Description

  • Die vorliegende Erfindung betrifft eine Glenoidkomponente einer Schulterprothese sowie eine Schultervollprothese, die eine derartige Komponente enthält.
  • Auf dem Gebiet der Schulterprothesen verwendet man heutzutage eine Glenoidkomponente, die einen konkaven Körper aus Polyethylen enthält, auf dem ein konvexer metallischer Kopf einer Humeruskomponente aufliegt. Dieses ist zum Beispiel der Fall in US-A-5,593,448 und US-A-6,406,495 . Es ist tatsächlich weithin anerkannt, dass der Humeruskopf das am stärksten mechanisch beanspruchte Teil während der Funktion der Prothese ist.
  • Diese allgemein angewendete Lösung führt jedoch zu einer Reihe von Nachteilen, die mit der Abnutzung des Glenoidkörpers aus Polyethylen in Zusammenhang stehen. Tatsächlich stellt man fest, dass sich dieser Polyethylenkörper recht schnell abnutzt, und dies umso mehr, als der Kontakt zwischen dem Polyethylenkörper und dem härteren Humeruskopf auf einer Oberfläche mit einem kleinerem Flächeninhalt für den Polyethylenkörper als für den Humeruskopf stattfindet. Außerdem wird im Falle einer schlechten Positionierung des Glenoidkörpers in Bezug auf den Humeruskopf die Abnutzung schlecht verteilt. Unter der Einwirkung von Umschwenkungsbeanspruchungen, die vom dem schlecht positionierten Humeruskopf erzeugt werden, besteht somit das Risiko, dass die Prothese entweder durch Ablösung der Glenoidkomponente oder durch Luxation des Gelenks beschädigt wird.
  • Um diese Nachteile zu überwinden, hat man in der Vergangenheit vorgeschlagen, entweder die Stärke des Polyethylenkörpers zu erhöhen, was jedoch die Artikulationsoberfläche mit dem Humeruskopf aus ihrer anatomischen Position verschiebt, oder zwischen den Polyethylenkörper und die knochige Gelenkpfanne einen metallischen Einsatz einzusetzen (dann spricht man von "metal back"-Glenoidkomponenten), wie in US-A-4,550,450 beschrieben. Diese letztere Lösung bleibt jedoch kostspielig und führt momentan vor allem dann, wenn die Positionierung zwischen den Humeruskopf und dem Glenoidkörper anatomisch nicht zufrieden stellend ist, zu einer Ablösung des Polyethylenkörpers in Bezug auf den Metalleinsatz.
  • Die Aufgabe der vorliegenden Erfindung ist es, eine Glenoidkomponente vorzuschlagen, die gleichzeitig die Risiken einer Beschädigung und einer Ablösung dieser Komponente beschränkt und effizient mit einer Humerusprothesenkomponente zusammenwirkt, und dabei ökonomisch und einfach zu implantieren ist.
  • Zu diesem Zweck ist der Gegenstand der Erfindung eine Glenoidkomponente einer Schulterprothese, die einen metallischen Körper umfasst, der auf einer Seite dafür ausgelegt ist, an der Gelenkpfanne einer Schulter immobilisiert zu werden, und der auf der gegenüberliegenden Seite eine konkave Artikulationsoberfläche trägt, die dafür ausgelegt ist, mit einer Humeruskomponente der Prothese zusammenzuwirken, wobei sich die Artikulationsoberfläche an der Peripherie in einer konvexen Oberfläche fortsetzt, die zumindest teilweise die Kante der Glenoidkomponente bildet.
  • Durch die Verwendung des metallischen Körpers der erfindungsgemäßen Glenoidkomponente wird die Steifheit dieser Komponente signifikant erhöht, auch ohne dass eine große Dicke erforderlich ist. Die Abnutzung dieses metallischen Teils beträgt praktisch Null, was somit die Risiken seiner Beschädigung stark verringert. Bei der Funktion ist die Krümmung der Artikulationsoberfläche so ausgestaltet, dass die Gefahren einer Luxation der Humeruskomponente beschränkt werden, und die abgestumpfte Peripherie dieser Oberfläche verhindert, dass die Humeruskomponente eingedrückt oder sogar eingekerbt wird. Außerdem ist die erfindungsgemäße Komponente verglichen mit den "metal back"-Glenoidkomponenten ökonomischer und leichter zu implantieren.
  • Weitere Eigenschaften dieser Glenoidkomponente, allein oder in allen technisch möglichen Kombinationen verwendet, sind in den abhängigen Ansprüchen 2 bis 12 dargelegt.
  • Gegenstand der Erfindung ist auch eine Schultervollprothese, die eine Glenoidkomponente, wie vorstehend definiert, und eine Humeruskomponente enthält, die eine konvexe Oberfläche für das gelenkige Zusammenwirken mit der konkaven Oberfläche der Glenoidkomponente trägt. Eine derartige Prothese stellt den Vorteil bereit, dass sie die Abnutzung auf die Humeruskomponente verschiebt, d. h. auf eine größere Oberfläche, die von einem dickeren Material getragen wird. Wenn es nach einigen Jahren notwendig ist, die Abnutzungsteile der Prothese auszuwechseln, muss außerdem nur der Kopf der Humeruskomponente ausgewechselt werden, was die entsprechende Entnahmeoperation schneller und einfacher macht.
  • Gemäß einem vorteilhaften Merkmal dieser Schultervollprothese wird die konvexe Oberfläche der Humeruskomponente von einem Teil dieser Komponente getragen, das aus einem weicheren Material als das Metall des Körpers des Glenoidkomponente und insbesondere aus einem Kunststoffmaterial besteht.
  • Die Erfindung wird leichter verstanden und ihre weiteren Vorteile werden deutlicher angesichts der folgenden Beschreibung, die nur als Beispiel gegeben wird und die sich auf die beigefügten Zeichnungen bezieht, in denen:

  • Die in 1 dargestellte Prothese 1 umfasst eine erste Komponente 2, die an der Gelenkpfanne G einer Schulter befestigt ist, sowie eine zweite Komponente 4, die im entsprechenden Numerus H fixiert ist.
  • Die Komponente 4 umfasst einerseits einen Schaft 6, der zur Verankerung im Markkanal des Numerus H bestimmt ist, und andererseits einen halbkugelförmigen Kopf 8, der eine konvexe Oberfläche S4 in Form eines Kugelabschnitts definiert.
  • Der Schaft 6 ist aus Metall, während der Kopf 8, zumindest in seinem peripheren Abschnitt, aus weicherem Material, insbesondere aus Polyethylen, insbesondere aus Polyethylen mit hoher Dichte (HDPE), besteht. Der Kopf 8 ist an dem Schaft 6 durch jedes bekannte Mittel fest angebracht, zum Beispiel durch Ineinanderstecken. Der Kopf 8 kann vorteilhafterweise an bereits bestehende Humerusschäfte angepasst werden.
  • Zur Erleichterung wird für die folgende Beschreibung, insbesondere für diejenige in Bezug auf die Glenoidkomponente 2, angenommen, dass sich diese Komponente in ihrer Implantationsposition gegenüber der Gelenkpfanne G befindet. Genauer gesagt, bezeichnen hinsichtlich der Komponente 2, die in den 2 und 4 dargestellt ist, die Begriffe "unten" und "oben" den linken bzw. rechten Teil der 2 und 4, die Begriffe "außen" und "innen" den oberen und unteren Teil der 2 und die Begriffe "anterior" und "posterior" den oberen bzw. unteren Teil der 4.
  • Die Komponente 2 der 2 bis 4 weist einen metallischen Körper 10 aus einem Stück auf, der in antero-posteriorer Richtung und von unten nach oben Abmessungen aufweist, die deutlich größer sind als seine Dicke in Richtung von innen nach außen. Der periphere Rand des Körpers 10 trägt die Bezugszahl 11.
  • Somit weist der Körper 10 einen Innenfläche 12 auf, die dafür ausgelegt ist, an der Gelenkpfanne G immobilisiert zu werden. Genauer gesagt, ist diese Fläche 12 mit einem vorspringenden Flügel 14 versehen, der dafür ausgelegt ist, sich tief in der knochigen Gelenkpfanne zu verankern, und in den eine Bohrung 16 gearbeitet ist, in deren Inneres eine Immobilisierungsschraube eingesetzt werden kann. Der Flügel 14 ist vorteilhafterweise aus Metall und mit dem Körper 10 stofflich verbunden.
  • Auf beiden Seiten des Flügels 14 umgrenzt die Fläche 12 konkave Oberflächen 18 für die Auflage an der Schulterhöhle des Schulterblatts des Patienten. Diese Oberflächen tragen vorteilhafterweise Rillen, die sich in Richtung von unten nach oben erstrecken und dazu bestimmt sind, mit der knochigen Wand der Gelenkpfanne G in Eingriff zu treten.
  • Vorteilhafterweise ist die innere Fläche 12 zumindest teilweise von einer Schicht aus Calciumhydroxyapatit bedeckt, welche die Osteointegration erleichtern soll, nachdem die Glenoidkomponente 2 implantiert worden ist.
  • Der Körper 12 weist außerdem eine äußere Fläche 20 auf, die eine konkave Oberfläche S2 trägt, die dafür ausgelegt ist, gelenkig mit der konvexen Oberfläche S4 des Humeruskopfes 8 zusammenzuwirken. Auf an sich bekannte Weise werden die jeweiligen Geometrien dieser Oberflächen S2 und S4 derart konzipiert, dass die anatomischen Gelenkverhaltensweisen der Schulter so naturgetreu wie möglich nachgeahmt werden. Außerdem ist die Krümmung der Oberfläche S2 derart ausgelegt, dass die Risiken einer Luxation des Humeruskopfes 8 bei den üblichen Beanspruchungen der Prothese 1 so weit wie möglich beschränkt werden.
  • Die konkave Oberfläche S2 setzt sich in ihrer Peripherie in einer konvexen Oberfläche 22 fort, die den Beginn der Kante 11 der Außenseite der Glenoidkomponente bildet. Anders gesagt, sind der innere Teil der Kante 11, der bei der Ausführungsform der 1 bis 3 im Wesentlichen eben ist, und die Oberfläche S2 des äußeren Fläche 20 fortlaufend durch die gekrümmte Oberfläche 22 verbunden. Die abgestumpfte Form dieser Oberfläche 22 beschränkt die Risiken, dass der Kopf 8 während der Beanspruchung der Prothese 1 geschnitten, eingedrückt oder eingekerbt wird. Als Beispiel liegt der minimale Krümmungsradius der Oberfläche 22 in der Größenordnung von 1 bis 3 mm bei einer Glenoidkomponente, die für einen Erwachsenen bestimmt ist.
  • Um das Erreichen der abgestumpften Oberfläche 22 zu erleichtern, ist Letztere vorzugsweise tangential mit der Artikulationsoberfläche S2 verbunden, wodurch somit ebenfalls jedes mögliche Risiko vermieden wird, dass der Humeruskopf 8 eingedrückt wird.
  • Wenn die Vollprothese 1 implantiert ist und belastet wird, liegt der Humeruskopf 8 auf der äußeren Fläche 20 des Glenoidkörpers 10 auf, wobei die Oberflächen S2 und S4 aufeinander artikulieren.
  • Verglichen mit den Prothesen des Standes der Technik, in denen die Abnutzung im Wesentlichen von einer Glenoidkomponente aus Polyethylen getragen wird, bietet die erfindungsgemäße Prothese den Vorteil, dass sie diese Abnutzung auf den Humeruskopf 8, d. h. auf die Oberfläche S4 mit einer größeren Kontaktfläche als diejenige der Oberfläche S2, überträgt. Die entsprechende Abnutzung wird dadurch verlangsamt. Außerdem kann die Dicke des Polyethylens, das die Oberfläche S4 trägt, größer vorgesehen werden, als sie für eine Glenoidkomponente des Standes der Technik in Betracht gezogen werden kann, wobei das nutzbare Volumen in Höhe des Humeruskopfes größer als das, welches im Allgemeinen für die Glenoidkomponente verfügbar ist, ausgenommen es wird eine Prothese hergestellt, deren Verhalten sehr weit von dem anatomische Verhalten entfernt ist. Somit erhöht man die Lebensdauer der Prothese 1.
  • Wenn die Prothese 1 ihre vorbestimmte Lebensdauer erreicht hat, wird das von dem Humeruskopf 8 gebildete Abnutzungsteil ausgetauscht, was einfacher und schneller geht als der Austausch der Glenoidkomponente, wie bei den derzeitigen Schulterprothesen.
  • Außerdem ist der Glenoidkörper 10, der aus Metall besteht, sehr starr und wird leichter in der Gelenkpfanne G verankert. Außer den vorstehend beschriebenen Befestigungsmitteln wird der Körper 10 bei einer nicht dargestellten Variante von hindurchgehenden Bohrungen durchbohrt, die auf der artikulierenden Oberfläche S2 münden und dafür ausgelegt sind, Schrauben zur Verankerung im Knochen, insbesondere selbstblockierende Schrauben, aufzunehmen. Bei einer derzeitigen Glenoidkomponente, deren Körper aus einem weicheren Material, wie Polyethylen, hergestellt ist, kann die Verwendung derartiger selbstblockierender Schrauben nicht in Betracht gezogen werden. Weil die erfindungsgemäße Glenoidkomponente eine wirksamere Befestigung im Knochen gestattet, kann somit erwogen werden, sie mit oder ohne Zement einzusetzen.
  • In den 4 und 5 ist eine Variante der Glenoidkomponente 2 dargestellt, die sich im Hinblick auf ihrer innere Seite im Wesentlichen von der Ausführungsform der vorhergehenden Figuren unterscheidet. Tatsächlich ist die innere Fläche 12, die keinen vorspringenden Flügel aufweist, dafür ausgelegt, an die Gelenkpfanne G zementiert zu werden. Zu diesem Zweck trägt die Fläche 12 in ihrem zentralen Teil eine Oberfläche 30, die dafür ausgelegt ist, das Abbinden des Zements zu erleichtern. Diese Oberfläche wird zum Beispiel mit Mikrokügelchen, Sand behandelt oder poliert.
  • Um die Komponente 2 fest an der Gelenkpfanne zu fixieren, ist der Körper 10 mit einer anterioren Lasche 32 und einer unteren Auflageklammer 34 versehen. Diese Elemente 32 und 34 liegen in Form metallischer Laschen vor, die mit dem Körper 10 stofflich verbunden sind, vom Körper in Richtung zur Gelenkpfanne auskragen und mit dem Körper in Höhe seiner Kante 11 verbunden sind.
  • Genauer gesagt, schließen die Lasche 32 und die Auflageklammer 34 tangential an periphere Teile an, die der abgestumpften Oberfläche 22 entsprechen.
  • Die Lasche 32 erstreckt sich genügend weit entlang des Glenoidknochens G, dass sie eine in 5 dargestellte Knochenverankerungsschraube 36 aufnehmen kann. Diese Schraube ist vorzugsweise selbstblockierend in Bezug auf die Lasche und verlängert sich vorteilhafterweise in einem Retroversionswinkel, der eine Knochenrekonstruktion der Gelenkpfanne durch Implantation eines Translantats G' in den posterioren Teil des Knochens gestattet.
  • Die Auflageklammer 34 ermöglicht ihrerseits die Bildung einer unteren Auflage für die Gelenkpfanne G.
  • In den 6 bis 8 ist eine andere Variante der Glenoidkomponente 2 dargestellt, die sich im Wesentlichen in ihrer inneren Seite von der Ausführungsform der 1 bis 3 unterscheidet. Tatsächlich ist der vorspringende Flügel 14 durch einen Bolzen 40 zur Verankerung im Knochen der Gelenkpfanne G ersetzt. Dieser Bolzen hat die Form eines im Großen und Ganzen kegelförmigen Zapfens mit der Achse X-X, der sich in Verlängerung der inneren Fläche 12 des Körpers 10 erstreckt, wobei er mit dem Körper stofflich verbunden ist. Die Oberfläche des Bolzens 40 divergiert von der Richtung der Fläche 12 derart, dass er leichter in den Knochen eingebracht werden kann, der gegebenenfalls zuvor auf im Wesentlichen passende Weise ausgehöhlt wird.
  • Der Bolzen 40 erstreckt sich vom oberen Teil der inneren Fläche 12, während der untere Teil eine hindurchgehende Bohrung 42 aufweist, die dafür ausgelegt ist, eine Schraube 44 zur Verankerung im Knochen im unteren Teil der Gelenkpfanne G aufzunehmen, wobei diese Schraube analog zu der Schraube 36 der 5 ist. Diese Schraube 44 ist vorzugsweise selbstblockierend in Bezug auf die Bohrung 42.
  • Wenn die Schraube 44 in der Bohrung 42 aufgenommen worden ist, erstreckt sie sich in der Länge um eine Mittelachse Y-Y, die mit der Achse X-X eine im wesentlichen vertikale Ebene definiert, die der Fläche der 8 entspricht. In dieser Ebene sind die Achsen X-X und Y-Y in Bezug zueinander geneigt und schneiden sich an einem Punkt P, der sich auf der Außenseite des Körpers 10, d. h. auf der Seite des Mittelpunkts der Krümmung der Oberfläche S2, befindet. Anders gesagt, divergieren die Achsen X-X und Y-Y voneinander von der inneren Seite des Körpers 10, was eine zuverlässige Verankerung gewährleistet, wobei gleichzeitig der obere und der untere Teil der Gelenkpfanne G belastet werden.
  • Um die Risiken eines Umschwenkens der Komponente 2 in eine im Großen und Ganzen vertikale Richtung zu beschränken, ist mit der Fläche 12 des Körpers 10 eine Rippe 46 stofflich verbunden, die sich in der Ebene der 8 von dem Bolzen 40 bis zum Umfang der Bohrung 42 mit ständig abnehmender Dicke verlängert.
  • Bei einer nicht dargestellten Variante kann der Bolzen 40 entlang seiner Achse X-X im Inneren ausgebohrt werden, so dass er eine weitere Knochenverankerungsschraube aufnehmen kann.
  • Andere Varianten und Ausgestaltungen der Glenoidkomponente und der Vollprothese, die vorstehend beschrieben sind, können ebenfalls in Betracht gezogen werden. Insbesondere sind die Mittel zur Befestigung an der Gelenkpfanne oder am Numerus, die vorstehend erwogen werden, keineswegs beschränkend und können vom Fachmann verändert werden. Ebenso kann/können je nach dem Zustand des Knochens der Gelenkpfanne und der Geometrie des Schnitts, der zum Einsetzten der Komponente 2 durchgeführt wird, die Auflagefläche(n) 18 oder 30 des Glenoidkörpers 10 im Wesentlichen eben sein, wie bei der Glenoidkomponente der 6 bis 8. Die Gelenkpfanne 2 kann aus mehreren Metallteilen bestehen, die miteinander verbunden werden.
  • Weil der Glenoidkörper 10 aus Metall besteht, kann man außerdem leicht dafür sorgen, dass sich dieser Körper bis zum akromialen Teil des Schulterblatts des Patienten erstreckt, so dass die Befestigung der Glenoidkomponente am Schulterknochen verbessert wird.

Claims (14)

  1. Glenoidkomponente (2) einer Schulterprothese (1) mit einer konkaven Artikulationsoberfläche (S2), wobei sich die Artikulationsoberfläche an der Peripherie in einer konvexen Oberfläche (22) fortsetzt, die zumindest teilweise die Kante (11) der Glenoidkomponente bildet, dadurch gekennzeichnet, dass die Glenoidkomponente einen metallischen Körper (10) umfasst, der auf einer seiner Flächen (12) dafür ausgelegt ist, an der Gelenkpfanne (G) einer Schulter immobilisiert zu werden, und der auf seiner gegenüberliegenden Seite (20) die konkave Artikulationsoberfläche (S2), die dafür ausgelegt ist, mit einer Humeruskomponente (4) der Prothese (1) zusammenzuwirken, und die konvexe Oberfläche (22) aufweist.
  2. Komponente nach Anspruch 1, dadurch gekennzeichnet, dass die konvexe Oberfläche (22) und die Artikulationsoberfläche (S2) tangential aneinander stoßen.
  3. Komponente nach einem der Ansprüche 1 oder 2, dadurch gekennzeichnet, dass der minimale Krümmungs-radius der konvexen Oberfläche (22) zwischen etwa 1 und 3 mm beträgt.
  4. Komponente nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der metallische Körper (10) zusammenhängend ist.
  5. Komponente nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der metallische Körper (10) mit mindestens einer hindurch gehenden Öffnung (42) versehen ist, die auf der Artikulations-oberfläche (S2) mündet und dafür ausgelegt ist, Mittel (44) zur Verankerung der Komponente (2) im Schulter-knochen, insbesondere selbstblockierende Verankerungsmittel, aufzunehmen.
  6. Komponente nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der metallische Körper (10) auf der Seite, die für die Immobilisierung an der Gelenkpfanne (G) bestimmt ist, mit mindestens zwei Knochenverankerungselementen (40, 44) ausgestattet ist, welche die jeweiligen Richtungen (X-X, Y-Y) des Ansetzens der Gelenkpfanne im Knochen definieren, die bei der Entfernung vom metallischen Körper voneinander divergieren.
  7. Komponente nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der metallische Körper (10) auf der Seite (12), die für die Immobilisierung an der Gelenkpfanne (G) bestimmt ist, mit mindestens einer vorspringenden Lasche (32, 34) ausgestattet ist, die sich in der Verlängerung eines Teils der konvexen Oberfläche (22) erstreckt.
  8. Komponente nach Anspruch 7, dadurch gekennzeichnet, dass die Lasche(n) (32, 34) mit dem Körper (10) stofflich verbunden ist (sind).
  9. Komponente nach einem der Ansprüche 7 oder 8, dadurch gekennzeichnet, dass die Lasche oder eine der Laschen vorzugsweise im anterioren Teil der Glenoidkomponente (2) eine Knochenbefestigungslasche (32) bildet, die dafür ausgelegt ist, eine Knochenverankerungsschraube (36) aufzunehmen.
  10. Komponente nach einem der Ansprüche 7 bis 9, dadurch gekennzeichnet, dass die Lasche oder eine der Laschen vorzugsweise im unteren Teil der Glenoidkomponente (2) eine Auflageklammer (34) für die Gelenkpfanne (G) bildet.
  11. Komponente nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Fläche (12) des metallischen Körpers (10), die für die Immobilisierung an der Gelenkpfanne (G) bestimmt ist, mit einer aktiven Schicht zur Ermöglichung der Osteointegration, insbesondere mit einer Calciumhydroxylapatit-Schicht bedeckt ist.
  12. Komponente nach einem der Ansprüche 1 bis 10, dadurch gekennzeichnet, dass die Fläche (12) des metallischen Körpers (10), die für die Immobilisierung an der Gelenkpfanne (G) bestimmt ist, einen Oberflächenzustand aufweist, um die Zementierung der Glenoidkomponente (2) zu ermöglichen.
  13. Schultertotalprothese (1), dadurch gekennzeichnet, dass sie eine Glenoidkomponente (2) nach einem der vorhergehenden Ansprüche und eine Humeruskomponente (4) enthält, die eine konvexe Oberfläche (S4) für das gelenkige Zusammenwirken mit der konkaven Oberfläche (S2) der Glenoidkomponente aufweist.
  14. Prothese nach Anspruch 13, dadurch gekennzeichnet, dass die konvexe Oberfläche (S4) der Humeruskomponente (4) sich auf einem Stück (8) dieser Komponente befindet, die aus einem weicheren Material als das Metall des Körpers (10) der Glenoidkomponente (2), insbesondere aus einem Kunststoffmaterial, besteht.
