Description
- Die vorliegende Erfindung betrifft einen Apparat für Wirbelsäulenprothesen.
- HINTERGRUND DER ERFINDUNG
- Die Spinalstenose ebenso wie die Spondylose, Spondylolisthese, Arthrose und andere degenerative Erscheinungen können Rückenschmerzen hervorrufen, insbesondere Schmerzen im unteren Rücken. Derartige Erscheinungen können durch Verengen des Spinalkanals durch eine Vielzahl von Ursachen hervorgerufen werden, die zum Quetschen des Rückenmarks und/oder der Nerven in der Wirbelsäule führen. Die Fusion von zwei oder mehr benachbarten Rückenwirbeln sollte den Rückenschmerz lindern. Die Fusion der Rückenwirbel kann jedoch missbilligt werden, weil die Fusion dazu neigt, degenerative Erscheinungen in den fusionierten Rückenwirbeln hervorzurufen, um zu benachbarten Rückenwirbelteilen zu wandern, die nicht fusioniert waren.
- Das
US-A-6063121 offenbart eine vertebrale Körperprothese mit oberen und unteren Platten, die jeweils ein Kugelstützelement und ein Hülsenstützelement tragen. Eine elastische Ringscheibe wird zwischen der oberen und unteren Platte angeordnet. - Die vorliegende Erfindung strebt danach, eine neuartige Wirbelsäulenprothese bereitzustellen, wie sie hierin nachfolgend detaillierter beschrieben wird. Die hierin offenbarten Prothesen sind insbesondere für den posterioren Abschnitt der Wirbelsäule vorteilhaft, aber die Erfindung ist nicht auf den posterioren Abschnitt der Wirbelsäule beschränkt.
- Es wird daher in Übereinstimmung mit einer Ausführung der vorliegenden Erfindung ein Artikel in Übereinstimmung mit dem nachfolgenden Patentanspruch 1 bereitgestellt.
- Die Wirbelsäulenprothese kann eines oder mehrere der folgenden charakteristischen Merkmale einschließen. Beispielsweise können das erste und zweite Befestigungselement mit Montagelöchern ausgebildet werden, die so ausgeführt sind, dass ein mechanisches Verbindungselement hindurch und in die Wirbelsäulenstruktur gelangen kann. Das erste und zweite Befestigungselement können Seitenwände einschließen, die durch einen Spalt getrennt sind, wobei der Spalt so ausgeführt ist, dass darin ein posteriorer Abschnitt der Wirbelsäule aufgenommen wird. Die Biegebaugruppe kann an dem ersten und zweiten Befestigungselement mittels einer Zapfenverbindung befestigt werden. Das erste und zweite Befestigungselement können mindestens ein Paar von entgegengesetzten elastischen Sperrklinken umfassen, die für ein Ergreifen des Abschnittes der Wirbelsäule ausgeführt sind. Die Biegebaugruppe kann mehr als ein Biegeelement zwischen dem ersten und zweiten Befestigungselement umfassen. Mindestens ein Abschnitt der Biegebaugruppe kann am ersten und zweiten Befestigungselement mit einer Vielzahl von Sperrelementen befestigt werden. Alternativ kann mindestens ein Abschnitt der Biegebaugruppe zusammenhängend mit dem ersten und zweiten Befestigungselement ausgebildet werden. Die Sperrelemente können plattenartige Elemente umfassen, die am ersten und zweiten Befestigungselement mit mechanischen Verbindungselementen gesichert werden. Die Sperrelemente können mindestens einen Ansatz umfassen, der sich im Allgemeinen senkrecht von den plattenartigen Elementen erstreckt, um die mindestens ein Anschlag in Eingriff gebracht wird. Die Biegebaugruppe kann eine Schutzkappe umfassen, die mindestens teilweise um die inneren Abschnitte der ersten und zweiten Befestigungselemente angeordnet wird, wobei die Schutzkappe mit dem ersten und zweiten Befestigungselement verbunden wird. Die Schutzkappe kann elastomer sein. Die Biegebaugruppe kann so ausgeführt sein, dass sie sich richtungsunabhängig biegt. Eine Vielzahl von Wirbelbogenwurzelschrauben kann an der Wirbelsäulenprothese befestigt oder damit zusammenhängend ausgebildet werden. Die Wirbelbogenwurzelschrauben können polyaxiale Wirbelbogenwurzelschrauben mit einem Gewindeschaft und einem polyaxialen Drehkopf umfassen. Der Gewindeschaft und/oder der polyaxiale Drehkopf können an der Wirbelsäulenprothese befestigt oder zusammenhängend damit ausgebildet werden. Das erste und zweite Befestigungselement können Montagearme umfassen, die drehbar in einem Gehäuse montiert sind, wobei ein Abschnitt eines jeden Montagearmes in einer hohlen Kammer angeordnet wird, die im Gehäuse gebildet wird, und wobei ein anderer Abschnitt eines jeden Montagearmes aus dem Gehäuse durch eine im Gehäuse gebildete Öffnung vorseht. Die Biegebaugruppe kann eine mehrteilige Gelenkbaugruppe umfassen, die ein erstes Gelenkelement umfasst, das eine konvexe Kontur aufweist, das mit einem zweiten Gelenkelement gelenkig verbunden ist, das eine konkave Kontur aufweist, die der konvexen Kontur des ersten Gelenkelementes entspricht und darüber gleitet, wobei das erste und zweite Gelenkelement an den Befestigungselementen befestigt werden oder zusammenhängend ein Teil davon sind. - Mit Bezugnahme auf
1A und1B wird eine Struktur einer menschlichen Wirbelsäule veranschaulicht, insbesondere die Lendenrückenwirbel. -
1A und1B veranschaulichen jeweils den vierten und fünften Lendenrückenwirbel L4 und L5 in einer seitlichen Ansicht (während sie sich in einer anatomischen Verbindung befinden) und in einer Draufsicht (separat). Die Lendenrückenwirbel (von denen insgesamt fünf vorhanden sind) befinden sich im unteren Rücken; sie werden ebenfalls als das „Kreuz" bezeichnet. - Wie es bei Rückenwirbeln typisch ist, werden die Rückenwirbel L4 und L5 durch eine intervertebrale Scheibe
125 getrennt. Die Konfiguration der Rückenwirbel L4 und L5 weicht etwas ab, aber jeder Rückenwirbel umfasst einen vertebralen Körper110 , der der anteriore massive Teil des Knochens ist, der der Wirbelsäule eine Festigkeit verleiht und das Körpergewicht aufnimmt. Ein vertebraler Bogen112 ist posterior zum vertebralen Körper110 vorhanden und wird durch linke und rechte Wirbelbogenwurzeln114 und die Lamina116 gebildet. Die Wirbelbogenwurzeln114 sind kurze, feste Fortsätze, die den vertebralen Bogen112 mit dem vertebralen Körper110 verbinden. Die Wirbelbogenwurzeln114 ragen posterior heraus, um mit zwei breiten, flachen Knochenplatten zusammenzupassen, die als die Lamina116 bezeichnet werden. - Sieben weitere Fortsätze erheben sich aus dem vertebralen Bogen
112 . Ein Dornfortsatz118 und zwei Querfortsätze120 ragen am dem vertebralen Bogen112 heraus und gewähren Befestigungen für die Muskeln, wodurch Hebel gebildet werden, die dabei helfen, dass die Muskeln die Rückenwirbel bewegen. Zwei superiore Gelenkfortsätze122 ragen superior aus dem vertebralen Bogen112 heraus, und zwei inferiore Gelenkfortsätze124 ragen inferior aus dem vertebralen Bogen112 heraus. Die superioren Gelenkfortsätze122 eines Rückenwirbels sind entgegengesetzt entsprechenden inferioren Gelenkfortsätzen124 eines benachbarten Rückenwirbels angeordnet. Gleichermaßen sind inferiore Gelenkfortsätzte124 entgegengesetzt entsprechenden superioren Gelenkfortsätzen122 eines benachbarten Rückenwirbels angeordnet. Der Schnitt der superioren Gelenkfortsätze122 eines Rückenwirbels mit den inferioren Gelenkfortsätzen124 eines benachbarten Rückenwirbels bildet ein Gelenk, das als ein Zygapophysial-Gelenk bezeichnet wird oder kurz als ein Facetgelenk oder Facet126 . Facetgelenke126 , die zwischen benachbarten superioren Gelenkfortsätzen122 und inferioren Gelenkfortsätzen124 längs der Wirbelsäule vorgefunden werden, gestatten eine Gleitbewegung zwischen den Rückenwirbeln L4 und L5. - Mit Bezugnahme auf
1C wird eine Wirbelsäulenprothese10 veranschaulicht, die in Übereinstimmung mit einer Ausführung der vorliegenden Erfindung konstruiert und funktionsfähig ist. - Die Prothese
10 kann ein oberes (superiores) vertebrales Befestigungselement20 , das an einem der Dornfortsätze118 (in1C nicht gezeigt) befestigt werden kann, und ein unteres (inferiores) vertebrales Befestigungselement21 umfassen, das an einem benachbarten Dornfortsatz118 befestigt werden kann. Die Befestigungselemente20 und21 kennen steif oder nichtsteif sein, aus Materialien hergestellt werden, die ein biokompatibles Material, wie beispielsweise ein Metall, beispielsweise nichtrostenden Stahl, Titan oder Titanlegierung, Kobalt-Chrom-Legierungen, Kunststoff oder andere harte, steife Materialien oder irgendeine Kombination der vorangehenden, einschließen, aber nicht darauf beschränkt sind. - Ein elastomeres Element
22 (worauf man sich ebenfalls als eine Biegebaugruppe bezieht) wird zwischen dem oberen und unteren vertebralen Befestigungselement20 und21 angeordnet und kann zusammenhängend damit gebildet oder daran befestigt werden. Das elastomere Element22 kann aus einem nachgiebigen Material hergestellt werden, einschließlich, aber nicht beschränkt auf, polyurethanhaltige Materialien, silikonhaltige Materialien, Elastomere auf Polyethylenbasis, Hydrogele und polypropylenhaltige Materialien. Das elastomere Element22 kann sich in jeder beliebigen Richtung biegen, was dabei helfen kann, die Spannung zwischen den Befestigungselementen20 und21 während der Bewegung der Wirbelsäule zu verringern. Die Biegung des elastomeren Elementes22 gestattet, dass sich die Befestigungselemente20 und21 relativ zueinander bewegen. Das kann dabei helfen, die Prothese im Körper einzusetzen, indem Versetzungen zwischen den Teilen der Prothese und den Montagelöchern im Körper überwunden werden. - Um die Prothese
10 einzusetzen, kann jegliche Substanz oder jegliche Körperstruktur, wie beispielsweise Bänder, entfernt oder vorübergehend zur Seite bewegt werden, um benachbarte Dornfortsätze freizulegen. Beispielsweise kann eine Laminektomie (Schneiden in die Lamina116 und Entfernen von mindestens einem Teil davon) durch eine posteriore Inzision durchgeführt werden. Danach können die Befestigungselemente20 und21 an den freigelegten Dornfortsätzen118 der benachbarten Rückenwirbel befestigt werden. - Bei der Ausführung in
1C werden, aber nicht zwangsläufig, das obere und untere vertebrale Befestigungselement20 und21 mit Montagelöchern24 ausgebildet, die so ausgeführt sind, dass ein mechanisches Verbindungselement (nicht gezeigt) hindurch und in die Dornfortsätze118 gelangt. Beispielsweise kann irgendein Verbindungselement oder Verbindungselemente, wie beispielsweise Schrauben, Schraubenbolzen, Nieten, Nägel, Täcks und Muttern, aber nicht darauf beschränkt, durch die Löcher24 hindurchgehen, um die Befestigungselemente20 und21 fest am Dornfortsatz118 zu befestigen. - Die vertebralen Befestigungselemente
20 und21 können Seitenwände25 aufweisen, die durch einen Spalt23 getrennt sind (die eine U-Form bilden). Der Dornfortsatz118 oder irgendein posteriorer Abschnitt der Wirbelsäule kann beispielsweise enganliegend (oder lose) im Spalt23 vor dem Befestigen mit dem mechanischen Verbindungselement aufgenommen werden. Ein Loch oder Löcher kann in oder genau durch den Dornfortsatz118 an einer Stelle entgegengesetzt den Löchern24 gebohrt werden, um die mechanischen Verbindungselemente zu verwenden, um die Befestigungselemente20 und21 fest an den Dornfortsätzen118 zu befestigen. - In der Ausführung in
1C kann, aber nicht zwangsläufig, das elastomere Element22 mittels einer Zapfenverbindung befestigt werden am oder zusammenhängend gebildet werden mit dem oberen und unteren vertebralen Befestigungselement20 und21 . Die Zapfenverbindung kann Zapfen27 (Vorsprünge) aufweisen, die zu Schlitzen (Nuten)29 passen. Die Erfindung ist jedoch nicht auf diese Art von Gelenk oder Verbindung beschränkt, und andere Arten von Gelenken, Verbindungen, Verbindungselementen, Klebstoffbindung und dergleichen können ebenfalls zur Anwendung gebracht werden. - Mit Bezugnahme auf
2 wird eine Wirbelsäulenprothese30 mit einem oberen und unteren vertebralen Befestigungselement32 und34 veranschaulicht, die an einem Dornfortsatz118 befestigt werden können, ohne dass in oder durch den Dornfortsatz gebohrt werden muss. Daher kann eine kleinere Inzision beim Einsetzen der Prothese30 angewandt werden als beim Einsetzen der Prothese10 . - Diese Ausführung kann ein oberes und unteres vertebrales Befestigungselement
32 und34 umfassen, die ein oder mehrere Paare von entgegengesetzten elastischen Sperrklinken36 aufweisen, die auf einer Basis38 montiert und für ein Ergreifen des Dornfortsatzes118 ausgeführt sind. Die Sperrklinken36 wenden eine Federkraft an, um den Dornfortsatz118 fest zu ergreifen. Die Befestigungselemente können daher an Ort und Stelle gedrückt werden und „schnappen" fest am Dornfortsatz118 „ein". (Der Dornfortsatz118 wird im Spalt33 aufgenommen, wie vorangehend.) - Bei den vorangehend beschriebenen Ausführungen kann die Prothese am Dornfortsatz
118 allein befestigt werden. Die Befestigungselemente können jedoch zusätzlich oder alternativ an anderen Teilen des Rückenwirbels befestigt werden, wie beispielsweise, aber nicht darauf beschränkt, an Teilen des Facet126 und/oder an den Wirbelbogenwurzeln114 oder irgendeinem posterioren Abschnitt der Wirbelsäule, nicht eben dem Dornfortsatz118 . Andere Ausführungen, die an den Wirbelbogenwurzeln114 befestigen, werden hierin nachfolgend weiter beschrieben. - Mit Bezugnahme auf
3 wird eine elastomere Wirbelsäulenprothese39 veranschaulicht, die in Übereinstimmung mit einer noch weiteren Ausführung der vorliegenden Erfindung konstruiert und funktionsfähig ist. Die Prothese39 kann beispielsweise am Dornfortsatz118 und an einer anderen vertebralen Struktur, beispielsweise einem Facet126 oder einer Wirbelbogenwurzel114 oder irgendeinem posterioren Abschnitt der Wirbelsäule (siehe1A und1B ; nicht in3 gezeigt), befestigt werden. - Die Prothese
39 kann ein oberes vertebrales Befestigungselement40 , das am Dornfortsatz118 , dem Facet126 und/oder der Wirbelbogenwurzel114 befestigt werden kann, und ein unteres vertebrales Befestigungselement41 einschließen, das an einem anderen Abschnitt der Wirbelsäule (nicht gezeigt) befestigt werden kann, wie beispielsweise einem benachbarten Dornfortsatz118 , Facet126 und/oder Wirbelbogenwurzel114 . Die Befestigungselemente40 und41 können steif oder nichtsteif sein, wie vorangehend. Ein oder mehrere elastomere Elemente42 (zwei werden in diesem Beispiel gezeigt; wobei man sich auf die elastomeren Elemente42 ebenfalls als eine Biegebaugruppe bezieht) können zwischen dem oberen und unteren vertebralen Befestigungselement40 und41 angeordnet und zusammenhängend damit gebildet oder daran befestigt werden. Das elastomere Element42 , das deutlich in6 gezeigt wird, kann aus einem nachgiebigen Material hergestellt werden, wie vorangehend. - Bei der Ausführung in
3 können, aber nicht zwangsläufig, das obere und untere vertebrale Befestigungselement40 und41 mit einem oder mehreren Montagelöchern ausgebildet werden, die so ausgeführt werden, dass ein mechanisches Verbindungselement (nicht gezeigt) durch und in den Abschnitt der Wirbelsäule gelangen kann. Beispielsweise kann irgendein Verbindungselement oder Verbindungselemente, wie beispielsweise, aber nicht darauf beschränkt, Schrauben, Schraubenbolzen, Nieten, Nägel, Täcks und Muttern durch die Montagelöcher hindurchgehen, um die Befestigungselemente40 und41 fest am Dornfortsatz118 zu befestigen. -
4 und5 sind posteriore perspektivische Ansichten von nicht einschränkenden Beispielen jeweils des oberen und unteren Befestigungselementes40 und41 , wie in3 gezeigt wird. Ein Montageloch44 wird beispielsweise für das Befestigen des Befestigungselementes40 posterior von einem der Dornfortsätze118 gebildet. Ein weiteres Montageloch46 im Element40 kann für eine Fixierung am rechten Facet126 des gleichen Rückenwirbels und ein noch weiteres Montageloch48 für eine Fixierung am linken Facet126 des gleichen Rückenwirbels vorhanden sein. Die Dreipunktfixierung oder Befestigung kann eine stabile und sicherere Befestigung des Elementes40 am Rückenwirbel posterior mit einem minimalen Eingriffsgrad bewirken. - In der Ausführung in
3 kann, aber nicht zwangsläufig, das elastomere Element mittels einer Zapfenverbindung befestigt werden am oder zusammenhängend gebildet werden mit dem oberen und unteren vertebralen Befestigungselement40 und41 . Die Zapfenverbindung kann Zapfen47 (Vorsprünge) aufweisen, die zu Schlitzen (Nuten)49 passen. Die Erfindung ist jedoch nicht auf diese Art von Gelenk oder Verbindung beschränkt, und andere Arten von Gelenken, Verbindungen, Verbindungselementen, Klebstoffbindung und dergleichen können ebenfalls zur Anwendung gebracht werden. - Es wird bemerkt, dass die Montagelöcher in vielerlei Weise angeordnet und ausgerichtet werden können. Beispielsweise gelangen in der Ausführung in
1C die Montagelöcher24 nicht durch eine Ebene, die das elastomere Element22 schneidet. Die Montagelöcher24 sind im Allgemeinen quer zu einer Längsachse des elastomeren Elementes22 . Im Gegensatz dazu geht bei der Ausführung in3 das Montageloch44 durch eine Ebene, die das elastomere Element42 schneidet. Die Montagelöcher44 ,46 und48 sind im Allgemeinen parallel zur Längsachse des elastomeren Elementes42 . Andere Anordnungen liegen ebenfalls innerhalb des Bereiches der Erfindung. - Mit Bezugnahme auf
7A ,7B ,8A und8B wird eine elastomere Wirbelsäulenprothese50 veranschaulicht, die in Übereinstimmung mit einer noch weiteren Ausführung der vorliegenden Erfindung konstruiert und funktionsfähig ist. Die Prothese50 kann beispielsweise am Dornfortsatz118 und Wirbelbogenwurzeln114 befestigt werden (siehe1A und1B ; nicht in7A ,7B ,8A und8B gezeigt). - Die Prothese
50 kann ein oberes (superiores) vertebrales Befestigungselement52 und ein unteres (inferiores) vertebrales Befestigungselement54 umfassen. Die Befestigungselemente52 und54 können an benachbarten Dornfortsätzen118 befestigt werden, aber, wie es hierin nachfolgend weiter beschrieben wird, kann die Prothese50 an Wirbelbogenwurzeln befestigt werden, ohne dass man die Befestigungselemente52 und54 an den Dornfortsätzen118 befestigen muss. Die Befestigungselemente52 und54 können steif oder nichtsteif sein, am Materialien hergestellt werden, die ein biokompatibles Material, wie beispielsweise ein Metall, beispielsweise nichtrostenden Stahl, Titan oder Titanlegierung, Kobalt-Chrom-Legierungen, Kunststoff oder andere harte, steife Materialien oder irgendeine Kombination der vorangehenden einschließen, umfassen, aber nicht darauf beschränkt sind. - Eine Biegebaugruppe
56 , die ein oder mehrere Biegeelemente (nachfolgend beschrieben) aufweist, kann zwischen dem oberen und unteren vertebralen Befestigungselement52 und54 angeordnet und zusammenhängend damit gebildet oder daran befestigt werden. Die Biegebaugruppe56 kann am oberen und unteren vertebralen Befestigungselement52 und54 mittels Sperrelementen58 befestigt werden. Die Sperrelemente58 können plattenartige Elemente59 einschließen, die am oberen und unteren vertebralen Befestigungselement52 und54 mit mechanischen Verbindungselementen60 gesichert sind, wie beispielsweise Schrauben, aber nicht darauf beschränkt. Die Sperrelemente58 können außerdem Ansätze61 umfassen, die sich im Allgemeinen senkrecht von den plattenartigen Elementen59 erstrecken, um die Anschläge62 sicher in Eingriff gebracht werden. Vier derartige Anschläge62 werden in der veranschaulichten Ausführung gezeigt; jedoch kann die Erfindung mit irgendeiner Anzahl von derartigen Anschlägen durchgeführt werden. Eine Schutzkappe64 kann zumindestens teilweise oder vollständig um die inneren Abschnitte der Befestigungselemente52 und54 angeordnet werden, beispielsweise zumindestens teilweise oder vollständig um die Sperrelemente58 und die Anschläge62 . Die Schutzkappe64 kann irgendeine geeignete Form oder Größe aufweisen, wie beispielsweise eine Ring-, Strumpf-, Ellipsoidform und andere Formen, ist aber nicht darauf beschränkt. Wie deutlich in8A und8B gesehen wird, wird die Schutzkappe64 schichtartig zwischen dem oberen und unteren vertebralen Befestigungselement52 und54 angeordnet und mit Sperrelementen58 , wie beispielsweise mittels, aber nicht zwangsläufig, der inneren Kanten65 der Schutzkappe64 , verbunden, die stationär mit den Nuten66 in Eingriff kommen, die in plattenartigen Elementen59 gebildet werden, wie am besten in8B zu sehen ist. Daher weist die Biegebaugruppe56 Anschläge62 und die Schutzkappe64 auf Die Biegebaugruppe56 kann aus einem nachgiebigen elastomeren Material konstruiert werden, einschließlich polyurethanhaltiger Materialien, silikonhaltiger Materialien, Elastomeren auf Polyethylenbasis, Hydrogelen und polypropylenhaltigen Materialien, aber nicht darauf beschränkt. Alternativ kann die Biegebaugruppe56 aus steifen Materialien konstruiert werden, wie beispielsweise nichtrostendem Stahl. Auf jeden Fall wird die Schutzkappe64 aus einem nachgiebigen Material hergestellt, wie beispielsweise einem Elastomer (z.B. Polyurethan) oder Stoff (Gewebe oder Vliesstoffe aus synthetischen oder natürlichen Fasern). - Die Biegebaugruppe
56 gestattet die Biegung der Prothese50 um zwei gegenseitig orthogonale Achsen67 und68 ebenso wie andere Richtungen für eine richtungsunabhängige Biegung in irgendeinem Freiheitsgrad. Beispielsweise, wie in8A gesehen wird, kann sich die Prothese50 elastisch um die Achse67 in der Richtung der Pfeile69 drehen. Wie in8B gesehen wird, kann sich die Prothese50 elastisch um die Achse68 in der Richtung der Pfeile70 drehen. Die Anschläge62 (ebenso wie die Schutzkappe64 in gewissem Maß) können die Biegung der Biegegruppe56 begrenzen, und sie können daher die relative Bewegung der Befestigungselemente52 und54 mit Bezugnahme zueinander begrenzen. - In Abhängigkeit von der Prothese
50 sind Wirbelbogenwurzelschrauben72 (7A und7B ) für das Einsetzen der Prothese50 in die Wirbelbogenwurzeln114 vorhanden. Die Wirbelbogenwurzelschrauben72 können ohne Einschränkung polyaxiale Wirbelbogenwurzelschrauben aufweisen, beispielsweise aus Titan oder Titanlegierung hergestellt, die kommerziell in vielen Größen und Formen von vielen Herstellern erhältlich sind. Es wird bemerkt, dass Titan sehr korrosionsbeständig und ermüdungsbeständig und MRI-kompatibel ist. Die Wirbelbogenwurzelschraube72 kann einen Gewindeschaft74 und einen beweglichen Drehkopf75 aufweisen, dessen Fähigkeit zur Drehung dabei helfen kann, eine vertebrale Spannung zu vermeiden. Die Drehköpfe75 können drehbar an abgerundeten Zinken76 , die aus dem oberen und unteren vertebralen Befestigungselement52 und54 herausragen, mittels Sicherungsmuttern77 befestigt werden, die zu den Köpfen75 passen. - Um die Prothese
50 einzusetzen, kann eine Laminektomie durchgeführt werden, wodurch ein Spalt zwischen den Dornfortsätzen118 gebildet wird, wie es hierin vorangehend erwähnt wird. Die Wirbelbogenwurzelschrauben72 können in die Wirbelbogenwurzeln114 geschraubt werden. Die Prothese50 kann in den Spalt zwischen den Dornfortsätzen118 eingesetzt werden, wobei die abgerundeten Zinken76 mit den Köpfen75 der Wirbelbogenwurzelschrauben72 ausgerichtet werden. Die Sicherungsmuttern77 können dann die Prothese50 an den Wirbelbogenwurzelschrauben72 sichern. Wie es vorangehend erwähnt wird, kann daher die Prothese50 an den Wirbelbogenwurzeln befestigt werden, ohne dass man die Befestigungselemente52 und54 an den Dornfortsätzen118 befestigen muss. Wahlfrei können die Befestigungselemente52 und54 ebenfalls an benachbarten Dornfortsätzen118 oder einer anderen Wirbelsäulenstruktur mittels irgendeines zweckmäßigen Verfahrens befestigt werden. Die Außenflächen der Befestigungselemente52 und54 können mit einem Material beschichtet werden, wie beispielsweise Hydroxy-Appatit (H/A), aber nicht darauf beschränkt, das das Knochenwachstum in deren Außenflächen unterstützt. - Mit Bezugnahme auf
8C und8D wird eine alternative Konstruktion der Biegebaugruppe56 der elastomeren Wirbelsäulenprothese50 veranschaulicht. (Die elastomere Schutzkappe64 wird der Deutlichkeit halber weggelassen.) Bei dieser alternativen Konstruktion kann die Biegebaugruppe56 so konstruiert werden, dass sie eine zweiteilige Gelenkbaugruppe aufweist, die aus einem ersten Gelenkelement101 konstruiert wird, das eine konvexe äußere Kontur102 mit einer abgeflachten Fläche103 aufweisen kann. Eine halbkreisförmige Aussparung104 kann aus der konvexen äußeren Kontur102 ausgehöhlt werden. Eine Platte105 kann das erste Gelenkelement101 am unteren Befestigungselement54 sichern, wie beispielsweise mit Schrauben (nicht gezeigt), die durch Montagelöcher106 hindurchgehen. - Der andere Teil der zweiteiligen Gelenkbaugruppe der Biegebaugruppe
56 kann ein zweites Gelenkelement107 umfassen, das eine konkave äußere Kontur108 aufweisen kann, die der konvexen äußeren Kontur des ersten Gelenkelementes101 entspricht und darüber hinweg gleitet. Eine Platte109 kann das zweite Gelenkelement107 am oberen Befestigungselement52 sichern, wie beispielsweise mit Schrauben (nicht gezeigt), die durch Montagelöcher110 hindurchgehen. Bin Anschlag111 kann bereitgestellt werden, entweder als ein Teil des zweiten Gelenkelementes107 oder als ein separates Teil, das am oberen Befestigungselement52 befestigt ist. Der Anschlag111 ragt in die halbkreisförmige Aussparung104 hinein. - Wie es vorangehend gleichermaßen beschrieben wird, gestattet diese Version der in
8C und8D gezeigten Biegebaugruppe56 die Biegung der Prothese50 um zwei gegenseitig orthogonale Achsen67 und68 ebenso wie andere Richtungen für eine richtungsunabhängige Biegung in irgendeinem Freiheitsgrad. Beispielsweise, wie in8A gesehen wird, kann sich die Prothese50 elastisch um die Achse67 in der Richtung der Pfeile69 drehen. Wie in8B gesehen wird, kann sich die Prothese50 elastisch um die Achse68 in der Richtung der Pfeile70 drehen. Der Anschlag111 kann die Biegung der Biegebaugruppe56 begrenzen, und er kann daher die relative Bewegung der Befestigungselemente52 und54 mit Bezugnahme zueinander begrenzen. - Es wird bemerkt, dass jede der hierin vorangehend beschriebenen Wirbelsäulenprothesen einen unitären Körper mit mindestens drei Befestigungsstellen aufweist, die an der Wirbelsäulenstruktur befestigt werden können. Mit „unitärer Körper" meint man, dass die Wirbelsäulenprothese an der Wirbelsäulenstruktur als eine vormontierte zusammenhängende Baugruppe befestigt werden kann. Der Chirurg kann einfach den gesamten unitären Körper während der Befestigung an der Wirbelsäulenstruktur an Ort und Stelle halten. Das steht im Gegensatz zu bekannten Prothesen, die an der Wirbelsäulenstruktur als mindestens zwei separate Teile befestigt werden müssen, die miteinander gelenkig verbunden werden können oder nicht. Bei jenen Prothesen muss der Chirurg jedes Teil separat an einer bestimmten Wirbelsäulenstruktur befestigen und kann nur „hoffen", dass die Teile nach dem Einsetzen richtig zusammenpassen. Die unitäre Konstruktion der vorliegenden Erfindung eliminiert dieses Problem und erleichtert in starkem Maß das Einsetzen der Prothese.
- Auch wenn vorangehend Ausführungen beschrieben wurden, bei denen die Befestigung der Befestigungselemente an entweder dem Dornfortsatz allein oder am Dornfortsatz und am Facet oder am Dornfortsatz und an der Wirbelbogenwurzel eines betreffenden Rückenwirbels erfolgte, ist es möglich, dass jegliche Kombination der vorangehenden Ausführungen ähnliche Ergebnisse bewirken (wie beispielsweise die Stabilisierung der Wirbelsäule, die Linderung der Schmerzen, die Implantation der Prothese, usw.), und die vorangehenden Beispiele sind nur Beispiele für Befestigungsstellen und in keiner Weise im Sinne einer Einschränkung. Beispielsweise kann ein superiores Befestigungselement an einem Dornfortsatz allein befestigt werden, und das inferiore Befestigungselement der Prothese kann am Dornfortsatz des benachbarten Rückenwirbels und seiner Wirbelbogenwurzel oder seinem Facet oder sogar einer Kombination beider befestigt werden. Gleichermaßen kann die Prothese dieser Erfindung in Verbindung mit einem Implantat verwendet werden, das zum Rückenmark anterior ist, wie beispielsweise eine totale Scheibenauswechselung.
- Obgleich die Erfindung in Verbindung mit spezifischen Ausführungen davon beschrieben wurde, sind für jene Fachleute viele Alternativen, Abwandlungen und Veränderungen offensichtlich. Dementsprechend fallen alle derartigen Alternativen, Abwandlungen und Veränderungen in den Bereich der folgenden Patentansprüche.
Claims (5)
- Artikel, der aufweist: eine Wirbelsäulenprothese (
50 ), die einen unitären Körper mit mindestens drei Befestigungsstellen aufweist, die an einer Wirbelsäulenstruktur befestigt werden können, wobei der unitäre Körper eine Biegebaugruppe (56 ) aufweist, die zwischen erstem und zweitem Befestigungselement (52 ,54 ) positioniert ist, wobei die Biegung der Biegebaugruppe (56 ) die Bewegung des ersten Befestigungselementes (52 ) relativ zum zweiten Befestigungselement (54 ) gestattet; gekennzeichnet durch eine Vielzahl von Wirbelbogenwurzelschrauben (72 ), die an der Wirbelsäulenprothese (50 ) befestigt werden oder zusammenhängend damit ausgebildet sind, wobei die Wirbelbogenwurzelschrauben (72 ) polyaxiale Wirbelbogenwurzelschrauben mit einem Gewindeschaft (74 ) und einem polyaxialen Drehkopf (75 ) aufweisen. - Artikel nach Anspruch 1, bei dem der Gewindeschaft (
74 ) an der Wirbelsäulenprothese (50 ) befestigt wird oder zusammenhängend damit ausgebildet ist. - Artikel nach Anspruch 1 oder Anspruch 2, bei dem der polyaxiale Drehkopf (
75 ) an der Wirbelsäulenprothese (50 ) befestigt wird oder zusammenhängend damit ausgebildet ist. - Artikel nach einem der Ansprüche 1 bis 3, bei dem die Biegebaugruppe (
56 ) eine mehrteilige Gelenkbaugruppe aufweist, die ein erstes Gelenkelement (101 ) umfasst, das eine konvexe Kontur (102 ) aufweist, das mit einem zweiten Gelenkelement (107 ) gelenkig verbunden ist, das eine konkave Kontur (108 ) aufweist, die der konvexen Kontur (102 ) des ersten Gelenkelementes (101 ) entspricht und darüber gleitet, wobei das erste und zweite Gelenkelement (101 ,107 ) an den Befestigungselementen (52 ,54 ) befestigt werden oder zusammenhängend ein Teil davon sind. - Artikel nach Anspruch 4, der außerdem einen Anschlag (
111 ) aufweist, der die Biegung der Biegebaugruppe (56 ) begrenzt, und der die relative Bewegung der Befestigungselemente (52 ,54 ) mit Bezugnahme zueinander begrenzt.
