- BEREICH DER ERFINDUNG
- Die vorliegende Erfindung bezieht sich im Allgemeinen auf eine Befestigungsvorrichtung zum Befestigen von Sehnen an Knochen und im Besonderen an eine chirurgische Schraube, die eine Unterlegscheibe zum Festklemmen von Sehnen innerhalb vorgeformter Löcher im Knochen aufweist.
- ALLGEMEINER STAND DER TECHNIK
- Es ist bekannt, dass beschädigte anteriore Kreuzbänder durch Transplantieren eines Teils von Patellasehne, die mit Knochenblöcken, die an jedem Ende physiologisch verbunden sind, entnommen wurde, zwischen den Femur und die Tibia am Kniegelenk repariert werden können. Die entnommenen Knochenblöcke werden typischerweise in vorgebohrten Löchern im Femur und in der Tibia positioniert.
- Dieses Verfahren der Reparatur von Bändern ist oft ein schwieriger und zeitaufwendiger Eingriff, der mit anschließender Sorge um das Wohlbefinden und die Sicherheit des Patienten einhergeht. Das Entnehmen von Patellasehne an sich kann auch Probleme für den Patienten verursachen. Das Sehnentransplantat ist bei vielen Patienten auch oft nicht in geeigneter physiologischer Verfassung, um verwendet zu werden. Deshalb ist es wünschenswert, Sehnen ohne den daran befestigten Knochen zu entnehmen und die Sehne dann an der erforderlichen Stelle so zu positionieren, dass das Knochenwachstum die Sehne am Knochen fixiert.
- Ein hauptsächliches Erfordernis zur Fixierung ist, dass das Sehnentransplantat fest gehalten wird, ohne dass es sich innerhalb eines Knochenlochs bewegt oder kolbenartig bewegt.
- Derzeitige Verfahren zur Fixierung einer Sehne umfassen eine Klammer, um die herum die Sehne positioniert wird, bevor die Klammer innerhalb des Knochens platziert wird. Dieses Verfahren hat zwei hauptsächliche Nachteile. Erstens wird, auf der Femurseite, die Klammer an der äußeren Fläche des Femurs angebracht und erfordert daher einen weiteren Einschnitt. Zweitens ermöglicht die Elastizität der Sehne ein kolbenartiges Bewegen unter Belastung, da die Fixierung außerhalb des Femurs erfolgt und die Sehne in einem Knochentunnel von ungefähr 40 mm Länge liegt und deshalb kann Heilung des Knochens des Sehnentransplantats ausbleiben oder eine schwache, faserige Heilung sein, die späteres Lösen ermöglicht.
- Eine Knochenklammer, die zwei durch ein Querelement verbundene parallele Beine beinhaltet, ist aus WO90/00370 bekannt.
- Typische chirurgische Befestigungsvorrichtungen bestehen aus Geräten zur Befestigung eines Endes eines Sehnentransplantats an einem Knochen, wobei in dem Knochen ein Loch gebildet ist, um die chirurgische Befestigungsvorrichtung aufzunehmen, wobei die chirurgische Befestigungsvorrichtung eine Schraube ist, die einen Kopf, einen Hals und einen Schaft mit einem Ende, das sich distal zum Kopf befindet, aufweist, wobei der Schaft ein Gewinde, das an mindestens einem Teil seiner Länge gebildet ist, aufweist, wobei der Kopf im Allgemeinen halbkugelförmig ist und einen Durchmesser aufweist, der größer als der Schaft ist, und wobei eine ringförmige Unterlegscheibe um den Hals der zwischen dem Kopf und dem Schaft liegenden Schraube angeordnet ist, wobei die Unterlegscheibe so angepasst ist, dass bei Gebrauch das eine Ende der Sehne an dieser befestigt und im Wesentlichen zwischen dem Kopf und dem Inneren des Lochs festgeklemmt werden kann.
- Im Längsschnitt an der Gesamtheit des Gewindes entlang, weist das Gewinde vorzugsweise keine äußerste Schnittlinie auf. In einer Ausführungsform weist die Schraube einen Schaft mit einem Gewinde auf, das vorzugsweise im Längsschnitt ungefähr oder vollständig sinusförmig ist, um ein glattes Gewinde ohne äußerste Schnittlinie darzustellen.
- Die Schraube weist vorzugsweise eine mit dem Kopf konzentrische Steuerhülse und eine zu dem Schaft konzentrische Längskanülierung auf.
- Bei anderen Befestigungsvorrichtungen ist die ringförmige Unterlegscheibe anliegend an den Kopf einer Schraube positioniert und weist vorzugsweise mindestens ein Loch in ihrem ringförmigen Teil auf. Die Unterlegscheibe weist noch besser vier Löcher in ihrem ringförmigen Teil auf, wobei die vier Löcher vorzugsweise auf eine Hälfte des ringförmigen Teils beschränkt sind.
- Bei anderen Befestigungsvorrichtungen ist ein aus Knochen hergestellter ringförmiger Pfropfen um den Hals der Schraube zwischen der ringförmigen Unterlegscheibe und dem Kopf angeordnet. Der Hals der Schraube ist bei diesen Befestigungsvorrichtungen vorzugsweise so ausgeführt, dass der Knochenpfropfen bei Verwendung der Schraube seitlich komprimiert wird.
- Befestigungsvorrichtungen können ein Sehenende an einem Knochen befestigen, was folgende Schritte beinhalten:
- (a) Bilden eines Lochs im Knochen, wobei das Loch ein erstes Ende und ein zweites Ende aufweist und zwei Teile beinhaltet, wobei das erste Teil anliegend an das erste Ende ist, wobei es einen Durchmesser aufweist, der größer als der Durchmesser des zweiten Teils ist, das sich distal zum ersten Ende befindet, so dass ein ringförmiges Widerlager im Loch gebildet wird;
- (b) Befestigen des einen Endes der Sehne an der Unterlegscheibe;
- (c) Ziehen der Unterlegscheibe mit der in dem Loch befestigten Sehne, so dass die Unterlegscheibe in der Nähe des ringförmigen Widerlagers positioniert ist; und
- (d) Schrauben der chirurgischen Schraube in das Loch, so dass die Unterlegscheibe mit dem ringförmigen Widerlager aneinander stößt und die Sehne festgeklemmt wird, wobei der Kopf der Schraube die Sehne mit dem Knochen innerhalb des ersten Teils des Lochs gegeneinander zwingt.
- Die Sehne weist vorzugsweise Nähte auf, die an ihrem einen Ende festgemacht sind, wobei die Nähte vorzugsweise an einem oder mehreren Löchern im ringförmigen Teil der Unterlegscheibe festgebunden sind. Die Nähte sind noch besser an vier Löchern in der Unterlegscheibe festgebunden, wobei die vier Löcher vorzugsweise auf eine Hälfte des ringförmigen Teils beschränkt sind.
- Die chirurgische Schraube ist vorzugsweise so lang, dass das eine Ende des Schafts den äußeren Kortex des Knochens erreicht, wenn die Schraube in den Knochen geschraubt ist. Die Schraube wird vorzugsweise unter Verwendung der Steuerhülse eingeführt.
- Der Knochen ist vorzugsweise ein Femur und das Loch im Knochen wird vorzugsweise in der Femurkondyle durch eine Bohrspitze gebildet.
- Die Schraube ist vorzugsweise kanüliert, um im Durchmesser größere Kirschnerdrähte oder dergleichen, die genaues arthroskopisches Führen der Schraube ermöglichen, aufzunehmen.
- Chirurgische Befestigungsvorrichtungen können ein Ende einer Sehne an einem Knochen befestigen, wobei sie die folgenden Schritte beinhalten:
- (a) Bilden eines Lochs im Knochen;
- (b) Befestigen des einen Endes der Sehne an der ringförmigen Unterlegscheibe;
- (c) Positionieren der Unterlegscheibe konzentrisch und in der Nähe des einen Endes des ringförmigen Knochenpfropfens;
- (d) Einführen der Unterlegscheibe und des Knochenpfropfens in das Loch unter Verwendung eines Einführmittels, so dass sich die Unterlegscheibe distal vom Eingang des Lochs befindet; und
- (e) Schrauben der chirurgischen Schraube in das Loch, so dass der Knochenpfropfen seitlich komprimiert wird, wodurch die Sehne zwischen der Wand des Lochs und einem wesentlichen Teil der Länge des Knochenpfropfens festgeklemmt wird.
- Wenn derartige Befestigungsvorrichtungen verwendet werden, wird es vorgezogen, dass der ringförmige Knochenpfropfen vor dem Einführen in das Knochenloch in einem Halter montiert wird, wobei der Halter ein erstes und zweites Ende aufweist und vorzugsweise den Knochenpfropfen umgibt. Bei einer anderen Befestigungsvorrichtung werden sowohl die Unterlegscheibe als auch der Knochenpfropfen in dem Halter montiert, so dass die an der Unterlegscheibe befestigte Sehne im Wesenlichen längs entlang der Länge des Knochenpfropfens liegt. Der Halter weist vorzugsweise eine Wölbung in der Nähe der Unterlegscheibe und des Knochenpfropfens auf, um die Sehne unterzubringen.
- Das Einführmittel beinhaltet vorzugsweise einen Tauchkolben, der am zweiten Ende des Halters, das ausgeführt ist, um den Knochenpfropfen und die Unterlegscheibe aus dem Halter in das Knochenloch hinauszutreiben, positioniert ist.
- Die ringförmige Unterlegscheibe, der Knochenpfropfen, der Halter und die chirurgische Schraube sind vorzugsweise auf einem Führungdraht, der innerhalb des Lochs zentral positioniert ist, positioniert.
- Die vorliegende Erfindung besteht aus einer chirurgischen Befestigungsvorrichtung, wobei die Befestigungsvorrichtung eine Klammer zum Befestigen eines Sehnentransplantats an einem Knochen ist, wobei die chirurgische Klammer zwei im Wesentlichen parallele Beine, die durch ein quergerichtetes Kopfelement miteinander verbunden sind, wobei die Beine jeweils im Wesentlichen gleich lang sind und dadurch gekennzeichnet sind, dass sie ein freies Endteil aufweisen, das im Wesentlichen in einem rechten Winkel mit einem Kopfteil verbunden ist, so dass das Kopfelement seitlich von den freien Endteilen versetzt ist, und eine Unterlegscheibe, die um die freien Endteile in der Nähe der rechtwinkligen Verbindung mit den Kopfteilen angeordnet ist, beinhaltet.
- Die chirurgische Klammer weist vorzugsweise zwei parallele Beine auf, wobei die Beine jeweils gleich lang sind und ein freies Endteil aufweisen, das in einem rechten Winkel mit einem Kopfteil verbunden ist.
- Das quergerichtete Kopfelement, das die zwei Kopfteile miteinander verbindet, ist vorzugsweise von der Ebene der Kopfteile aus nach unten hin abgefast.
- Die chirurgische Klammer beinhaltet vorzugsweise ein lineares Element, das in die erforderliche Form gefaltet ist. Das lineare Element ist vorzugsweise ein Draht, dessen freie Endteile vorzugsweise eine scharfe Pfeilspitze aufweisen, um das Einführen der Klammer in den Knochen zu erleichtern. Die Teile des Drahts, die voraussichtlich mit der Sehne in Kontakt kommen, sind vorzugsweise abgerundet, um einer Beschädigung der Sehne vorzubeugen. Der Draht ist vorzugsweise ein biologisch inertes Metall.
- Die Unterlegscheibe weist vorzugsweise eine ovale Form mit abgerundeten Kanten auf, um einer Beschädigung des Sehnentransplantats vorzubeugen. Es gibt vorzugsweise für jedes Bein zwei seitlich versetzte Löcher in der Unterlegscheibe. Die Unterlegscheibe ist vorzugsweise aus einem biologisch inerten Metall hergestellt.
- In einer weiteren Ausführungsform beinhaltet die vorliegende Erfindung ein Verfahren zum Befestigen einer Sehnenschlaufe an einem Knochen unter Verwendung der chirurgischen Befestigungsvorrichtung gemäß dem dritten Aspekt der Erfindung, das die folgenden Schritte beinhaltet:
- (a) Positionieren der Unterlegscheibe durch die Schlaufe der Sehne;
- (b) Einführen des freien Endteils der Klammerbeine durch die Unterlegscheibe, um die Sehnenschlaufe innerhalb der Klammer zu positionieren; und
- (c) Einführen der Klammer in den Knochen, so dass die Sehne an dem Knochen sowohl unter der Unterlegscheibe als auch unter dem seitlich versetzten Kopfteil sicher festgemacht ist, so dass jegliches Kippen der Klammer unter Belastung dazu dienen wird, das Kopfelement noch mehr auf die Sehne zu drücken, wodurch sich die Sehne und die Klammer weniger vom Knochen lösen.
- Die Klammer wird vorzugsweise in den Knochen an der erforderlichen Stelle unter Verwendung eines angemessenen hammerartigen Werkzeugs eingeführt.
- Lediglich mittels Beispielen werden Beispiele von chirurgischen Verschlussmitteln unter Bezugnahme auf die beiliegenden Zeichnungen beschrieben werden, in denen;
-
1 eine Längsschnittansicht einer chirurgischen Befestigungsvorrichtung ist; -
2 eine Draufsicht einer ringförmigen Unterlegscheibe ist; -
3 eine Längsschnittansicht der chirurgischen Schraube und der Unterlegscheibe aus1 bzw.2 ist, nach dem Einführen in einen Knochen; -
4 eine Längsschnittansicht einer chirurgischen Befestigungsvorrichtung ist; -
5 eine Längsschnittansicht der chirurgischen Schraube aus4 ist, nach dem Einführen in einen Knochen; -
6 eine Längsschnittansicht des Knochenpfropfenhalters ist; -
7 eine Längsschnittansicht eines weiteren Knochenpfropfenhalters ist; -
8 ein Aufriss vom Ende eines Halters aus7 ist; und -
9 eine Perspektivansicht der vorliegenden Erfindung ist. - Eine chirurgische Schraube zum Befestigen eines Sehnentransplantats an einem Knochen ist im Allgemeinen als
10 in1 gezeigt. - Die chirurgische Schraube
10 umfasst einen im Allgemeinen halbkugelförmigen Kopf11 , einen Hals12 und einen Schaft13 . Der Schaft13 weist ein Gewinde14 auf, das im Längsschnitt ungefähr sinusförmig ist, um ein glattes Gewinde ohne äußerste Schnittlinie darzustellen. Eine sechseckige Drehinbusfassung15 ist innerhalb des Kopfs11 konzentrisch positioniert. Die Schraube10 weist auch eine zentrale Kanülierung16 entlang der Länge der Schraube10 auf. - Eine ringförmige Unterlegscheibe
17 ist um den Hals12 der Schraube10 angeordnet. Wie in2 deutlicher abgebildet, weist die ringförmige Unterlegscheibe17 vier Löcher18 in ihrem ringförmigen Teil19 auf. Die Löcher18 sind auf eine Hälfte des ringförmigen Teils19 beschränkt. - Die Position der chirurgischen Schraube
10 beim Befestigen einer Sehne an einem Knochen ist im Allgemeinen in3 abgebildet. - Um eine Sehne unter Verwendung der Schraube
10 und der Unterlegscheibe17 aus1 und2 zu befestigen, wird zuerst ein zwei Teile22 und23 beinhaltendes Loch21 in den Knochen24 gebohrt. Das erste Teil22 weist einen Durchmesser auf, der größer als der des zweiten Teils23 ist, so dass in dem Loch21 ein ringförmiges Widerlager25 gebildet wird. - Eine Sehne
26 , an deren einem Ende (nicht abgebildet) Nähte festgemacht sind, wird dann unter Verwendung der Löcher18 an der Unterlegscheibe17 festgebunden. Die Unterlegscheibe17 mit der daran befestigten Sehne26 wird dann um den Führungsdraht27 in das Loch21 gezogen, so dass die Unterlegscheibe17 in der Nähe des ringförmigen Widerlagers25 positioniert wird. - Die chirurgische Schraube
10 wird dann unter Führung durch den zentralen Führungsdraht27 in das Loch21 geschraubt, so dass die Unterlegscheibe17 und das ringförmige Widerlager25 gegeneinander gezwungen werden. Die Sehne26 wird auch gegen die Wand des ersten Teils22 des Lochs21 durch den Kopf11 der Schraube10 festgeklemmt. - Eine zweite Ausführungsform einer chirurgischen Schraube ist im Allgemeinen als
30 in4 gezeigt. - Die chirurgische Schraube
30 umfasst einen Kopf31 , einen spitz zulaufenden Hals32 und einen Schaft33 . Der Schaft33 weist ein Gewinde34 auf, das im Längsschnitt ungefähr sinusförmig ist, um ein glattes Gewinde ohne äußerste Schnittlinie darzustellen. Eine sechseckige Steuerhülse35 ist innerhalb des Kopfs31 konzentrisch positioniert. Die Schraube30 weist auch eine zentrale Kanülierung36 entlang der Länge der Schraube30 auf. - Eine ringförmige Unterlegscheibe
37 ist um den Hals32 der Schraube30 angeordnet. Ein ringförmiger Knochenpfropfen38 ist um den Hals32 zwischen der Unterlegscheibe37 und dem Kopf31 angeordnet. - Die Position der chirurgischen Schraube
30 beim Befestigen einer Sehne an einem Knochen ist im Allgemeinen in5 abgebildet. - In dieser Ausführungsform, wenn die Schraube
30 in das Loch39 im Knochen41 eingeschraubt wird, wird der Knochenpfropfen38 seitlich komprimiert und klemmt dadurch die Sehne26 , die an der Unterlegscheibe37 befestigt ist, gegen die Wand des Lochs39 fest. Das Festklemmen der Sehne26 zwischen dem Knochenpfropfen38 und der Wand des Lochs39 gewährleistet eine gute Fixierung der Sehne26 am Knochen41 und reduziert somit das kolbenartige Bewegen der Sehne und verkürzt den Heilungszeitraum. - Um den Knochenpfropfen
38 und die Unterlegscheibe37 in das Loch39 einzuführen, wird eine Vorrichtung, die im Allgemeinen als 50 in6 gezeigt ist, angewendet. - Vor dem Einführen des Knochenpfropfens
38 und der Unterlegscheibe37 in das Loch39 wird ein Führungsdraht51 innerhalb des Lochs39 zentral positioniert. Ein Halter52 wird dann auf dem Führungsdraht51 positioniert. Der Halter52 ist eine zylindrische Muffe, in die der Knochenpfropfen38 eingeführt wird. Der Knochenpfropfen38 ist kanüliert und gleitet deshalb entlang dem Führungsdraht51 in den Halter52 . - Eine Unterlegscheibe
37 , an der eine Sehne26 befestigt ist, wird dann über den Führungsdraht51 geglitten und in der Nähe eines Endes53 des Halters52 positioniert. Ein Einführmittel, das einen auf dem anderen Ende55 des Halters52 montierten Tauchkolben54 beinhaltet, treibt den Knochenpfropfen38 aus dem Halter52 in das Loch39 entlang dem Führungsdraht51 . Die Unterlegscheibe37 , an der die Sehne26 befestigt ist, wird vor dem Knochenpfropfen38 positioniert und wird deshalb vor dem Knochenpfropfen38 in das Loch39 eingeführt. - Sobald die Unterlegscheibe
37 und der Knochenpfropfen38 in dem Loch39 positioniert sind, wird der Tauchkolben54 zurückgezogen, und der Halter52 wird vom Führungsdraht51 entfernt. - Nach dem Entfernen des Halters
52 wird die chirurgische Schraube30 über den Führungsdraht51 geglitten und in das Loch39 eingeschraubt. Der Hals32 der Schraube30 komprimiert den Knochenpfropfen38 und klemmt dadurch die Sehne26 zwischen dem Knochenpfropfen38 und der Wand des Lochs39 fest. - Eine andere Ausführungsform des Halters
52 ist in7 und8 abgebildet. In dieser Ausführungsform des Halters52 ist auf einer Seite der Muffe des Halters52 eine Wölbung55 bereitgestellt. Bei Gebrauch dieser Ausführungsform des Halters52 werden der Knochenpfropfen38 sowie die Unterlegscheibe37 , an der eine Sehne26 befestigt ist, in den Halter52 eingeführt. Die Sehne26 ist im Wesentlichen längs entlang dem Knochenpfropfen38 positioniert und ist innerhalb der Wölbung55 des Halters52 untergebracht. - Eine Ausführungsform der vorliegenden Erfindung ist im Allgemeinen als
60 in9 gezeigt. - Die chirurgische Klammer
60 umfasst zwei parallele Beine61 , die durch ein abgerundetes, quergerichtetes Kopfelement62 miteinander verbunden sind. Jedes der Beine61 beinhaltet ein freies Endteil63 und einen Kopfteil64 , die in rechten Winkeln65 verbunden sind, so dass das Kopfelement62 seitlich von dem freien Endteil63 der Beine61 versetzt ist. Die chirurgische Klammer60 umfasst auch eine um die freien Endteile63 in der Nähe der rechten Winkel65 in den Beinen61 angeordnete Unterlegscheibe66 . - Die freien Endteile weisen eine pfeilförmige Spitze
67 auf, um das Einführen der Klammer60 in den Knochen zu erleichtern. - Die Unterlegscheibe
66 weist eine ovale Form sowie abgerundete Kanten68 auf, um einer Beschädigung der Sehne26 , die um die Unterlegscheibe66 geschlungen ist, vorzubeugen. - Um die Sehne
26 an einem Knochen festzumachen, wird die Klammer60 in einen Knochen eingehämmert, wobei die Sehne26 an der Stelle der Unterlegscheibe66 sowie des quergerichteten Kopfelements62 festgemacht wird. - Jedes Lösen der Klammer
60 unter Belastung dient dazu, dass das quergerichtete Kopfelement62 noch mehr auf die Sehne26 gedrückt wird, wodurch sich die Sehne26 weniger vom Knochen löst. - Fachleuten werden verstehen, dass zahlreiche Variationen und/oder Modifizierungen an der Erfindung, wie in den Ausführungsformen gezeigt, vorgenommen werden können, ohne den Sinn oder Bereich der Erfindung, wie allgemein beschrieben, zu verlassen. Die vorliegenden Ausführungsformen sollen daher in jeglicher Hinsicht als veranschaulichend und nicht beschränkend gesehen werden.
Claims (6)
- Eine chirurgische Befestigungsvorrichtung, wobei die Befestigungsvorrichtung eine Klammer zum Befestigen eines Sehnentransplantats (
26 ) an einem Knochen ist, wobei die chirurgische Klammer (60 ) zwei im Wesentlichen parallele Beine (61 ), die durch ein quergerichtetes Kopfelement (62 ) miteinander verbunden sind, wobei die Beine jeweils im Wesentlichen gleich lang sind und dadurch gekennzeichnet sind, dass sie ein freies Endteil (63 ) aufweisen, das im Wesentlichen in einem rechten Winkel (65 ) mit einem Kopfteil (64 ) verbunden ist, so dass das Kopfelement seitlich von den freien Endteilen (63 ) versetzt ist, und eine Unterlegscheibe (66 ), die um die freien Endteile (63 ) in der Nähe der rechtwinkligen Verbindung (65 ) mit den Kopfteilen (64 ) angeordnet ist, beinhaltet. - Chirurgische Befestigungsvorrichtung gemäß Anspruch 1, wobei die chirurgische Klammer (
60 ) zwei parallele Beine (61 ) aufweist, wobei die Beine jeweils gleich lang sind und ein freies Endteil (63 ) aufweisen, das in einem rechten Winkel (65 ) mit einem Kopfteil (64 ) verbunden ist. - Chirurgische Befestigungsvorrichtung gemäß Anspruch 1 oder 2, wobei das quergerichtete Kopfelement (
62 ), das die zwei Kopfteile miteinander verbindet, von der Ebene der Kopfteile aus nach unten hin abgefast ist. - Chirurgische Befestigungsvorrichtung gemäß einem der Ansprüche 1–3, wobei die chirurgische Klammer ein lineares Element beinhaltet, das in die erforderliche Form (
60 ) gefaltet ist. - Chirurgische Befestigungsvorrichtung gemäß Anspruch 4, wobei das lineare Element ein Draht ist, wobei die freien Endteile eine scharfe Pfeilspitze (
67 ) aufweisen, um das Einführen der Klammer in den Knochen zu erleichtern, und die Teile des Drahts, die voraussichtlich mit der Sehne in Kontakt kommen, abgerundet sind, um einer Beschädigung der Sehne vorzubeugen. - Chirurgische Befestigungsvorrichtung gemäß einem der Ansprüche 1–5, wobei die Unterlegscheibe (
66 ) eine ovale Form mit abgerundeten Kanten (68 ) aufweist, um einer Beschädigung des Sehnentransplantats vorzubeugen, und für jedes Bein zwei seitlich versetzte Löcher aufweist.
