die erste Außenfläche (
60 ,97 ) und die zweite Außenfläche (70 ,99 ) jeweils eine mit einem Gewinde versehene Außenfläche (80 ) aufweisen. - Wirbelsäulenimplantat nach einem der vorstehenden Ansprüche, das ferner stabilisierende Mittel zur vorüberge henden, starren Kopplung des ersten Elements (
20 ) an das zweite Element (22 ) aufweist. - Wirbelsäulenimplantat nach einem der vorstehenden Ansprüche, wobei das stabilisierende Mittel eine Endkappe (
90 ) aufweist, die angepasst ist, um einen Teilabschnitt des ersten Elements (20 ) und einen Teilabschnitt des zweiten Elements (22 ) lösbar zu beaufschlagen. - Wirbelsäulenimplantat nach Anspruch 14, wobei das stabilisierende Mittel mindestens ein biologisch resorbierbares Element (
82 ) aufweist, das angepasst ist, um im Anschluss an die Implantation absorbiert bzw. resorbiert zu werden. - Wirbelsäulenimplantat nach einem der vorstehenden Ansprüche, wobei die erste und die zweite Außenfläche (
60 ,70 ) des ersten und zweiten Elements (20 ,22 ) konvex ausgebildet sind und zusammenwirken, um einen im wesentlichen zylinderförmigen Implantatkörper zu bilden. - Wirbelsäulenimplantat nach einem der vorstehenden Ansprüche 1 bis 16, wobei die erste Außenfläche (
60 ) und die zweite Außenfläche (70 ) jeweils eine halbzylindrische Außenfläche aufweisen. - Wirbelsäulenimplantat nach einem der Ansprüche 1 bis 15, wobei die erste und zweite Außenfläche (
97 ,99 ) konvex ausgebildet sind und zusammenwirken, um einen im wesentlichen elliptisch geformten Implantatkörper zu bilden. - Wirbelsäulenimplantat nach einem der Ansprüche 1 bis 15 oder 18, wobei die erste konvexe Außenfläche (
97 ) und die zweite Außenfläche (99 ) jeweils eine halbelliptische Außenfläche (97 ,99 ) aufweisen.
