Description
- Bei der Behebung von Schäden des Skelettgewebes besteht eine Geweberegenerationstherapie im lokalen Aufbringen von autologen (vom Wirt abgeleiteten) Zellen, um die Rekonstruktion von Gewebedefekten, die durch Traumata, Krankheiten oder chirurgische Maßnahmen hervorgerufen worden sind, zu fördern. Das Ziel der Geweregenerationstherapie besteht in der Bereitstellung von reparaturkompetenten Zellen (oder Zellen, die durch Beeinflussung der lokalen Umgebung kompetent werden können) in hoher Dichte an der Defektstelle in einem Format, das die anfängliche Wundmechanik und schließlich die Bildung von neuem Gewebe optimiert. Bei der Reparatur von weichem Gewebe besteht die Wahrscheinlichkeit, dass ein oder mehrere Implantatträger erforderlich sind, um (1) die autologen Zellen zu transportieren und an der Defektstelle festzuhalten und (2) eine anfängliche mechanische Stabilität an der chirurgischen Stelle bereitzustellen. Bei einem optimalen System ist es wahrscheinlich, dass der Träger langsam biologisch mit einer Geschwindigkeit abgebaut wird, die mit der Geschwindigkeit der Bildung von neuem Gewebe und der Entwicklung einer festen Beschaffenheit im Reparaturgewebe vergleichbar ist (1 ).
- Der Weg der Geweberegenerationstherapie steht im deutlichen Gegensatz zu stärker passiven Möglichkeiten zur Wundreparatur, bei denen kein Versuch gemacht wird, Reparaturzellen zur Defektstelle zu bringen oder dort festzuhalten. Beispielsweise hängt im Fall einer Reparatur des vorderen Kreuzbandes (ACL) mit synthetischen (vermutlich "inerten") polymeren Transplantaten der Heilungsvorgang vollständig von lokalen zellulären Reaktionen ab, um die Einverleibung eines permanenten Implantats einzuleiten und zu steuern (
2 ). - Kürzlich wurden aktivere Vorrichtungen unter Verwendung von Matrixgerüsten getestet, die so konzipiert waren, zelluläre Prozesse auszulösen und/oder zu dirigieren. Hierzu gehören beispielsweise die Sehnen- oder ACL-Reparatur (
3 –7 ), die Meniskus-Reparatur (8 –11 ) und die Gelenkknorpel-Reparatur (12 –15 ). Alternativ wurde auch die Verwendung von lokal abgegebenen Peptidfaktoren untersucht, die dazu vorgesehen waren, die Schaffung von Reparaturzellen und deren Anhaften und/oder Differenzierung zu stimulieren (16 –19 ). - Bei den bisher vermutlich am besten dokumentierten Versuchen zur Sehnenreparatur beschrieben Silver, Dunn und Mitarbeiter eingehende Untersuchungen des Verhaltens von Kollagenfaser-Prothesen für die Reparatur der Achilles-Sehne (
3 –5 ) und das vordere Kreuzband (ACL) bei Kaninchen (6 ,7 ). Diese Autoren berichten, dass beim Defekt der Achilles-Sehne52 Wochen nach der Implantation die rekonstruierte Sehne (Prothese + Reparaturgewebe) eine Festigkeit von etwa 66% des normalen Gewebes für alle getesteten Implantate aufwies, einschließlich eines autologen Sehnentransplantats und mit Glutaraldehyd oder Carbodiimid vernetzte Kollagenfaser-Verbundstoffe (5 ). Sowohl bei den autologen Implantaten als auch bei den mit Carbodiimid vernetzten Prothesen wurde ein rascher biologischer Abbau festgestellt, wonach mit der Bildung von neuem Gewebe die Festigkeit rasch wiedergewonnen wurde. Mit Glutaraldehyd vernetzte Prothesen zeigten einen wesentlich langsameren biologischen Abbau beim Modell der Achilles-Sehne, wobei in der Umgebung eine dicke Kapsel entstand, die schließlich den Abbauvorgang stoppte. Die bei diesen Untersuchungen entwickelte neue Sehne war zwar der normalen Sehne ähnlich, jedoch nicht identisch. Beispielsweise war der Kräuselungswinkel (crimp angle) des Neusehnen-Kollagens bei sämtlichen Implantaten ähnlich wie bei einer normalen Sehne, jedoch betrug die Länge der Neusehnen-Kräuselung bei sämtlichen Kollagen-Prothesevorrichtungen weniger als etwa 30% des Normalwerts. Ferner waren die Modulwerte der aus den sich rascher abbauenden Implantaten gebildeten neuen Sehnen (autologe Sehnen und mit Carbodiimid vernetzte Kollagenfasern) deutlich niedriger als bei normalen Sehnen. Schließlich wurde festgestellt, dass sich die neuen Sehnen nicht mit der Bündel-Mikroarchitektur von normalen Sehnen vereinigten. Diese Arbeitskreise zogen den Schluss, dass die Geschwindigkeit des Abbaus der Prothese und die anschließende Übertragung der Belastung auf das neue Gewebe möglicherweise bei der Festlegung der letztendlichen Eigenschaften des Reparaturgewebes ebenso wichtig wie die anfängliche Zugfestigkeit der Prothese sind (5). Eine ähnliche Bildung von neuen Bändern wurde bei ACL-Implantaten nach 20 Wochen festgestellt, obgleich die Wiedergewinnung der Festigkeit des Gewebes in der gefäßlosen Synovialumgebung möglicherweise etwas langsamer ist. - Auf der Grundlage dieser Befunde ist es klar, dass zumindest beim gesunden Tier reparaturkompetente Zellen aus den die Defekte in Sehnen und Bändern umgebenden Geweben gewonnen werden können und dass diese Zellen die Bildung von neuem Gewebe einleiten. Aus diesen Untersuchungen geht aber nicht klar hervor, in welchem Umfang die gewonnenen Zellen differenzierte Phänotypen (z. B. Sehnen-Fibroblasten) repräsentierten, im Gegensatz zu undifferenzierten pluripotenten Stammzellen, oder ob eine erhöhte Anzahl von derartigen Zellen die Geschwindigkeit der Synthese oder die Mikroarchitektur und die mechanischen Eigenschaften des neu gebildeten Gewebes verstärkt.
- Zahlreiche zellvermittelte Vorgänge, die mit der Bildung von Skelettgewebe im Zusammenhang stehen, hängen von der Anzahl der beteiligten Zellen ab, und zwar sowohl in Bezug auf die Geschwindigkeit als auch auf die Größe des Effekts. Beispielsweise hängt bei der in vitro-Bildung von Bindegewebe die Geschwindigkeit der Kollagen-Gelkontraktion durch in das Gel eingebettete Fibroblasten von der Anzahl der in der Kultur vorhandenen Zellen ab (20). Eine ähnliche gelkontrahierende Aktivität steht auch im Zusammenhang mit der von der Zelldichte abhängigen Sekretion eines kontraktionsfördernden Faktors durch Endothelzellen (
21 ). Ferner steht das Ausmaß der Fibroblastenorientierung in Kulturen, die auf Kollagengelen gezüchtet worden sind, in direktem Zusammenhang mit der anfänglichen Zelldichte (22 ). Dieser Zellorientierungseffekt korreliert mit der Feststellung von "Organisationszentren" in der Kultur, deren Anzahl als direkter Indikator der morphogenetischen Kapazität auf molekularem und zellulärem Niveau angesehen wurde (23 ). - Eine von der Zelldichte abhängige Differenzierung wurde klar in einer Kultur von Hühnergliedmaßen-Keimzellen gezeigt (
24 ). Bei Züchtung in einer sehr geringen Dichte (106 Zellen/35 mm-Schale) zeigten diese Zellen keine chondrogenen oder osteogenen Eigenschaften. Bei "mittleren" Zellkulturdichten (2 × 106 Zellen/35 mm-Schale) wiesen die Zellen die maximale Frequenz der Osteogenese auf, wobei bei einer noch höheren Dichte (5 × 106 Zellen) die maximale Frequenz von Chondrozyten-Phänotypen beobachtet wurde. - Bei jedem der vorgenannten Fälle beeinflusst die Anzahl der ursprünglich vorhandenen Zellen in starkem Maße die Natur von zellvermittelten Vorgängen, die bei der Skelettgewebebildung beteiligt sind, und die Geschwindigkeit, mit der diese Entwicklungsvorgänge und physiologischen Vorgänge ablaufen. Daher haben Caplan und Mitarbeiter in Bezug auf Reparaturvorgänge von Skelettgewebe die Hypothese aufgestellt, dass ein gewisser minimaler Schwellenwert der Zellzahl an der Reparaturstelle erforderlich ist, bevor es zur Bildung von "normalem" neuen Gewebe kommen kann (25). Ferner kann in zahlreichen Fällen dieser minimale Schwellenwert die Anzahl an gewinnbaren Reparaturzellen übersteigen, unter Einschluss von weniger involvierten Zellen, die zu reparaturkompetenten Phänotypen differenzieren können. Daher kann das Ausmaß, in dem der Reparaturvorgang erfolgen kann, durch diesen einzigen Parameter eingeschränkt sein.
- Voruntersuchungen über den Weg der Geweberegenerationstherapie haben in letzter Zeit zu einem Sehnen-Reparaturmodell der Achilles-Sehne des Kaninchens geführt (
25 ). Drei Komponenten trugen zu diesem Modell bei: der Defekt, die Zellen und der Träger zur Abgabe der Zellen an der Defektstelle. Der Abgabeträger bei diesem Modell muss die Zellen an der Defektstelle zurückhalten, die Gewebemechanik stabilisieren und sodann mit Bildung von neuem Gewebe einem langsamen biologischen Abbau unterliegen. - US-5 061 283 beschreibt eine Vorrichtung, die sich zur Reparatur einer gerissenen oder verletzten Sehne, insbesondere einer Beugesehne der Hand, eignet und einen Körper in Form eines flachen Bandes aufweist, wobei die gegenüberliegenden Enden des Körpers so konzipiert sind, dass sie zur Verankerung von Verbindungsnähten dienen. Eine derartige Vorrichtung umfasst einen länglichen Körperbereich mit einer Struktur in Form eines flachen Bandes, wobei der Körperbereich an den gegenüberliegenden Enden zur Verbindung mit mindestens einem Nadel-Nahtmaterial geeignet ist. Der Körper der Vorrichtung und die damit verbundene Naht (oder Nähte) können mit einem oder mehreren Gelüberzügen bedeckt sein, z. B. mit Calciumalginat. Zur Reparatur einer beschädigten Sehne unter Verwendung der Vorrichtung gemäß US-5 061 283 müssen Schlitze in die einzelnen Enden der geschädigten Sehne eingeschnitten werden, wobei die Schlitze jeweils zur Aufnahme eines Teils der Vorrichtung dienen, die sodann durch Annähen der Vorrichtung an die Sehne befestigt werden. US-5 061 283 erwähnt keine lebenden Zellen als Bestandteil der beschriebenen Vorrichtung oder zu deren Herstellung.
- US-5 226 914 beschreibt allgemein verschiedene Verfahren und Vorrichtungen zur Verwendung von isolierten und in Kultur vermehrten, von Knochenmark abgeleiteten Mesenchymzellen (mesenchymale Stammzellen) zur Behandlung von Störungen des Skelettgewebes und anderer Bindegewebe. In dieser Druckschrift wird ausgeführt, dass die vorerwähnten mesenchymalen Stammzellen unter bestimmten Bedingungen zur Differenzierung in verschiedene Typen von erwünschtem Bindegewebe, wie Knochen oder Knorpel bildende Zellen, induziert werden können. Als Faktoren, die die Differenzierung der in Kultur vermehrten, von Knochenmark abgeleiteten Mesenchymzellen induzieren, werden mechanische, zelluläre und biochemische Reize aufgeführt. Jedoch handelt es sich bei der einzigen Differenzierung von mesenchymalen Stammzellen, die in US-5 266 914 beschrieben ist, um die Differenzierung zu Knochen, die an poröser Calciumphosphat-Keramik ohne Bildung von Knorpel erfolgte.
- Die vorliegende Erfindung betrifft ein Implantat zur Reparatur eines Gewebedefekts gemäß den Angaben in Anspruch 1.
- Ein bevorzugtes Material für die in Anspruch 1 verwendete Gelmatrix besteht aus gereinigten Kollagenfibrillen vom Typ I. Gleichermaßen können aber auch andere Materialien verwendet werden, z. B. 1) zellkontrahierte Kollagengele mit einem Gehalt an anderen Komponenten der extrazellulären Matrix, wie Proteoglycane oder Glycosaminoglycane, Glycoproteine (Fibronectin, Laminin und dergl.), andere Kollagene, (z. B. die Typen II oder IV), Elastin und/oder bioaktive Peptid-Wachstumsfaktoren oder Cytokine; 2) andere Biopolymere, wie Fibrin; 3) synthetische, biologisch abbaubare Fasern aus Polymeren, wie Polymilchsäure oder Polyglykolsäuren, Polycaprolactone oder Polyaminosäuren oder deren Copolymere, die zu Nahtmaterial gegossen oder gewickelt oder auf Nahtmaterial gegossen oder gewickelt werden können; oder 4) Verbundstrukturen, wie Kollagen/Polymilchsäure-Strukturen.
- Neben einfachen Einzelfilament-Nahtmaterialien können auch Multifilament-Vorrichtungen, die durch Flechten, Weben oder Wirken von biologisch abbaubaren faserigen Materialien, einschließlich Nahtmaterialien oder Kollagenfasern oder dergl., hergestellt worden sind, verwendet werden. Zellen können allgemein an derartige Vorrichtungen durch zellvermittelte Vorgänge, wie Kontraktion von Kollagengelen, oder durch nicht-zellvermittelte physikalische oder chemische Verfahren, wie das Gießen eines Gelatinegels oder das Umwickeln der Vorrichtung mit zellhaltigen faserigen oder Membranenstrukturen, angeheftet werden. Derartige Implantationsvorrichtungen können eine oder mehrere Nadeln aufweisen, die an einem Ende der Vorrichtung angebracht sind, um die Befestigung an weichen oder harten Geweben zu erleichtern. Die Vorrichtungen können verschiedene geometrische Anordnungen aufweisen, einschließlich eine planare, zylindrische oder röhrenförmige Bauweise, je nach dem speziellen, zu reparierenden Gewebe, der Art der Befestigung des Implantats und/oder dem Verfahren, das zum Anheften der zellhaltigen Biomatrixkombination an die Implantationsvorrichtung herangezogen wird.
- Die vorliegende Erfindung betrifft ein Implantat zur Verwendung in einem Verfahren zur Implantation eines beliebigen Typs von Zellen, die die Gewebereparatur herbeiführen. Obgleich die Erfindung auf keinen speziellen Zelltyp beschränkt ist, umfasst eine besonders bevorzugte Ausführungsform humane mesenchymale Stammzellen (MSCs) oder beliebige übertragene oder differenzierte Nachkommen davon. Die Zellen werden vorzugsweise von dem Tier gewonnen, für das das Implantat vorgesehen ist. Vorzugsweise werden sie vor der Implantation in Kultur vermehrt. Unter "Tier" ist vorzugsweise ein Mensch zu verstehen.
- Es wurden Verfahren zum Züchten von MSCs oder Sehnen-Fibroblasten an Doppelnadel-Dexon-Nahtmaterialien dargelegt, indem man die Zellen dazu veranlasst, Kollagengele um den Mittelbereich der Nahtmaterialien zu kontrahieren. Die autologe Zell/Kollagengel/Nahtmaterial-Verbundvorrichtung kann direkt zwischen den freien Enden von Defekten von voller Dicke an Sehnen, z. B. zur Reparatur der menschlichen Achilles-Sehne, Bändern, z. B. zur Reparatur des vorderen Kreuzbandes, oder Knorpeln, z. B. zur Reparatur von Gelenkknorpeln, Meniskus oder der Scheibe des mandibulotemporalen Gelenks, implantiert werden.
- In der in
1 dargestellten Ausführungsform umfasst ein Implantat1 einen Strang von Nahtmaterial2 und eine Gelmatrix4 , die Reparaturzellen6 enthält und um den Mittelbereich8 des Nahtmaterials2 kontrahiert worden ist. Das Nahtmaterial2 weist freie Enden10 und12 auf, die zur erneuten Verbindung des den Defekt benachbarten Gewebes T verwendet werden. Wie dargestellt, sind die freien Enden10 und12 an den Körper des Gewebes angenäht, wodurch nicht nur die Enden der Sehne an Ort und Stelle gehalten werden, sondern auch die Gelmatrix4 an der Defektstelle in Position gehalten wird. -
2 zeigt eine Formanordnung15 , die zur Bildung eines erfindungsgemäßen Implantats verwendet werden kann. Die Formanordnung15 umfasst eine Form16 , in der die zellhaltige Gelmatrix um das Nahtmaterial2 herum gebildet ist, wobei das Nahtmaterial hier an seinen Enden mit Nadeln3 und5 dargestellt ist. Ein Spanndraht18 hält das Nahtmaterial2 unter Spannung in der Form16 und in der Inkubationsschale20 , in der das Matrixpräparat inkubiert wird, um das Gel zu verfestigen. Eine spezielle Ausführungsform davon wird im nachstehenden Beispiel beschrieben. - Beispiel 1
- Eine Formanordnung wurde zur Herstellung eines Implantats zur erfindungsgemäßen Reparatur eines Gewebedefekts verwendet. Kleine Glaszylinder der Abmessungen 5 mm × 27 mm, deren Enden durch Abschmelzen verschlossen waren, wurden in Längsrichtung in der Mitte zerschnitten, um kanuförmige Formen aus Glas zu bilden. Steife chirurgische Drähte wurden zu kleinen, bogenförmigen Spanndrähten gebogen, wobei die Enden genau 2 mm tief in die Glasformen ragten. Die Glasform wurde in eine 100 mm-Kulturschale gelegt, wobei ein Nahtmaterial, das den Spanndraht überbrückte, in der Formmitte bei der Herstellung der zu gießenden Gelsuspension angeordnet war.
- Autologe mesenchymale Stammzellen (4 × 106 Zellen) wurden in 0,5 mm 2 × DMEM-LG suspendiert und gründlich vermischt, wodurch man eine Einzelzellsuspension erhielt. Anschließend wurde die Zellsuspension mit 0,5 ml sterilisierter Lösung von Kollagen vom Typ I (Pancogene SR, Gattefossé SA, Lyon, Frankreich; 3 mg/ml; dialysiert in 0,001 M HCl) versetzt und auf- und abpipettiert, um im Gel eine homogene Suspension von Zellen zu bilden. Diese Gelsuspension wurde sofort in die vorbereitete Glasform in der Kulturschale gegossen. Ein Deckel wurde auf die Schale gelegt. Die Schale wurde zur Verfestigung des Gels 15–20 Minuten in einen Inkubator von 37°C gelegt. Nach beendeter Gelbildung wurde die Schale mit Medium ohne Serum geflutet, bis die Glasform bedeckt war, und sodann erneut 4–6 Stunden in den Inkubator gestellt. Die Kontraktion des Gels durch die Zellen erfolgte so weit, dass sich das Gel von den Wänden der Form löste und eine Verringerung von Durchmesser und Länge um etwa 10% erfuhr. Wenn die Zellen in dieser Vorrichtung etwa 20 Stunden gezüchtet werden, kontrahiert sich das Gel auf etwa 60% seiner ursprünglichen radialen Abmessung. Nach 4 Stunden haftete das Gel fest an der Mittelnaht, so dass das Nahtmaterial und die Spannfeder aus dem Medium entfernt, die Spannfeder entnommen und das Gel gemäß den gemachten Angaben in den chirurgischen Defekt implantiert werden konnte.
- Nach diesem Verfahren hergestellte Gewebereparaturvorrichtungen wurden in ein Defektmodell der Achilles-Sehne des Kaninchens mit oder ohne eine Vicryl-Hülle implantiert. Histologische Beobachtungen dieser Implantate nach 1, 3 und 8 Wochen zeigen, dass sich durch dieses Verfahren neue Sehnengewebe bereits nach 1 bis 3 Wochen bilden. Diese im frühen Stadium auftretenden neuen Sehnengewebe weisen eine morphologische Ähnlichkeit mit Geweben auf, die aus Sehnenzellen- oder MSC-Implantaten im Vicryl-Hüllen-Reparaturmodell zu späteren Zeitpunkten gebildet wurden.

Claims (11)
- Implantat (
1 ) zur Reparatur eines Gewebedefekts in einem humanen Körper oder nicht-humanen Tierkörper, wobei das Implantat (1 ) folgendes umfasst: – ein physiologisch verträgliches Lasttrageelement (2 ) mit Mitteln zur Befestigung von Gewebe, das dem zu reparierenden Gewebe benachbart ist, unter Spannung, – um das eine kontrahierte Gelmatrix (4 ) angeordnet ist; und – mesenchymale Stammzellen (6 ) innerhalb der kontrahierten Gelmatrix (4 ), – wobei die Zellen (6 ) die Gelmatrix (4 ) kontrahiert haben; und – wobei die Gelmatrix (4 ) während des Anlegens einer Zugkraft an das physiologische lasttragende Element (2 ) kontrahiert worden ist. - Implantat (
1 ) nach Anspruch 1, wobei es sich bei der Gelmatrix (4 ) um ein Kollagengel handelt. - Implantat (
1 ) nach einem der Ansprüche 1 bis 2, wobei das Implantat (1 ) zur Behandlung eines Weichgewebedefekts in einem humanen Körper oder nicht-humanen Tierkörper geeignet ist. - Implantat (
1 ) nach Anspruch 3, wobei es sich beim Weichgewebedefekt um einen Sehnendefekt, einen Knorpeldefekt, einen Gelenkknorpeldefekt oder einen Bänderdefekt handelt. - Implantat (
1 ) nach Anspruch 1, umfassend eine faserige, abbaubare Fixiervorrichtung als lasttragendes Element (2 ), das an der Gelmatrix (4 ) angrenzt und an dieser zwischen einer ersten und zweiten Stelle, die im Abstand voneinander angeordnet sind, befestigt ist, so dass sie sich über den Defekt erstreckt. - Implantat (
1 ) nach Anspruch 5, wobei das lasttragende Element (2 ) zwischen der ersten und zweiten Stelle gestreckt ist. - Implantat (
1 ) nach Anspruch 5 oder 6, wobei es sich beim lasttragenden Element (2 ) um ein Nahtmaterial handelt. - Implantat (
1 ) nach Anspruch 5 bis 7, wobei das lasttragende Element (2 ) aus der Gruppe Einzelfilament, Mehrfachfilament, biologisch abbaubare faserige Materialien, gewebte Filamente, Kollagenfasern und mindestens ein flexibles, geschmeidiges Filament ausgewählt ist. - Implantat (
1 ) nach Anspruch 5 bis 8, wobei es sich beim lasttragenden Element (2 ) um mindestens ein Filament mit entsprechenden Enden (10 ,12 ) handelt, die sich über die erste und zweite Stelle hinaus erstrecken. - Implantat (
1 ) nach Anspruch 1, wobei das lasttragende Element (2 ) eine Zuglast aufweist, während die Gelmatrix (4 ) kontrahiert wird, wobei die Last auf das lasttragende Element (2 ) am ersten und zweiten Ende des lasttragenden Elements aufgebracht wird, wobei sich das Gel (4 ) zwischen diesen Enden befindet. - Implantat (
1 ) nach Anspruch 5 bis 10, wobei es sich beim lasttragenden Element (2 ) um ein Nahtmaterial handelt, dessen Enden (10 ,12 ) zur Befestigung am Gewebe (T) vorgesehen sind, wobei sich das Nahtmaterial und die Gelmatrix (4 ) über den Defekt erstrecken.
