Abstract

The invention concerns a medicament application device for syringe pumps, the device comprising at least two hydraulic elements which are interconnected by a fluid-filled pression-resistant pipe and of which one, disposed remote from the patient, is used as a pump and the other, disposed near the patient, is used as an advance device for a medicament syringe.

Description

Medikamenten-Applikationsvorrichtung für Spritzenpumpen Drug application device for syringe pumps
The invention relates to a hydraulic medication application infusion device according to the preamble of patent claim 1.
For the precise, intravenous and subcutaneous medication application infusion, expensive, complicated, heavy and bulky, electrically operated infusion pumps with complex alarm devices are used almost exclusively today, which, moreover, are usually only used for the application of large quantities of liquid, but not, for example, for a few milliliters of a drug. For keeping intravenous access open, especially venous catheters, for example during transport to the hospital, for small surgical interventions or special examinations such as X-ray diagnostics, nuclear medicine, metabolism, NMR spectroscopy and other measures, but also for postoperative pain therapy, especially that of the patient Controlled, when transporting with the ambulance and not least with outpatient pain and infusion therapy at home, other, simpler drug application infusion systems would be desirable. These systems should be simple and safe to use, inexpensive, lightweight and small in size, and should have a largely maintenance-free drive and at the same time enable precise iv medication application and infusion therapy. Systems on which such considerations are based represent, for example, the already available, light-weight, easier-to-use, largely maintenance-free special spring-driven infusion syringe pumps which, in combination with fixed flow restrictors connected by hoses, for certain flow volumes per unit of time are supplied.
Noch bestehende Probleme hierbei sind aber, daßProblems that still exist here are that
1. die als Durchflußbegrenzer verwendeten Microlumenschläuche oder gezogenen Glasmikrokapillaren schwer, exakt in Serie mit den geforderten geringen Toleranzen herstellbar sind und deswegen nur zu einem noch sehr hohen Preis angeboten werden können,1. the microlumen tubes or drawn glass microcapillaries used as flow restrictors are difficult to manufacture exactly in series with the required small tolerances and can therefore only be offered at a very high price,
Dosing inaccuracies of these flow limiters occur when the viscosity of the infusion solutions changes, not only as a result of additives, but above all as a function of the temperature.
Too large dead space volumes of these tubes with their connectors occur, especially if the smallest infusion volumes per unit of time, e.g. should be applied for pain therapy,
4. ein rascher, unkomplizierter Wechsel der Infusionsge¬ schwindigkeit nicht ohne weiteres möglich ist,4. a quick, uncomplicated change of the infusion speed is not readily possible,
Long preparation times when filling the voluminous infusion lines with their connectors, filters and adapters without air bubbles have to be accepted, above all when using flow restrictors (microlumen tubing, capillaries) for very low infusion speeds,
6. noch zu große schwere Infusionspumpen und zu umständlich zu bedienende Infusionssysteme verwendet werden müssen und6. still too large heavy infusion pumps and cumbersome to use infusion systems must be used and
Small, usual amounts of medication in the range of 1-10 ml cannot be applied.
The object of the invention is to develop a generic medication application infusion device so that it enables a technically simple, exact medication application infusion at low cost.
This object is achieved by a device having the features of claim 1. Advantageous refinements and developments are the subject of the dependent claims.

This is inserted with its syringe cylinder 1 provided with a Luer lock connection 2, which also carries the two tabs 3, into the fixed holder of the syringe pump 4, which is shown schematically in broken lines. The syringe plunger 5, on which the compression spring 6 acts, is connected to the syringe plunger 7 with the pressure plate 8. This pressure plate 8 is inserted into the holding device of the syringe pump feed 9, shown schematically in dashed lines. Part B, the pressure-resistant infusion line 10 via its connectors 11, 12 are not detachably connected to the Luer lock connection 13 of the syringe barrel 14 of part C via the Luer lock connection 2. In this cylinder 14, a tightly closing piston 15 with a pressure transmission disc 16 is fitted. This transfers the pressure to the pressure plate 17, the stamp 18 and the piston 19 connected to it. Part D with cylinder 20, Luer lock connection 21 and the two tabs 23 is a sterile disposable syringe which is filled with medication. This syringe is locked with its tabs 23 in the locking element formed by the base plate 24 and cover plate 25. Via the male Luer lock connection 21, part D can be detachably connected and locked to a line leading to the patient P and provided with a female Luer lock connector.
Fig. 2 shows the in an exploded perspective view hydraulic feed device with syringe locking element. The feed device consists of the syringe cylinder 14 with the two tabs 22 and the piston 15 with pressure transmission disk 16. The syringe locking element 23 consists of the base plate 24, provided with the round cutout 27 and the two connecting holes 28 ¬ the spacer rollers 29 and the two rivets 30, the thin, resilient cover plate 25 with its oval cutout 31 via the two connecting holes 32 with the rigid base plate 24 and the two tabs 22 of the syringe barrel is firmly connected to form a unit.
Bei stärkerer als der gezeichneten Medikamentenfüllung des Sprit¬ zenzylinders 20 wird bei verschlossenem Anschluß 21 der Kolben 19 mit Stempel 18 und Druckplatte 17, mechanisch geschützt im Zylinder 14 angeordnet, die Druckscheibe 16 und den Kolben 15 in Richtung des Luerlockanschlusses 13 schieben.3 shows a perspective view of the hydraulic feed unit with syringe locking element, completely assembled with an inserted and locked disposable medication syringe D. As can be seen, the syringe cylinder 20 with its two tabs 23 was first inserted into the oval-horizontal opening 31 and subsequently locked by a rotation of 90 degrees around its longitudinal axis, and is now held between the rigid base plate 24 and the resilient cover plate 25. If the medication filling of the syringe cylinder 20 is stronger than that shown, the piston 19 with the stamp 18 and Pressure plate 17, mechanically protected in the cylinder 14, push the pressure plate 16 and the piston 15 in the direction of the Luer lock connection 13.
As schematically shown in FIG. 1, the medicament application device consists of the three parts A, B and C, which are preferably delivered completely assembled, not separable, filled with liquid approximately half free of air bubbles. Part A, the 10-60 ml infusion syringe with compression spring 6 acting on the plunger 5 is now placed in the holder of the syringe pump 4 (dashed line) for the tabs 3 of the cylinder 1 inserted. Now the small medication syringe D is filled with the desired amount of medication solution without air bubbles and subsequently its outlet connector 21, which is provided with a Luer lock connector, is closed with a closure cap. Your protruding syringe plunger 18 with pressure plate 17 is now inserted manually into the cylinder 14 of part C through the locking element formed from the base plate 24 and cover plate 25 and thereby pushes the piston 15 with the pressure disk 16 in the direction of the Luer lock connecting piece 13 Piston 15 displaced liquid from the cylinder 14 enters the cylinder 1 (part A) via the pressure-resistant connecting hose 10, part B, and pushes the piston 5 back somewhat. By rotating the medicament syringe D about its longitudinal axis, preferably by 90 degrees, it is locked between the base plate 24 and cover plate 25. Now the piston plunger 7 with its pressure plate 8 is inserted and fixed in the feed bracket 9 of the syringe pump. The desired infusion quantity per unit of time can now be set on the infusion pump, for example after multiplication by a factor specified on the medication application device, and the pump can be started. Now the patient is still connected via the connecting section 26. The infusion syringe pump now pushes the piston 5 against the defined resistance of the compression spring 6 in the direction of the Luer lock connection 2. The liquid that is displaced thereby passes through the pressure hose 10 into the cylinder 14, pushes the piston there 15 with pressure plate 16 and pressure plate 17 with plunger 18 and piston 19 in the direction of the arrow towards the syringe outlet connector 21. As a result, the medication syringe D is now slowly emptied into the connected patient. If a new medication application is to be carried out after the syringe has been emptied, the procedure described above will be followed, but additionally before the newly filled medicament syringe D is inserted first release the pressure plate 8 from the syringe pump feed bracket 9 so that the piston 15 is pushed back from part C and the liquid displaced can flow back into the cylinder 1 of part A via pressure line 10. That, for example, even when the infusion line is closed to the patient, the infusion pressures are not too high, e.g. B. over max. 1.2 bar can occur, is ensured by the pressure exerted by the compression spring 6 in combination with the back-up liquid on the piston 5, which is transmitted to the syringe pump feed 9 by means of a punch 7 with a pressure plate 8, which then activates the automatic shutdown of the syringe pump . With syringe pumps with variably adjustable pressure, e.g. B. if a lower cut-off pressure is set, the compression spring in cylinder 1 is omitted. It is also possible to provide a similarly acting compression spring in the cylinder 14.
Colored or uncolored water, oil, aqueous glycerin solution or glycerin can preferably be used as the hydraulic fluid.
To avoid impermissibly high pressures, it is also possible to provide alarm pressure settings on the syringe pumps, for example to values of 1.2 to 0.5 bar.
In a second exemplary embodiment of the medication application device (not shown in the drawing), as shown in FIGS. 1-3, the hydraulic elements (1, 14) are not designed as hydraulic cylinders with a tightly fitted piston, but occur here instead of the piston, a tightly fitted roller membrane. In a further third exemplary embodiment (not shown), the piston of the hydraulic element is produced by a bellows not fitted in the hydraulic cylinder, preferably made of plastic or an elastomer. tigt, replaced. This elimination of the pistons, which was carried out in the further exemplary embodiments, makes it possible to circumvent their problems which may arise due to aging processes, in particular changes in their sliding properties in the hydraulic cylinder.
It consists of two infusion syringe cylinders 35, 45 with tightly fitted pistons 37, 42. The cylinders 33, 34 are fixed at a certain distance from one another by holding devices 33, 34. The cylinder 35 contains a spiral compression spring 36 which loads the tightly fitted piston 37 in the direction of arrow 38 and thus moves the adjacent syringe plunger 39 with piston 40 in the syringe cylinder 41 in the direction of its outlet cone 44. The syringe cylinder 41 is part of the disposable syringe 43 flanged over a locking device 42. The second cylinder 45 is closed with an air-permeable cover 46 and contains the tightly fitted piston 47. This is detachably connected to a round piston rod 48 which ends in a handle 51 and has on its outer half a thread 49 on which a stop nut 50 is adjustably arranged. A metering module 52 is locked with the luer connectors 53, 54 of the syringe cylinders 35, 45.
5 shows schematically and in section the medicament application device, ready for use with the patient. The disposable syringe 43, which is filled with medication solution without air bubbles, is connected with its outlet cone 44 to a connecting line (not shown) to the patient. The compressed spiral compression spring 55 expands in the direction of arrow 56 and thus displaces piston 37 and the adjoining one Syringe plunger 39 with piston 40 in the syringe cylinder 41. Since this cylinder 41 is firmly connected to the locking device 42, medication solution is dispensed to the patient. However, in order that this process can take place slowly, as desired, the movement of the compression spring 55, which is striving for expansion, is hydraulically controlled in a controlled manner by drawing in liquid from the syringe cylinder 45 serving as liquid reservoir 57 with piston 47 via the metering module 52 becomes. This module contains a particle filter 59 upstream of the metering nozzle 58 (metering tube, hose) (to keep away any abrasion dirt particles that may occur) and the bypass a one-way valve 60 with low flow resistance and the lowest backflow, which is closed.
The delivery of infusion solution to the patient, controlled by the dosing module, will now continue until the stop nut 50 is stopped by the closure cover 46 and thus also the piston 47 connected via the piston rod 48. In this way, preselected partial volumes can also be achieved na are delivered from the drug syringe to the patient at a controlled, low speed.
Nach kurzer Zeit ist der Zustand, wie in Fig. 1 dargestellt, wiederhergestellt.FIG. 6 shows the state after the drug infusion has ended and the syringe 43 has been removed, as well as the measures required for the device to be used again for the drug application. The spring 61, which has been largely relaxed in the cylinder 35, can now either by manually exerting pressure in the direction of arrow 62 with the aid of the plunger 39, a new injection syringe filled with medication solution, firmly closed on the outlet cone, on the piston 37 or by manually withdrawing the piston 47 the piston rod 48 and the handle 51 be compressed again. In this case, the liquid can flow back into the liquid reservoir 57 quickly and with little resistance via the open one-way valve 63 (even in a very small amount via the nozzle 58). After a short time, the state, as shown in Fig. 1, is restored.
Figuren 7 bis 11 zeigen schematisch im Schnitt weitere Ausfüh¬ rungsbeispiele.FIGS. 7 to 11 show schematically in section further exemplary embodiments.
Damit kann die neue medikamentengefüllte Einmal¬ spritze eingelegt werden und die erneute Infusion beginnen.7 shows schematically in section a pneumatic-hydraulic feed unit after restoration of the operating state with an inserted medication-filled syringe, now ready for use again. When the medication application is started, compressed air flows through the inlet connection 74, for example from a hand blower with a manometer and inflatable cuff, as used for blood pressure measurement, into the feed cylinder 73 with piston 75, piston rod 72 and return spring 76. The piston compresses in the process 75, as shown in FIG. 8, the return spring 76 and displaces the liquid present in the cylinder 73. This flows slowly, only via the metering nozzle (capillary) 58 and the particle filter 59 (one-way valve 66 closed) of the metering module 52 into the reservoir cylinder 57 with piston 47 and vent opening 65. Strictly coupled with the movement of the piston 75 the connected holding device 71 with inserted syringe plunger 70 and thus also the disposable syringe plunger 40 in the drug-filled cylinder 43, the tabs 68 of which are fixed in the holding groove 69, is also advanced in the direction of the syringe outlet nozzle 44 via the piston rod 72. This outlet connector is inserted into the holding device 67 and releases the squeezed medicament to the patient via a connected infusion tube (not shown). If a new medication is to be ment-filled disposable syringe are administered, the holding device 71 is pulled out / back to the stop, for example manually, with the piston rod 72 and the connected piston 75. Liquid thus flows quickly from the reservoir cylinder 57 through the now open one-way valve 64 and in small quantities also through the particle filter 59 and metering nozzle 58 of the metering module 52 into the cylinder 73 while simultaneously relieving the return spring 76. The new drug-filled disposable spring can thus be used syringe and start the infusion again.
To simplify the drawing, the disposable syringe filled with medication and its holders are omitted, since they largely correspond to those of FIGS. 7 and 8. 9 shows the pneumatic syringe feed unit, prepared for the insertion of the drug-filled disposable syringe. As can be seen, a non-rotatable piston rod 77, which carries the holding device 71 for the syringe plunger 39 (only this part of the disposable syringe is shown), is retracted to the maximum.
On the piston rod 77 running in the seals 82, 83, the two attached pistons 78, 79 are each in their associated cylinders in their end position. Now the infusion can be started after the medication syringe has been inserted. For this purpose, a cylinder inlet connection 87 is connected to a compressed air source (not shown). Compressed air now flows into the right cylinder chamber 85 and pushes the working piston 78 seated on the piston rod 77 and the brake piston 79 to the left in the drawing. This process is shown in FIG. 10. In this case, 78 is released from the working piston left cylinder chamber 87, which is separated from the oil-filled brake cylinder by the partition 85, is expelled through the vent opening 89. This movement of the working piston 78 can, however, only take place very slowly, since the brake piston 79 arranged in the oil-filled cylinder can only move to the left as quickly as the oil from its left chamber 91 via a nozzle 80 provided in the piston 79 (one-way valve 81 is closed) can flow into the right chamber 90. This state is shown in FIG. 10 shortly before the medication infusion is closed. If the empty medication syringe is now to be replaced by a full new syringe, the empty syringe must be removed and the piston rod 77 with the syringe plunger holding device 71 must be pulled back manually, for example. This can be accomplished quickly with little effort, since, as shown in FIG. 9, the oil in the brake cylinder can flow quickly and without resistance from the right chamber 90 through the now wide open one-way valve 81 in the piston 79 into the left half of the chamber 91. This makes it possible to quickly reinsert a new drug-filled syringe. This integration of the pneumatic cylinder and metering module, implemented as an oil brake cylinder in a cylinder tube 86 with a partition 85, enables a very compact, flow-prone, precise, easy-to-use, inexpensive medication application device to be implemented.
Fig. 11 zeigt eine weitere Ausführungsform schematisch. Here, a tension spring 93 under tension, which is arranged in a cylinder 92 with a piston 94 and holding device 97 for a syringe, serves as the drive element. A syringe 98 filled with hydraulic fluid, which serves as a reservoir, is inserted into this holding device. From it by the pressure of the piston 94 on syringe ram 95 and piston 96 hydraulic fluid via the pressure connection hose 99 and the intermediate pressure-limiting metering module 100 - (with one-way valve 101 (flow direction to the reservoir 98) and in the bypass the metering nozzle 102 with a downstream pressure-limiting device 103 (constructed, for example, from a pressure capsule as a trigger for a manually spring-loaded pinch valve)) - via a connecting element 104 into the feed device having a cylinder 105 with piston 106 and syringe locking element 109. This causes the plunger 106 to empty the syringe plunger 107 and plunger 108 of the disposable syringe 110 filled with medication and locked with the feed device in the direction of arrow 111 into an infusion hose system, not shown, which is connected to the patient. After the syringe has been emptied, a new full syringe can be inserted effortlessly, as already described, since the piston 106 can be pushed back by squeezing hydraulic fluid via hose 99 and one-way valve 101 into the reservoir syringe 98 with the piston 96 being pushed back.
The main advantage of this device shown and described in FIGS. 4-11 compared to other known systems that only work with a spiral spring drive is that the disposable single-use metering device with its expensive, difficult to manufacture metering hoses (capillaries) can be dispensed with .
14 dargestellten Hydraulikkassetten, in welche als sterile Einmalartikel die Medikamenten-Applikations-Infusions-Beutel eingelegt werden.FIGS. 12 to 14 show a hydraulic medication application infusion device for multiple use, in which only the medication apoplication infusion bags with filler neck and discharge tube that are in direct connection with the patient are sterile disposable articles. It consists of the spring-driven hydraulic device shown in FIG. 12 with a downstream metering device, as Hydraulic pump and the in Fig. 13 and fig. 14 shown hydraulic cassettes, in which the medication application infusion bags are inserted as sterile disposable items.
Fig. 13 zeigt die Hydraulikkassette mit dem eingelegten Medikamenten-Applikations-Infusions-Beutel 133 am Ende des Füllvorgangs. Fig. 14 zeigt noch die Hydraulikkassette mit einem weitgehend entleerten Medikamenten-Applikations-In- fusions-Beutel. As shown in FIG. 12, the hydraulic device 112 consists of two cylindrical, can-shaped parts, a base part 114 and a cover part 115, which are detachably screwed together by means of an applied threaded piece 113. The bag 116 inserted in the lid part 115 and filled with hydraulic fluid is compressed by the spiral pressure fields 117 in the bottom part 114 via the pressure plate 118. Hydraulic fluid is delivered to the metering device 120 via the bag outflow nozzle and the connected pressure line 119. From here it passes through the particle filter 122 to the precision metering capillary 123 and from there via the pressure hose 124 and 136 to the inlet port 125 of the hydraulic cassette. 13 shows the hydraulic cassette with the medication application infusion bag 133 inserted at the end of the filling process. It consists of two cylindrical, nozzle-shaped parts, a base part 126 with inlet connection piece 125 and a cover part 127, which are screwed to one another in a readily detachable manner by means of an applied threaded piece 128. In the cover part 127 there is the medication application filled with medication infusion solution via the large syringe 129, the connection piece 130, the Y-piece 131 (one leg of which is closed), the bag connection hose and connection piece 132 Infusion bag 133 (dashed lines), inserted. As indicated by arrows, he presses the roller membrane 34 so deeply into the bowl-shaped bottom part 126 that only a gap-shaped space 135 remains which is filled with hydraulic fluid. In the medication bag filling process, the hydraulic fluid was pressed by pressure on the roller membrane 134 via the connector 125, the connection hose 136, the connecting hose 124 and the disposable valve 121 of the metering device 120 and via hose 119 into the hydraulic bag 116 of the spring-driven hydraulic device 112. 14 also shows the hydraulic cassette with a largely empty medication application infusion bag. As can be clearly seen, the pressurized hydraulic fluid which has flowed in via the connecting hose 124 and 136 as well as the connector 125 has greatly expanded the space 137 delimited by the rolling membrane 134 with compression and emptying of the medication application infusion bag 140 via the outlet connector and hose 138, the Y-piece 131 and the connecting hose to the patient 139. The second leg of the Y-piece with the connector 130 is securely closed by the closure piece 141. In order to be able to start a new medication application infusion therapy, all that is required is to insert a new sterile medication application infusion bag after manually unscrewing the cover part 127 of the hydraulic cassette and to fill it with the hydraulic cassette screwed on, as described . However, it is even easier and more hygienically safe to use industrially pre-filled medication application infusion bags.
