Description
- Die vorliegende Erfindung bezieht sich auf ein Pumpenkommunikationssystem, das ein Computersystem für eine ortsgebundene und eine Fernkommunikation mit einer Pumpe umfasst.
- Es sind verschiedene medizinische Vorrichtungen bekannt, um Patienten an einer von der Praxis des Pflegers oder Arztes entfernt angeordneten Stelle aus zu behandeln und/oder zu überwachen. Ein Beispiel einer ortsungebundenen medizinischen Vorrichtung ist eine Arzneimittelverabreichungsvorrichtung wie eine Arzneipumpe, um den Patienten periodisch oder kontinuierlich mit einer Arzneimittelverabreichung zu versorgen, wenn er sich von der Praxis der Pflegeperson entfernt befindet.
- Bestimmte Medikamente erreichen über herkömmliche Injektionstechniken kaum ihre therapeutische Höchstwirkung. Viele Arzneien erreichen ihr volles Potential nur durch eine genaue Verabreichung über einen längeren Zeitraum. Mit einer kontrollierten Arzneimittelinfusion über eine Arzneipumpe kann das Medikament mit einer präzisen Rate gegeben werden, welche die Arzneimittelkonzentration innerhalb der therapeutischen Grenze und außerhalb des toxischen Bereichs hält. Ortsungebundene Arzneimittelpumpen können eine angemessene Arzneimittelverabreichung an den Patienten mit einer steuerbaren Rate bereitstellen, die keine häufige medizinische Betreuung erfordert und es dem Patienten ermöglicht, das Krankenhaus oder die Pflegerpraxis zu verlassen.
- Das Versäumnis, die Arzneipumpe und den Gebrauch der Arzneipumpe durch den Patienten angemessen zu überwachen, kann die Vorteile reduzieren oder zunichte machen, die ein Patient aus einer richtigen Arzneimittelverabreichungstherapie gezogen haben kann. In manchen Fällen können Arzneitherapien ernsthafte gesundheitliche Folgen für den Patienten haben, wenn die Arzneien nicht ordnungsgemäß verabreicht werden.
- Es treten mehrere Probleme auf, wenn der Patient die verschiedenen ortsungebundenen medizinischen Vorrichtungen an entfernten Stellen nutzt. Falls die ortsungebundene medizinische Vorrichtung Daten hinsichtlich der Vorrichtung und/oder des Patienten sam melt, müssen die Daten für die Pflegeperson leicht zugänglich sein, um es ihr zu ermöglichen, die Vorrichtung und den Patienten zu überwachen. Es kann sein, dass der Patient oftmals beim Zugriff auf die Daten und bei der Kommunikation mit der Pflegeperson von einer entfernten Örtlichkeit aus von wenig oder gar keiner Hilfe ist. Muss die Pflegeperson die Vorrichtung physikalisch betätigen, um auf die Daten zuzugreifen, müssen Vorrichtung und Pflegeperson an denselben Ort gebracht werden.
- Ähnliche Probleme können auch dadurch auftreten, dass die medizinischen Vorrichtungen an den entfernten Stellen ausfallen oder unsachgemäß verwendet werden. Arbeitet die Vorrichtung nicht ordnungsgemäß, kann es sein, dass sie zur Pflegeperson gebracht werden muss oder die Pflegeperson den Patienten aufsuchen muss. Falls die Pflegeperson eine Fehlersuche von der entfernten Stelle aus versucht, kann es sein, dass der Patient physisch und mental nicht in der Lage ist, ausreichend mit der Pflegeperson zusammenzuarbeiten, damit diese die Fehlersuche von der entfernten Stelle aus durchführen kann. Betriebsprobleme können zu unpassenden Zeiten wie in der Nacht oder an Wochenenden und Feiertagen auftreten. Es kann auch sein, dass die Pflegeperson auf einfache Weise den Gebrauch der medizinischen Vorrichtung durch den Patienten überwachen können muss, um zu beobachten, ob der Patient die vorgeschriebenen Behandlungen einhält, während er sich an der entfernten Stelle befindet.
- Es kann darüber hinaus auch notwendig werden, dass die Pflegeperson auf das Steuersystem wie den Prozessor und die Programme zugreifen muss, die den Betrieb der ortsungebundenen medizinischen Vorrichtungen steuern. Beispielsweise verändern sich im Falle von Arzneimittelverabreichungsvorrichtungen einige Therapien mit der Zeit. Dann kann es sein, dass die Pflegeperson auf das Betriebsprogramm zugreifen muss, um den Betrieb der Arzneimittelverabreichungsvorrichtung zu ändern. Darüber hinaus können einige Arzneimittelverabreichungsvorrichtungen beispielsweise für sehr unterschiedliche Anwendungen verwendet werden, die unterschiedliche Betriebsprogramme erfordern. Diese Betriebsprogramme können sich ändern, wenn sich der Einsatz der Arzneimittelverabreichungsvorrichtung ändert. Es kann sein, dass Betriebsmittel nicht effizient genutzt werden, wenn die Pflegeperson und das Gerät an dieselbe Örtlichkeit gebracht werden müssen, damit die Pflegeperson auf den Prozessor und die Betriebsprogramme zugreifen kann.
- Eine spezielle Situation tritt oftmals im Falle einer Arzneipumpe auf, bei welcher die Pflegeperson auf das Steuergerät der Arzneipumpe zugreifen muss. Die Pflegeperson kann den Patienten bei sich zu Hause zu irgendeinem Zeitpunkt, nachdem die Arzneimitteltherapie unter Verwendung der Arzneipumpe begonnen hat, aufsuchen. Die Pflegeperson kann eine Blutprobe des Patienten nehmen. Die Pflegeperson kehrt in die Praxis zurück und die Blutprobe wird analysiert. Die Ergebnisse der Blutuntersuchung können anzeigen, dass der Patient nicht die richtige Arzneimitteltherapie erhält. Dies bedeutet für gewöhnlich, dass das Steuergerät der Arzneipumpe eingestellt werden muss, um dem speziellen Patienten die ordnungsgemäße Therapie angedeihen zu lassen. Um das Steuergerät einzustellen, muss die Pflegeperson auf das Steuergerät zugreifen können.
- Ein weiteres Problem, dem Pflegepersonal auf dem Gebiet ortsungebundener medizinischer Vorrichtungen gegenübersteht, bezieht sich auf die Leichtigkeit der Fernkommunikation mit den Geräten, falls eine solche Fernkommunikationsfähigkeit vorgesehen ist. Eine Fernkommunikation mit einer ortsungebundenen medizinischen Vorrichtung, die einen PC oder eine Basisstation verwendet, kann für manche Pflegepersonen zu einem schwierigen Fall werden, wenn der PC/die Basisstation anders als die medizinische Vorrichtung arbeitet. Die Pflegeperson muss sich dann sowohl im Betrieb der medizinischen Vorrichtung als auch im Betrieb des PCs/der Basisstation auskennen. Dieses Problem trifft auch auf die lokale Kommunikation mit der ortsungebundenen medizinischen Vorrichtung zu.
- Im Falle einer Fernkommunikation mit der medizinischen Vorrichtung eines Patienten, die einen PC/eine Basisstation verwendet, muss die Pflegeperson auch in der Nähe des PCs/der Basisstation bleiben oder sie mit sich tragen, falls die Pflegeperson mit der medizinischen Vorrichtung des Patienten weiter in Verbindung bleiben möchte.
- Manchmal kann es notwendig werden, mit einem PC oder einer Basisstation entweder lokal oder entfernt zu kommunizieren, um beispielsweise die größere Datenbearbeitungs- oder Datenverarbeitungsfähigkeit des PCs oder der Basisstation zu nutzen. Diese Fähigkeit der Pflegeperson, mit der Pumpe auf einfache Weise zu kommunizieren, indem ein PC oder eine Basisstation verwendet wird, kann unter diesen Umständen wichtig sein.
- Zusätzlich muss der Patient nicht sehr viel dazu tun, die Pflegeperson beim Anschließen der entfernt angeordneten Vorrichtung an den lokalen PC/ die lokale Basisstation zu unterstützen.
- Es besteht ein Bedarf nach Systemen und Verfahren, um sowohl lokal als auch entfernt mit ortsungebundenen medizinischen Vorrichtungen wie Arzneimittelverabreichungsvorrichtungen zu kommunizieren, welche sich den vorstehenden und weiteren Problemen zuwenden.
- Die EP-A-319272 offenbart ein über mehrere Betriebsarten verfügendes Arzneiinfusionssystem mit einer Benutzeroberfläche.
- Die US-A-5100380 offenbart ein fernprogrammierbares Infusionssystem zur Verabreichung von mehreren Infusionen mit unterschiedlichen Raten, Volumina und Sequenzen.
- Von einem Gesichtspunkt aus gesehen, stellt die vorliegende Erfindung ein wie in Anspruch 1 definiertes Pumpenkommunikationssystem bereit.
- Die Arzneipumpe kann mit einem Pumpensteuergerät zur Steuerung des Betriebs der Pumpe versehen sein. Das Steuergerät kann einen Prozessor und einen dazugehörigen Speicher umfassen, um Betriebsprogramme und andere Daten abzuspeichern. Die Pumpe kann eine Pumpeinrichtung umfassen, die vom Pumpensteuergerät gesteuert wird, um einem Patienten in der normalen Pumpbetriebsart durch Pumpen Fluid zuzuführen.
- Es kann eine Verbindungseinrichtung wie Modems vorgesehen sein, um eine Kommunikation zwischen den jeweiligen Prozessoren über ein Kommunikationsmedium wie ein herkömmliche Telefonverbindungsleitung zuzulassen. Die Telefonleitungen ermöglichen auch eine anfängliche Sprachkommunikation zwischen dem Patienten und der Pflegeperson unter Verwendung herkömmlicher Telefone oder anderer Geräte. Die Modems können Lautsprecher umfassen, um die Bedienungsfreundlichkeit bei Patienten zu begünstigen, die aufgrund mentaler und/oder physischer Einschränkungen bei der Kooperation mit der Pflegeperson Schwierigkeiten haben.
- Die Pumpe kann eine Bedienereingabeeinrichtung umfassen, um Bedienereingaben in das Steuergerät der Pumpe vorzunehmen. Die Bedienereingabeeinrichtung ist nützlich, um die gewünschten Pumpensteuerungsprogramme oder die gewünschten Pumpendaten, oder beides, aus dem Speicher des Prozessors der Pumpe auszuwählen. Die Datenauswahleinrichtung kann eine Tastatur mit einer Vielzahl von Tasten umfassen.
- Die Anzeigeeinrichtung an der Pumpe ist mit der Prozessoreinrichtung des Computersystems verbunden. Die Anzeigeeinrichtung zeigt Daten über Betriebsabläufe der Pumpe an, die von der Prozessoreinrichtung an die Anzeigeeinrichtung geschickt wurden. Die Anzeigeeinrichtung kann gewünschte Daten aus dem Speicher des Prozessors wie Daten bezüglich der normalen Pumpbetriebsart des Pumpmechanismus anzeigen. Andere Daten, die angezeigt werden können, können sich auf verschiedene Sensorausgänge zur Prozessoreinrichtung beziehen. Die Anzeigeeinrichtung kann eine LCD- oder Flüssigkristall-Punktmatrixanzeige umfassen.
- Die entfernt angeordnete Patientenpumpe kann in das Steuergerät einprogrammierte Daten besitzen, die die Patientenpumpe anweisen, zur richtigen Zeit in die untergeordnete Betriebsart überzugehen. In der untergeordneten Betriebsart kann die Patientenpumpe ihre Tastatureingabesignale in erster Linie von der Prozessoreinrichtung bekommen. Die Patientenpumpe kann dann ihre Anzeigesignale an das Computersystem schicken. In einer Ausführungsform sperrt die untergeordnete Betriebsart der Patientenpumpe die Patientenpumpe, damit sie kein Fluid während der Kommunikation mit dem Computersystem pumpt.
- In einer Ausführungsform umfasst die Patientenpumpe Programmiereinrichtungen, um sowohl in der übergeordneten als auch in der untergeordneten Betriebsart zu arbeiten. Eine automatische Auswahl der der normalen Pumpbetriebsart übergeordneten oder untergeordneten Betriebsart steht bereit, wenn ein Nachrichtenkabel an die Pumpe angeschlossen ist.
- Es werden verschiedene Programmeinrichtungen und -abfolgen zur Kommunikation zwischen dem Computersystem und der entfernt angeordneten Pumpe offenbart, um eine effiziente Kommunikation zu erleichtern und gleichzeitig einen einfachen Gebrauch durch die Pflegeperson und den Patienten hervorzuheben, wovon einige Schwierigkeiten haben können, Anweisungen der Pflegeperson über das Telefon auszuführen.
- Das System kann für eine Kommunikation zwischen einer entfernt angeordneten Pumpe und einem lokalen Computer wie einem PC eines Pflegers sorgen. Solch eine Kommunikation kann unter Verwendung der Datenbearbeitungsfähigkeit des Computers eine umfangreichere Datenverarbeitung zulassen. Auch kann die Kommunikation mit der entfernt angeordneten Pumpe unter Einsatz des lokalen Computers nützlich sein, um die entfernt angeordnete Pumpe periodisch einer erneuten Qualitätsprüfung zu unterziehen, was manchmal im Hinblick auf medizinische Geräte erforderlich ist. Der lokale Computer kann auch beim Ändern oder Einstellen des Pumpenbedienungsprogramm der entfernt angeordneten Pumpe nützlich sein. Die Modemkommunikationseinrichtung ermöglicht eine Kommunikation zwischen der entfernt angeordneten Pumpe und dem lokalen Computer über ein Kommunikationsmedium wie herkömmliche Telefonleitungen.
- In einer weiteren Ausführungsform ist eine Modemkommunikationseinrichtung vorgesehen, welche eine lokale Pumpe mit einem lokalen Computer verbindet. Diese Modemkommunikation ermöglicht: (1) eine Kommunikation einer lokalen Pumpe und eines lokalen Computers, und (2) eine Kommunikation einer entfernt angeordneten Pumpe und eines lokalen Computers.
- Das Computersystem ist mit einem Prozessor und einem Speicher ausgestattet, welcher so programmiert ist, dass er ein Bild einer Pumpe, die der vom Patienten verwendeten Pumpe gleicht, auf einer Anzeige anzeigt. Das Bild umfasst Darstellungen der Tasten und der Anzeige der tatsächlichen Pumpe. Der Computer ist darüber hinaus so programmiert, dass das Bild der Pumpe dazu verwendet wird, mit einer tatsächlichen entfernt angeordneten Pumpe oder einer tatsächlichen lokalen Pumpe zu kommunizieren, indem die Tasten auf dem Bild der Pumpe betätigt oder aktiviert werden. Die Tasten des Bilds können durch solche Verfahren wie durch den Einsatz einer Maus oder eines Berührungsbildschirms aktiviert werden. Eine Aktivierung der Tasten des Bilds der Pumpe simuliert für die Pflegeperson das Vorhandensein einer tatsächlichen Pumpe. Dies erleichtert Verkürzungen der Schulungszeit, um die Pflegeperson darin zu schulen, mit der Pumpe des Pati enten zu kommunizieren, da die Pflegeperson höchstwahrscheinlich mit der Bedienung der Pumpe des Patienten schon vertraut ist.
- Das Computersystem mit dem Pumpenbildprogramm kann auch als Simulator verwendet werden, um die Pflegeperson oder den Patienten darin zu schulen, wie die Pumpe zu verwenden ist. Der Simulator umfasst verschiedene Programme, um für den zu schulenden Patienten und/oder Pfleger verschiedene Pumpenbedienungssituationen mit einer Patientenpumpe zum Pumpen von Flüssigkeit zu simulieren. Der Simulator umfasst auch verschiedene Programme, um für den zu schulenden Patienten und/oder Pfleger verschiedene Kommunikationssituationen mit einer Patientenpumpe zu simulieren.

- Nunmehr ist mit Bezug auf
1 ein System10 zur Kommunikation zwischen einer lokalen Arzneipumpe20 (oder Pflegerpumpe) und einer entfernt angeordneten Arzneipumpe40 (oder Patientenpumpe) gezeigt. Im System10 fungiert die lokale Pumpe20 als Pflegerpumpe zur Verwendung durch die Pflegeperson an der Stelle32 , wo sich die Pflegeperson befindet, wie etwa das Büro der Pflegeperson. Bei der Stelle32 kann es sich während Bereitschaftszeiten um das Haus der Pflegeperson oder auch um das Auto der Pflegeperson handeln, wenn Letzteres mit irgendwelchen Kommunikationsfähigkeiten ausgestattet ist, um Signale hinsichtlich einer anderen Stelle zu senden und zu empfangen. Im System10 handelt es sich bei der Pflegeperson, welche die Pumpe20 bedient, typischerweise um eine Krankenschwester, einen Arzt, einen Therapeuten oder anderes medizinisches Personal. - Im System
10 fungiert die entfernt angeordnete Pumpe40 als ortsungebundene Patientenpumpe, um dem Patienten durch Pumpen Medikamente zuzuführen, und befindet sich beim Patienten an einer von der Pflegerpumpe20 entfernt angeordneten Stelle, wie dem Haus52 des Patienten. 20 ,40 umfassen jeweils einen Pumpmechanismus, welcher Fluid aus einem Fluidbehälter zu einem Patienten pumpen kann. Spezielle Bestandteile der Patientenpumpe40 sind ausführlicher in1A gezeigt. Die Pflegerpumpe20 ist vorzugsweise im Hinblick auf die in1A gezeigten Merkmale mit der Pflegerpumpe40 identisch. - Die wie in
1A gezeigte Pumpe40 umfasst einen Pumpmechanismus43 und einen Fluidbehälter47 , die durch eine Fluidverbindungseinrichtung47a wie einen Schlauch miteinander verbunden sind. Der Fluidbehälter47a kann eine an der Pumpe40 angebrachte Kassette oder ein Behältnis sein, das sich an einer von der Pumpe40 entfernt angeordneten Stelle befindet, die beide mit einem Fluidverbindungsschlauch47a mit der Pumpe40 verbunden sind. Um Fluid zu einem Patienten zu pumpen, ist die Pumpe auch mit dem Patienten wie etwa über einen Schlauch40a verbunden. Der Pumpmechanismus43 pumpt Fluid aus dem Behälter47 durch den am Patienten angeschlossenen Schlauch40a zum Patienten. - Die Pumpen
20 ,40 können irgendwelche aus einer Reihe von Arzneimittelverabreichungspumpen sein. Ein Beispiel einer Arzneimittelverabreichungspumpe ist im US-Patent Nr. 4,559,038 beschrieben, das hier durch Bezug mitaufgenommen wird. Im US-Patent Nr. 4,559,038 ist eine rotierende Nockenwelle vorgesehen, welche zwei Hubventile und eine hin- und hergehende Austreibungsvorrichtung in Eingriff nimmt. Die Ventile und die Austreibungsvorrichtung nehmen einen Schlauch in Eingriff, der in Flüssigkeitsverbindung zwischen einem Fluidbehälter und dem Patienten angeschlossen ist. Die rotierende Nockenwelle bewegt die Ventile und die Austreibungsvorrichtung, um Fluid durch den Schlauch zu pumpen. - Im Kommunikationssystem
10 von1 wird nur die Patientenpumpe40 genutzt, um Fluid zum Patienten zu pumpen. Die Pflegerpumpe20 wird von der Pflegeperson verwendet, um mit der Patientenpumpe40 zu kommunizieren. Die Pflegerpumpe20 wird vorzugsweise nicht dazu hergenommen, um während der Pumpen-zu-Pumpen-Kommunikation mit der Patientenpumpe40 Fluid zu pumpen. Dennoch könnte die Pflegerpumpe20 dazu verwendet werden, falls notwendig vor oder nach der Kommunikation mit der Patientenpumpe40 Fluid zum Patienten zu pumpen. In einigen Fällen könnte die Pflegerpumpe20 dazu verwendet werden, während der Pumpen-zu-Pumpen-Kommunikation mit der Patientenpumpe40 Fluid zu pumpen. - Die Pumpen
20 ,40 umfassen jeweils ein Steuergerät, um den Betrieb des Pumpmechanismus zu steuern, der zum Pumpen von Fluid eingesetzt wird. Die in1A ausführlicher gezeigte Pumpe40 umfasst ein Steuergerät41 , das über eine Anschlusseinrichtung43a mit einem Pumpmechanismus43 verbunden ist. Das Steuergerät41 umfasst vorzugsweise einen Prozessor und Speicher, die mit ausgewählten Funktionen programmiert werden können, um den Betrieb des Pumpmechanismus43 zu steuern. Wie in1a gezeigt ist, kann das Steuergerät41 auch an ein oder mehrere elektrische Sensorelement/e in der Pumpe wie die Sensoren45 ,49 angeschlossen werden. - Eine Reihe von Sensoren wie der Sensor
45 können je nach Pumpentyp und beabsichtigtem Gebrauch der Pumpe vorgesehen sein. Da das Fluid von der Pumpe durch einen Schlauch40a zum Patienten gepumpt wird, kann die Pumpe beispielsweise einen nachgeordneten Hochdrucksensor umfassen, um zu erfassen, ob ein Verschluss des Schlauchs aufgetreten ist. Ein Sensor kann auch vorgesehen sein, um zu erfassen, ob ein Unterdruck im Schlauch47a der Pumpe vorgeordnet zwischen dem Pumpmechanismus43 und dem Fluidbehälter47 entstanden ist, der einen Mangel an zu pumpendem Fluid anzeigt. Es können weitere Sensoren vorgesehen sein, um den elektrischen Stromversorgungszustand der Pumpe zu erfassen. Noch weitere Sensoren können vorgesehen sein, um den Betrieb der mechanischen Komponenten einschließlich des Pumpmechanismus zu überwachen. Die Sensoren wie der Sensor45 senden typischerweise alle über die Verbindungseinrichtung wie die Einrichtung45a ein geeignetes elektrisches Signal an den Prozessor des Steuergeräts41 , das die erfassten Zustände angibt. Das Steuergerät ist zweckmäßigerweise so programmiert, dass es die eingehenden Sensorsignale empfängt und verarbeitetet. Der Sensor49 steht in Bezug zu einem bestimmten Sensor, der während der Pumpen-zu-Pumpen-Kommunikation von Nutzen ist, um die Bediener (Patient sowohl als Pflegeperson) bei der Kommunikation zwischen den Pumpen zu unterstützen, was später noch ausführlicher erläutert wird. - Das Steuergerät
41 umfasst einen Speicher, um verschiedene Programme und Daten zu speichern, die mit dem Betrieb der Pumpe zusammenhängen. Der Prozessor lässt die gewünschten Betriebsprogramme ablaufen, um den Betrieb des Pumpmechanismus43 zu steuern. Der Prozessor spricht darüber hinaus auf die Sensoreingangssignale an, indem er geeignete Steuerausgangssignale entsprechend seiner einprogrammierten Steuerlogik erzeugt. Ein Zugriff auf den Prozessor wird über einen außenliegenden Kommunikationsanschluss42 bereitgestellt. Für die Pflegerpumpe20 wird ein Zugriff auf den Prozessor über einen außenliegenden Kommunikationsanschluss22 bereitgestellt (siehe1 ). - Sowohl die Pflegerpumpe
20 als auch die Patientenpumpe40 können dazu verwendet werden, dem Patienten durch Pumpen ein Medikament zuzuführen oder zu verabreichen, wenn die jeweilige Pumpe an den Patienten angeschlossen ist. Die Pumpen20 ,40 sind hinsichtlich der elektrischen und mechanischen Fluidpumpfunktionen identisch. Ein Vorteil davon ist, dass die Pflegerpumpe20 als ungebrauchte Ersatzpatientenpumpe40 ver wendet werden kann. Wie nachstehend noch erläutert wird, können die jeweiligen Steuergeräte der Pumpen20 ,40 unterschiedlich programmiert werden, um während der Pumpen-zu-Pumpen-Kommunikation in der geeigneten gewünschten Weise zu arbeiten. Wie auch noch erläutert wird, kann diese Programmierung als eine lokale oder Fernprogrammierung erfolgen. - Vorzugsweise umfassen die Pumpen
20 ,40 geeignete Programme, um während der Pumpen-zu-Pumpen-Kommunikation entweder als übergeordnete oder untergeordnete Pumpe zu arbeiten. In einigen Fällen sind die Programme in jeder Pumpe, die den Steuerbetrieb des Pumpmechanismus steuern, unterschiedlich. Dies wird auch nachstehend noch ausführlicher erläutert. - Ein bevorzugter Prozessor, der in Verbindung mit den Pumpen
20 ,40 verwendet werden kann, ist ein MC68HC11E9, eine Komplementär-Metalloxid-Halbleiter (HCMOS)-Hochleistungsmikrocontrollereinheit (MCU) von Motorola. Ein solcher Prozessor umfasst einen elektrisch löschbaren programmierbaren ROM (EEPROM) mit 512 Bytes, und einen RAM mit 512 Bytes. - Die Pumpen
20 ,40 umfassen jeweils eine Bedienereingabeeinrichtung, um es einem Bediener der jeweiligen Pumpe zu ermöglichen, mit dem Steuergerät der Pumpe, insbesondere dem internen Prozessor der Pumpe und der Information im internen Speicher zu kommunizieren. In einer bevorzugten Ausführungsform sind mehrere Bedienertasten24 an der Pflegerpumpe20 vorgesehen, die von der Pflegeperson gedrückt werden sollen. Die Tasten24 senden ein Signal zum Steuergerät der Pumpe20 , das die von der Pflegeperson gedrückte Taste anzeigt. Das Steuergerät der Pumpe20 spricht auf die gewünschte Weise an, wenn durch die Pflegeperson ein verwertbarer Tastendruck erfolgt. - Die Patientenpumpe
40 weist Tasten44 auf, die vorzugsweise identisch mit den Tasten24 sind. Die Tasten44 senden ein Signal an das Steuergerät der Patientenpumpe40 , das die gedrückte Taste angibt. Das Steuergerät der Patientenpumpe40 spricht auf die gewünschte Weise an, wenn durch den Patienten ein verwertbarer Tastendruck erfolgt ist. Wie in1A gezeigt ist, ist die Tastatur44 über eine Verbindungseinrichtung44a mit dem Steuergerät41 zusammengeschaltet. - Die Pflegerpumpe
20 umfasst eine Anzeige26 , um ausgewählte Daten anzuzeigen, die im Steuergerät gespeichert sind. In einer bevorzugten Ausführungsform umfasst die Anzeige26 eine LCD-Punktmatrixanzeige. Die LCD-Punktmatrixanzeige26 ist mit dem Steuergerät der Pflegerpumpe20 zusammengeschaltet. Die vom Steuergerät der Pflegerpumpe20 verschickten Anzeigesignale ermöglichen das Anzeigen von Daten auf der Anzeige26 , die mit dem Betrieb der Pumpe20 zusammenhängen. Eine bevorzugte LCD-Punktmatrixanzeige ist eine vierzeilige Anzeige mit einundzwanzig Zeichen pro Zeile. - Die Patientenpumpe
40 besitzt eine Anzeige46 , die vorzugsweise identisch mit der Anzeige26 der Pflegerpumpe20 ist. Die vom Steuergerät der Patientenpumpe40 verschickten Anzeigesignale zeigen Daten auf einer Anzeige46 an, die mit dem Betrieb der Pumpe40 zusammenhängen. Wie in1A gezeigt ist, ist die Anzeige46 über eine Verbindungseinrichtung46a mit dem Steuergerät41 zusammengeschaltet. - Der Kommunikationsanschluss
22 der Pflegerpumpe20 ermöglicht den Anschluss des Steuergeräts der Pflegerpumpe20 an ein Modem28 , das im Hinblick auf die Pflegerpumpe20 lokal angeordnet ist. Die Pflegerpumpe20 ist über eine Verbindungseinrichtung30 wie ein serielles RS232-Kabel an das Modem28 angeschlossen. Die Pflegerpumpe20 und das Modem28 können sich im Büro32 der Pflegeperson, während Bereitschaftszeiten im Haus der Pflegeperson oder auch an einem mobilen Ort wie dem Fahrzeug der Pflegeperson befinden. - Der Kommunikationsanschluss
42 ermöglicht den Anschluss des Steuergeräts der Patientenpumpe40 mit einer Verbindungseinrichtung50 wie einem seriellen RS232-Kabel an ein Modem48 . Die Patientenpumpe40 und das Modem48 sind beide fern von der Pflegerpumpe20 und dem Modem28 angeordnet, wie etwa im Haus oder am Arbeitsplatz52 des Patienten oder an einem anderen Ort fern von der Pflegerpumpe20 . - Eine Kommunikation zwischen der Pumpe
20 und der Pumpe40 über die Modems28 ,48 findet über ein Kommunikationsmedium54 wie herkömmliche Telefonleitungen, Zellulartelefone, Lichtleiterverbindungen, Satellitenverbindungen, Mikrowellenverbindungen, usw. statt. Die Modems28 ,48 kommunizieren vorzugsweise mit 9600 bps und umfassen gegenüber herkömmlichen Telefonleitungen Fehlerkorrektur- und Datenkomprimierungsmerkmale. - Ein Vorteil der vorliegenden Erfindung besteht darin, dass der Pfleger mit der Patientenpumpe
40 kommunizieren kann, indem er eine ähnliche Pumpe, nämlich die Pflegerpumpe20 benutzt. Der Pfleger kennt vermutlich den Betrieb der Patientenpumpe40 . Diese Kenntnis ist beim Gebrauch der Pflegerpumpe20 zur Kommunikation mit der Patientenpumpe40 wichtig, um auf den Prozessor der Patientenpumpe40 von einer entfernt angeordneten Stelle aus zuzugreifen. - Die Kommunikation zwischen dem Steuergerät der entfernt angeordneten Patientenpumpe
40 und dem Steuergerät der lokal angeordneten Pflegerpumpe20 ermöglicht eine Datenfernerfassung aus der entfernt angeordneten Patientenpumpe durch die lokal angeordnete Pflegerpumpe. Solch eine Datenerfassung kann nützlich sein, um die Patientenpumpe40 während ihres Gebrauchs an der entfernt angeordneten Stelle periodisch zu überwachen. Eine Datenerfassung kann auch am Ende des Gebrauchs der Patientenpumpe nützlich sein. - Eine Kommunikation zwischen der entfernt angeordneten Patientenpumpe
40 und der lokal angeordneten Pflegerpumpe20 ermöglicht eine Fehlersuche im Hinblick auf die entfernt angeordnete Patientenpumpe, ohne dass sich die Pflegeperson am selben Ort befindet wie die Pumpe des Patienten. Eine Fernfehlersuche ist in dem Fall von Nutzen, wenn Patienten mit den einigen oder mehreren Bedienungsvorgängen ihrer Pumpe nicht vertraut sind. Auch ist die Fernfehlersuche unter Verwendung des Pumpen-zu-Pumpen-Kommunikationssystems bei Patienten von Nutzen, die Schwierigkeit haben, mündlich mit dem Pfleger über das Telefon zu kommunizieren. - Eine Kommunikation mit der entfernt angeordneten Patientenpumpe
40 ist auch von Nutzen, um auf die Pumpenbetriebsprogramme zuzugreifen, um den Betrieb der entfernt angeordneten Patientenpumpe von der lokalen Stelle aus zu verändern oder einzustellen, wodurch die Pflegeperson und der Patient Zeit sparen, weil sie keinen persönlichen Besuch machen müssen. - In das Steuergerät der Pflegerpumpe
20 einprogrammierte Daten ermöglichen es, dass die Pflegerpumpe20 zur angemessenen Zeit aus der normalen Pumpbetriebsart in eine übergeordnete Betriebsart versetzt werden kann. In der übergeordneten Betriebsart verschickt die Pflegerpumpe20 über den Anschluss22 ein Tastatureingabesignal an die Patientenpumpe40 , das eine Taste24 angibt, die von der Pflegeperson gedrückt wurde. In der übergeordneten Betriebsart empfängt die Pflegerpumpe20 von der Patientenpumpe40 ihre Anzeigesignale in erster Linie über den Kommunikationsanschluss22 . In der übergeordneten Betriebsart greifen die Tastenbetätigungen der Tasten24 nicht auf den Speicher der Pflegerpumpe20 zu, um diesen zu programmieren oder Daten zur Anzeige auszuwählen, die sich auf die Pflegerpumpe20 beziehen. Die übergeordnete Betriebsart ist in erster Linie dazu da, es der Pflegeperson zu ermöglichen, mit dem Steuergerät der Patientenpumpe40 zu kommunizieren, um den Speicher der Patientenpumpe40 zu programmieren oder Daten zur Anzeige aus dem Speicher der Patientenpumpe40 auszuwählen, die sich auf die Patientenpumpe40 beziehen. - Im Hinblick auf die Patientenpumpe
40 ermöglichen es die in ihr Steuergerät einprogrammierten Daten, dass die Patientenpumpe40 zur angemessenen Zeit aus der normalen Pumpbetriebsart in eine untergeordnete Betriebsart versetzt werden kann. In der untergeordneten Betriebsart empfängt die Patientenpumpe40 Tastatureingabesignale von der Pflegerpumpe20 in erster Linie über den Kommunikationsanschluss42 . Die Patientenpumpe40 sendet ihre Anzeigesignale vom Kommunikationsanschluss42 zur Pflegerpumpe20 . - Um über die Modems
28 ,48 zwischen der Pflegerpumpe20 und der Patientenpumpe40 zu kommunizieren, befindet sich die Pflegerpumpe20 nicht in der normalen Pumpbetriebsart, sondern in der übergeordneten Betriebsart. Entsprechend befindet sich die Patientenpumpe40 nicht in der normalen Pumpbetriebsart, sondern in der untergeordneten Betriebsart. In manchen Fällen können Steuergeräte mit ausreichender Kapazität vorgesehen werden, bei denen die Pumpen20 ,40 gleichzeitig in der normalen Pumpbetriebsart und der über- oder untergeordneten Betriebsart arbeiten. - Beim System
10 ist zumindest die Patientenpumpe40 so programmiert, dass sie in zwei Betriebsraten betrieben werden kann, nämlich der normalen Pump- und der untergeord neten Betriebsart. Typischerweise ist es nicht notwendig, dass die Patientenpumpe40 in der übergeordneten Betriebsart betrieben werden kann, wenn der Patient sie besitzt. Darüber hinaus ist beim System10 zumindest die Pflegerpumpe20 in der übergeordneten Betriebsart betreibbar. Dennoch wird Situationen zuvorgekommen, bei denen es wünschenswert ist, dass eine oder beide Pumpen20 ,40 Programme für einen Betrieb in der normalen Pumpbetriebsart, der untergeordneten und der übergeordneten Betriebsart umfassen. In einigen Fällen kann die Pflegerpumpe20 eine ungebrauchte Patientenersatzpumpe sein. Zu einem späteren Zeitpunkt kann dann die ungebrauchte Patientenersatzpumpe als Patientenpumpe benötigt werden. Dies würde das Betriebsprogramm für die untergeordnete Betriebsart und ein bestimmtes Programm für die normale Betriebsart notwendig machen, das sich für den Patienten eignet. Es kann für die Pflegeperson sinnvoller sein, wenn das Steuergerät jeder Pumpe20 ,40 so vorprogrammiert ist, dass es sowohl das Programm für die untergeordnete als auch das Programm für die übergeordnete Betriebsart enthält. Die Auswahl der übergeordneten oder untergeordneten Betriebsart kann durch die Pflegeperson erfolgen, indem die Patientenpumpe40 so vorkonfiguriert wird, dass sie während der Pumpen-zu-Pumpen-Kommunikation in die untergeordnete, aber nicht in die übergeordnete Betriebsart übergeht. Die Pflegeperson könnte dann die Pflegerpumpe20 so vorkonfigurieren, dass sie, falls die Pflegeperson das wünscht, während der Pumpen-zu-Pumpen-Kommunikation nur in die übergeordnete, aber nicht in die untergeordnete Betriebsart übergeht. Zu irgendeinem späteren Zeitpunkt könnte die Pflegeperson die Pflegerpumpe20 so umkonfigurieren, dass sie während der Pumpen-zu-Pumpen-Kommunikation nur in die untergeordnete Betriebsart übergeht, wenn sie als Patientenpumpe gebraucht wird. - Im System
10 von1 , welche die Verbindung der Pflegerpumpe20 mit der Patientenpumpe40 zeigt, kann die Pflegeperson auf das Steuergerät der Patientenpumpe40 zugreifen, unter Verwendung der Pflegerpumpe20 diverse Eingaben vornehmen und Anzeigeeingaben von der Patientenpumpe40 derart zurückerhalten, dass die Pflegeperson die Anzeigeeingaben auf der Anzeige26 der Pflegerpumpe20 sehen kann. Eine solche Kommunikation kann stattfinden, wenn sich die Patientenpumpe40 an einer von der Pflegerpumpe20 entfernt angeordneten Stelle befindet. Dies ist besonders vorteilhaft beim Einsparen von Betriebsmitteln, indem die Anzahl der persönlichen Besuche zwischen Pflegeperson und Patient reduziert werden. - In einer Ausführungsform ist ein Sperrprogramm im Hinblick auf die Pflegerpumpe vorgesehen, um den Pumpmechanismus der Pflegerpumpe
20 während der Pumpen-zu-Pumpen-Kommunikation so zu sperren, dass er abgeschaltet wird. Entsprechend ist ein Sperrprogramm für die Patientenpumpe40 vorgesehen, um den Pumpmechanismus der Patientenpumpe40 während der Pumpen-zu-Pumpen-Kommunikation auszuschalten. Dies kann aufgrund von Einschränkungen der Prozessorleistung notwendig sein. Dies kann aber auch ein Sicherheitsmerkmal sein, um zu verhindern, dass eine Pflegeperson den Betrieb der Patientenpumpe von einer entfernt angeordneten Stelle her anlaufen lässt. Dennoch kann es in manchen Situationen wünschenswert sein, dass die Pflegerpumpe20 den Betrieb des Pumpmechanismus der Patientenpumpe40 an einer von der Örtlichkeit der Pflegeperson entfernt angeordneten Stelle während der Pumpen-zu-Pumpen-Kommunikation anlaufen lässt. Falls ein geeignetes Steuergerät vorgesehen ist, kann es möglich sein, den Pumpmechanismus der Patientenpumpe40 zu betreiben, während sie mit der Pflegerpumpe20 kommuniziert. - Nunmehr ist mit Bezug auf
4 ein Ablaufdiagramm200 gezeigt, das eine bevorzugte Betriebsablaufsequenz der Patientenpumpe40 hinsichtlich der normalen Pumpbetriebsart und der untergeordneten Betriebsart darstellt.5 ist ein Ablaufdiagramm250 , das eine bevorzugte Betriebsablaufsequenz der Pflegerpumpe20 hinsichtlich der normalen Pumpbetriebsart und der übergeordneten Betriebsart darstellt. Die4 und5 stellen die Betriebsablaufsequenzen für jede Pumpe hinsichtlich der normalen Pumpbetriebsart oder der Betriebsart der Pumpen-zu-Pumpen-Kommunikation dar (untergeordnete und übergeordnete Betriebsart). Die4 und5 zeigen speziell die Sequenzen hinsichtlich der Kommunikation der Tasteneingabesignale und Anzeigesignale zwischen den Pumpen. - Nunmehr ist mit Bezug auf die
6A und6B ein Ablaufdiagramm300 für speziellere Betriebsablaufsequenzen gezeigt, die jeweils während der Verbindung und Kommunikation zwischen der Pflegerpumpe20 und der Patientenpumpe40 aufeinander folgen. Das Ablaufdiagramm300 umfasst auch Maßnahmen, die vom Patienten und Pfleger während der Verbindung und Kommunikation zwischen den beiden Pumpen ergriffen werden sollen. - Das Ablaufdiagramm

40 über die Telefonleitung54 überwachen oder programmieren möchte, würden sie und der Patient zuerst wie etwa durch die Sprachkommunikation über die Telefone29 ,49 , wie in1 gezeigt ist, in Kontakt treten, um die Pumpen-zu-Pumpen-Kommunikation einzuleiten. Die Telefone29 ,49 können herkömmliche Telefone mit Einrichtungen zum Anwählen eines anderen Telefons und ein Handteil oder eine andere Vorrichtung sein, um eine Sprachkommunikation mit dem Gesprächspartner am anderen Ende zu ermöglichen. Vor dem Einleiten der Pumpen-zu-Pumpen-Kommunikation befinden sich beide Pumpen20 ,40 in der normalen Pumpbetriebsart. Oben an jeder Spalte302 ,304 von6A befinden sich die Pumpen20 ,40 in der normalen Pumpbetriebsart. Wie in6A gezeigt ist, würde die Pflegeperson den Patienten zuerst anweisen, die Pumpe40 an das Modem48 anzuschließen, wodurch die Pumpe automatisch angehalten und das geeignete Prozessorprogramm für die untergeordnete Betriebsart ausgewählt wird. - Die automatische Auswahl der untergeordneten Betriebsart und das Anhalten der normalen Pumpbetriebsart bei der Patientenpumpe
40 wird bewerkstelligt, indem eine geeignete Logikschaltungs- und Sensoreinrichtung wie der Sensor49 in1A verwendet wird, um das Vorhandensein des Kabels50 zu erfassen. Die automatische Auswahl der untergeordneten Betriebsart und das Anhalten der normalen Pumpbetriebsart durch Einstecken des Kabels50 in die Patientenpumpe40 ist nutzvoll, da es eine oder mehrere Tastatureingabe/n unnötig macht, die andernfalls vom Patienten oder der Pflegeperson vorgenommen werden müssten, um die Patientenpumpe40 aus der normalen Pumpbetriebsart in die untergeordnete Betriebsart zu versetzen. Alternativ kann es sein, dass der Patient eine vorbestimmte Taste44 drücken oder einen passenden Schalter umlegen muss, um die normale Pumpbetriebsart zu verlassen und in die untergeordnete Betriebsart einzutreten, falls keine automatische Auswahl der untergeordneten Betriebsart und kein automatisches Anhalten der normalen Pumpbetriebsart vorgesehen ist. - In
1A ist der Sensor49 über eine geeignete Anschlusseinrichtung49b an den Kommunikationsanschluss42 und über eine geeignete Anschlusseinrichtung49a an das Steuergerät41 angeschlossen. Der Sensor49 kann zwei voneinander beabstandete Stifte umfassen, welche das Kommunikationskabel50 in Eingriff nehmen, um eine elektrische Leitungsschleife zu schließen, wenn das Kabel50 funktionsbereit im Kommunikationsanschluss42 angeordnet wird. Durch das Schließen der Leitungsschleife wird ein geeignetes Signal, dass das Kabel50 vorhanden ist und eine Pumpen-zu-Pumpen-Kommunikation, d. h. das Betriebsprogramm der untergeordneten Betriebsart gewünscht wird, an das Steuergerät41 geschickt. - Das Betriebssystem der Pflegerpumpe
20 wird dann zulassen, dass die Pumpe aus der normalen Pumpbetriebsart in die Pflegerbetriebsart zur Fernprogrammierung versetzt wird. Die automatische Auswahl der übergeordneten Betriebsart und das automatische Anhalten der normalen Pumpbetriebsart in der Pflegerpumpe20 wird bewerkstelligt, indem eine geeignete Logikschaltungs- und Sensoreinrichtung verwendet wird, um das Vorhandensein des Kabels30 mit etwa einem dem Sensor49 der Patientenpumpe40 ähnlichen Sensor zu erfassen. Alternativ kann es sein, dass die Pflegeperson eine vorbestimmte Taste24 drücken oder einen geeigneten Schalter umlegen muss, um die normale Pumpbetriebsart zu verlassen und in die übergeordnete Betriebsart einzutreten, falls keine automatische Auswahl vorgesehen ist, die mit dem Einstecken des Kabels30 verbunden ist. - Die in der übergeordneten Betriebsart arbeitende Pflegerpumpe
20 umfasst Programmeinrichtungen, um eine Modemverbindung zu der in der untergeordneten Betriebsart arbeitenden Patientenpumpe40 einzuleiten, wie im Ablaufdiagramm300 der6A und6B gezeigt ist. Es wird angenommen, dass die Telefonverbindung zur Patientenpumpe40 steht. Wenn die Modemverbindung zwischen den Pumpen hergestellt ist, wird jedes Mal, wenn die Pflegeperson eine Taste26 an der Pflegerpumpe20 drückt, ein Tasteneingabesignal durch die Programmeinrichtung im Steuergerät der Pflegerpumpe20 an die Patientenpumpe40 geschickt. Die Programmeinrichtung in der Patientenpumpe40 spricht auf das Signal an, als wäre auf ihrer eigenen Tastatur eine Taste gedrückt worden. - Wenn der Prozessor der Patientenpumpe
40 auf das Tasteneingabesignal anspricht, schickt die Programmeinrichtung im Steuergerät der Patientenpumpe40 das aktualisierte Anzeigesignal an die Pflegerpumpe20 . - Wie in
6B gezeigt ist, schickt die Patientenpumpe40 ihre aktuelle Anzeige an die Pflegerpumpe20 , wenn die Pumpen erst einmal miteinander verbunden sind. Bei der Pumpen-zu-Pumpen-Kommunikation sind die Steuergeräte vorzugsweise menügesteuert, und die aktuelle Anzeige lässt die Pflegeperson den aktuellen Status der Patientenpumpe40 sehen, bevor die Pflegeperson damit beginnt, Tasteneingabesignale an die Patientenpumpe40 zu schicken, um die gewünschten Daten aus der Patientenpumpe40 zu erhalten. Nach der Anzeige auf der Anzeige26 der aktuellen Daten auf der Anzeige46 , empfängt die Pflegerpumpe20 ihre an die Pflegerpumpe20 geschickten Anzeigen im Ansprechen auf die Tasteneingaben an der Pflegerpumpe20 , welche zur Patientenpumpe40 geschickt werden. - Statt einer automatischen Einleitung der Modemverbindung zur in der untergeordneten Betriebsart arbeitenden Patientenpumpe
40 , könnten die Pflegeperson sowie der Patient, nachdem die Modemkabel angeschlossen worden sind, beide ihre jeweiligen Telefone einhängen. Die Patientenpumpe40 ist so programmiert, dass sie das im Haus des Patienten installierte Modem anweist, ans Telefon zu gehen, wenn es das nächste Mal läutet. Der Patient würde dann auf den Rückruf der Pflegeperson warten. Die Pflegerpumpe20 ist so programmiert, dass sie das Modem28 anweist, den Patienten zurückzurufen. Ist das Modem28 erst einmal mit dem Modem48 verbunden, findet eine Kommunikation zwischen den jeweiligen Steuergeräten im Hinblick auf Tasteneingabesignale und Anzeigesignale statt. - In einer Ausführungsform zeigt die Anzeige
46 der Patientenpumpe40 alles an, was an die Anzeige26 der Pflegerpumpe20 geschickt wird. In einer anderen Ausführungsform ist das Steuergerät der Patientenpumpe40 so programmiert, dass es ein Sperrprogramm umfasst, um einen Teil oder alle Daten, die vom Steuergerät der Patientenpumpe40 zum Steuergerät der Pflegerpumpe20 geschickt werden, während der Pumpen-zu-Pumpen-Kommunikation zu sperren. Dies kann dann vorteilhaft sein, wenn bestimmte Daten dem Patienten vorenthalten werden sollen wie Zugriffscodes für das Steuergerät, die verwen det werden, um über die Tasten44 oder die Tasten24 während der Pumpen-zu-Pumpen-Kommunikation auf den Prozessor der Patientenpumpe40 zuzugreifen. - Falls die Kommunikationssitzung durch eine schlechte Telefonverbindung unterbrochen wird, könnte die Patientenpumpe
40 unverändert oder teilweise programmiert bleiben. Während der Programmierung der Patientenpumpe40 könnte die Pflegerpumpe20 durch eine geeignete Programmierung in der Pflegerpumpe20 und der Patientenpumpe40 einen kontinuierlich aktualisierten Statusbericht aus der Patientenpumpe40 bekommen. Die Pflegeperson könnte in den Statusbericht nach dem Trennen der Pumpe vom Modem Einsicht nehmen, um zu überprüfen, ob die Patientenpumpe40 so programmiert wurde, wie die Pflegeperson es gewünscht hat. - Das Steuergerät jeder Pumpe
20 ,40 steuert den Betrieb des jeweiligen Modems28 ,48 , das als Peripheriegerät angeschlossen ist. Das Steuergerät jeder Pumpe20 ,40 weist sein jeweiliges Modem an, abzuheben und die Verbindung mit dem Telefon beim Einleiten der Pumpen-zu-Pumpen-Kommunikation zu unterbrechen. Die Steuergeräte steuern darüber hinaus den Betrieb eines jedem Modem zugehörigen Modemlautsprechers, so dass der Patient und die Pflegeperson mündlich miteinander kommunizieren können, bevor die Pumpen-zu-Pumpen-Kommunikation beginnt und nachdem die Sprachübertragungs- und Sprachempfangsmerkmale eines Telefonhandteils oder beider Telefonhandteile beendet wurden. Die Modemlautsprecher sind nützlich, wenn der Patient oder die Pflegeperson das Telefonhandteil nicht benutzen und eine solche Kommunikation gewünscht wird. Der eine Gesprächspartner kann den anderen über den Lautsprecher auffordern, den Telefonhörer abzunehmen. Auch lässt der Modemlautsprecher während der Verbindung die Pflegeperson vorbestimmte Signale aus der Patientenpumpe40 hören, um ihn wissen zu lassen, wann die Patientenpumpe40 für den Beginn der Pumpen-zu-Pumpen-Kommunikation bereit ist, wie im Ablaufdiagramm der6A und6B gezeigt ist. - Das Steuergerät jeder Pumpe
20 ,40 kann mit Maskierprogrammen ausgestattet sein, um die Tasten24 ,44 zu maskieren, die während der Pumpen-zu-Pumpen-Kommunikation inaktiv sind. - Nun ist mit Bezug auf
7 ein Ablaufdiagramm400 einer pflegerausgelösten Unterbrechungsbetriebssequenz gezeigt, um die Verbindung der Pflegerpumpe20 mit der Patientenpumpe40 zu trennen. Eine solche Unterbrechungsbetriebssequenz ist wünschenswert, wenn die Pflegeperson nicht länger mit der Patientenpumpe40 kommunizieren möchte. Im Ablaufdiagramm400 von7 ist eine Pflegersequenz402 in der linken Spalte und einen Patientensequenz404 in der rechten Spalte gezeigt. Das Ablaufdiagramm400 beschreibt die verschiedenen Schritte während der Unterbrechungssequenz, die von der Pflegeperson ausgelöst wird. Oben an jeder Spalte402 ,404 von7 befinden sich die Pumpen20 ,40 in der übergeordneten bzw. untergeordneten Betriebsart. - Ein Versuch, die Pflegerpumpe
20 zu starten, ist ein Verfahren, dem Steuergerät zu signalisieren, die Pumpen-zu-Pumpen-Kommunikation zu beenden. Das Steuergerät der Pflegerpumpe20 beginnt mit der Unterbrechungssequenz im Hinblick auf das Modem28 . Das Steuergerät der Pflegerpumpe20 fordert darüber hinaus an, dass die Patientenpumpe40 mit der Unterbrechungssequenz im Hinblick auf das Modem48 beginnt. - Das Betriebssystem jedes Steuergeräts lässt am Ende der Pumpen-zu-Pumpen-Kommunikation ein automatisches Abschalten der jeweiligen Modems zu, wie in
7 gezeigt ist. Die Steuergeräte steuern darüber hinaus den Betrieb des Modemlautsprechers, so dass der Patient und die Pflegeperson nach der Pumpen-zu-Pumpen-Kommunikation mündlich miteinander sprechen können, so dass die Pflegeperson den Patienten vor der Sprachkommunikation über Telefon durch den Hörer des Patienten während der Abschaltvorgänge instruieren kann. - Eine Programmeinrichtung ist bei der Patientenpumpe
40 vorgesehen, um den Betrieb eines Piepsers, eines anderen Tonalarms oder einer anderen Anzeige von der Pumpe zum Patienten über die bevorstehende Unterbrechung zu informieren, wie im Ablaufdiagramm400 von7 festgehalten ist. Das Piepsen oder das andere Signal kann anhalten, bis die Unterbrechung abgeschlossen ist. Dies ist dabei nützlich, den Patienten darüber zu informieren, wann der Unterbrechungsvorgang der Pumpen-zu-Pumpen-Kommunikation beginnt und wann er aufhört, so dass die Pumpe wieder zu einem Einsatz in der normalen Pumpbetriebsart bereit ist. - Nun ist mit Bezug auf
8 ein Ablaufdiagramm500 einer patientenausgelösten Unterbrechungsbetriebssequenz gezeigt. Das Ablaufdiagramm von8 zeigt eine Pflegersequenz502 in der linken und eine Patientensequenz504 in der rechten Spalte. Oben in jeder Spalte502 ,504 von8 befinden sich die Pumpen20 ,40 in der über- bzw. untergeordneten Betriebsart. - In den Ablaufdiagrammen der
7 und8 ist festgehalten, dass die Pflegeperson und der Patient die Taste STOP/START drücken. Die Taste STOP/START ist eine weitere der Tasten24 ,44 der Pumpen20 ,40 . Befinden sich die Pumpen nicht in der normalen Pumpbetriebsart, wie es in einer Ausführungsform der Fall ist, ist die Taste STOP/START ein bequemes Mittel, ein Signal an die jeweiligen Steuergeräte zu schicken, dass eine Beendigung der Pumpen-zu-Pumpen-Kommunikationssitzung vom Patienten oder der Pflegeperson gewünscht wird. - Wie in
8 gezeigt ist, ist ein Versuch, die Patientenpumpe40 unter Verwendung der Taste STOP/START an der Patientenpumpe40 zu starten, ein Verfahren, um dem Prozessor zu signalisieren, die Pumpen-zu-Pumpen-Kommunikation zu beenden. Die Patientenpumpe40 fordert die Pflegerpumpe20 auf, mit den Unterbrechungssequenzen zu beginnen. Die Patientenpumpe40 wartet darauf, dass die Pflegerpumpe20 den Abschaltprozess einleitet. Das Steuergerät der Pflegerpumpe20 beginnt die Unterbrechung mit dem Modem28 , und das Steuergerät der Pflegerpumpe20 fordert darüber hinaus das Steuergerät der Patientenpumpe40 auf, mit der Unterbrechungssequenz im Hinblick auf das Modem48 zu beginnen. Es sollte klar sein, dass, wenn die Patientenpumpe40 ein Tasteneingabesignal vom Patienten erhält, um mit der Unterbrechung der Pumpen-zu-Pumpen-Kommunikationsverbindung zu beginnen, die Pflegerpumpe20 solange weitermachen kann, bis die Pflegeperson die Pumpen-zu-Pumpen-Kommunikationsverbindung zu beenden wünscht. Die Pflegeperson drückt, wie in8 festgehalten ist, eine Taste wie die Taste STOP/START, um zum gewünschten Zeitpunkt mit der Unterbrechungssequenz zu beginnen. - Wie bei der pflegerausgelösten Unterbrechungssequenz von
7 , schließt auch die Unterbrechungssequenz von8 die Steuergeräte ein, die den Betrieb der Modemlautsprecher so steuern, dass der Patient und die Pflegeperson nach der Pumpen-zu-Pumpen- Kommunikation mündlich miteinander sprechen können, so dass die Pflegeperson den Patienten vor der telefonischen Sprachkommunikation über den Hörer während der Unterbrechungsvorgänge instruieren kann. - Eine Programmeinrichtung ist bei der Patientenpumpe
40 vorgesehen, um den Betrieb eines Piepsers, eines anderen Tonalarms oder einer anderen Anzeige für den Patienten zu steuern, um den Patienten darüber zu informieren, dass die Unterbrechung bevorsteht, wie im Ablaufdiagramm500 von8 festgehalten ist. Das Piepsen oder das andere Signal hält an, bis die Unterbrechung abgeschlossen ist. Das akustische oder ein anderes Patientensignal kann in Kombination mit oder als Alternative für den Einsatz des patientenseitigen Modemlautsprechers verwendet werden, um dem Patienten anzuzeigen, wann das Kabel50 aus der Patientenpumpe40 ausgesteckt werden soll. - Eine Programmeinrichtung bei der Pflegerpumpe
20 ist vorgesehen, um den Betrieb eines Piepsers oder eines anderen Tonalarms oder einer Pflegeranzeige zu steuern, um die Pflegeperson zu informieren, dass die Unterbrechung vom Patienten angefordert wurde. Dies ist eine funkrufartige Funktion, bei der die Pflegeperson mit der Pumpen-zu-Pumpen-Kommunikation weitermachen oder die Unterbrechungssequenz starten kann, indem sie die Taste STOP/START drückt. - In einigen Anwendungen wird das Steuergerät der Patientenpumpe
40 zumindest teilweise über ein Zugriffscodeprogramm gesperrt, um zu verhindern, dass der Patient das Pumpenbetriebssystem verändert oder auf andere Daten im Speicher zugreift. In einer bevorzugten Ausführungsform kann die Pflegeperson die Pumpensperre der Patientenpumpe40 von einer entfernt angeordneten Stelle aus über das Pumpen-zu-Pumpen-Kommunikationssystem entsperren. Vorzugsweise kann die Pflegeperson die Pumpensperre der Patientenpumpe40 wieder sperren, nachdem die Pflegeperson das Pumpenbetriebssystem eingestellt oder verändert hat. Es kann eine Programmeinrichtung zum automatischen Neusperren vorgesehen sein, um das Steuergerät am Ende des Zugriffs der Pflegeperson auf das Steuergerät, um die Betriebsprogramme zu ändern, automatisch wieder zu sperren. - Nun stellt das Schema mit Bezug auf
2 , in der eine alternative Ausführungsform gezeigt ist, ein Kommunikationssystem60 zur Kommunikation zwischen einer Pumpe62 und einem Computer80 dar, die sich beide im Büro66 der Pflegeperson befinden. Bei der Pumpe62 kann es sich um eine Pflegerpumpe wie die Pflegerpumpe20 , oder eine Patientenpumpe wie die Patientenpumpe40 handeln. -
2 stellt auch die Kommunikation zwischen einer Patientenpumpe64 , die sich im Haus68 des Patienten befindet, und sowohl dem Computer80 als auch der Pumpe62 dar. Es sollte klar sein, dass in einigen Anwendungen die Pumpe62 nicht vorhanden sein kann. Es sollte auch klar sein, dass in einigen Anwendungen die Pumpe64 nicht vorhanden sein kann. Bei jeder dieser Anwendungen würde der Computer80 dann nur mit der verbleibenden Pumpe kommunizieren. - Ein Beispiel für den Computer

Eine geeignete Tastatur kann beim Computer80 vorgesehen sein, um Bedienereingaben in den Mikroprozessor vorzunehmen. - Wie in
2 gezeigt ist, kann der Computer80 darüber hinaus mit einem zweiten Computer86 kommunizieren, um Daten und/oder Programme über ein Kommunikationsmedium87 wie herkömmliche Telefonleitungen zum und aus dem Computer86 zu übertragen. Beispielsweise kann sich der Computer80 im Büro66 der Pflegeperson wie etwa in einem Krankenhaus befinden. Der zweite Computer86 kann sich im Pumpenhersteller /Pumpendienstleisterbetrieb88 befinden. Der zweite Computer kann Daten empfangen und an mehrere Computer80 weiterleiten. Diese Anordnung kann nützlich sein, um eine Vielzahl von Patientenpumpen64 über eine Vielzahl von Pflegerbüros66 zu warten. Auch können sich verbesserte Medikamententherapien ergeben, wenn der Pumpenhersteller-/Pumpendienstleister schnellen Zugriff auf die Benutzerdaten der Patientenpumpe hat. - Die Pumpen
62 ,64 sind mechanisch und elektrisch vorzugsweise identisch mit den zuvor beschriebenen Pumpen20 ,40 . In manchen Situationen können die Steuergeräte der Pumpen62 ,64 unterschiedlich programmiert sein, je nachdem, wie die Pumpen eingesetzt werden. Die Pumpe64 wird typischerweise als Patientenpumpe verwendet. Als Patientenpumpe benötigt die Pumpe64 das Betriebsprogramm für die normale Pumpbetriebsart und mindestens das Betriebsprogramm für die untergeordnete Betriebsart. - Im System
60 kann die Pumpe62 eine Patienten- oder Pflegerpumpe sein. Als Patientenpumpe benötigt die Pumpe62 das Betriebsprogramm für die normale Pumpbetriebsart und mindestens das Betriebsprogramm für die untergeordnete Betriebsart. Als Patientenpumpe befindet sich die Pumpe62 an derselben Stelle wie der Computer80 , wie wenn etwa die Pumpe62 vom Patienten gebracht wird. In manchen Situationen kann die Pumpe62 eine Patientenpumpe sein, die sich im Pflegerbüro66 zur Datenerfassung, Fehlersuche und/oder für Programmänderungen oder Programmabwandlungen befindet. Auch ist die Pumpe62 , bevor der Patient das Pflegerbüro66 verlässt, im Pflegerbüro66 zur Verwendung als Patientenpumpe im Pflegerbüro vorhanden. Als Pflegerpumpe benötigt die Pumpe62 mindestens das Betriebsprogramm für die übergeordnete Betriebsart. Die Pumpe62 , die als Pflegerpumpe arbeitet, kann auch das Betriebsprogramm für die normale Pumpbetriebsart und das Betriebsprogramm für die untergeordnete Betriebsart umfassen. - Um eine Kommunikation zwischen der Pumpe
62 , dem Computer80 und der Pumpe64 zu ermöglichen, ist ein Modem100 vorgesehen. Nunmehr ist mit Bezug auf3 das Modem100 zusätzlich im Detail gezeigt. Das Modem100 umfasst ein lokales Pumpenkommunikationskabel102 zum elektrischen Anschluss des Modems100 an eine wie in2 gezeigte lokale Pumpe62 . Das Kabel102 ist wie die Kabel30 ,50 von1 . Das Kabel102 erstreckt sich vom Modemsteuergerät110 für einen selektiven Anschluss an die Pumpe62 . - Das Modem
100 umfasst darüber hinaus einen Kommunikationsanschluss104 für den Computer, um das Modem100 wie in2 gezeigt elektrisch mit dem Computer80 zu verbinden. - Das Modem
100 umfasst darüber hinaus einen Kommunikationsanschluss106 für das Telefonsystem, um das Modem100 über das Kommunikationsmedium54 elektrisch mit entfernt angeordneten Stellen zu verbinden. In der gezeigten Ausführungsform ermöglicht der Kommunikationsanschluss106 über die Telefonleitungen54 eine Zusammenschaltung des Modems100 mit dem Modem48 , wie in2 gezeigt ist. Eine solche Zusammenschaltung ermöglicht eine Kommunikation zwischen dem Modem100 und der Patientenpumpe64 , die sich an einer vom Modem100 entfernt angeordneten Stelle befindet. - Das Modem
100 umfasst darüber hinaus einen lokalen Telefonkommunikationsanschluss108 , um das Zusammenschalten eines (nicht gezeigten) lokalen Telefons mit dem Modem100 zu ermöglichen. Der lokale Telefonanschluss108 ermöglicht die Verwendung eines lokalen Telefons, so dass die Pflegeperson, bevor eine Modemverbindung hergestellt wird und nachdem die Modemverbindung beendet wurde, durch herkömmliche telefonische Sprachkommunikation mit dem Patienten kommunizieren kann. - Das Modem
100 ermöglicht, dass vom Computer80 Daten über die Telefonleitungen54 an eine entfernt angeordnete Stelle übertragen werden können. Das Modem100 ermöglicht darüber hinaus, dass die Pumpe62 über die Telefonleitungen54 mit der entfernt angeordneten Stelle kommunizieren kann. Eine Fernkommunikation über die Telefonleitungen durch das Modemsteuergerät110 und das Modembauteil112 mit der Pumpe64 findet vorzugsweise mit 9600 bps (Bits pro Sekunde) statt. Das Modem100 umfasst vorzugsweise Fehlerkorrektur- und Datenkomprimierungsfunktionen. - Das Modem
100 erlaubt außerdem eine Kommunikation zwischen der Pumpe62 und dem Computer80 . Vorzugsweise findet eine solche Kommunikation über das Modemsteuergerät in einer Durchlaufkonfiguration mit 19.200 bps statt. Die Pumpe62 ist vorzugsweise nicht direkt mit dem Computer80 verbunden, weil die Pumpe62 mit anderen Stromversorgungen als den herkömmlichen 110 V-Stromversorgungen ausgestattet sein kann, die zur elektrischen Versorgung des Computers80 verwendet werden. Auch ist es wünschenswert, die Pumpe62 vom Computer80 elektrisch zu isolieren, um die Pumpe62 und den Patienten vor den Gefahren der Stromquelle, die den Computer80 versorgt, wie einem elektrischen Schlag zu schützen. Vorzugsweise kommuniziert das Modem100 über eine angemessene Verbindungseinrichtung wie ein serielles RS232-Kabel mit dem Computer80 . -
2 zeigt die Pumpe62 auch an ihren unabhängigen Drucker72 angeschlossen, der mit der Verbindungseinrichtung74 wie einem seriellen RS232-Kabel verbunden ist. Anstatt einer seriellen Verbindung kann auch eine Parallelverbindung verwendet werden. Der Drucker72 stellt einen Papierausdruck der im Steuergerät der Pumpe62 gespeicherten Daten her. In manchen Fällen wäre der Drucker72 nur über den Kommunikationsanschluss einsetzbar, welcher die Pumpe62 am Modem100 anschließt, wie in2 gezeigt ist. Mit anderen Worten kann die Pumpe62 nicht gleichzeitig mit dem Drucker74 und dem Modem100 verwendet werden. Ein geeignet bemessenes Steuergerät und zwei Kommunikationsanschlüsse würden einen solchen Einsatz zulassen. - Eine Kommunikation zwischen dem Computer
80 und einer Pumpe, sei es nun eine Fernkommunikation (mit der Pumpe64 ) oder eine lokale Kommunikation (mit der Pumpe62 ), ist aus mehreren Gründen von Nutzen. Erstens kann der Computer80 mit einem größeren Speicher und größeren Datenverarbeitungsfähigkeiten ausgestattet werden, als bei den einzelnen Pumpen62 ,64 vorhanden sind. Die Druckfähigkeiten können mit einem elektrisch an den Computer80 angeschlossenen Drucker82 größer sein. Die Anzeigefähigkeiten können mit einem an den Computer80 angeschlossenen Monitor84 größer sein. Auch kann der Computer80 dabei von Nutzen sein, die Qualität der Pumpe periodisch neu zu prüfen, wie es erforderlich ist, um die Betriebsfähigkeit und Genauigkeit der Pumpen zu überprüfen. - Der Computer
80 kann auch von Nutzen sein, um Programmeinstellungen oder Anwendungsveränderungen bei den Pumpen62 ,64 vorzunehmen. Zum Beispiel können Arzneimittelverabreichungsvorrichtungen in verschiedenen normalen Pumpbetriebsarten oder Anwendungen eingesetzt werden wie 1) Schmerzkontrolle, 2) Ernährung, 3) Chemotherapie und 4) Behandlung mit Antibiotika. Andere Anwendungen sind möglich. - Jede der Anwendungen kann verschiedene Betriebsabläufe des Pumpmechanismus mit sich bringen. Jede Anwendung kann eine oder mehrere patientenspezifische Variationen des Betriebs des Pumpmechanismus und anderer Pumpensteuerungsfunktionen aufwei sen. Auch können einige der Anwendungen eine gewisse Patienteneingabe beinhalten, wie es manchmal bei Schmerzkontrollanwendungen der Fall ist. Wenn der Patient beispielsweise zu einer bestimmten Zeit Schmerz verspürt, kann die Pflegeperson innerhalb festgelegter Parameter wie Zeit und Menge nach Bedarf des Patienten für erhöhte Dosen sorgen. In dieser Anwendung muss der Pumpmechanismus automatisch und nach Ermessen des Patienten innerhalb der festgelegten Parameter betriebsbereit sein. Das Betriebsprogramm enthält die geeigneten Pumpensteuerungsbefehle, um den Pumpmechanismus und die Funktionen der Tasten zu steuern, welche eine Steuerung des Pumpmechanismus durch den Patienten zulassen. Die anderen Anwendungen können andere Pumpensteuerungsbefehle und andere Funktionen der Tasten umfassen. In allen vier vorstehend aufgelisteten Anwendungen kann es verschiedene unterschiedliche Pumpenbetriebsprogramme geben. Das Steuergerät der Patientenpumpe
40 kann von der Pflegeperson für den speziellen Patientengebrauch angemessen programmiert werden. - Es sollte klar sein, dass die Pflegerpumpe
20 und die Patientenpumpe40 nicht dasselbe Anwendungsprogramm zum Betrieb des Pumpmechanismus umfassen müssen. Ein signifikanter Vorteil der vorliegenden Erfindung ist, dass die Pflegeperson nacheinander mit verschiedenen Patienten kommunizieren kann, wobei bei jedem Patienten eine andere Pumpenanwendung oder dieselbe Pumpenanwendung mit unterschiedlichen patientenspezifischen Funktionen betroffen sein kann. Mindestens wird vorgezogen, dass die Pflegerpumpe20 das Betriebsprogramm für die übergeordnete Betriebsart und die Patientenpumpe40 das Betriebsprogramm für die untergeordnete Betriebsart oder das Betriebsprogramm für die normale Pumpbetriebsart umfasst, um den Pumpmechanismus so zu betreiben, dass er Fluid zum Patienten pumpt. Vorzugsweise ist es bequemer für die Pflegeperson, wenn die Pflegerpumpe20 und die Patientenpumpe40 beide das Betriebsprogramm für die untergeordnete Betriebsart und das Betriebsprogramm für die übergeordnete Betriebsart und auch mindestens ein Betriebsprogramm für die normale Pumpbetriebsart umfassen. - Erwartungsgemäß kann die Pflegeperson die Steuergeräte der Pumpen
20 ,40 ,62 ,64 lokal oder fernprogrammieren, so dass die Pumpe beim Einleiten der Pumpen-zu-Pumpen-Kommunikationssequenzen entweder in 1) der normalen Pumpbetriebsart oder der unter geordneten Betriebsart oder 2) der normalen Pumpbetriebsart oder der übergeordneten Betriebsart arbeiten kann. - In den Steuergeräten der Pumpen
20 ,40 können verschiedene pumpenbezogene Daten gespeichert sein. Daten, die während des Gebrauchs der Patientenpumpe40 gesammelt werden können, umfassen Datum und Zeit von: - 1) Pumpenfehlerzuständen, bei denen beispielsweise der Pumpmechanismus zu arbeiten aufgehört hat und ein geeigneter Sensor ein Pumpenstillstandssignal an den Prozessor schickt.
- 2) Einem Hochdruckalarm bei stromabwärts der Pumpe aufgetretenen Blockierungen, wobei ein geeigneter Drucksensor ein Hochdrucksignal an den Prozessor schickt.
- 3) Einem Alarm bei einem stromaufwärts der Pumpe auftretenden Verschluss, wobei ein geeigneter Drucksensor ein Verschlusssignal an den Prozessor schickt.
- 4) Jedem Fluidbehälterentnahmeereignis, wobei ein geeigneter Sensor ein Behälterentnahmesignal an den Prozessor schickt.
- 5) Jedem Fluidbehälteranbringungsereignis, wobei von einem geeigneten Sensor ein geeignetes Signal an den Prozessor geschickt wird, möglicherweise demselben Sensor wie demjenigen zum Erfassen eines Fluidbehälterentnahmeereignis.
- 6) Jedem internen Batterieereignis, wobei ein geeigneter Sensor erfasst, ob die Batterie gewechselt wurde, schwach oder leer ist.
- 7) Jedem AC-Adapterereignis, wobei ein geeigneter Sensor erfasst, ob der Adapter angeschlossen, abgekoppelt oder aus der Wandsteckdose herausgezogen ist.
- 8) Jedem Ereignis bei einer tragbaren Stromversorgung, wobei ein geeigneter Sensor erfasst, ob die Stromversorgung angeschlossen, abgekoppelt oder leer ist.
- 9) Jedem Ereignis bei einer Ferndosierungsleitung (welche es dem Patienten ermöglicht, von der Ferne aus eine Taste wie die Dosistaste DOSE auf der Tastatur zu drücken), wobei ein geeigneter Sensor erfasst, ob die Dosierungsleitung angeschlossen oder nicht angeschlossen ist.
- 10) Jedem Kommunikationskabelereignis (um es der Pumpe zu ermöglichen, mit einem Drucker, einer anderen Pumpe oder einem Computer zu kommunizieren), wobei ein geeigneter Sensor erfasst, ob das Kabel angeschlossen oder nicht angeschlossen ist.
- 11) Jedem Mal, dass die Vorrichtung mit der Taste START/STOP erfolgreich ein- oder ausgeschaltet wird, wobei ein geeigneter Sensor erfasst, ob die Pumpe nach dem Tastendruck erfolgreich ein- oder ausgeschaltet wurde.
- 12) Jeder Sperrgradveränderung und der neue Sperrgrad in Situationen, bei denen der Zugriffsbetrag des Patienten zum Prozessor verändert wird, d. h. voller Zugriff, etwas Zugriff, kein Zugriff.
- 13) Jeder Veränderung des Betriebsprogramms der Patientenpumpe oder Reset-/Freigabestatus im neuen Programm.
- 14) Jedem Mal, dass die Taste PRIME (ANSAUGEN) gedrückt wird und wie viel Fluid angesaugt wird, um, wie etwa beim Hochfahren der Pumpe, Luft aus der Pumpe auszutreiben.
- 15) Jeder abgegebenen Patientendosis und die Menge, die abgegeben wird, indem der Patient die Taste DOSIS drückt, um dem Patienten eine zusätzliche Fluidmenge zu verabreichen.
- 16) Jeder Veränderung anderer Merkmale (Einheiten, Einstellzeit oder Einstelldatum, Änderung der automatischen Sperre, Änderung der Anwendung) und den neuen Daten oder dem neuen Zustand.
- 17) Jedem Mal, dass die Pumpe erneut erfolgreich qualitätsgeprüft wird.
- 18) Anzahl der Betätigungsvorgänge des Pumpmechanismus und Dauer des Einsatzes.
- Verschiedene Funktionen sind für jede der Tasten an jeder Pumpe
20 ,40 vorgesehen. Jede Taste hat mindestens eine Funktion. Beispiele potentieller Funktionen der verschiedenen Tasten umfassen: - 1) eine Taste NEXT SCREEN (NÄCHSTE BILDSCHIRMMASKE), um verschiedene Bildschirmmasken zu durchlaufen, indem ein Programm „Nächste Bildschirmmaske" ablaufen gelassen wird;
- 2) eine Taste UP ARROW (AUFWÄRTSPFEIL) und eine Taste DOWN ARROW (ABWÄRTSPFEIL), um das mit einem Hervorhebungsbalken zu durchblättern, was auf der Bildschirmmaske angezeigt wird, oder um Zahlenwerte durchzublättern, um den gewünschten Wert hervorzuheben und/oder anzuzeigen;
- 3) eine Taste PRIME (ANSAUGEN), um ein Pumpenansaugprogramm ablaufen zu lassen, um die Pumpe zum Ansaugen zu bringen;
- 4) eine Taste START/STOP, um einen Pumpenstartprogramm und ein Pumpenstopprogramm ablaufen zu lassen;
- 5) eine Sperrtaste LOCK, um über ein Zugriffsprogramm eine Zugriffssteuerung zum Prozessor bereitzustellen;
- 6) eine Dosistaste DOSE, um ein Patientenpumpensteuerungsprogramm ablaufen zu lassen, um eine Steuerung des Pumpmechanismus durch den Patienten zuzulassen;
- 7) eine Hilfetaste HELP, um Hilfeinformation auf der Anzeige bereitzustellen.
- Nun ist mit Bezug auf
9 ein Monitor oder Computerbildschirm90 als weitere Ausführungsform der Erfindung gezeigt. Das Bild einer Pumpe92 (Vorderansicht) ist durch eine geeignete Grafikfähigkeit auf dem Bildschirm90 angezeigt. Der Bildschirm90 ist operativ mit einem Prozessor eines Computersystems wie dem Prozessor des Computers80 verbunden. Das Bild92 auf dem Bildschirm90 kann mittels eines Bildschirminteraktionsprogramms unter Verwendung einer Maus oder eines Berührungsbildschirms so betätigt werden als wäre es eine Pumpe. Der Computer80 ist so programmiert, dass er je nachdem, wie die verschiedenen Tasten94 des Pumpenbilds92 unter Verwendung der Maus oder des Berührungsbildschirms gedrückt werden, verschiedene Programme ablaufen lässt. Dies ermöglicht eine Simulation der Pumpe20 mit Hilfe des Computers80 . Eine Anzeigefläche95 wie die Anzeige26 der Pumpe20 kann dann Information anzeigen. - Der Computerbildschirm
90 und der Computer80 werden dazu eingesetzt, mit einer an einer entfernten Stelle befindlichen Pumpe wie der Pumpe64 von2 zu kommuni zieren. Ist die Pumpe62 wie die Pumpe von2 lokal angeordnet, ist die Kommunikation mit einer wie in den Figuren gezeigten unmittelbaren Durchschaltung direkt. - Die Anordnung, die den Computerbildschirm von
9 umfasst, ist nützlich, wenn die Pflegeperson mit einer entfernt angeordneten oder einer lokal angeordneten Patientenpumpe kommuniziert. Die Pflegeperson kann das Computersystem leichter durch den Einsatz der Tasten und der Anzeige verwenden, da sie mit dem Betrieb einer Pumpe bereits vertraut ist. Eine Betätigung der Tasten94 des Bilds92 und der Einsatz der Anzeige95 des Pumpenbilds simuliert für die Pflegeperson das Vorhandensein einer wirklichen Pumpe. Dies erleichtert Reduzierungen bei der Schulungszeit, um die Pflegeperson darin zu schulen, mit der Patientenpumpe zu kommunizieren, weil die Pflegeperson höchstwahrscheinlich schon mit dem Betrieb der Patientenpumpe vertraut ist. - Das Computersystem mit dem Pumpenbildprogramm kann auch als Simulator zur Schulung der Pflegeperson und/oder des Patienten verwendet werden, wie die Pumpe zu benutzen ist. Der Simulator umfasst verschiedene Programme, um den Betrieb einer Patientenpumpe zum Pumpen von Fluid zu simulieren. Der Simulator umfasst auch verschiedene Programme, um verschiedene Kommunikationssituationen mit einer Patientenpumpe zu simulieren.
- In
10 ist ein Computerbildschirm96 eines Schulungssimulatorsystem gezeigt. Auf dem Computerbildschirm100 werden das Pumpenbild92 und Simulatordaten98 angezeigt. Die Simulatordaten werden dazu verwendet, um durch Simulationsprogramme verschiedene Bedingungen auszuwählen, um einen Betrieb einer Patientenpumpe zu simulieren. Die Pumpe92 könnte als Pflegerpumpe fungieren, um eine Pflegeperson darin zu schulen, wie die Pflegerpumpe einzusetzen ist, um mit einer Patientenpumpe zu kommunizieren. Falls beispielsweise die zu schulende Pflegeperson eine Kommunikationssitzung mit einer entfernt angeordneten Patientenpumpe simulieren möchte, deren Batteriestand niedrig ist, würde die Pflegeperson zum Beispiel das Programm in der Auflistung98 wählen, das eine Pumpen-zu-Pumpen-Kommunikationssituation simulieren würde, bei der die Pflegeperson die Patientenpumpe auf Fehler hin untersuchen würde, um festzustellen, ob die Situation einer schwachen Batterie besteht. Der Simulator verfügt auch über ein Simulationsprogramm, um zumindest einige der Sequenzen zum Anschließen und Abkoppeln der Pumpen entsprechend den Ablaufdiagrammen der6A ,6B ,7 und8 zu simulieren. - Das Simulatorsystem umfasst auch Simulationsprogramme in der Information
98 , um den Betrieb der Pumpe auf dem Bildschirm96 als Patientenpumpe zu simulieren, um einem Patienten durch Pumpen Fluid zuzuführen. Dies wäre dazu nützlich, einen Patienten und eine Pflegeperson darin zu schulen, wie die Pumpe in der normalen Pumpbetriebsart zu bedienen ist. - Die vorliegende Erfindung bezieht sich speziell auf einen Pumpenkommunikationssimulator zur Schulung eines Pumpenbedieners, welcher Simulator umfasst: ein Computersystem mit einer Prozessoreinrichtung und einer Anzeigeeinrichtung, die elektrisch mit der Prozessoreinrichtung des Computersystems verbunden ist; eine erste Programmeinrichtung zum Anzeigen eines Bilds der Pumpe auf der Anzeigeeinrichtung, wobei das Bild eine Eingabefläche und eine Anzeigefläche aufweist; eine zweite Programmeinrichtung, um eine Betätigung der Eingabefläche des Bilds der Pumpe auf der Anzeigeeinrichtung durch den Pumpenbediener so zuzulassen, dass Zugriff auf die Prozessoreinrichtung des Computersystems erhalten wird; und eine dritte Programmeinrichtung, um im Ansprechen auf die Betätigung der Eingabefläche der Bilds durch den Pumpenbediener eine vorbestimmte Nachricht an die Anzeigefläche des Bilds der Pumpe zu schicken.
Claims (14)
- Pumpenkommunikationssystem, umfassend: (a) eine Pumpe (
20 ,40 ,62 ,64 ) mit: (i) einer Einrichtung zum Pumpen von Fluid (43 ); (ii) eine unabhängig programmierbare und betreibbare Pumpenprozessoreinrichtung (41 ), um den Betrieb der Einrichtung zum Pumpen von Fluid zu steuern; (b) ein Computersystem (80 ) mit einer Prozessoreinrichtung, und eine Anzeigeeinrichtung (90 ), die elektrisch mit der Prozessoreinrichtung des Computersystems zusammengeschaltet ist; (c) eine Kommunikationseinrichtung (28 ,48 ,100 ), um Kommunikation zwischen der Prozessoreinrichtung des Computersystems und der Pumpenprozessoreinrichtung zuzulassen; dadurch gekennzeichnet, dass es darüber hinaus umfasst: (d) eine erste Programmiereinrichtung des Computersystems, um ein Bild (92 ) der Pumpe auf der Anzeigeeinrichtung anzuzeigen; und (e) eine zweite Programmiereinrichtung des Computersystems, um eine gezielte Veränderung des Bilds der Pumpe auf der Anzeige zuzulassen und dadurch über die Pumpenprozessoreinrichtung mit der Einrichtung zum Pumpen von Fluid zu kommunizieren. - Pumpenkommunikationssystem nach Anspruch 1, bei dem die Pumpe (
20 ,40 ,62 ,64 ) ortsungebunden ist. - Pumpenkommunikationssystem nach Anspruch 2, bei dem die Einrichtung zum Pumpen von Fluid (
43 ) zwei hin- und hergehende Ventile und eine hin- und hergehende Austriebsvorrichtung umfasst, welche einen Schlauch (40a ) in Eingriff nehmen, der in Fluidverbindung zwischen einem Fluidvorratsbehälter (47 ) und einem Patienten angeschlossen ist. - Pumpenkommunikationssystem nach Anspruch 1, bei dem die Pumpe (
20 ,40 ,62 ,64 ) eine Vorderfläche hat, und bei dem das Bild (92 ) der Vorderfläche der Pumpe gleicht. - Pumpenkommunikationssystem nach Anspruch 1, bei dem die Pumpe (
20 ,40 ,62 ,64 ) eine Tastatur (44 ) umfasst, um Befehle an die Pumpenprozessoreinrichtung (41 ) einzugeben. - Pumpenkommunikationssystem nach Anspruch 5, bei dem das Bild (
92 ) ein Tastaturbild umfasst, das die Tastatur (44 ) symbolisiert. - Pumpenkommunikationssystem nach Anspruch 5, bei dem die Tastatur (
44 ) mehrere Tasten umfasst, und das Bild (92 ) mehrere Tastenbilder umfasst, wovon jedes einer der Tasten entspricht, wobei die zweite Programmiereinrichtung eine gezielte Veränderung des Bilds zulässt, um ein Drücken der Tastenbilder zu simulieren, und wobei das Drücken der Tastenbilder veranlasst, dass dieselben Befehle an die Pumpenprozessoreinrichtung (41 ) geschickt werden, wie sie nach Drücken der entsprechenden Tasten auf der Tastatur geschickt werden. - Pumpenkommunikationssystem nach Anspruch 7, bei dem (a) die Tastatur (
44 ) so konfiguriert und eingerichtet ist, dass sie im Ansprechen auf ihre Betätigung Tastatursignale generiert; (b) die Prozessoreinrichtung des Computersystems (80 ) so konfiguriert ist, dass sie im Ansprechen auf eine gezielte Veränderung des Bilds (92 ) Tastatursignale generiert; und (c) die Kommunikationseinrichtung (28 ,48 ,100 ) so konfiguriert ist, dass sie die von der Prozessoreinrichtung des Computersystems generierten Tastatursignale an die Pumpenprozessoreinrichtung (41 ) übermittelt. - Pumpenkommunikationssystem nach Anspruch 1, bei dem die Pumpe (
20 ,40 ,62 ,64 ) in einer untergeordneten Betriebsart betrieben werden kann, bei der die Pumpenprozessoreinrichtung (41 ) über die Tasten eingegebene Befehle aus dem Computersystem (80 ) entgegennimmt. - Pumpenkommunikationssystem nach Anspruch 9, bei dem die Pumpe (
20 ,40 ,62 ,64 ) darüber hinaus eine Anzeige (26 ) umfasst, um Daten anzuzeigen, die aus der Pumpenprozessoreinrichtung (41 ) eingegangen sind, wobei die Pumpenprozessoreinrichtung die Anzeigedaten an das Computersystem (80 ) zur Anzeige auf der Anzeigeeinrichtung (90 ) schickt, wenn sich die Pumpe in der untergeordneten Betriebsart befindet. - Pumpenkommunikationssystem nach Anspruch 10, bei dem die Pumpenprozessoreinrichtung (
41 ) so programmiert ist, dass sie ein Sperrprogramm umfasst. - Pumpenkommunikationssystem nach Anspruch 9, bei dem die Pumpenprozessoreinrichtung (
41 ) in der untergeordneten Betriebsart über die Kommunikationseinrichtung (28 ,48 ,100 ) Tastenanschlagsignale aus der Prozessoreinrichtung erhält. - Pumpenkommunikationssystem nach Anspruch 1, bei dem die Pumpe (
20 ,40 ,62 ,64 ) eine Anzeige (26 ) zum Anzeigen von Daten umfasst, und das Bild (92 ) darüber hinaus ein Anzeigebild (95 ) umfasst, das die Anzeige (26 ) symbolisiert. - Pumpenkommunikationssystem nach Anspruch 1, bei dem die Kommunikationseinrichtung (
28 ,48 ,100 ) ein Telefonmodem umfasst.
