Description
- Die vorliegende Offenbarung bezieht sich im Allgemeinen auf Vorrichtungen und Techniken zur Behandlung von Wirbelsäulenstörungen und im Besonderen auf eine künstliche Bandscheiben-Prothese, die anschließend an die Entfernung einer beschädigten oder erkrankten Bandscheibe sowohl die Höhe als auch die Form des Bandscheibenraumes wiederherstellt, wobei die natürlichen Biomechanismen des Wirbelsäulen-Bewegungssegmentes erhalten bleiben.
- Beschreibung des Stands der Technik
- Das Ziel eines Bandscheiben-Ersatzes ist es, eine Scheibenprothese zur Verfügung zu stellen, die sowohl Stabilität zur Unterstützung der hohen Belastung der Rückenwirbel des Patienten als auch Flexibilität zur Ausstattung des Patienten mit ausreichender Mobilität und geeigneter Wirbelsäulen-Belastungsverteilung kombiniert. Bei dem Versuch, diese Balance zu treffen, wurden im Allgemeinen vier Grundtypen von künstlichen Bandscheiben zur Ersetzung eines Teiles der oder der gesamten entfernten Scheibe entwickelt, und zwar Elastomer-Scheiben, Kugelgelenk-Scheiben, Mechanische Feder-Scheiben und Hybrid-Scheiben. Elastomer-Scheiben beinhalten typischerweise ein Elastomer-Kissen, welches zwischen eine obere und eine untere steife Endplatte eingelegt ist. Die Elastomer-Scheiben sind insofern vorteilhaft, als dass das Elastomer-Kissen im mechanischen Verhalten ähnlich wie das entfernte Bandscheiben-Gewebe wirkt. Jedoch liegt ein Nachteil dieses Scheibentyps darin, dass das Elastomer-Kissen längerfristig von Mikrobrüchen stammende in-vivo Probleme erleidet, die seine Brauchbarkeit als Ersatzmöglichkeit schmälern. Weiterhin zeigt die Befestigung des flexiblen Elastomer-Kissens an den steifen Endplatten zusätzliche Schwierigkeiten. Beispiele für Elastomer-Kissen sind in den U.S.-Patenten Nr. 5,702,450 von Bisserie; 5,035,716 von Downey; 4,874,389 an Downey; und 4,863,477 von Monson offenbart.
- Kugelgelenk-Scheiben beinhalten typischerweise zwei Plattenelemente, die zusammenarbeitende Kugelgelenk-Abschnitte aufweisen, welche eine deutliche/gelenkbildende (articulating) Bewegung der Elemente während der Bewegung der Wirbelsäule erlauben. Die Kugelgelenk-Anordnung ist in der Lage, die „Bewegung" der Wirbelsäule wiederherzustellen, aber in der Nachbildung der natürlichen Steifheit der Bandscheibe dürftig. Verrenkungen und Verschleiß sind weitere Sorgen bei diesem Scheibentyp. Beispiele für Kugelgelenk-Scheiben sind in den U.S.-Patenten Nr. 5,507,816 von Bullivant und 5,258,031 von Salib u. a. offenbart.
- Mechanische Federscheiben vereinen üblicherweise eine oder mehrere zwischen zwei metallenen Endplatten angeordnete Spiralfedern. Die Spiralfedern definieren vorzugsweise eine kumulative Federkraft die fortdauernd ausreicht, die räumliche Anordnung der benachbarten Bandscheiben aufrecht zu erhalten und normale Bewegungen der Bandscheiben während der Beugung und Streckung der Feder zu erlauben. Nachteile des mechanischen Federscheibentyps betreffen die Befestigung der Spiralfedern an den metallenen Endplatten und damit verbundenen Verschleiß an den Befestigungspunkten. Beispiele für mechanische Federscheiben sind in den U.S.-Patenten Nr. 5,458,642 von Beer u. a. und 4,309,777 von Patil offenbart.
- Der vierte Typ künstlicher Bandscheiben, nämlich der Hybrid-Typ, vereint zwei oder mehr Prinzipien der vorhergehend beschriebenen Scheibentypen. Beispielsweise enthält eine bekannte Hybrid-Scheiben-Anordnung einen von einem Elastomerring umgebenen Kugelgelenk-Satz. Diese Hybrid-Scheibe stellt einige Vorteile in Bezug auf die Lasttragefähigkeit zur Verfügung, ist aber im Allgemeinen kompliziert und erfordert eine Reihe von individuellen Komponenten. Weiterhin bleiben längerfristig in-vivo Schwierigkeiten mit den Elastomer-Kissen eine Sorge, ebenso wie ein Verschleiß der Kugelgelenk-Anordnung.
- Ein weiterer Typ von Bandscheiben-Prothese ist in dem U.S.-Patent Nr. 5,320,644 von Baumgartner offenbart. Mit Bezug auf die
1 –3 stellt die Baumgartner '644 – Vorrichtung ein aus einem kräftigen, elastisch verformbaren Material hergestelltes Eiheitsbandscheiben-Element1 dar. Das Scheiben-Element1 weist rechtwinklig zur Achse des Scheiben-Elementes angeordnete parallele Schlitze5 auf. Die parallelen Schlitze5 überlappen einander teilweise, wobei sie überlappende Bereiche6 zwischen benachbarten Schlitzen definieren. Die überlappenden Regionen6 gestalten Blattfedern7 zur Übertragung von Kräften von einer Rückenwirbel-Befestigungsoberfläche auf die andere aus. In Bereichen nicht überlappender benachbarter Schlitze5 ist die Federtätigkeit der Blattfedern7 durch Fixierungszonen9 aus solidem Prothesenmaterial unterbrochen. Die auf die Bandscheibe einwirkenden Kräfte werden über die Fixierungszonen9 von einer Blattfeder-Ebene auf die nächste übertragen. - Jedoch sind die Lastwege durch die wenigen Fixierungszonen
9 an sich unvermittelt sprunghaft (inherently abrupt) mit hoher lokaler Lastenübertragung. Es gibt sogar Umstände, bei denen die vollständige Last durch eine einzige Fixierungszone9 in der Mitte der Scheibe getragen wird. Die unvermittelten Lastenwege können zu hohen Stressbereichen führen, die die angemessenen biomechanischen Leistungen der Prothese, wie z. B. Stärke, Flexibilität und Bewegungsbereich, schmälern. - Daher besteht der Bedarf an einer Prothesenscheibe, die einfach herzustellen ist und eine geeignete Ausgewogenheit von Flexibilität und Stabilität durch verbesserte Lastenverteilung zur Verfügung stellt.
- ZUSAMMENFASSUNG
- Gemäß der vorliegenden Erfindung wird eine Bandscheiben-Prothese zur Verfügung gestellt, welche ein Bandscheiben-Element umfasst, das zum Einsetzen in den Bandscheiben-Raum zwischen benachbarten Rückenwirbeln dimensioniert ist, um zumindest einen Teil einer hiervon entfernten Bandscheibe zu ersetzen, wobei das Scheibenelement eine Längsachse, welche sich entlang der Höhe des Scheibenelements erstreckt, sowie eine radiale Achse quer zu der Längsachse definiert, wobei das Scheibenelement eine äußere Wand, welche eine Vielzahl von Schlitzen darin aufweist, definiert, wobei benachbarte Schlitze in zumindest teilweise Überlappender Beziehung in Bezug auf die Längsachse angeordnet sind, und dadurch gekennzeichnet ist, dass: jeder Schlitz eine erste Längskomponente einer Richtung parallel zu der Längsachse und eine zweite Komponente einer Richtung, welche von der ersten Komponente einer Richtung abweicht, definiert.
- Vorzugsweise beinhaltet die äußere Wand eine Vielzahl spiralförmiger Schlitze, wobei benachbarte Schlitze in radialer Beziehung in Bezug auf die Längsachse angeordnet sind, wodurch die Lastenübertragung entlang der äußeren Wand geschieht. Die Schlitze geben der äußeren Wand eine Flexibilität, die in Einklang mit der natürlichen Bandscheibe steht.
- Die Scheibe kann weiterhin eine innere Aushöhlung beinhalten. Vorzugsweise erstreckt sich der Schlitz/erstrecken sich die Schlitze von einer äußeren Wandoberfläche der äußeren Wand bis zu einer inneren Wandoberfläche dieser in Verbindung mit der inneren Aushöhlung. Erste und zweite längs gegenübergestellte Stützoberflächen sind an den Längsenden der Scheibe angeordnet. Die Stützoberflächen sind so dimensioniert, dass sie abstützend in Rückenwirbelabschnitte der jeweiligen Rückenwirbel eingreifen. Wenigstens eine der ersten und der zweiten Stützoberflächen weist eine sich dort hindurch erstreckende Öffnung in Verbindung mit der inneren Aushöhlung auf.
- Eine Endkappe kann entfernbar an der Stützoberfläche befestigt und zumindest teilweise innerhalb der Öffnung der Stützoberfläche positionierbar sein. Die Endkappen können eine innere Öffnung enthalten, welche so dimensioniert ist, dass sie deren Steifheit reduziert.

- Bezugnehmend auf die Zeichnungen, in denen gleiche Bezugszeichen gleiche oder ähnliche Elemente durch die verschiedenen Ansichten hinweg ausweisen, und Bezugnehmend im Besonderen auf die
4 –6 , wird nun die künstliche Bandscheiben-Prothese gemäß der vorliegenden Offenbarung dargestellt. Die Bandscheiben-Prothese100 ist dazu gedacht, teilweise oder insgesamt die Stützfunktion einer erkrankten Bandscheibe zu ersetzen, welche vorher durch ein Discectomie – Verfahren oder ähnlich entfernt wurde. Die Bandscheiben-Prothese100 ist vorteilhaft zur Positionierung zwischen benachbarten Wirbeln in stützender Kontaktbeziehung mit deren Wirbel-Endplatten dimensioniert, um die benachbarten Wirbel in geeigneter räumlicher Lage während der Wiederherstellung der natürlichen Biomechanik (z. B. Steifheit, Bewegungsbereich und Stärke beinhaltend) der Wirbelsäulen- oder Wirbelsegmente zu halten. - Die Bandscheiben-Prothese
100 beinhaltet zwei Grundkomponenten, nämlich das Scheiben- oder Körperelement102 und die ersten und zweiten Endkappen104 ,106 , welche entfernbar auf dem Körperelement102 befestigt sind. Das Körperelement102 weist, wie abgebildet, eine allgemeine Form (z. B. Nierenform) einer Bandscheibe auf und definiert eine sich entlang der Höhe des Elementes102 erstreckende Längsachse „a" und eine im Allgemeinen quer zu der Längsachse „a" liegende radiale Achse „b". Eine Winkelbeziehung ist wie abgebildet durch „c" definiert (5 ). Das Körperelement102 beinhaltet eine erste und zweite längs gegenübergestellte (z. B. obere und untere) Stützoberfläche108 ,110 , welche abstützend in die jeweiligen Stirnseiten der benachbarten Wirbel beim Einsetzen der Prothese eingreifen, und die sich zwischen den Stützoberflächen108 ,110 erstreckende innere Wand112 . Die Stützoberflächen108 ,110 sind jeweils von bogenförmiger Gestalt, wobei sie eine leichte Außenkrümmung definieren, die vorzugsweise mit der leichten Innenkrümmung der Wirbel-Endplatten korrespondiert, um die Positionierung und die Retention der Prothese innerhalb des Wirbelraumes zu erleichtern. - Das Körper-Element
102 beinhaltet weiterhin einen zentral gelegenen Kanal (cannulation)116 in allgemeiner Ausrichtung zur Längsachse „a" und sich durch die Stützelemente108 ,110 (5 ) erstreckend. Der Kanal oder die Bohrung116 definiert eine innere Aushöhlung114 und zentrale Öffnungen118 der Stützoberflächen108 ,110 . In der in4 veranschaulichten Ausführungsform sind die Öffnungen118 korrespondierend dimensioniert, um zumindest teilweise ihre jeweiligen Endkappen104 ,106 aufzunehmen. Eine vergrößerte umlaufende Vertiefung120 ist innerhalb jeder Stützoberfläche108 ,110 um die Peripherie jeder Öffnung118 herum definiert, um den Kopf-Abschnitt auf den Endkappen104 ,106 aufzunehmen. Wie abgebildet befinden sich die einmal eingeführten Endkappen104 ,106 im Allgemeinen auf gleicher Ebene mit den oberen und unteren Oberflächen114 . Die Endkappen104 ,106 stellen weitere Oberflächen134 zur Knochenbefestigung zur Verfügung und verhindern Knochenwachstum in das Körper-Element102 hinein. Die Eingriffsoberflächen142 ,144 der Endkappen104 ,106 berühren während hoher Belastung einander und dienen verschiedenen Zwecken: (1) Sie bewahren die äußeren Wände112 vor einer Überbelastung, indem sie einen alternativen Lastenweg (durch das Zentrum der Scheibe) zur Verfügung stellen; (2) Sie erhöhen die Gesamtsteifheit der Scheibe100 auf ähnliche Weise wie die natürliche Scheibe, welche bei höheren Lasten steifer wird; und sie verhindern eine vollständige Verschließung der im Wesentlichen spiralförmigen Schlitze122 , wobei sie einen „Einklemm"-Effekt auf umgebendes weiches Gewebe vermindern. Die innere Bohrung138 mit ihren einbezogenen geschlitzten Öffnungen140 vermindert effektiv die Steifheit der Endkappen104 ,106 , sodass die Gesamtsteifheit der Scheibe100 stärker in Einklang mit der natürlichen Bandscheibe steht. - Weiterhin mit Bezug auf die
4 –6 weist die äußere Wand112 eine Vielzahl von darin definierten Schlitzen122 auf, welche sich, in der bevorzugten Ausführungsform, vollständig durch die äußere Wand von ihrer äußeren Oberfläche124 bis zu ihrer inneren Oberfläche126 in Verbindung mit der inneren Aushöhlung114 (6 ) erstrecken. Jeder Schlitz122 ist im Allgemeinen von spiralförmiger Gestalt, d. h., jeder Schlitz122 verfügt, wie abgebildet, über eine Längskomponente der Ausrichtung und eine Winkelkomponente der Ausrichtung. Diese verschiedenen Richtungskomponenten, z. B. eine Längs- und eine Seitenrichtung, führen zu einem multi-direktionalen Pfad für jeden der Schlitze122 . Die Schlitze122 sind vorzugsweise um die äußere Wand herum an vorbestimmten regelmäßigen radialen Plätzen angeordnet, wobei sich die benachbarten Schlitze122 in teilweise überlappender Anordnung befinden. In der dargestellten Ausführungsform sind fünf Schlitze vorgesehen, welche regelmäßig in 72° – Intervallen radial angeordnet sind, auch wenn eine alternative Anzahl an Schlitzen und andere regelmäßige Intervalle vorgesehen sind. Die Schlitze122 erstrecken sich, wie abgebildet, um etwa 180° um die äußere Wand112 , auch wenn sie sich um mehr als 180° oder weniger als 180° erstrecken können. Es kann ein einziger, im Wesentlichen spiralförmiger Schlitz benutzt werden, jedoch sieht die bevorzugte Ausführungsform eine Vielzahl von im Wesentlichen spiralförmigen Schlitzen122 vor. Die spiralförmigen Schlitze122 sind in radialem Verhältnis in Bezug auf die radiale Achse „b" und den Winkel „c" versetzt. Der verbleibende Lastenweg128 der Scheibenwand112 weist ein federförmiges Erscheinungsbild auf, ähnlich einer Druck- oder Spiralfeder. Die Vielzahl von Lastenwegen128 bildet eine flexible Scheibenwand112 und erlaubt den Lastentransfer zwischen der oberen Stützoberfläche108 und der unteren Stützoberfläche110 auf gleichmäßige Weise ohne abrupte Lastenwege. - Auch wenn spiralförmige Schlitze abgebildet sind, ist es ebenfalls vorgesehen, dass andere multi-direktionale Schlitze, die z. B. eine seitliche Richtungskomponente und eine Längsrichtungskomponente aufweisen, benutzt werden können. Dies kann glatte, teilweise glatte, offen geschlungene Schlitze etc. beinhalten.
- Weiterhin mit Bezug auf die
4 –6 definieren die Endkappen104 ,106 jeweils einen umlaufenden Leisten- oder Kopfabschnitt130 und einen Hauptabschnitt132 von verminderter Abmessung. Die Endkappen104 ,106 werden zumindest teilweise innerhalb der zentralen Öffnungen118 der Stützoberflächen108 ,110 auf eine Weise erreicht, durch die der umlaufende Kopfabschnitt130 in einer entsprechend dimensionierten umlaufenden Vertiefung120 der Stützoberfläche108 ,110 liegt und sich der Hauptabschnitt132 innerhalb des Kanals116 erstreckt. Die äußere Oberfläche134 jeder Endkappe104 ,106 weist vorzugsweise eine Bogenform auf, wobei sie im Wesentlichen mit der bogenförmigen Gestalt der äußeren Stützoberfläche108 ,110 korrespondiert, um einen weichen Übergang von den äußeren Stützoberflächen108 ,110 zu den Endkappen auszuformen. Jede Endkappe104 ,106 beinhaltet weiterhin einen in der äußeren Stützoberfläche134 definierten Einschnitt136 zur Befestigung eines Instrumentes, um die Endkappen104 ,106 während des Einsetzens in die zentralen Öffnungen118 des Hauptkörpers102 entfernbar zu halten. Der Einschnitt136 ist im Allgemeinen kleeblattförmig, auch wenn andere Formen vorgesehen sind, einschließlich rechtwinklige, sechseckige usw., um geeignete Instrumente aufzunehmen. Der Hauptabschnitt132 jeder Endkappe104 ,106 weist eine zentrale innere Bohrung oder Aushöhlung138 auf, welche sich durch seine äußere Wand erstreckt, um eine Vielzahl (z. B. 4) von radial angeordneten geschlitzten Öffnungen140 zu definieren. Die interne Bohrung138 mit ihren einbezogenen radialen Öffnungen140 reduziert effektiv die Steifheit der jeweiligen Endkappen104 ,106 . Die Kappen können zur weiteren Reduzierung der Steifheit alternativ spiralförmige Schlitze an Stelle der Öffnungen140 aufweisen. - Die Komponenten der Bandscheiben-Prothese
100 werden aus geeignetem starrem Material wie rostfreiem Stahl, Titan oder geeignetem polymerischen Material gefertigt. Vorzugsweise ist das Körper-Element102 monolithisch als einzelne Einheit geformt, auch wenn es vorgesehen ist, dass in einer alternativen Ausführungsform das Körper-Element102 aus separaten Komponenten zusammengesetzt ist, wovon jedes, wie oben diskutiert, die strukturellen Merkmale, wie z. B. spiralförmige Schlitze und innere Aushöhlung, aufweisen würde. Beispielsweise können drei Komponenten benutzt werden, welche, wenn sie in Nebeneinanderstellung in dem Wirbelraum platziert werden, die Nierenform von4 ausformen. - Einsetzen der künstlichen Bandscheibe
- Das Einsetzen der künstlichen Bandscheibe wird mit Bezug auf die
7 –8 diskutiert. Der zwischen den benachbarten Wirbeln „V1, V2" definierte Wirbelraum „i" wird durch Einsatz von Rückhalteinstrumenten oder ähnlich zugänglich gemacht. Danach wird eine teilweise oder vollständige Discectomie durchgeführt, um den erkrankten Abschnitt der Scheibe zu entfernen. Die benachbarten Wirbel werden durch geeignete Distractor – Instrumente auseinandergerückt, um den Wirbelraum freizulegen. Die künstliche Bandscheiben-Prothese100 wird dann innerhalb des Wirbelraumes „i" positioniert. Bei der Platzierung greifen die oberen und unteren Stützoberflächen108 ,110 in stützendem Verhältnis in die jeweiligen Wirbel-Endplatten der benachbarten Wirbel ein. Wie oben erwähnt, nähern sich die durch die äußeren Stützoberflächen134 der Endkappen104 ,106 und die äußeren Oberflächen der oberen und unteren Stützoberflächen108 ,110 definierten gebogenen Konturen der gebogenen Kontur der Wirbel-Endplatten an, um angeschmiegt in die benachbarten Wirbel zu passen und die Retention innerhalb des Wirbelraumes zu erleichtern. - Wie vorhergehend angezeigt, ist die künstliche Bandscheiben-Prothese
100 dadurch charakterisiert, dass sie eine ausreichende Steifheit zur Aufrechterhaltung der benachbarten Wirbel in regelmäßiger Beziehung aufweist, während sie eine angemessene Flexibilität besitzt, um Beugebewegungen der Wirbelsäule zuzulassen. Die auf die Bandscheiben-Prothese100 angewandten Belastungen werden durch die äußere Wand112 zwischen den oberen und unteren Stützoberflächen108 ,110 entlang von im Wesentlichen ununterbrochenen Wegen über die spiralförmige Schlitz-Anordnung122 und die Vielzahl von Lastenwegen128 übertragen. - Alternative Ausführungsform(en)

9 stellt eine alternative Ausführungsform der vorliegenden Offenbarung dar. Die Bandscheiben-Prothese200 beinhaltet ein Scheiben- oder Körperelement202 , welches im Wesentlichen ähnlich dem Körper-Element102 der Ausführungsform aus4 ist. Jedoch sind in Übereinstimmung mit dieser Ausführungsform die Endkappen104 ,106 eliminiert, sodass die Stützoberflächen208 ,210 ununterbrochen sind. Ebenfalls existieren keine Öffnungen118 innerhalb der Stützoberflächen wie in der Ausführungsform aus4 (siehe Oberflächen108 ,110 ). Die Aushöhlung oder Bohrung (nicht abgebildet) erstreckt sich intern zwischen den Oberflächen208 ,210 . Daher handelt es sich, in Übereinstimmung mit dieser Ausführungsform, um eine einzige, monolithisch geformte Einheit. Die Prothese200 kann interne „Kappen" beinhalten, die sich untereinander bei schwerer Last berühren, um dadurch in ähnlicher Weise wie die Kappen104 ,106 der Prothese100 aus4 zu funktionieren. -
10 stellt eine weitere alternative Ausführungsform der vorliegenden Offenbarung dar. Die Prothese300 ist im Wesentlichen ähnlich der Prothese100 aus4 , jedoch beinhaltet die äußere Wand312 , in Übereinstimmung mit dieser Ausführungsform, einen einzelnen ununterbrochenen spiralförmigen Schlitz302 , welcher sich von einer der oberen Stützoberfläche308 benachbarten Position bis zu einer der unteren Stützoberfläche310 benachbarten Position erstreckt. Die Lastenwege werden durch die Bezugsnummer308 bezeichnet. Dies stellt eine höhere Flexibilität zur Verfügung. Der ununterbrochene Schlitz302 definiert überlappende Bereiche, worin längsverschobene Abschnitte des ununterbrochenen Schlitzes in teilweise überlappender Beziehung stehen. Diese überlappenden Bereiche des ununterbrochenen Schlitzes302 sorgen ebenfalls für einen ununterbrochenen Lastentransfer von der oberen Stützoberfläche108 zu der unteren Stützoberfläche110 , wobei der Vorteil einer solchen Anordnung vorhergehend diskutiert wurde. Die Endkappen104 und106 können optional vorgesehen sein. - Verschmelzungskäfig mit spiralförmigem Schlitz (spiralförmigen Schlitzen)
- Die vorliegende Offenbarung beinhaltet ebenfalls einen einzigartigen Verschmelzungskäfig, wie in
11A und11B dargestellt und im Allgemeinen mit der Bezugsnummer500 bezeichnet. Beim Einsatz von Wirbelsäulen-Verschmelzungskäfig ist eine Lastenteilung mit der innerhalb des Käfigs eingepackten Knochenverpflanzung notwendig, um die Knochenverpflanzung in eine solide Knochen-Arthrodese umzuformen. Bei herkömmlichen Verschmelzungskäfigen, wie solchen, die aus Titanlegierungen gefertigt sind, ist der Käfig steif, was dazu führt, dass der Käfig zum maßgeblichen Lastenweg während des Verschmelzungs-Vorganges wird. - Der Verschmelzungskäfig der vorliegenden Offenbarung ist vorzugsweise aus einer Titanlegierung zusammengesetzt. Jedoch beinhaltet der Käfig eine Schlitz-Anordnung zur Reduzierung der Steifheit. Das bedeutet, dass die spiralförmigen Schlitze
522 den Käfig mit einer zusätzlichen Flexibilität versehen, so dass sie sich unter Last biegen, was zu einer größeren Lastenteilung mit der Verpflanzung führt. Wie anzunehmen, weist der Verschmelzungskäfig500 die identische spiralförmige Schlitz-Anordnung wie die Scheiben-Prothese aus4 auf, weshalb die Schlitz-Anordnung nicht ein weiteres Mal beschrieben wird. Es ist zu beachten, dass das Schlitz-Design aus10 ebenfalls genutzt werden kann. - Der Käfig
500 beinhaltet eine interne Aushöhlung502 zur Aufnahme von Knochenverpflanzungsmaterial „g" (siehe11A ). Endkappen (nicht abgebildet) können vorgesehen sein, um zu helfen, dass Knochenverpflanzungsmaterial festzuhalten und die Beugung wie oben beschrieben zu begrenzen, vorausgesetzt, dass die Kappen Öffnungen aufweisen, die mit der inneren Aushöhlung502 kommunizieren, um den Kontakt zwischen dem Knochenverpflanzungsmaterial und den Wirbeln sicherzustellen. Ist erst einmal der Käfig500 in dem Wirbelraum „i" platziert, wobei die Stützoberflächen508 ,510 die Wirbel berühren, verschmilzt mit der Zeit dieses Knochenverpflanzungsmaterial innerhalb des Käfigs502 mit den benachbarten Wirbeln. Wie in11A dargestellt, ist wie bei herkömmlichen Verschmelzungskäfigen der Käfig500 schmaler als der gesamte Scheibenraum. Es ist vorgesehen, auch wenn nur ein Käfig abgebildet ist, dass zwei oder mehr Käfige500 Seite an Seite in dem Scheibenraum platziert werden können. - Weil der Verschmelzungskäfig
500 nicht den vollständigen Scheibenraum ausfüllt, sind auch andere Formen als die Nierenform aus11A und11B vorgesehen, sofern sie die Schlitz-Anordnung zur Reduzierung der Gesamtflexibilität enthalten. - Selbstverständlich können verschiedene Modifikationen der hier offenbarten Ausführungsform vorgenommen werden. Daher sollte die obige Beschreibung nicht als Einschränkung sondern lediglich als eine Veranschaulichung einer bevorzugten Ausführungsform aufgefasst werden.
Claims (14)
- Bandscheiben-Prothese (
100 ), welche ein Bandscheiben-Element (102 ) umfasst, welches zum Einsetzen in den Bandscheiben-Raum zwischen benachbarten Rückenwirbeln dimensioniert ist, um zumindest einen Teil einer hiervon entfernten Bandscheibe zu ersetzen, wobei das Scheibenelement (102 ) eine Längsache (a), welche sich entlang der Höhe des Scheibenelements erstreckt, sowie eine radiale Achse (b) quer zu der Längsache def iniert, wobei das Scheibenelement (102 ) eine äußere Wand (112 ), welche eine Mehrzahl von Schlitzen darin aufweist, definiert, wobei benachbarte Schlitze in zumindest teilweise überlappender Beziehung in Bezug auf die Längsachse angeordnet sind, und dadurch gekennzeichnet, dass jeder Schlitz eine erste Längskomponente einer Richtung parallel zu der Längsachse (a) und eine zweite Komponente einer Richtung, welche von der ersten Komponente einer Richtung abweicht, definiert. - Bandscheiben-Prothese gemäß Anspruch 1, wobei Teile der zumindest drei Schlitze (
122 ) in überlappender Beziehung angeordnet sind. - Bandscheiben-Prothese gemäß einem der voranstehenden Ansprüche, wobei benachbarte Schlitze in radialer Beziehung mit Bezug zu der Längsachse angeordnet sind.
- Bandscheiben-Prothese gemäß einem der voranstehenden Ansprüche, wobei die zweite Richtung eine schräge Komponente der Richtung definiert.
- Bandscheiben-Prothese gemäß einem der voranstehenden Ansprüche, wobei das Bandscheiben-Element eine erste (
108 ) und eine zweite (110 ) Stützoberfläche beinhaltet, welche an den jeweiligen Längsenden des Bandscheiben-Elements angeordnet ist und so dimensioniert ist, dass sie abstützend in Rückenwirbelabschnitte der jeweiligen Rückenwirbel eingreift. - Bandscheiben-Prothese gemäß Anspruch 5, wobei das Bandscheiben-Element eine zentrale Öffnung (
118 ) beinhaltet, welche sich durch eine oder mehrere der Abstützoberflächen (108 ,110 ) erstreckt. - Bandscheiben-Prothese gemäß Anspruch 5 oder 6, des weiteren beinhaltend eine Endkappe (
104 ), welche entfernbar an der ersten Abstützoberfläche (108 ) befestigt ist, und wobei die Endkappe innerhalb der Öffnung (118 ) positionierbar ist. - Bandscheiben-Prothese gemäß Anspruch 5, beinhaltend eine erste (
104 ) und eine zweite (106 ) Endkappe, welche entfernbar an der jeweiligen ersten (108 ) und zweiten (110 ) Abstützoberfläche befestigt ist, wobei die Endkappen innerhalb der jeweiligen Öffnungen (118 ) der ersten und zweiten Abstützoberflächen positionierbar ist. - Bandscheiben-Prothese gemäß einem der Ansprüche 5 bis 8, wobei die Endkappen (
104 ,106 ) eine innere Öffnung beinhalten, welche so dimensioniert ist, dass sie die Steifheit der Endkappen minimiert. - Bandscheiben-Prothese gemäß einem der Ansprüche 5 bis 9, wobei das Bandscheiben-Element monolithisch ausgeformt ist und damit ausgeformte erste (
108 ) und zweite (110 ) Stützoberflächen aufweist. - Bandscheiben-Prothese gemäß einem der voranstehenden Ansprüche, wobei das Bandscheiben-Element eine innere Aushöhlung (
114 ) beinhaltet. - Bandscheiben-Prothese gemäß Anspruch 11, wobei die Wand (
112 ) eine äußere Oberfläche und eine innere Wandoberfläche definiert und wobei jeder Schlitz (122 ) sich von der äußeren Wandoberfläche zu der inneren Wandoberfläche erstreckt, um mit der inneren Aushöhlung zu kommunizieren. - Bandscheiben-Prothese gemäß einem der voranstehenden Ansprüche, wobei jeder der Schlitze (
122 ) im Wesentlichen spiralförmig ist. - Bandscheiben-Prothese gemäß einem der voranstehenden Ansprüche, wobei die auf das Bandscheiben-Element einwirkenden Längskräfte entlang der äußeren Wand (
112 ) übertragen werden.
